HAMILTON HOUSE

The Columbus District Golf Association will send $5 per player fromeach entry fee to the Capital Campaign for the new Evans ScholarHouse at the Ohio State University. The new house, located at 52East 14th Avenue is named the Hamilton House in honor of Earl“Bones” Hamilton. $5 per player of the entry fee qualies as acharitable donation for federal income tax purposes.

EVANS SCHOLARSHIP

Evans Scholars Foundation administers the nation’s largest privatelyfunded college scholarship program. Since 1930, over 8,500deserving young caddies have earned their college degrees through

Evans Scholarships. Today, more than 820 caddies attend collegeeach year as Evans Scholars. The Evans Scholarship was founded bygolf great Charles “Chick” Evans Jr. (1890-1979).

ELIGIBILITY Entrants must be a full member of a CDGA Afliated Club. CDGA GHIN # mustbe on application. Team participants must be full, non-restrictive golng members of the clubthey are representing.

RULES USGA rules will govern all play except for local rules approved by CDGA.

FORMAT Field limited to 28 teams, one from each club. Any open remaining spots will be lledby draw after the deadline. 18 Holes, each event. Points awarded for top nishers, total pointsafter three events determines champion team. All play is scratch. OSU – 2 of 4 Balls, Lancaster –two 2-man scrambles, switching partners after nine, Pinnacle – Points, All four balls count.

ENTRY Entry is subject to approval or rejection at any time by CDGA Tournament Committee.No practice round is included in entry.

ENTRY FEE $750 per team. Entry fee for teams from Lancaster, OSU and Pinnacle is $675(10% discount as a host club member). Entry includes cart, range and continental breakfastat each event. Fee will not be refunded after deadline unless team submits a written medicalexcuse.

DEADLINE May 3, 2012, 4 p.m. (or when eld lls). No Tee Entries.CARTS Golf carts and caddies are permitted. Players may carry clubs if host club permits.

CADDIES The following constitutes a condition of the competition and is authorized byAppendix I, Part C, Section 3 of the Rules of Golf. “A player is prohibited from having aparent, guardian or another player in the same competition serve as his caddie during thestipulated round.”

TEE TIMES Requested tee times can not be guaranteed. Copies of starting times will be sent toCDGA Pro Shops approximately one week in advance of event. Contestants may also checkOHIOGOLF.ORG for tee times of the events. Entry Fee: $750 per Team
