2.1 Batu Saluran Kemih
2.1.1 Proses Pembentukan Batu Saluran Kemih
Batu saluran kemih merupakan agregat polycrystalline yang
terbentuk dari berbagai macam kristaloid dan matriks organik.
Terbentuknya batu dipengaruhi oleh saturasi urin. Saturasi urin bergantung
pada pH urin, ion-ion, konsentrasi zat terlarut, dan lain lain.
Hubungan antara konsentrasi zat terlarut dengan terbentuknya batu
sangat jelas. semakin besar konsentrasi ion, maka kemungkinan ion akan
mengendap akan semakin tinggi. Apabila konsentrasi ion meningkat, ion
akan mencapai suatu titik yang disebut solubility product (Ksp). Bila
konsentrasi ion meningkat diatas titik ini, maka akan dimulai proses
perkembangan kristal dan nukleasi.

saturation is called the thermodynamic solubility product, Ksp, which is the point at which the dissolved and crystalline components are in equilibrium for a specific set of conditions. At this point, addition of further crystals to the saturated solution will cause the crystals to precipitate unless the conditions of the solution, such as pH or temperature, are changed.
In urine, despite concentration products of stone-forming salt components such as calcium oxalate that exceed the solubility product, crystallization does not necessarily occur because of the presence of inhibitors and other molecules that allow higher concentrations of calcium oxalate to be held in solution before precipitation or crystallization occurs. In this state of saturation, urine is consid-ered to be metastable with respect to the salt. As concentrations of the salt increase further, the point at which it can no longer be held in solution is reached and crystals form. The concentration product at this point is called the formation product, Kf.
The solubility product and the formation product differentiate the three major states of saturation in urine: undersaturated, meta-stable, and unstable (Fig. 452). Below the solubility product, crystals will not form under any circumstances and dissolution of crystals is theoretically possible. At concentrations above the for-mation product, the solution is unstable and crystals will form. In the metastable range, between the solubility product and the formation product and in which the concentration products of most common stone components reside, spontaneous nucleation
WaterThe beneficial effect of a high fluid intake on stone prevention has long been recognized. In two large observational studies, fluid intake was found to be inversely related to the risk of incident kidney stone formation (Curhan et al, 1993, 1997). Furthermore, in a prospective, randomized trial assessing the effect of fluid intake on stone recurrence among first-time idiopathic calcium stone formers, urine volume was significantly higher in the group assigned to a high fluid intake compared with the control group receiving no recommendations, and, accordingly, stone recurrence rates were significantly lower (12% vs. 27%, respectively) (Borghi et al, 1996).
Geographic differences in the incidence of stone disease have been ascribed in some cases to differences in the mineral and electrolyte content of water in different areas. Although several investigators reported a lower incidence of stone disease in geo-graphic regions with a hard water supply compared with a soft water supply, where water hardness is determined by content of calcium carbonate (Churchill et al, 1978; Sierakowski et al, 1979), others found no difference. Schwartz and colleagues (2002) found no association between water hardness and incidence of stone episodes, although they did observe a correlation between water hardness and urinary magnesium, calcium, and citrate levels.

PHYSICOCHEMISTRYThe physical process of stone formation is a complex cascade of events that occurs as the glomerular filtrate traverses the nephron. It begins with urine that becomes supersaturated with respect to stone-forming salts, such that dissolved ions or molecules precipi-tate out of solution and form crystals or nuclei. Once formed, crystals may flow out with the urine or become retained in the kidney at anchoring sites that promote growth and aggregation, ultimately leading to stone formation. The discussion that follows describes the process of stone formation from a physicochemical standpoint.
State of SaturationA solution containing ions or molecules of a sparingly soluble salt is described by the concentration product, which is a mathematic expression of the product of the concentrations of the pure chemi-cal components (ions or molecules) of the salt. For example, the concentration product (CP) expression for sodium chloride is CP = [Na+][Cl]. A pure aqueous solution of a salt is considered satu-rated when it reaches the point at which no further added salt crystals will dissolve. The concentration product at the point of

Teori nukleasi menegaskan bahwa batu saluran kemih terbentuk dari
kristal-kristal atau benda asing dari urin yang kadarnya jenuh. Akan tetapi,
batu tidak selalu terbentuk dari pasien yang tinggi tingkat eksresinya atau
beresiko dehidrasi. Teori inhibitor kristal merupakan teori lain pada
pembentukan batu. Menurut teori ini, batu terbentuk karena rendahnya
konsentrasi ion-ion yang menjadi inhibitor alami dari batu tersebut seperti
magnesium, sitrat dan pirofosfat. Akan tetapi, validitas teori ini masih
dipertanyakan, akibat banyak orang yang mengalami defisiensi ion-ion
tersebut tidak mengalami gangguan batu saluran kemih (Stoller, 2008)
Bahan utama pembentuk batu adalah komponen kristalin. Terdapat
beberapa tahap dalam pembentukan kristal yaitu nukleasi, growth, dan
agregasi. Nukleasi merupakan awal dari proses pembentukan batu dan
dipengaruhi oleh berbagai substansi seperti matriks proteinaceous, benda
asing, dan partikel lain. Nukleasi heterogen (epitaxy) merupakan jenis
nukleasi yang umum terjadi pada pembentukan batu. Hal ini disebabkan
nukleasi heterogen membutuhkan energi yang lebih sedikit daripada
nukleasi homogen. Sebuah tipe kristal akan menjadi nidus untuk nukleasi
tipe kristal lain, contohnya kristal asam urat akan menjadi nidus untuk
nukleasi kalsium oksalat (Stoller, 2008)
Komponen matriks pada batu bervariasi tergantung jenis batu.
Komponen matriks biasanya hanya 2-10% dari berat batu tersebut.
Komposisi matriks yang dominan adalah protein dengan sedikit hexose atau
hexosamine. Peran matriks pada inisiasi pembentukan batu masih belum
diketahui secara sempurna. Matriks dapat berperan sebagai nidus untuk
agregasi kristal atau sebagai perekat komponen-komponen kristal kecil
(Stoller, 2008).
Urin normal mengandung chelating agent seperti sitrat, yang
menghambat proses nukleasi, pertumbuhan dan agrefasi kristal-kristal yang
mengandung ion kalsium. Inhibitor lainnya adalah calgranulin, Tamm-
Horsfall protein, glycosaminoglycans, uropontin, nephrocalcin, dan lain
lain. Mekanisme biokimia mengenai hubungan antara substansi tersebut
dengan pembentukan batu masih belum dipahami seluruhnya, akan tetapi

