Bismillahirahmanirrohiiim. Puji dan syukur kami panjatkan kehadirat
Allah SWT, berkat rahmat dan karuniaNya, maka buku Referensi ini
dapat diselesaikan.
Ini adalah buku referensi yang berisi tentang manfaat aktivitas
antioksidan biji pinang terhadap sel kanker mulut, yakni karsinoma sel
skuamosa mulut. Buku ini disusun untuk memenuhi kebutuhan
pembelajaran dan referensi lanjutan dalam penelitian tentang terapi
adjuvan karsinoma sel skuamosa mulut yang berasal dari bahan herbal
alami, yakni biji pinang Aceh. Buku ini menjelaskan tentang aktivitas
antioksidan biji pinang yang selama ini kurang banyak diketahui oleh
masyarakat. Kemampuan aktivitas antioksidan biji pinang terhadap
sel-sel kanker mulut juga akan dianalisis melalui beberapa uji dengan
hasil yang signifikan.
Buku ini disusun secara ringkas dan sistematis agar mudah
dipelajari dan dipahami. Penulis menyadari ada beberapa masalah
yang belum dibahas dalam penulisan buku ini, namun diharapkan
buku ini sebagai langkah awal untuk menguak tabir ilmu pengetahuan
herbal alami di bidang Ilmu Kedokteran gigi khususnya Ilmu
Penyakit Mulut. Penulis mengucapkan banyak terima kasih kepada
para guru dan pembimbing. Semoga buku ini bermanfaat bagi dunia
akademik dan menjadi ladang pahala bagi penulis

KATA SAMBUTAN
Puji dan syukur kita panjatkan kehadirat Allah SWT, berkat rahmat
dan karunia Nya, buku referensi “Aktivitas Antioksidan dan
Sitotoksisitas Biji Pinang pada Karsinoma Sel Skuamosa Mulut” dapat
diterbitkan. Buku ini diharapkan dapat memberikan pemahaman baru
tentang Ilmu Kedokteran Gigi khususnya Ilmu Penyakit Mulut.
Karsinoma sel skuamosa mulut (KSSM) merupakan keganasan
pada sel-sel keratinosit epitel rongga mulut yang berpotensi untuk
menyebar melalui jaringan limfatik atau pembuluh darah dan
merupakan kasus keganasan yang paling sering terjadi yakni mencapai
94% dari seluruh kasus dalam rongga mulut. Terapi pilihan untuk
KSSM adalah pembedahan disertai kemo/radioterapi, tetapi teknik ini
banyak menimbulkan efek samping yang cukup parah. Buku ini
bertujuan untuk memaparkan kemampuan biji pinang yang memiliki
aktivitas antioksidan dan sitotoksik terhadap sel kanker namun
bersifat aman bagi sel normal.
Saya berharap buku referensi ini dapat menjadi pedoman
pembelajaran untuk pemahaman terhadap peran antioksidan biji
pinang terhadap sel kanker mulut. Saya mengucapkan selamat kepada
penulis yang telah menyelesaikan tulisannya dan mengharapkan dapat
ditingkatkannya produktivitas penelitian dan buku referensi oleh staf
pengajar Fakultas Kedokteran Gigi Universitas Syiah Kuala.

PENDAHULUAN
Karsinoma Sel Skuamosa Mulut (KSSM) merupakan kasus keganasan
yang paling sering terjadi yakni mencapai 94% dari seluruh kasus
dalam rongga mulut serta menempati urutan kelima dari kejadian
kanker diseluruh dunia.1,2
Insidensi geografis menunjukkan 3%-6%
kasus KSSM terjadi di negara-negara barat dan 30% di negara bagian
timur, termasuk di Indonesia.3 Amerika Serikat merupakan salah satu
negara yang memiliki 35.000 kasus baru KSSM sepanjang tahun
2008, dan menimbulkan lebih dari 7500 kematian setiap tahun.4
Rerata angka keselamatan selama lima tahun untuk kanker tipe ini
menunjukkan angka yang paling rendah yakni kurang dari 50% jika
dibandingkan dengan jenis kanker yang lain dan kondisi ini tidak
mengalami perubahan selama empat puluh tahun terakhir terakhir.4, 5
Penelitian yang dilakukan oleh Ascani dkk. menunjukkan bahwa
lokasi yang paling sering terkena kanker rongga mulut adalah lidah
(lateral posterior dan ventral) diikuti dengan dasar mulut, gusi,
mukosa bukal, retromolar pad, dan yang terakhir adalah palatum
durum.3
Karsinoma sel skuamosa mulut merupakan keganasan pada
sel-sel keratinosit epitel rongga mulut yang berpotensi untuk
menyebar melalui jaringan limfatik atau pembuluh darah.6, 7
Faktor
risiko yang paling berpengaruh diantaranya adalah sirih, tembakau,
dan alkohol. Infeksi Human Papilloma Virus (HPV) dan faktor
genetik juga berperan dalam etiopatogenesis KSSM.8 Walaupun

