お母さんの薬の中のヨウ素が、胎盤をとおして赤ちゃんに移行し、赤ちゃんの甲状腺にたまって甲状腺機能低下症を起こしたと考えられ、それ以後、妊婦での服用が止められました 。

Transient Hypothyroidism or Persistent Hyperthyrotropinemia in Neonates Born to Mothers with Excessive Iodine Intake

Perinatal exposure to excess iodine can lead to transient hypothyroidism in the newborn. In Japan, large quan- tities of iodine-rich seaweed such as kombu (Laminaria japonica) are consumed. However, effects of iodine from food consumed during the perinatal period are unknown. The concentration of iodine in serum, urine, and breast milk in addition to thyrotropin (TSH), free thyroxine (FT4
), and
thyroglobulin was measured in 34 in- fants who were positive at congenital hypothyroidism screening. Based on the concentration of iodine in the urine, 15 infants were diagnosed with hyperthyrotropinemia caused by the excess ingestion of iodine by their mothers during their pregnancy. According to serum iodine concentrations, these infants were classified into group A (over 17 µ,g/dL) and group B (under 17 µ,g/dL) of serum iodine. During their pregnancies these moth- ers consumed kombu, other seaweeds, and instant kombu soups containing a high level of iodine. It was cal- culated that the mothers of group A infants ingested approximately 2300–3200 µ,g of iodine, and the mothers of group B infants approximately 820–1400 µ,g of iodine per day during their pregnancies. Twelve of 15 infants have required levo-thyroxine (LT

) because hypothyroxinemia or persistent hyperthyrotropinemia was pres- ent. In addition, consumption of iodine by the postnatal child and susceptibility to the inhibitory effect of io- dine may contribute in part to the persistent hyperthyrotropinemia. We propose that hyperthyrotropinemia re- lated to excessive iodine ingestion by the mother during pregnancy in some cases may not be transient.

熊本県で新生児マススクリーニングをうけた赤ちゃんで、ろ紙血中のTSHが高値で、先天性甲状腺機能低下症が疑われた例の中に、
お母さんが昆布や昆布だしを沢山摂取し、そのため赤ちゃんがヨウ素過剰状態となって、甲状腺機能低下症となった例が、大勢いたことがわかったという報告です

まず:海産物昆布を摂食する際の甲状腺機能に関する効果を、正常日本人成人でスタディーしました。15-30g/ 7-10日間の短期間、毎日摂食すると、明らかに血清TSH濃度が上昇し、いくつかの検体では正常上限を超えていました。血清FT4やFT3は、正常範囲内でしたが、軽度低下しました。 昆布を55-87日間毎日15gという長期間摂食した場合、同様のTSHレベルは上昇し、FT4・FT3値がほぼ変化なく維持されました。 尿中ヨード排泄は、昆布摂食中は明らかに増加していました。これらの異常値は、昆布摂食を止めたあと、7-40日で初期のレベルまで戻りました。 昆布摂食している間、甲状腺機能が抑制されたというこれらの知見から、効果はリバーシブルでしたが、
我々としては、日本人の方々に、過剰な多量の海藻摂食を避けるように提案するものです。

1」本例に認めた血中Tg値の上昇は, 本症の病態が有機化障害であること, 甲状腺腫と関不することなどを示唆し, 長期フォロー時の指標となる. 2」本症発症の要因として, 危険因子が2つ以上存在することが重要であった. HSG2回, 妊娠中に加えて授乳中もイソジンうがい連日, ヨード含有の多い 食品を2品目連日など. また, HSG1回例では, 母体からのヨード排泄遅延 が2つ目の因子と考えられた. 3」母児に対して, ヨード過剰の可能性がある医療行為を完全に禁止するこ とが望ましい. ただ, 実施される場合には, 本症発症リスクに対する十分な 説明と同意がなされ, さらに, 母児に対する甲状腺機能のその後のフォロー が必要である.

健康な大人の場合は、必要以上にヨードを甲状腺の中に取り込まない仕組みが働いて、やがて甲状腺機能低下症が解消しますしかし、お母さんのお腹の中にいる赤ちゃん(胎児)や生まれたばかりの赤ちゃんでは、ヨード過剰による甲状腺機能低下症になりやすく、またそれが長引きやすいことが知られています。
