ANTICOAGULACIÓN EN PACIENTE ANCIANO CON FIBRILACIÓN AURICULAR

•Arritmia más frecuente en nuestro medio (4.5 millones de personas en Europa).
• Prevalencia e Incidencia aumentan con la edad. • Incidencia (4.1/1000 en < 75 años, 26.3/1000 en > 75 años)• Prevalencia (0.1% < 55 años, 9% > 80 años)
• Importancia: morbimortalidad conlleva riesgo tromboembólico.• Responsable del 50% de los ictus de origen cardioembólico y del 20% de todos los ictus.
• Incremento esperanza de vida el % de pacientes con FA no valvular con 80 años se cuadriplicará en 2050, llegando a ser > 50% de los pacientes con FA.
• Incremento riesgo Ictus entre los 80-89 años en un 25-36%.

PACIENTE ANCIANOAproximadamente el 35% de los octogenarios con FA no reciben terapia anticoagulante oral y cuando lo hacen es en dosis reducidas.

Datos en contra:• Beneficio clínico neto positivo de la anticoagulación en pacientes 90 años sobre antiagregación o no tratar. • La prevalencia de FA aumenta notablemente con la edad por lo tanto el % total de Ictus evitados con el tratamiento anticoagulante es mucho mayor.• La evidencia disponible sugiere que la edad incrementa el riesgo de Ictus más que el de
sangrado.

SITUACIONES ESPECIALES
• Desnutrición (< 60kg):
• Disminuir Edoxaban
• 1 de los criterios disminución Apixaban
• Hipoproteinemia: no recomendaciones específicas
• Disfagia: Rivaroxaban, Apixaban.
Desnutrición-Disfagia
• Individualizar (edad, PA, origen, dosis, antiagregantes, riesgo ictus, localización, gravedad)
• Uso ACO bajo riesgo
• HIC esperar 4-8 semanas

SITUACIONES ESPECIALES
• Inicio la anticoagulación: razones de la no adherencia y estrategias cumplimiento.
• Razones falta adherencia: patología neuropsiquiátrica, situación social, falta de entendimiento enfermedad o complejidad del tratamiento.
• Ventajas ACOD : dosificación fija y ausencia de monitorización.
• Regímenes dosificación cada 24h mejor adherencia > aumento complicaciones.
Mal cumplimiento-Aislamiento social
LIMITACIONES
- Heterogeneidad de los estudios pivotales debido a las distintas propiedades farmacológicas de los ACOD, las diferentes estrategias de reducción de dosis y la diversidad de las características de los diferentes
estudios .
- Ausencia de estudios específicos diseñados en población anciana.
- Resultados no extrapolables en > 85 años, escasa representación en los estudios .
CONCLUSIONES• Primera elección:
• Apixaban 5/2.5 mg según la SEGG: clasificación FORTA solo Apixaban entra en la categoría A (muy beneficioso). Concordamos ya que es superior evitar Ictus/ES que warfarina y similar a Dabigatran 150 mg, conllevando una reducción de las hemorragias graves HIC y extracraneales. Único ACOD comparación con AAS, demostró 6 veces menos riesgo de ictus y mismo riesgo de sangrado.
• Consideramos Dabigatran 150mg es de primera elección en < 72 años (ÚNICO que reduce Ictus isquémicos y reduce hemorragias graves respecto a Warfarina)
• Segunda elección: Edoxaban 60/30 mg: no superior a Warfarina y aumenta hemorragias digestivas. Reduce hemorragias graves (restringido a pacientes con CRD, es decir reciben dosis de 30 mg).
• Tercera elección: Dabigatran 110 mg, no superior a Warfarina y no reduce hemorragias graves.
• Rivaroxaban: no superior a Warfarina en prevención de Ictus, no reduce hemorragias mayores ni las HIC es > 75 años y aumenta un 44% las hemorragias mayores en > 80 años.
• Casos seleccionados podría plantearse como alternativa Dabigatran 150 mg: pacientes entre 75-80 años con buena función renal, HAS-BLEED 1-2, sin riesgo de sangrado GI, preferentemente varón y sin bajo peso.