bila pada pemeriksaan substansi tersebut kadarnya dibawah normal, maka
akan terjadi agregasi kristal yang akan membentuk batu (Coe et al, 2005).
Nephrocalcin ,glikoprotein yang bersifat asam dan disekresikan oleh ginjal,
dapat menghambat nukleasi, pertumbuhan dan agregasi dari kalsium oksalat
(Pearle et al, 2012)
Batu saluran kemih biasanya terbentuk dari kombinasi berbagai
faktor, dan jarang terbentuk dari kristal yang tunggal. Batu lebih sering
terbentuk pada pasien dengan konsumsi protein hewani yang tinggi atau
konsumsi cairan yang kurang. Batu juga dapat terbentuk dari kondisi-
kondisi metabolic seperti distal renal tubular acidosis, Dents disease,
hyperparathyroidism, dan hyperoxalouria (Coe et al, 2005)
2.1.2 Jenis Batu Saluran Kemih
2.1.2.1 Batu Kalsium
Kalsium yang didapat dari makanan diserap sebanyak 30-40% di
usus halus dan 10% diserap di usus besar. Absorpsi kalsium bervariasi
bergantung pada konsumsi kalsium tersebut. Kalsium diserap pada fase
ionik, dan penyerapan kalsium tidak sempurna karena pembentukan
kompleks kalsium pada lumen usus. Substansi yang dapat menghasilkan
kompleks kalsium adalah fosfat, sitrat, oksalat, sulfat dan asam lemak
(Pearle et al, 2012).
Kalsifikasi dapat berlangsung dan berakumulasi pada duktus
pengumpul, menghasilkan batu saluran kemih. Kira-kira 80-85% dari
seluruh kejadian batu adalah batu kalsium. Batu kalsium sangat sering
terjadi akibat kenaikan kadar kalsium dalam urin, kenaikan kadar asam urat
dalam urin, naiknya kadar oksalat dan menurunnya sitrat dalam urin (Stoller,
2008)
Hiperkalsiuria merupakan kelainan yang paling sering ditemukan
pada pasien dengan batu kalsium. Akan tetapi, peran hiperkalsiuria pada
pembentukan batu masih kontroversial. Investigasi terakhir menyatakan
bahwa plak adalah perkursor yang potensial pada pembentukan batu
kalsium dan angkanya berhubungan langsung dengan kadar kalsium dalam

urin dan angka kejadian batu. (Pearle et al, 2012). Konsentrasi kalsium
dalam urin yang tinggi menyebabkan meningkatnya saturasi garam kalsium
pada urin dan menurunnya aktivitas inhibitor seperti sitrat dan kondroitin
sulfat (Stoller, 2008).
a. Absorptive Hypercalciuric Nephrolithiasis
Konsumsi kalsium normal rata-rata per hari adalah 900-1000 mg.
Kira-kira 150-200 mg akan dieksresikan melalui urin. Absorptive
hypercalciuria (AH) adalah suatu keadaan meningkatnya absorpsi kalsium
pada usus halus, terutama jejunum. Hal ini diakibatkan meningkatnya
jumlah kalsium yang disaring oleh glomerulus, mengakibatkan surpresi dari
hormon paratiroid. Selanjutnya, reabsorpsi kalsium pada tubulus ginjal akan
menurun, mengakibatkan hiperkalsiuria. Kaskade fisiologis ini adalah
sebagai respon dari meningkatnya absorpsi kalsium di usus halus. (Stoller,
2008)
AH terbagi atas 3 tipe yaitu tipe I, II, dan III. Tipe I AH bersifat
independen dari diet dan merupakan 15% dari seluruh kasus batu kalsium.
Pada AH tipe I, terdapat peningkatan kadar kalsium dalam urin meskipun
dilakukan restriksi diet kalsium. Tipe II AH merupakan penyebab batu
saluran kemih yang cukup umum dan bergantung pada diet. Pada tipe II AH,
eksresi kalsium normal pada restriksi kalsium diet. Pasien harus membatasi
konsumsi kalsium sekitar 400-600mg/hari. Tipe III AH disebabkan
kebocoran fosfat pada ginjal. Menurunnya kadar fosfat mengakibatkan
meningkatnya sintesis 1, 25-dihidroksivitamin D. Kaskade fisiologis
tersebut akan meningkatkan absorpsi fosfat dan kalsium pada usus dan
meningkatnya eksresi kalsium dari ginjal, mengakibatkan hiperkalsiuria
(Pearle et al, 2012).
b. Resorptive Hypercalciuric Nephrolithiasis
Sekitar separuh dari pasien dengan hiperparatiroid primer
mengalami batu saluran kemih. Pasien dengan batu kalsium fosfat, wanita
dengan batu kalsium berulang harus dicurigai memiliki hiperparatiroid.

Hiperkalsemia merupakan tanda umum dari hiperparatiroid. Hormon
paratiroid menghasilkan peningkatan kadar fosfor dalam urin dan
menurunnya kadar fosfor dalam plasma, diikuti dengan meningkatnya
kalsium plasma dan urin.
c. Renal Hypercalciuria
Ginjal menyaring sekitar 270 mmol kalsium dan melakukan
reabsorpsi lebih dari 98% diantaranya untuk mempertahankan homeostasis
kalsium. Sekitar 70% reabsorpsi kalsium berlangsung di tubulus proksimal.
Reabsorpsi kalsium tersebut berlangsung secara paraselular.
Pada hiperkalsiuria renal, kerusakan pada tubulus ginjal
mengakibatkan gangguan pada reabsorpsi kalsium. Hal ini menyebabkan
meningkatnya kadar kalsium dalam urin. Kadar kalsium dalam serum tetap
normal disebabkan ginjal yang kehilangan kalsium dikompensasi oleh
meningkatnya absorpsi kalsium melalui pencernaan dan mobilisasi kalsium
dari tulang diakibatkan peningkatan hormon paratiroid (Pearle et al, 2012).
d. Hyperoxalouric calcium nephrolithiasis
Hyperoxalouric calcium nephrolithiasis disebabkan oleh
meningkatnya kadar oksalat dalam urin yaitu diatas 40 mg dalam 24 jam.
Biasanya hal ini ditemukan pada pasien dengan inflammatory bowel disease,
diare kronik, dan dehidrasi berat dan jarang ditemukan yang diakibatkan
oleh konsumsi oksalat yang berlebih.
Diare kronik yang menyebabkan malabsorpsi mengakibatkan
meningkatnya kadar lemak dan empedu. Kalsium intralumen akan berikatan
dengan lemak, menyebabkan terjadinya proses sponifikasi. Kadar kalsium
yang rendah menyebabkan kalsium yang seharusnya berikatan dengan
oksalat menurun. Oksalat yang bebas siap untuk diserap dan tidak
terpengaruh dengan inhibitor-inhibitor. Absorpsi oksalat yang meningkat
mengakibatkan meningkatnya pembentukan produk dari kalsium oksalat.
Hal ini mengakibatkan potensi terjadinya nukleasi dan pertumbuhan kristal
(Pearle et al, 2012).