2.1 Senyawa Antioksidan Alami Dalam Biji Pinang
Menurut salah satu penelitian, 90%-95% kanker disebabkan oleh
faktor gaya hidup, dan hanya 5%-10% disebabkan oleh kerusakan
genetik.56
Hal ini menunjukkan bahwa kanker adalah penyakit yang
dapat dicegah oleh perubahan gaya hidup. Kanker mulut terjadi
melalui proses multifaktorial yang sangat kompleks. Gaya hidup yang
paling banyak memberikan kontribusi penyebab kanker mulut di
negara barat adalah tembakau dan alkohol.63
Perbaikan pola makan
dan gaya hidup sehat sangat dibutuhkan untuk mengurangi risiko
timbulnya kanker.56
Penemuan yang menunjukkan bahwa spesies oksigen reaktif
terlibat dalam perubahan sel prekanker menjadi kanker, menyebabkan
adanya usaha untuk mengontrol produksi spesies oksigen reaktif.44
Perhatian peneliti terhadap antioksidan tanaman semakin berkembang
pesat selama dua dekade terakhir karena penggunaannya yang aman,
availabilitas terjamin, harga terjangkau, serta khasiatnya yang cukup
efektif.44
Antioksidan terbukti dapat berperan sebagai kemoprevensi
dan kemoterapi tergantung dari tipe kanker dan konsentrasi yang
digunakan. Beberapa antioksidan alami tanaman yang terkait dalam
penelitian ini adalah polifenol, flavonoid, dan katekin.

3.1 Karakteristik Sel Kanker
Hanahan dan Weinberg menggambarkan enam karakteristik sel kanker
yang membedakannya dengan sel normal. Karakteristik sel tersebut
berupa :
1. Sel Kanker Mampu Mencukupi Sinyal Pertumbuhan
Sel normal membutuhkan sinyal pertumbuhan eksogen untuk
menstimulasi proliferasi. Sinyal pertumbuhan ini dihantarkan dari
reseptor permukaan sel yang kemudaian secara bertahap mengaktivasi
jalur sinyal intraseluler yang menghasilkan proliferasi. Selama terjadi
karsinogenesis, terjadi peningkatan ikatan reseptor faktor
pertumbuhan dengan ligannya yang mengakibatkan stimulasi autokrin
tanpa faktor eksogen. Peningkatan ekspresi reseptor Epidermal
Growth Factor (EGFR), dan Transforming Growth Factor (TGF), dan
beberapa sinyal protein intraseluler memegang peran penting dalam
perkembangan tumor.
Molekul EGFR akan mengaktivasi jalur MAPK,
fosfatidillinositol-3-kinase (PI3K), AKT, mammalian Target of
Rapamycin (mTOR), Janus Kinase (Jak), Signal Transducer and
Activator of Transcription (STAT), dan Protein Kinase C (PKC) yang
menginduksi proliferasi dan keselamatan sel kanker, invasi,
metastasis, dan angiogenesis. Hepatocyte Growth Factor/c-Met

