IntroductionDementia is not caused by aging Most researchers regard it as

resulting from injuries, infections, brain diseases, tumors, or other disorders

Introduction Dementia has been consistently

shown to be associated with an increased risk of falls, with rates double that of cognitively intact older people

A fall related event with or without fracture is the most common reason for hospitalisation in people with dementia, accounting for approximately 26% of all admissions

No evidence to guide fall prevention

ResultsAdherence to recommendations for home hazard reduction

simple changes:◦changing footwear◦rearranging small items of furniture, and recommendations

◦Double sided tape securing mats◦fluorescent tape to highlight step◦Edges and using sensor lights. ◦Not leaving bags on floors to trip over straps.

ResultsAdherence to exercise intervention

Intervention group (n=11) were prescribed between 2 and 6 exercises (mean 4.3) at the start of the program, and these were to be completed three times per week.

Page 26: A feasibility study and pilot randomised trial of a tailored prevention program to reduce falls in older people with mild dementia

ResultsFallsThere were fewer falls (n=5) in the intervention than the control group (n=11), (IRR= 0.34 (95% CI 0.06-1.91)).Most people in the control group who fell did so in their own home environment with only one fall recorded outdoors in a park.

ResultsSafety and adverse events

Intervention group - No serious adverse events related ◦Minor complaints-Stiffness◦dizziness ◦mild joint pain

Discussion

Page 31: A feasibility study and pilot randomised trial of a tailored prevention program to reduce falls in older people with mild dementia

DiscussionThe primary aim of the study was to

develop and pilot the feasibility of a novel approach to fall prevention for people with dementia.

Page 32: A feasibility study and pilot randomised trial of a tailored prevention program to reduce falls in older people with mild dementia

DiscussionMild dementia but was still a

relatively heterogeneous group in terms of both physical and cognitive abilities. -Acceptable adherence rates

Reduction in the rate of falls and increased activity in the intervention group the intervention shows potential benefits.

Page 33: A feasibility study and pilot randomised trial of a tailored prevention program to reduce falls in older people with mild dementia

Discussion

Adherence to home safety recommendations was goodParticipants simple recommendations: securing matshighlighting step edgeschanging footwearre-arranging furniture to improve clear

access

Page 34: A feasibility study and pilot randomised trial of a tailored prevention program to reduce falls in older people with mild dementia

DiscussionAdherence to exercise was

also very good ─ Simple ankle cuff weights have

been part of the current successful home based programs with cognitively intact people

Page 35: A feasibility study and pilot randomised trial of a tailored prevention program to reduce falls in older people with mild dementia

DiscussionOccupational therapist and

physiotherapist are trained to implement both components.

Allen’s model was useful in guiding the modification of cognitive demands of the program.

Page 36: A feasibility study and pilot randomised trial of a tailored prevention program to reduce falls in older people with mild dementia

DiscussionPersonal goals achieved were not

always directly related to falls prevention.

For example, one participant started walking to the bus on her own as she felt more confident.

Page 37: A feasibility study and pilot randomised trial of a tailored prevention program to reduce falls in older people with mild dementia

DiscussionThis feasibility study is the first

falls study to include people with dementia in a home based intervention.

Page 38: A feasibility study and pilot randomised trial of a tailored prevention program to reduce falls in older people with mild dementia

DiscussionCarer stressors

Related to other life roles for example, family and work issues.

Home safety recommendationsTraining of the carers in

management and coping skills

Page 39: A feasibility study and pilot randomised trial of a tailored prevention program to reduce falls in older people with mild dementia

DiscussionLimitations

Not been tested in people with a moderate-severe dementia.

Same for control and intervention, it may be underestimated if the person with dementia does not remember to report all falls.

-Lived alone.-Missing data

Page 40: A feasibility study and pilot randomised trial of a tailored prevention program to reduce falls in older people with mild dementia

Conclusions

Page 41: A feasibility study and pilot randomised trial of a tailored prevention program to reduce falls in older people with mild dementia

ConclusionsThe pilot demonstrated that a

home-based tailored intervention was feasible and acceptable to people with mild dementia and their carers.

Page 42: A feasibility study and pilot randomised trial of a tailored prevention program to reduce falls in older people with mild dementia

Conclusions Future research on fall prevention

for people with dementia should consider the following:

Recruitment for a larger trial Require multiple approachesAssessors should have dementia-

specific experienceCompleteness of data collectionIntegrating therapy approachesProvide specific carer training session