e. Hypocitraturic calcium nephrolithiasis
Sitrat merupakan inhibitor penting dari batu saluran kemih.
Meningkatnya permintaan metabolic di mitokondria sel-sel ginjal
menyebabkan menurunnya eksresi urin. Hal ini terjadi pada asidosis
metabolik, hipokalemia, puasa, hipomagnesia, androgen dan
glukoneogenesis (Pearle et al, 2012).
Bila membentuk kompleks dengan kalsium, akan menurunkan
konsentrasi kalsium dan menurunnya energi untuk nukleasi. Sitrat juga
menghambat agglomerasi, nukleasi spontan dan pertumbuhan kristal dari
kalsium oksalat dan menurunkan kadar monosodium urat (Pearle et al,
2012).
2.1.2.2 Batu Struvite
Menurut Griffith (1978) dalam Sellaturay (2011), batu struvite
dibentuk dari magnesium, ammonium dan fosfat. Pertama kali ditemukan
oleh Ulex, seorang geologis asal Swedia pada abad ke-18. Nama struvite
berasal dari diplomat dan ilmuwan Rusia H.C.G von Struve. Brown
menemukan bahwa bakteri akan memecah urin dan memfasilitasi
pembentukan batu. Ia mengisolasi Proteus vulgaris dari inti batu yang
sekarang diketahu mensekresikan urease.
Batu struvite umumnya ditemukan pada wanita dan sering berulang
dalam waktu singkat. Mikroorganisme lain yang memecah urea dan dapat
menyebabkan batu struvite adalah Proteus, Pseudomonas, Providencia,
Klebsiella, Staphylococci, dan Mycoplasma. Kadar amonia yang tinggi dari
organisme-organisme tersebut mengakibatkan alkalinisasi pH urin sampai
7,2 sehingga kristal MAP akan mengendap (Stoller, 2008).
Untuk membentuk batu struvite, urin harus mengandung amonia dan
ion trivalent fosfat pada saat yang sama. Tubulus ginjal hanya menghasilkan
amonia apabila organisme mengeksresikan asam, akan tetapi ion trivalent

fosfat tidak tersedia pada saat urin bersifat asam, oleh karena itu batu
struvite tidak terbentuk saat kondisi fisiologis. Pada kondisi patologis,
dimana terdapat bakteri yang menghasilkan urease, urea akan dipecah
menjadi amonia dan asam karbonat. Selanjutnya, amonia akan bercampur
dengan air untuk menghasilkan ammonium hidroksida pada kondisi basa,
dan akan menghasilkan bikarbonat dan ion karbonat. Alkalinisasi urin oleh
reaksi urease tadi menghasilkan NH4, yang akan membentuk ion karbonat
dan ion trivalent fosfat. Inilah yang akan membentuk batu struvite
(Sellaturay, 2011)

The dissociation of hydrogen phosphate under alkaline conditions provides phosphate, thereby completing the generation of con-stituent ions for infection stone formation:

This chemical cascade, along with physiologic concentrations of magnesium, provides the constituents necessary for precipitation of struvite. In addition, the concentrations of calcium, phosphate, and carbonate allow precipitation of carbonate apatite and hydroxyapatite, thereby comprising the components of infection stones (Fig. 4514).
Although infection stones are a direct result of persistent or recurrent infection with urease-producing bacteria, they may also be associated with or exacerbated by urinary obstruction or stasis (Bichler et al, 2002). As such, growth of infection stones can prog-ress at a rapid rate (Hinman, 1979).
BacteriologyAlthough the family Enterobacteriaceae comprises the majority of urease-producing pathogens, a variety of gram-positive and gram-negative bacteria and some yeasts and Mycoplasma species have the capacity to synthesize urease (Table 454). The most common urease-producing pathogens are Proteus, Klebsiella, Pseudomonas, and Staphylococcus species (Griffith and Osborne, 1987), with Proteus mirabilis the
most common organism associated with infection stones (Silverman and Stamey, 1983). Although Escherichia coli is a common cause of urinary tract infections, only rare species of E. coli produce urease (Bichler et al, 2002). Bacterial urease can be detected by the Urea-Rapid Test, a urea-indole medium from Bio-Merieux, Inc. (Durham, NC) (Bichler et al, 2002).
Bacteria may be involved in stone formation by damaging the mucosal layer of the urinary tract, resulting in both increased bacterial colonization and crystal adherence (Parsons et al, 1984; Grenabo et al, 1988). It has been proposed that ammonium, gen-erated as a result of urealysis, may alter the glycosaminoglycan layer present on the surface of the transitional cell layer and sig-nificantly increase bacterial adherence to normal bladder mucosa, further exacerbating infection risk (Parsons et al, 1984). In addi-tion, a study in rats found that injury to the bladder mucosa increased crystal adherence to the bladder wall, a process that was potentiated by the presence of common bacteria such as Proteus, E. coli, Enterococcus, and Ureaplasma urealyticum (Grenabo et al, 1988). Another potential mechanism for increased stone forma-tion in the presence of bacteria is the finding that particular bacteria, such as E. coli and Proteus, may alter the activity of uro-kinase and sialidase, whereas organisms not typically associated

2.1.2.3 Batu Asam Urat
Batu asam urat merupakan jenis batu yang lazim ditemukan pada
pria dan memiliki angka kejadian 5% dari seluruh kejadian batu. Pasien
dengan gout, penyakit proliferatif, penurunan berat badan yang cepat serta
riwayat penggunaan obat-obat sitotoksik memiliki insiden yang tinggi pada
batu asam urat. Tidak seluruh pasien dengan batu asam urat mengalami
hiperurisemia,. Naiknya kadar asam urat dalam urin dipicu oleh kurangnya
cairan dan konsumsi purin yang berlebihan.

Terdapat 3 faktor utama pada pembentukan batu asam urat yaitu pH
urin yang rendah, volume urin yang rendah dan hyperuricosuria. Faktor
patogenesis utama adalah pH urin yang rendah karena umumnya pasien
dengan batu asam uran memiliki kadar eksresi asam urat yang normal (Pak
et al, 2003).

