โรคขอเสื่อมโรคขอเสื่อม เปนการเสื่อมสภาพของ articularcartilage และ matrix มีความไมสมดุลระหวางการสรางและทําลาย articularcartilage มีการหนาตัวของกระดูกใตชั้นกระดูกออนผิวขอ และมีกระดูกงอก ที่ขอที่เกิดการเสื่อม

Cytokines (เชน TNF-α, IL-1),proteolytic enzymes (เชน MMPs,collagenase), nitric oxide, oxygen radical มีสวนรวมในพยาธิกําเนิด

Acetaminophen

• 10-20 mg/kg ทุก 4-6 hr.• เปนยาขนานแรกที่แนะนําใน

ผูปวย OA ในการบรรเทาอาการปวด (ACR, EULAR, สมาคมโรคขอและรูมาติสซั่ม)

NSAIDs

• Analgesic และ anti-inflammatory effectsอาการที่เกดิขึ้นใน OA สวนหนึ่งอาจเกิดจาก mild synovitis, หรือ periarticular inflammation

ระยะสั้น NSAID ลดอาการปวดไดดกีวา placebo ระยะยาว ไดผลดีกวา placebo แตมีปญหาการติดตามการใชยา และไมสามารถใชยาไดติดตอกันนาน

NSAIDs

• ไมมีความแตกตางกันระหวาง NSAID แตละชนิด (รวมทั้ง COX II inhibitors)

• การใชยาขึ้นกับผูปวย การตอบสนอง และความเหมาะสมดานอื่นๆ

NSAIDs ตัวไหน ?Conventional or specific COX II inhibitor

ในผูปวยที่มีปจจัยเสี่ยงตอการเกิดผลขางเคียง ควรเลี่ยงใชยา หรือ ใชรวมกับ misoprostol, H2 blocker หรือ PPI / หรือใชยา COX II inhibitor

Acetaminophen for OATowheed TE at al Cochrane Database Syst Rev. 2005

• In the placebo-controlled RCT, acetaminophen was shown to be clearly superior to placebo with a similar safety profile.

• Acetaminophen was less effective overall than NSAIDs in term of pain reduction and global assessment BUT both drugs had similarefficacy in term of improvement in functional status.

คําแนะนํา ใชเปนทางเลอืกหนึ่งในการรักษา OA knee (ACR, EULAR)ใชในขอที่มี effusion, mild inflammationไมแนะนําใหใชใน chronic stable knee painเจาะขอดูด synovial fluid ออกมาใหมากที่สุดกอน ไมทําบอยกวา 3 เดือนตองแนใจวาไมมีการติดเชื้อ

Therapeutic effect of IA hyaluronic acid

- Beneficial effect: Adams (1995), Altman (1998), Huskisson (1999) Brandt (2003)

- No benefial effect: Dahlberg (1994), Henderson (1995), Lohanmander (1996)

Brandt (2003): RCT hyaluronan vs. saline. IA saline มี significant improvement i.e. 40% in saline group compared with 58% in HA group (improved > 5 units on WOMAC pain score)

Overall, ...... IA HA being modestly effective in treatment for pain associated with moderate knee OA, and the effect lasts for several months, but saline injections also produce large effect sizes over the same period of time.........

Therapeutic effect of IA hyaluronic acid

Recommodation:1. ใชในรายที่ไมตอบสนองตอ non-pharmacologic therapy

และการใชยาแกปวดทัว่ไป2. รายที่มีขอหามในการใช NSAID หรือมีผลขางเคียงตอการใช NSAID

(ควรใชในรายที่ moderately severe OA) ผลที่ไดคงอยูประมาณ 2-6 เดอืน

1. Pain reduction during movement (VAS 100 mm.) - 3.8 mm after 2-6 weeks- 4.3 mm after 10-14 weeks- 7.1 mm after 22-30 weeks

This effect was NOT compatible with a clinically “meaningful difference”. (expected to be about 15 mm on the visual analogue scale)

2. No improvement in knee function was observed at any points.

Content up to July 2003, and Cochrane Central Register of Controlled Trials. Specialised journals and reference lists of RCTs and pertinent review articles up to April 2004 were handsearched.

