Take Home Messages (I)
� Painful neuropathy is relatively frequent in IGTas compared with NGT (and IFG).
� CVD risk factors (obesity, low physical activity)are associated with DPN and CAN, while peripheralarterial disease is a major comorbidity of DPN.
� A broad spectrum of abnormalities in the somato-sensory, autonomic, and central nervous system canbe detected at the time of diagnosis of type 1 andtype 2 diabetes.
� Small and large nerve fiber alterations develop inparallel in both type 1 and type 2 diabetes.
� Near-normoglycemia maintained from the time ofdiagnosis of type 1 diabetes may completely preventthe development of DPN and CAN over 24 years andstabilizes nerve dysfunction at later stages.
� Early detection of incipient nerve dysfunction and adequate consideration of associated risk factors and comorbidities should open up new strategies to prevent the clinical manifestations of neuropathies in diabetes and prediabetes.

� Plasma markers of oxidative stress (superoxide anion) predict the development and progression of DPN and CAN over 6 years.
Conclusions
� The predictive value and usefulness of the novelmarkers as surrogate endpoints in clinical trials remainto be established in long-term prospective studies.
� Diabetic neuropathy is a predictor of cardiovascularmorbidity and mortality, and its prevalence is increasedeven in prediabetes.
� Recently diagnosed diabetic subjects show an early lossof epidermal nerve fibers, Langerhans cells and corne alnerve fibers as well as evidence of systemic oxidative and carbonyl stress and inflammation.