Uric acid is a weak acid with a pKa of 5.35 at 37 C. At that pH, half of the uric acid is present as the urate salt and half as free uric acid. Because sodium urate is approximately 20 times more soluble than the free acid, the relative proportion present as free uric acid strongly determines the risk of stone formation. Urine pH is a critical factor in determining uric acid solubility; at pH 5, even modest amounts of uric acid exceed uric acid solubility, whereas at pH 6.5, concentrations of uric acid exceeding 1200 mg/L remain soluble (see Fig. 4510 on the Expert Consult website) (Asplin, 1996). Under normal conditions, the limit of uric acid solubility is approximately 96 mg/L, a level readily exceeded by normal daily uric acid excretion, which averages 500 to 600 mg/L. Consequently, urine may reach supersaturation, particularly at pH less than 6. Low urine pH increases concentrations of sparingly soluble undissociated uric acid, which leads to direct precipitation of uric acid. Of note, uric acid and sodium urate can also serve as a nidus for calcium oxalate stones through heterologous nucle-ation, and, thus, low urine pH is a risk factor for both uric acid and calcium oxalate stones (Coe and Kavalach, 1974; Levy et al, 1995).
The process of uric acid stone formation once uric acid crystals precipitate has not been fully elucidated. Although some investi-gators have suggested that uric acid crystal adhesion to kidney epithelial cells (Koka et al, 2000) and inhibitors such as glycosami-noglycans (Ombra et al, 2003) may play a role in uric acid stone formation, the involvement or importance of these factors in uric acid stone formation is unclear (Pak et al, 1976a).
The three main determinants of uric acid stone forma-tion are low pH, low urine volume, and hyperuricosuria (Fig. 4511). The most important pathogenetic factor is low urine pH because most patients with uric acid stones have normal uric acid excretion but invariably demon-strate persistent low urine pH (Pak et al, 1985, 2003a). Uric acid stones can develop as a result of congenital, acquired, or idiopathic causes. Congenital disorders associated with uric acid stones involve renal tubular urate transport or uric acid metabo-lism, leading to hyperuricosuria. Acquired causes of uric acid stones such as chronic diarrhea, volume depletion, myeloprolif-erative disorders, high animal protein intake, and uricosuric drugs may affect any of the three factors determining uric acid stone formation. Patients with gouty diathesis or idiopathic low urine pH typically demonstrate decreased fractional excretion of urate and do not have gout (Maalouf et al, 2004a). Patients with gouty diathesis differ from those with hyperuricosuric calcium nephro-lithiasis in that the former generally have normal urinary uric acid levels and acidic urine, whereas the latter have hyperuricosuria and normal urine pH. Patients with hyperuricosuria frequently have high urinary sodium and calcium levels leading to increased
excretion of stone-forming substances such as calcium and uric acid owing to impaired renal function.
HypomagnesuriaHypomagnesuria is a rare cause of nephrolithiasis, affecting less than 1% of stone formers as an isolated abnormality, although it can be found in conjunction with other abnormalities in 6% to 11% of cases (Levy et al, 1995; Schwartz et al, 2001). Magnesium complexes with oxalate and calcium salts, and therefore low magnesium levels result in reduced inhibitory activ-ity. Low urinary magnesium is also associated with decreased urinary citrate levels, which may further con-tribute to stone formation (Preminger et al, 1989; Schwartz et al, 2001). Whether low magnesium is the cause or effect of low citrate is not clear. Low magnesium levels occur with poor dietary intake (Pfab et al, 1985) or as a result of reduced intestinal absorp-tion associated with intestinal abnormalities producing chronic diarrheal syndrome (Rudman et al, 1980).
Although a number of studies in rats have implicated hypomag-nesuria as a factor in stone formation (Rushton and Spector, 1982), others (Faragalla et al, 1963; Borden et al, 1969; Rattan et al, 1993) have questioned the impact of magnesium (Su et al, 1991). Clini-cal studies regarding the role of magnesium are contradictory. Schwartz and colleagues (2001) found that hypomagnesiuric patients had higher stone recurrence rates than patients with normal urinary magnesium. However, other studies found no dif-ference in magnesium excretion between stone patients and con-trols (Johansson et al, 1980; Esen et al, 1991). Of note, the lack of difference in mean magnesium levels may be a result of the small fraction of stone formers with low urinary magnesium levels.
Although magnesium has been shown to increase urinary pH, citrate, and magnesium and therefore to decrease urinary satura-tion of calcium oxalate in vitro (Khan et al, 1993) and in vivo (Curhan et al, 2001), two randomized trials comparing magne-sium oxide with placebo or no treatment in stone formers have failed to demonstrate clinical benefit (Wilson et al, 1984; Ettinger et al, 1988).
Uric Acid StonesMost mammals, except humans and Dalmatians, synthesize the hepatic enzyme uricase, which catalyzes the conversion of uric acid to allantoin, the end product of purine metabolism (Yu, 1981; Bannasch et al, 2004). Consequently, humans accumulate signifi-cantly higher levels of uric acid in their blood and urine (Watts, 1976; Yu, 1981). Because allantoin is 10 to 100 times more soluble in urine than uric acid, humans are prone to uric acid stone formation.

Hiperurikosuria menjadi faktor predisposisi pada pembentukan batu
asam urat dan batu kalsium oksalat karena menyebabkan supersaturasi urin.
Pasien dengan kadar asam urat dalam urin dibawah 600mg/hari memiliki
batu yang lebih sedikit dari pasien yang memiliki kadar asam urat diatas
1000mg/hari dalam urin.
Batu asam urat dapat dihasilkan secara kongenital, didapat, atau
idiopatik. Kelainan congenital yang berhubungan dengan batu asam urat
melibatkan transpor urat di tubulus ginjal atau metabolisme asam urat
menyebabkan hiperurikosuria. Kelainan didapat dapat berupa diare kronik,
turunnya volume urin, penyakit-penyakit myeloproliferatif, tingginya
konsumsi protein hewani, dan obat obatan yang menyebabkan 3 faktor
diatas (Pearle et al, 2012).
2.1.3. Manifestasi Klinis dan Evaluasi Pasien Batu Saluran Kemih

