OBJECTIF :DÉTERMINER SI LA PERFUSION DE SOLUTÉS RICHES EN CHLORE EST NOCIF POUR LES REINS DES PATIENTS DE RÉANIMATION

PATIENTS ET MÉTHODES

� Étude pilote prospective ouverte 2008-2009� Service de réanimation polyvalente de 22 lits, Melbourne, Australie� Protocole appliqué à l’ensemble des patients entrant dans le service sauf cas

particuliers (œdème cérébral, hyponatrémie)� Validé par un comité d’éthique

� Période contrôle de 6 mois (février à aout 2008) : protocole habituel de remplissage, riche en chlore (sérum physiologique, gélofusine, et albumine à4%).

� 6mois (aout 2008-février 2009), information délivrée aux participants : utiliser des solutés avec moins de chlore : du Ringer lactate, du plasmalyte-148 et de l’albumine à 20%,

� puis pendant 6 mois mise en pratique de cette approche (février à aout 2009).

PATIENTS ET MÉTHODES

� Patients inclus: � tous les patients admis en réanimation de février àaout 2008 puis de

février à aout 2009

� Exclus:� déjà dialysés, � patients épurés pour toxique dialysable.

� Recueil :� Age, sexe, score APACHE, SAPSII, caractéristiques cliniques à

l’admission,� créatininémie de base M-1 admission (classification RIFLE), sinon

PHYSOPATHOLOGIE

� L’augmentation du chlore (et/ou l’acidose) associée àla perfusion de sérum physiologique est responsable d’une diminution de la perfusion rénale.

� Si sur une courte période ceci n’a pas d’impact, chez des patients avec déjàun parenchyme rénal mal perfusé (hypotension) ajouter en plus de l’hypoperfusion peut précipiter la situation vers la nécrose tubulaire aigue.

� Selon le concept de Stewart, les troubles acidobasiques résultent des modifications d’une des 3 variables indépendantes que sont : le strong ion difference (SID), la masse totale des acides faibles (albuminate + phosphate = Atot) pour les troubles métaboliques et la PaCO2 pour les troubles respiratoires. SIDa (meq·l–1) = [Na+ + K+ + Ca++ + Mg++] – [Cl– + lactate–] = 40 ± 2 meq·l–1

� Les acidoses métaboliques se traduisent par une baisse du SID, en rapport le plus souvent avec une augmentation d’anions forts organiques (classiques acidoses métaboliques à trou anionique élevé) ou de l’anion fort chlore (classique acidose hyperchlorémique à trou anionique normal).

� Abstract

� OBJECTIVE:

� We compared the effects of intravenous infusions of 0.9% saline ([Cl] 154 mmol/L) and Plasma-Lyte 148 ([Cl] 98 mmol/L, Baxter Healthcare) on renal blood flow velocity and perfusion in humans using magnetic resonance imaging (MRI).

� BACKGROUND:

� Animal experiments suggest that hyperchloremia resulting from 0.9% saline infusion may affect renal hemodynamics adversely, a phenomenon not studied in humans.

� METHODS:

� Twelve healthy adult male subjects received 2-L intravenous infusions over 1 hour of 0.9% saline or Plasma-Lyte 148 in a randomized, double-blind manner. Crossover studies were performed 7 to 10 days apart. MRI scanning proceeded for 90 minutes after commencement of infusion to measure renal artery blood flow velocity and renal cortical perfusion. Blood was sampled and weight recorded hourly for 4 hours.

� RESULTS:

� Sustained hyperchloremia was seen with saline but not with Plasma-Lyte 148 (P < 0.0001), and fall in strong ion difference was greater with the former (P = 0.025). Blood volume changes were identical (P = 0.867), but there was greater expansion of the extravascular fluid volume after saline (P = 0.029). There was a significant reduction in mean renal artery flow velocity (P = 0.045) and renal cortical tissue perfusion (P = 0.008) from baseline after saline, but not after Plasma-Lyte 148. There was no difference in concentrations of urinary neutrophil gelatinase-associated lipocalin after the 2 infusions (P = 0.917).

� CONCLUSIONS:

� This is the first human study to demonstrate that intravenous infusion of 0.9% saline results in reductions in renal blood flow velocity and renal cortical tissue perfusion. This has implications for intravenous fluid therapy in perioperative and critically ill patients. NCT01087853.

� Abstract

� Animal studies have shown that large volumes of IV lactated Ringer's solution (LR) decrease serum osmolality, thereby increasingcerebral water. These studies have led to recommendations to limit LR to avoid cerebral edema in neurosurgical patients. Eighteen healthy human volunteers aged 20-48 yr received 50 mL/kg LR over 1 h on one occasion and 0.9% sodium chloride (NS) on another. Venous samples were taken at baseline (T1), at infusion end (T2), and 1 h after T2 (T3). Time until first urination was noted. With LR, serum osmolality decreased by 4+/-3 mOsm/kg from T1 to T2 and increased insignificantly with NS. At T3, osmolality returned almost to baseline in the LR group. Blood pH increased from T1 to T2 with LR by 0.04+/-0.04 and decreased with NS by 0.04+/-0.04. These pH changes persisted at T3. Subjective mental changes occurred only with NS. Abdominal discomfort was more common with NS. Time until first urination was longer with NS (106+/-11 min) than with LR (75+/-10 min) (P < 0.001). In healthy humans, an infusion of large volumes of LR, but not NS, transiently decreased serum osmolality, whereas acidosis associated with NS persisted and urinary output was slower with NS.

� IMPLICATIONS:

� Large volumes of lactated Ringer's solution administered to healthy humans produced small transient changes in serum osmolality. Large volumes of sodium chloride did not change osmolality but resulted in lower pH.

� Abstract

� OBJECTIVE:

� To assess the association of 0.9% saline use versus a calcium-free physiologically balanced crystalloid solution with major morbidity and clinical resource use after abdominal surgery.

� BACKGROUND:

� 0.9% saline, which results in a hyperchloremic acidosis after infusion, is frequently used to replace volume losses after major surgery.

� METHODS:

� An observational study using the Premier Perspective Comparative Database was performed to evaluate adult patients undergoing major open abdominal surgery who received either 0.9% saline (30,994 patients) or a balanced crystalloid solution (926 patients) on the day of surgery. The primary outcome was major morbidity and secondary outcomes included minor complications and acidosis-related interventions. Outcomes were evaluated using multivariable logistic regression and propensity scoring models.

� RESULTS:

� For the entire cohort, the in-hospital mortality was 5.6% in the saline group and 2.9% in the balanced group (P < 0.001). One or more major complications occurred in 33.7% of the saline group and 23% of the balanced group (P < 0.001). In the 3:1 propensity-matched sample, treatment with balanced fluid was associated with fewer complications (odds ratio 0.79; 95% confidence interval 0.66-0.97). Postoperative infection (P = 0.006), renal failure requiring dialysis (P < 0.001), blood transfusion (P < 0.001), electrolyte disturbance (P = 0.046), acidosis investigation (P < 0.001), and intervention (P = 0.02) were all more frequent in patients receiving 0.9% saline.

� CONCLUSIONS:

� Among hospitals in the Premier Perspective Database, the use of a calcium-free balanced crystalloid for replacement of fluid losses on the day of major surgery was associated with less postoperative morbidity than 0.9% saline.