63 trials were identified. (37 trials: HA vs. placebo, 9 trials: HA vs. IA steroids, and 5 trials: HA vs. NSAIDs)

Viscosupplementation for the treatment of OA of the knee.Bellamy N et al. Cochrane Database Syst Rev 2005 Apr

Main results:1. The pooled analyses of the effects of HA against placebo controls

generally supported the efficacy of this class of intervention.5-13 week post inj. showed improvement from baseline (11-54% for pain and 9-15% for function)

2. Comparable efficacy was noted against NSAIDs and longer-term benefits were noted in comparisons against IA steroids

Overall, the aforementioned analyses support the use of the HA class ofproducts in the treatment of knee OA.

• ใน Europe และ Asia จัดเปนยา แตใน USA และ Canada จัดเปน food supplement

การศึกษาชนิด systematic review และ mata-analysis• มีการศึกษาแสดงใหเห็นวา GS และ CS มีประสิทธิภาพเหนือ placebo และ/

หรือ NSAID ในการลดอาการปวด และชวยใหมีการทํากิจกรรมไดมากขึ้น (Towheed et al 2003, Leeb et al 2000, McAlindon et al 2000)

• มีการศึกษา ที่แสดงใหเห็นวา GS มีประสิทธิภาพในการชะลอการทําลายของกระดูกออนผิวขอ และยาทั้ง 2 ตัว มีประสิทธิภาพในการลดอาการปวดและเพิ่มความสามารถในการทํากิจกรรม (Richy et al 2003)

ขอสรุปจาก Richy et al; Arch Intern Med 2003;163:1514-15221. Oral GS และ CS ประสิทธิภาพในการในการลดอาการปวด ชวยใหการทํากิจกรรมและเคลื่อนไหวขอไดดีขึ้น เมื่อเปรียบเทียบกับ placebo2. GS มีประสิทธิภาพในการชะลอการดําเนินของ OA เมื่อเทียบกบั placebo แตผลมีนอยมาก (0.27 มม. ใน 3 ป) ไมมีขอมูลสวนนี้ที่ชัดเจนใน CS3. ขอมูลประสิทธิภาพของยาทั้งสองเปรียบเทียบกับ placebo ที่มีนัยสําคัญทางคลินิก คือ ประสิทธิภาพในการลดอาการปวดและการชวยการเคลื่อนไหวของขอใหดีขึ้นเทานั้น

8 trials with adequate allocation concealment failed to show benefit of glucosamine for pain and WOMAC function.The 20 analysed RCTs found glucosamine favoured placebo with an improvement in pain and function the Lequesne Index. WOMAC pain, function and stiffness outcome DID not reach statistical significance.The results are not uniformly positive….. reasons …unexplained.Rotta preparation, glucosamine was found to be superior for pain, and function using the Lequesne Index. …… (but not WOMAC)

Authuor’s Conclusions:Update 20 trails with 2750 patients; pooled results from studied using non-Rotta

preparation or adequate allocation concealment failed to show benefit of glucosamine for pain and WOMAC function.

Studies evaluating Rotta preparation show that glucosamine was superior to placebo in the treatment of pain and functional impairment resulting from symptomatic OA. the Lequesne Index. …… (but not WOMAC)

WOMAC outcomes of pain, stiffness, and function DID NOT show a superiority of glucosamine over placebo for both Rotta and non-Rotta preparations.

Diacerein• ลดการสรางหรือหลั่ง IL-1 จากเยื่อบุขอ ลดการทํางานของ IL-1 receptor ที่มี

ตอchondrocyte• Diacerein vs. tenoxicam in OA hip 288 ราย: ประสิทธิภาพของขอพอๆ กัน

เมื่อประเมินที่เดือนที่ 2 แตกลุมที่ได diacerein มีความตองการยาแกปวดมากกวากลุมที่ได tenoxicam

• การใชยาระยะปานกลาง diacerein vs. placebo: diacerein ไดผลดีกวา placebo ในการลดอาการปวด

• การใชยาในระยะยาว ใน hip OA เทียบกับ placebo พบวายาสามารถชะลอการสูญเสียของความกวางของชองขออยางมีนัยสําคัญทางสถิติ (50% vs. placebo 60%; p = 0.036) โดยการวิเคราะหขอมลูพบวายาสามารถชะลอการสูญเสียความกวางของชองขอได 0.15 มม. ตอป แตอยางไรก็ตามการศึกษานี้ไมพบความแตกตางของอาการที่ดีขึ้นเมื่อเทียบกับ placebo