Banyak gejala serta tanda yang dapat menyertai penyakit batu
saluran kemih. Walaupun begitu, ada juga beberapa batu yang tidak
menunjukkan gejala atau tanda khusus tetapi ditemukan pada hasil
pemeriksaan radiologi. Gejala-gejala yang sering timbul pada pasien dapat
berupa nyeri, hematuria, mual, muntah, demam, dan gangguan buang air
kecil seperti frekuensi, urgensi dan disuria. (Pahira & Pevzner, 2007).
Nyeri merupakan gejala yang paling sering menyertai penyakit batu
saluran kemih, mulai dari nyeri sedang sampai nyeri berat yang memerlukan
pemberian analgesik. Nyeri biasanya terjadi pada batu di saluran kemih
bagian atas, dengan karakter nyeri bergantung pada lokasi batu, ukuran batu,
derajat obstruksi, dan kondisi anatomis setiap orang yang berbeda-beda.
Nyeri yang terjadi dapat berupa kolik maupun nonkolik (Pearle et al, 2012)
Nyeri kolik pada ginjal biasanya terjadi diakibatkan meregangnya
ureter atau collecting duct, diakibatkan adanya obstruksi saluran kemih.
Obstruksi juga menyebabkan meningkatnya tekanan intraluminal,
meregangnya ujung-ujung saraf, dan mekanisme lokal pada lokasi obstruksi
seperti inflamasi, edema, hiperperistaltik dan iritasi mukosa yang
berpengaruh pada nyeri yang dialami oleh pasien (Stoller, 2008)
Pada obstruksi di renal calyx, nyeri yang terjadi berupa rasa nyeri
yang dalam pada daerah flank atau punggung dengan intensitas bervariasi.
Nyeri dapat muncul pada konsumsi cairan yang berlebihan. Pada obstruksi
renal pelvic dengan diameter batu diatas 1 cm, nyeri akan muncul pada
sudut costovertebra. Nyeri yang timbul dapat berupa nyeri yang redup
sampai nyeri yang tajam yang konstan dan tidak tertahankan, dan dapat
merambat ke flank dan daerah kuadran abdomen ipsilateral (Stoller, 2008).
Obstruksi di proximal ureter menimbulkan nyeri pada sudut
kostovertebra yang intens dan dapat merambat sepanjang dermatom dari
saraf spinal yang terpengaruh. Pada obstruksi ureter bagian atas, nyeri
merambat ke daerah lumbal, sementara pada obstruksi midureter nyeri
merambat ke daerah lower abdomen. Obstruksi di ureter bagian distal
cenderung menyebabkan nyeri yang merambat ke daerah lipat paha dan
testis pada pria atau labia mayora pada wanita. Rambatan nyeri tersebut

dihantarkan melalui nervus ilioinguinal atau cabang genital dari nervus
genitofemoral (Stoller, 2008).
Insiden hematuria pada pasien batu saluran kemih diperkirakan
mencapai 90% berdasarkan teori yang ada. Akan tetapi, tidak adanya
hematuria tidak menjadi jaminan bahwa batu saluran kemih tidak terjadi.
Diperkirakan 10% pasien memiliki hasil negatif pada pemeriksaan
mikroskopi dan dipstick (Lallas et al, 2011)
Pemeriksaan urinalisis lengkap diperlukan untuk memastikan
diagnosa batu saluran kemih berdasarkan hematuria dan kristaluria dan pH
urin. Pasien biasanya mengeluhkan warna urin yang seperti teh pekat. Pada
10-15 % kasus, mikrohematuria tidak terjadi akibat obstruksi komplit dari
ureter.
Demam yang berhubungan dengan adanya batu saluran kemih
menunjukkan suatu kondisi hjjjjj gawat darurat. Demam merupakan salah
satu dari gejala sepsis selain takikardi, hipotensi dan vasodilatasi. Sementara
itu, mual dan muntah terjadi akibat kolik yang dirasakan oleh pasien (Stoller,
2008).

Setelah menggali riwayat pasien, evaluasi yang dilakukan adalah
pemeriksaan fisik. Pemeriksaan fisik yang detail merupakan komponen
penting dalam evaluasi pasien dengan batu saluran kemih. Hal-hal yang
dapat dilihat seperti takikardia, berkeringat, mual, demam, dan
menyingkirkan kemungkinan kemungkinan kelainan pada abdomen dan
lumbal (Pahira dan Pevzner, 2007)

membantu bila diagnosa belum pasti. NCCT dapat mendeteksi batu asam
urat dan batu xanthine yang bersifat radiolucent pada foto polos. NCCT
memiliki sensitivitas 97% dan spesivisitas 96% (Turk et al, 2013).
2.1.4 Penatalaksanaan Batu Saluran Kemih
2.1.4.1 Penatalaksanaan Konservatif
Penatalaksanaan konservatif diberikan pada pasien tanpa riwayat
batu saluran kemih. Penatalaksanaan non-farmakologis dapat mengurangi
insiden rekuren batu per 5 tahun sampai 60%. Penatalaksanaan konservatif
berupa :
1. Konsumsi cairan minimal 8-10 gelas per hari dengan tujuan
menjaga volume urin agar berjumlah lebih dari 2 liter per hari
2. Mengurangi konsumsi protein hewani sekitar 0,8 1,0
gram/kgBB/hari untuk mengurangi insiden pembentukan batu
3. Diet rendah natrium sekitar 2-3 g/hari atau 80-100 mEq/hari
efektif untuk mengurangi eksresi kalsium pada pasien dengan
hiperkalsiuria
4. Mencegah penggunaan obat-obat yang dapat menyebabkan
pembentukan batu seperti calcitrol, suplemen kalsium, diuretic
kuat dan probenecid
5. Mengurangi makanan yang berkadar oksalat tinggi untuk
mengurangi pembentukan batu. Makanan yang harus dikurangi
seperti teh, bayam, coklat, kacang-kacangan dan lain-lain (Pearle
et al, 2012)

