• 6. Etiology/manifestation convention (“code first”, “use
additional code” and “in diseases classified elsewhere” notes)
• Certain conditions have both an underlying etiology and multiple body system manifestations due to the underlying etiology. For such conditions, the ICD-9-CM has a coding convention that requires the underlying condition be sequenced first followed by the manifestation. Wherever such a combination exists, there is a “use additional code” note at the etiology code, and a “code first” note at the manifestation code. These instructional notes indicate the proper sequencing order of the codes, etiology followed by manifestation.

• Deve ter-se em atenção as notas de inclusão nas seguintes 3 categorias:
• 652 - Malposition and mal presentation of fetus
• 653 - Disproportion • 654 - Abnormality of organs and soft tissues
of pelvis • Estas categorias referem que deve ser
codificado em primeiro lugar a obstrução.

• Na categoria 652 deve associar-se o código 660.0 "Obstruction caused by mal position of fetus at onset of labor",
• na 653, associar-se-á o código 660.1 “Obstrution by bony pelvis”,
• e na 654 o código 660.2 “Obstrution by abnormal pelvic soft tissues”.

• 4) When a delivery occurs • When a delivery occurs, the principal diagnosis should correspond
to the main circumstances or complication of the delivery. In cases of cesarean delivery, the selection of the principal diagnosis should be the condition established after study that was responsible for the patient’s admission. If the patient was admitted with a condition that resulted in the performance of a cesarean procedure, that condition should be selected as the principal diagnosis. If the reason for the admission/encounter was unrelated to the condition resulting in the cesarean delivery, the condition related to the reason for the admission/encounter should be selected as the principal diagnosis, even if a cesarean was performed.

Conforme os serviços e o codificador podem existir diferenças na codificação dos episódios cujo procedimento principal foi um parto distócico.
Para condições semelhantes pode resultar uma codificação diferente consoante os termos utilizados pelo obstetra.

Proposta:
• Uniformizar os partogramas e a nomenclatura utilizada, com exigência de especificação do motivo do parto distócico.
• Sensibilizar médicos para importância dos registos clínicos
• Indicar se houve parto obstruído
