Conclusions• La jonction neuromusculaire recèle encore bien des secrets• La curarisation améliore les conditions d’ IOT / opératoires• La durée d’action des curares est variable.• Cette variabilité prédispose à la curarisation résiduelle (40%)• La curarisation résiduelle conserve en 2011 des conséquences
respiratoires graves (1%)• La néostigmine, bien que globalement efficace, souffre de
nombreuses limitations (seuil, dose, délai)• Le sugammadex provoque une décurarisation très rapide, ce
qui favorise l’utilisation clinique d’un bloc neuromusculaire profond, en faveur de la chirurgie.
• Le monitoring objectif est recommandé pour suivre toute curarisation, assurer l’adéquation dose / effet des drogues administrées et, garantir l’absence de curarisation résiduelle avant l’extubation.