hypercalciuria but prevented complications commonly associated with sodium cellulose phosphate therapy.
Although side effects are generally mild, they occur in about 30% to 35% of patients treated with thiazide. Side effects are usually seen on initiation of treatment but disappear with contin-ued treatment. Lassitude and sleepiness are the most common symptoms and can occur in the absence of hypokalemia. Potas-sium supplementation should always be considered, par-ticularly in patients with evident potassium deficiency, patients on digitalis therapy, and those individuals who develop hypocitraturia. Addition of potassium citrate has been documented to prevent occurrence of hypokalemia and hypochloremic metabolic acidosis in patients undergoing long-term thiazide therapy (Odvina et al, 2003). Occasionally, thiazides unmask primary hyperparathyroidism (i.e., thiazide challenge). Thiazides may also cause impaired carbohydrate tolerance and hyperuricemia. A more distressing complication is decreased libido or sexual dysfunction, which is seen in a small percentage of patients.
Sodium Cellulose PhosphateSodium cellulose phosphate (SCP), given orally, is a nonabsorbable ion exchange resin that binds calcium and inhibits calcium absorption (Blacklock and Macleod, 1974; Pak et al, 1974; Backman et al, 1980). Unfortunately, despite early enthusiasm, the
treatment, however, the rise in bone density stabilizes and the hypocalciuric effect of thiazide becomes attenuated. These results suggest that thiazide treatment may cause a low turnover state of bone that interferes with a continued calcium accretion in the skeleton. The rejected calcium would then be excreted in urine. In contrast, bone density is not significantly altered in renal hyper-calciuria, in which thiazide has been shown to cause a decline in intestinal calcium absorption commensurate with a reduction in urinary calcium.
Further work on this topic has been recently reported (Pak et al, 2003a). In this study, 28 patients with absorptive hypercalciuria type 1 were managed with thiazide (20) or indapamide (8) and potassium citrate for 1 to 11 years while maintained on diet low in calcium oxalate. Serum and urinary chemistry studies and bone mineral density were measured at baseline and at the end of treat-ment. During treatment, urinary calcium significantly decreased but urinary oxalate did not change. Urinary pH and citrate signifi-cantly increased, and urinary saturation of calcium oxalate signifi-cantly decreased by 46%. Stone formation rate decreased significantly from 2.94 to 0.05 per year. Notably, L2-L4 bone mineral density increased significantly by 5.7% compared with normal peak value, and by 7.1% compared with normal age- and gender-matched values. The authors concluded that dietary restriction of calcium and oxalate, combined with thia-zide and potassium citrate, satisfactorily controlled

2.1.4.2 Penatalaksanaan Spesifik
1. Batu kalsium
Untuk Absorptive hypercalciuria tipe I dapat diberikan diuretik
tiazid 25-50 mg untuk menurunkan kadar kalsium dalam urin sampai 150
mg/hari. Hal ini terjadi melalui turunnya volume urin yang mengakibatkan
kompensasi meningkatnya reabsorpsi natrium dan kalsium di tubulus
proksimal. Alternatif lain yang dapat diberikan yaitu chlorthalidone 25-50
mg, indapamide 1,25-2,5 mg/hari (Stoller, 2008).
Pada AH tipe II, dilakukan restriksi diet kalsium 600 mg/hari.
Restriksi diet natrium juga penting untuk menurunkan hiperkalsiuria. Tiazid

dan suplemen kalium sitrat juga dapat diberikan apabila penatalaksanaan
konservatif tidak efektif. Pada AH tipe III, diberikan orthophospate yang
akan menurunkan kadar 1,25(OH)2D3 dan meningkatkan kadar inhibitor
dalam urin.
Tiazid juga diberikan pada renal hiperkalsiuria untuk meningkatkan
reabsorpsi kalsium di tubulus. Hal ini akan menormalkan kadar kalsium
dalam serum dan menurunkan kadar hormon paratiroid. Diet natrium juga
dikurangi menjadi 2 g/hari dan menjaga natrium urin dibawah 100
mEq/hari.
Pada hiperoksalouria primer, pyridoxine dapat menurunkan produksi
oksalat endogen. Dosis pyridoxine yang dianjurkan adalah 100-800 mg/hari.
Orthophospate oral juga dapat diberikan dalam dosis 4 kali sehari.
Magnesium oral, suplemen kalium sitrat dan konsumsi cairan yang
ditambah dapat membantu terapi (Turk et al, 2013)
Pasien dengan hipositraturia diberikan kalium sitrat untuk
meningkatkan pH intraselular dan produksi sitrat. Selain kalium sitrat,
konsumsi jus lemon setiap hari yang dilarutkan dalam 2 liter air akan
meningkatkan kadar sitrat dalam urin (Stoller, 2008).
2. Batu asam urat
Untuk pasien dengan batu asam urat, penatalaksanaan harus
dilakukan adalah penatalaksanaan konservatif dibantu dengan pemberian
obat-obatan. Pemberian acetazolamide 250-500 mg pada malam hari akan
berguna untuk mengontrol pH urin. Allupurinol diberikan apabila kadar
asam urat dalam darah diatas 800 mg/hari dan pH urin diatas 6,5.
Suplementasi kalium sitrat berguna untuk menjaga pH urin tetap bersifat
alkali sekitar 6,5. Kadar pH dalam urin harus tetap dijaga agar tidak naik
sampai keatas 7, untuk mengurangi resiko terbentuknya batu kalsium fosfat
(Pearle et al, 2012).

3. Batu sistin
Pasien dengan batu sistin harus meningkatkan konsumsi cairan agar
mendapatkan urin sekitar 3,5 liter setiap harinya untuk disolusi maksimal
dari batu sistin. Alkalinisasi urin menggunakan kalium sitrat atau sodium
bikarbonat digunakan untuk menjaga pH urin 7,5-8,5. Urin yang alkali akan
meningkatkan larutnya sistin dalam urin (EAU Guideline, 2013).
Bila pengobatan diatas tidak berhasil dan kadar sistin dalam urin
diatas 3 mmol per hari, maka dapat diberikan tiopronin. Dosis tiopronin
yang digunakan adalah 250 mg per hari. Tiopronin dianggap lebih baik dari
pendahulunya yaitu D-penicillamine yang dianggap menimbulkan banyak
efek samping (EAU Guideline, 2013).
2.1.4.3 Modalitas terapi
1. Percutaneous Nephrolithotomy (PCNL)
Tehnik PCNL dilakukan melalui akses pada lower calyx, selanjutnya
dilakukan dilatasi menggunakan balloon dilator atau Amplatz dilator dengan
bantuan fluoroscopy dan batu dihancurkan menggunakan elektrohidrolik,
ultrasonic atau litotripsi laser (Pearle et al, 2012)
Indikasi melakukan PCNL adalah batu staghorn, batu ginjal dengan
ukuran diatas 3 cm, batu sistin, adanya abnormalitas ginjal dan saluran
kemih bagian atas, kegagalan pada ESWL dan uretroscpy, dan batu pada
ginjal hasil transplantasi. PCNL tidak dapat dilakukan pada kondisi
perdarahan, infeksi saluran kemih yang tidak terkontrol, dan faktor-faktor
yang mengakibatkan PCNL tidak optimal seperti obesitas dan splenomegaly
(Stoller, 2008)
2. Uretroscopy (URS)
URS merupakan baku emas untuk penatalaksanaan batu ureter
tengah dan distal. Penggunaan uretroskop dengan kaliber yang kecil dan
balloon dilatation meningkatkan stone-free rate secara dramatis. Terdapat
variasi pada lithotries yang dapat ditempatkan pada uretroscope termasuk
elektrohidrolik, probe ultrasonic, laser dan system pneumatic seperti Swiss