Ekstrak Biji Pinang Positif terhadap Tanin, Flavonoid, dan
Alkaloid
Skrining fitokimia ekstrak biji pinang menunjukkan bahwa
kandungan ekstrak positif terhadap tanin, flavonoid, dan alkaloid.
Senyawa tanin positif menghasilkan filtrat hijau kehitaman, yakni
senyawa tannin jenis katekol. Senyawa flavonoid positif dengan
adanya warna merah kecoklatan. Senyawa alkaloid positif dengan
adanya endapan putih kekuningan dengan pereaksi Mayer dan
endapan jingga dengan Dragendorff yang menunjukkan terbentuknya
garam alkaloid. Endapan ini timbul akibat alkaloid yang merupakan
senyawa golongan basa nitrogen bereaksi dengan asam klorida
membentuk garam yang tidak larut sehingga timbul endapan.
Total Fenolik dan Flavonoid dalam Ekstrak Biji Pinang
Kandungan total fenolik yang terdapat dalam ekstrak biji
pinang menggunakan tanin sebagai standar. Data absorbansi
spektrofotometri menunjukkan persamaan garis kurva standar fenolik
y = 4.807x - 0.0038, dengan nilai R² 99,99%. Nilai konsentrasi total
fenolik di dalam 1 gram ekstrak adalah 80,3 mg TAE/gr ekstrak.
Konsentrasi fenolik paling tinggi umumnya terdapat dalam pelarut
metanol, aseton, dan air. Konsentrasi ini tergantung dari besarnya
polaritas pelarut yang digunakan saat ekstraksi. Tingginya kelarutan

Efek Sitotoksisitas Ekstrak Biji Pinang Lebih Kuat pada
Sel HSC-3 daripada sel HSC-2
Uji MTS dilakukan untuk melihat efek sitotoksisitas ekstrak biji
pinang pada lima dosis yang lebih besar, yakni 160, 320, 640, 1280,
dan 2560 μg/ml pada sel HSC-2, HSC-3 dan HaCat. Sitotoksisitas
ekstrak dinyatakan dengan persentase viabilitas. Semakin tinggi
konsentrasi ekstrak maka semakin rendah persentase viabilitas yang
terjadi. Hasil penelitian menunjukkan bahwa nilai IC50 ekstrak biji
pinang pada sel HSC-2 adalah 629,50 μg/ml, efek sitotoksik dimulai
pada konsentrasi 320 μg/ml. Nilai IC50 HSC-3 terjadi pada konsentrasi
yang rendah yakni 164,06 μg/ml. Efek sitotoksik dimulai dari
konsentrasi yang terkecil yakni antara 160-640 μg/ml, namun pada
konsentrasi diatas 1280 μg/ml, ekstrak mulai menimbulkan aktifitas
proliferasi. Ekstrak biji pinang memberikan efek sitotoksisitas yang
lebih kuat pada sel HSC-3 dibandingkan sel HSC-2. Kurva grafik
sitotoksisitas kedua galur tampak pada Gambar 5.1. Ekstrak biji
pinang tidak menimbulkan sitotoksisitas pada sel HaCat. Aktifitas
proliferasi terjadi diatas 100% viabilitas sel, dimulai dari konsentrasi
terkecil (160 μg/ml) dan meningkat sesuai dengan bertambahnya
konsentrasi ekstrak.

PEMBAHASAN
Mayoritas masyarakat saat ini lebih memilih pengobatan
herbal karena bahan alami dianggap bersifat lebih aman dan murah
jika dibandingkan dengan obat kimia. Faktor pendorong
meningkatnya penggunaan obat herbal di negara maju adalah akibat
besarnya efek samping penggunaan obat kimia, tingginya biaya
pengobatan modern, serta meningkatnya usia harapan hidup pada saat
prevalensi penyakit kronik meningkat sehingga obat herbal menjadi
pilihan alternatif yang diyakini dapat mencakup berbagai lapisan
masyarakat.152
Perkembangan penelitian obat herbal di Indonesia
dapat menciptakan peluang bagi Indonesia untuk menjadi pusat obat
herbal dunia karena Indonesia dikenal secara luas sebagai pusat
keanekaragaman hayati terbesar kedua setelah Brazil yang terdiri dari
tumbuhan tropis dan biota laut. Indonesia memiliki 30.000 jenis
spesies tumbuhan, dimana 960 spesies diantaranya telah tercatat
sebagai tumbuhan berkhasiat dan 283 jenis diantaranya merupakan
tumbuhan penting bagi industri obat tradisional.152
Biji pinang adalah salah satu biji tanaman yang sudah banyak
digunakan secara empiris dalam masyarakat, namun potensi
antioksidannya terhadap pengobatan kanker terutama KSSM belum
ditemukan dalam literatur. Biji ini juga merupakan salah satu etiologi
timbulnya KSSM. Kanker ini dapat timbul akibat mengunyah biji
pinang dalam waktu yang panjang dan tidak terkontrol. Namun,