lithoclast. Lithotrites elektrohidrolik memiliki tenaga 120 volt yang dapat
menghasilkan gelombang kejut. Lithotrites ultrasonik memiliki sumber
energi piezoceramic yang dapat mengubah energi listrik menjadi gelombang
ultrasonik 25.000 Hz, sehingga dapat efektik mengakibatkan fragmentasi
pada batu tersebut (Stoller, 2008)
URS efektif digunakan pada batu ureter dengan tingkat keberhasilan
98-99% pada batu ureter distal, 51-97% pada batu mid ureter dan 58-88%
pada batu ureter atas. URS memiliki komplikasi seperti abrasi mukosa,
perforasi ureter, dan striktur ureter (Stoller, 2008).
2.2 Extracorporeal Shock Wave Lithotripsy (ESWL)
2.2.1 Cara Kerja ESWL
Prinsip kerja alat ESWL adalah menggunakan gelombang kejut.
Gelombang kejut adalah gelombang tekanan yang berenergi tinggi yang
dapat dialirkan melalui udara maupun air. Ketika berjalan melewati dua
medium yang berbeda, energi tersebut dilepaskan, menyebabkan batu
terfragmentasi. Gelombang kejut tidak menyebabkan kerusakan bila
melewati substansi dengan kepadatan yang sama. Oleh karena air dan
jaringan tubuh memiliki kepadatan yang sama, gelombang kejut tidak
merusak kulit dan jaringan dalam tubuh. Batu saluran kemih memiliki
kepadatan akustik yang berbeda, dan bila dikenai gelombang kejut, batu
tersebut akan pecah, Setelah batu terfragmentasi, batu akan keluar dari
saluran kemih (Pahira dan Pevzner, 2007).
Terdapat beberapa mekanisme dalam pemecahan batu melalui
ESWL bergantung pada energi yang digunakan, yaitu :

Pada generator elektrohidrolik, gelombang kejut yang berbentuk
bulat dihasilkan oleh percikan air. Voltase yang tinggi diberikan pada dua
elektroda yang berhadapan dengan jarak 1 mm. Voltase yang tinggi tersebut
menyebabkan air menguap pada ujung elektroda. Selanjutnya gelombang
kejut yang terbentuk difokuskan pada batu, dengan meletakkan elektroda
pada suatu fokus dan elektroda lain pada target fokus. Dengan ini, mayoritas

gelombang kejut yang dihasilkan oleh elektroda akan mengenai batu pada
F1. Kekurangan generator elektrohidrolik ini adalah tekanannya yang
berfluktuasi dan daya hidup elektroda yang singkat.
2. Generator elektromagnetik
Generator elektromagnetik menggunakan gelombang kejut yang
berbentuk silinder atau datar. Gelombang yang datar akan difokuskan oleh
sebuah lensa akustik sementara gelombang silinder akan direfleksikan oleh
sebuah reflector parabolik. Prinsip kerja generator ini cukup sederhana,
yaitu sebuah shock tube yang diisi air mengandung 2 plat silinder yang
dipisahkan oleh lembaran pelindung. Ketika arus listrik dikirimkan satu atau
kedua konduktor, gerakan plat terhadap air dan sekitarnya menghasilkan
suatu gelombang tekanan. Tenaga elektromagnetik terbentuk yang disebut
dengan tekanan magnetik menyebabkan gelombang kejut di air. Energi dari
gelombang kejut yang dihasilkan dikonsentrasikan pada target melalui lensa
akustik. Selanjutnya, tenaga akan difokuskan pada satu titik fokal dan

In both systems the pressure pulse has only one focal point (F2) that is positioned on the target.
Electromagnetic generators are more controllable and reproduc-ible than electrohydraulic generators because they do not incor-porate a variable in their design such as the underwater spark discharge. Other advantages include the introduction of energy into the patients body over a large skin area, which may cause less pain. In addition, a small focal point can be achieved with high-energy densities, which may increase its effectiveness in breaking stones. This generator will deliver several hundred thou-sand shockwaves before servicing, thereby eliminating the need for frequent electrode replacement, which is required with most electrohydraulic machines. A disadvantage of this design may be that the small focal region of high energy results in an increased rate of subcapsular hematoma formation. The rate of subcapsular hematoma formation for the Storz Modulith has been suggested to be 3.1% to 3.7% (Dhar et al, 2004). Piper and associates (2001) suggested that perinephric hematomas may occur in up to 12% of patients treated with a DoLi S lithotripter. In contrast, perineph-ric hematomas were reported to occur in approximately 0.6% of patients undergoing SWL with the unmodified Dornier HM3 machine (Chaussy and Schmiedt, 1984; Knapp et al, 1987).
Piezoelectric Generator. The piezoelectric lithotripter also produces plane shockwaves with directly converging shockfronts. These generators are made of a mosaic of small, polarized, polycrystalline, ceramic elements (barium titanate), each of which can be induced to rapidly expand by the application of a high-voltage pulse (Fig. 4817). Owing to the limited power of a single piezoelectric element, 300 to 3000 crystals are necessary for the generation of a sufficiently large shock pressure. The piezo-electric elements are usually placed on the inside of a spherical dish to permit convergence of the shockfront. The focus of the system is at the geometric center of the spherical dish.
The advantages of this generator include the focusing accuracy, a long service life, and the possibility of an anesthetic-free treatment because of the relatively low-energy density at the skin entry point of the shockwave. For this reason, piezoelectric lithotripters in general tend to produce less discomfort than do lithotripters with other energy sources. A major disadvantage of this system is the insuf-ficient power it delivers, which hampers its ability to effectively break renal stones. The piezoelectric energy sources produce some of the highest peak pressures of any litho-tripter, but the actual energy delivered to the stone per shockwave pulse is several orders of magnitude lower than that delivered by an electrohydraulic machine because of the extremely tiny volume of F2.
Other Generators. Microexplosive generators have also been produced but have not gained widespread acceptance. The explo-sion of tiny lead azide pellets within a parabolic reflector generates the devices shockwave (Kuwahara et al, 1987). Despite the effec-tiveness of this type of generator in producing shockwaves, this technology has not met with commercial success because of con-cerns about the storage and handling of the volatile lead azide pellets. Still other methods of shockwave generation use a laser

Generator elektromagnetik lebih mudah dikontrol dan mudah
diproduksi. Keuntungan lainnya adalah pajanan terhadap tubuh pada daerah
yang luas menyebabkan nyeri yang kurang. Titik fokal yang kecil dengan
energi yang besar meningkatkan efektifitas dari pemecahan batu. (Pearle et
al, 2012). Akan tetapi, hal ini juga meningkatkan resiko hematoma
subkapsular sekitar 3,1-3,7% menurut Dhar (2004) dalam Pearle (2012).

3. Generator piezoelektrik
Litotripter piezoelektrik menghasilkan gelombang kejut yang datar
dan konvergen. Generator ini dibuat dari elemen barium titanate yang kecil
dan terpolarisasi yang dapat dengan cepat menghantarkan arus gelombang.
Oleh karena ukurannya yang kecil, diperlukan 300-3000 kristal

issue of stone detection is the fact that it is almost impossible to view a kidney stone in areas such as the middle third of the ureter or when there is an indwelling ureteral catheter. Once a stone is fragmented, it is difficult to identify each individual stone piece. Unfortunately, these disadvantages tend to overshadow the advan-tages of ultrasound imaging.
Combination of Ultrasonography and Fluoroscopy. As the demand for interdisciplinary lithotripters has increased, the lithotripsy industry has responded, in some cases combining ultrasonogra-phy and fluoroscopy for stone localization. There are clearly advantages to these setups, but each system has a drawback that limits one of the functions of the system.
Anesthesia. The approach to anesthesia for lithotripsy has changed considerably since clinical SWL began in 1980. At that time, regional or general anesthesia was used in all instances because the unmodified HM3 device (15.6-cm ellipsoid; 80-nF generator) produced a powerful shockwave and treatment at rec-ommended energy levels caused intolerable pain. Subsequently, urologists and lithotripter manufacturers recognized that the HM3 is considerably more powerful at the recommended energy setting than is necessary for the fragmentation of most renal calculi, an observation that spawned interest in less powerful lithotripters with lessened anesthesia requirements (Marberger et al, 1988; Wilbert et al, 1987). Several researchers have noted that the origi-nal HM3 lithotripter without modification produces excellent clinical results when it is used at lower energy settings (Pettersson et al, 1989; Tiselius, 1991; Tolley et al, 1991). In addition, such settings create a smaller lesion at F2 in experimental animals (Connors et al, 2000).
The discomfort experienced during SWL is related directly to the energy density of the shockwave as it passes through the skin as well as the size of the focal point. In the past decade several new and useful anesthetic tech-niques adaptable to SWL have been produced that were not avail-able at the time SWL was introduced and include short-acting parenteral sedative-narcotics and topical agents.
Short-acting agents, such as the narcotic alfentanil and the sedative-hypnotics midazolam and propofol, have been used in various combinations to allow most SWL treatments with any lithotripter (including the unmodified Dornier HM3) to be accom-plished comfortably for the patient without the need for general
beam or a multistage light gas gun, but these too have not been well received commercially.
Imaging Systems. There are three basic designs used by lithotripter manufacturers for stone localization. They are fluoroscopy alone, ultrasonography alone, and the combination of ultrasonography and fluoroscopy.
Fluoroscopy Alone. The original Dornier HM3 lithotripter used two x-ray converters arranged at oblique angles to the patient and 90 degrees from each other to localize the stone effectively at F2. To reduce the cost of lithotripters, an adjustable C-arm has been subsequently introduced on many devices. There is presently a remarkable similarity in the fluoroscopic systems used among manufacturers. This appears to be primarily the result of a common theme in the industry to develop multifunctional tables around these machines. The fluoroscopic system typically consists of a high-quality digitized x-ray imaging system mounted on a rotat-able C-arm with an isocentrically integrated shockwave source. Because the shockwave head can be rotated out of the field of the fluoroscopic system, the table can be used for routine urologic fluoroscopic applications.
The primary advantages of fluoroscopy still include its familiarity to most urologists, the ability to visualize radiopaque calculi throughout the urinary tract, the ability to use iodinated contrast agents to aid in stone localization, and the ability to display anatomic detail. The disadvantages include the exposure of the staff and patient to ionizing radiation, the high maintenance demands of the equip-ment, and the inability to visualize radiolucent calculi without the use of radiographic contrast agents.
Ultrasonography Alone. Ultrasonic localization was initially designed to aid multifunctional lithotripters for treatment of both urinary and biliary stones. It is presently used in several low-cost machines because it is inexpensive to manufacture and to main-tain compared with fluoroscopic systems. Another major advan-tage of this technology is in the treatment of children and infants when one is concerned about the dose of ionizing radiation. In addition, ultrasonography can localize slightly opaque or non-opaque calculi.
Despite its advantages there are a number of significant disad-vantages of ultrasound imaging. Sonographic localization of a kidney stone requires a highly trained operator. To complicate the

Keuntungan dari generator ini adalah fokus yang akurat, dan
kemungkinannya untuk dilakukan tindakan tanpa anastesi karena kekuatan
energi yang tendah pada kulit saat gelombang kejut memasuki tubuh. Oleh
karena itu, litotripter piezoelektrik menjai pilihan karena merupakan pilihan
yang paling nyaman dibandingkan sumber energi lain. Kekurangannya
adalah tenaga yang dihasilkan kurang sufisien, sehingga memperlambat
proses pemecahan batu secara efektif. Piezoelektrik menghasilkan tekanan
puncak yang paling besar dibandingkan dengan litotripter lain, akan tetapi
dikarenakan volume dari piezoelektrik yang kecil maka energi yang
dihantarkan menjadi berkurang (Pearle et al, 2012)
2.2.2 Indikasi dan Kontraindikasi Tindakan ESWL

Tindakan ESWL hanya dapat dilakukan pada batu dengan lokasi
ginjal dan ureter. Lebih dari 90% batu pada orang dewasa dapat
ditatalaksana dengan ESWL. ESWL merupakan pilihan utama terapi pada
batu proksimal ureter dengan ukuran dibawah 10 mm dan 10-20 mm, baik
pada ureter proksimal maupun distal.
Tingkat kesuksesan tindakan ESWL untuk batu dengan ukuran kurang dari
20 mm adalah 80-90%. Batu yang terletak di lower calyx dan ureter
memiliki tingkat fragmentasi 60-70%. Akan tetapi, tingkat kesuksesan juga
ditentukan oleh komposisi batu dan pelaksanaan ESWL (Stoller, 2008)
Kontraindikasi pelaksanaan ESWL terbagi 2, yaitu kontraindikasi
absolut dan kontraindikasi relatif.
