Convergent evolution of Salmonella typhoid agents

To date, over 2500 Salmonella serovars have been known and more than 1400 may cause infections in warm blooded animals. In humans, most Salmonella pathogens cause usually self-limited gastroenteritis, but a few, e.g., Salmonella typhi and S. paratyphi C, cause typhoid, a potentially fatal systemic infection. It is not known whether the typhoid agents have evolved from a common ancestor (by divergent processes) or acquired similar pathogenic traits independently (by convergent processes). Comparison of different typhoid agents with non-typhoidal Salmonella lineages will provide excellent models for studies on how similar pathogens might have evolved. We sequenced a strain of S. paratyphi C, RKS4594, and compared it with previously sequenced Salmonella strains. RKS4594 contains a chromosome of 4,833,080 bp and a plasmid of 55,414 bp. We predicted 4,640 intact coding sequences (4,578 in the chromosome and 62 in the plasmid) and 152 pseudogenes (149 in the chromosome and 3 in the plasmid). RKS4594 shares as many as 4346 of the 4,640 genes with a strain of S. choleraesuis, which is primarily a swine pathogen, but only 4008 genes with another human-adapted typhoid agent, S. typhi. Comparison of 3691 genes shared by all six sequenced Salmonella strains placed S. paratyphi C and S. choleraesuis together at one end, and S. typhi at the opposite end, of the phylogenetic tree, demonstrating separate ancestries of the human-adapted typhoid agents. S. paratyphi C seemed to have suffered enormous selection pressures during its adaptation to man as suggested by the differential nucleotide substitutions and different sets of pseudogenes, between S. paratyphi C and S. choleraesuis.

禽流感和人類的關係

流行性感冒病毒屬於正黏液病毒科 (Orthomyxoviridae family),根據其核蛋白及基質蛋白的抗原差異分為甲、乙或丙型(Type A,B,C)。禽流感病毒(AIV)屬於甲型(Type A),目前分為高病原性禽流感病毒(HPAI)和低病原性禽流感病毒(LPAI),依國際疫會規定只要是H5或H7不論HPAI或LPAI均需通報,然後依一定程序鑑定後採取既定防疫措施。禽流感病毒冇種表面抗原-血液凝集素 (haemagglutinin, H)及神經胺酸酶 (neuraminidase, N),能引起特定亞型及免疫反應,目前有16種H亞型 (H1-16) 及 9種N亞型 (N1-9)。禽流感病毒對鳥類的內皮起源 (腸,肺) 之上皮組織的主要受體種類-α2,3連結的唾液酸接受體結合位置(receptor-binding site)有高親合性,因此主要影響禽鳥,對雞隻和火雞具有高致病力,但因病毒會不定時基因突變,衍生新品種,導致原來僅感染禽類的流感病毒,變得可以影響人類;此外,已研究證實人類氣管、支氣管和小支氣管內有一定數目的纖毛細胞同樣帶有較低密度像禽類之接受體,對於與受感染的活家禽、被其糞便嚴重污染的表面或物件有密切接觸的人,感染的風險極高。1997年於香港爆發H5N1,當時受感染的18人中有6人死亡,在流行病學上與活禽市場的高度接觸有關;2003年末再度發生H5N1亞型高病原性禽流感,接著由東北亞、東南亞並傳至蘇聯和歐洲,由於這些突變的流感病毒對人類是全新的病毒,因此曾和病禽或其病源接觸的人導致嚴重病症,自2003年起累計至2009年8月31日止全球共發生440病例,262人死亡。由於這高致病性禽流感H5N1病毒株有特異性能跳躍不同物種類障礙和在人類引起高致死率嚴重疾病,文獻指出A型流感病毒在不同物種間的傳播以及重組曾見於豬、人、鴨及火雞,引起1957及1968年世界大流行的人類流感病毒均含有類似禽類流感病毒的基因片斷,因此,全球仍關注其是否在人類提升另一全世界流行性感冒大流行的可能性,當人(高度風險群)同時感染禽流感與和流行性感冒兩種病毒,這兩種病毒在其體內可能經互換基因而衍生新變種病毒,如果這新病毒會發生直接從人對人 (代替只從鳥-禽-畜對人)傳染,預期將會遇見全世界流行性感冒大流行開始。今年以來雖然因為在全球多國發生H1N1新型流感的泛流行疫情而轉移焦點,但仍應提高警覺,隨時注意高致病性禽流感H5N1的後續發展。

O2 以Multiplex RT-PCR與PCR/RFLP技術快速檢測及區分H5、H6亞型禽流感病毒及高病原性新城病毒

本研究建立以multiplex RT-PCR配合RFLP技術,以快速鑑定及區分禽流感病毒(AIV)與新城病毒(NDV),並可快速區分AIV之H5或H6,以及N1或N2亞型,另外也可區分NDV之疫苗株與田間強毒株。在AIV與 NDV之區分方面,以AIV之NP基因引子對配合NDV之F基因引子對進行multiplex RT-PCR,可得最佳檢測敏感度與特異性。在H5/H6亞型與N1/N2亞型之區分方面,以H5-310引子對配合H6-730引子對,以及N1-365引子對配合H6-639引子對,進行multiplex RT-PCR,可得最佳檢測敏感度與特異性。將RT-PCR產物以限制脢HaeIII及AluI切割進行RFLP分析,可確認RT-PCR之結果,並可區分美洲型(H5N2)與亞洲型(H5N1)毒株。本研究所建立之multiplex RT-PCR及 PCR/RFLP技術,可於5-6小時內診斷並確認H5N1、H5N2、H6N1及NDV感染,且不須經由定序之步驟(定序需1-2天),可大幅提昇AIV與NDV診斷之準確性與時效性。

Abstract

This study developed a multiplex RT-PCR and RFLP method for rapid identification and differentiation of avian influenza virus (AIV) and Newcastle disease virus (NDV). This method could also differentiate between H5, H6, N1 and N2 subtypes of AIV and differentiate vaccine strain from field strains of NDV. For identification of AIV and NDV, multiplex RT-PCR using primer pairs specific for the NP gene of AIV and for the F gene of NDV gave the best sensitivity and specificity. For differentiating between H5/H6 and N1/N2, multiplex RT-PCR using primer pairs H5-310/H6-730 and N1-365/H639 gave the best sensitivity and specificity. For RFLP analyses, the RT-PCR product amplified from the H5 gene was subjected to HaeIII and AluI digestion and the RFLP patterns obtained could differentiate H5 viruses (H5N2) of American lineage from those of Eurasian lineage (H5N1). In summary, the multiplex RT-PCR and RFLP method developed in this study could be used for rapid identification and differentiation of H5N1, H5N2, H6N1 virues and NDV in 5-6 hrs, and circumvent the need for sequence analyses.

O3 禽流感之基因重組桿狀病毒疫苗之開發

Baculovirus display系統已成為研發次單位疫苗之重要工具。被表現之蛋白質可表現於桿狀病毒之封套,形成所謂 ”重組桿狀病毒”。此重組桿狀病毒可作為次單位疫苗。家禽流行性感冒病毒(avian influenza virus ; AIV)為正黏液病毒科(Orthomyxoviridae)之A型流行性感冒病毒。高病原性家禽流行性感冒(highly pathogenic avian influenza, HPAI)對雞或火雞造成高度傳染或急速死亡。Hemagglutinin (HA)和Neuraminidase (NA)為家禽流行性感冒病毒套膜上的主要免疫原。傳統的作法利用桿狀病毒系統所表現的目標蛋白存在於昆蟲細胞細胞質中。我们成功將H5N2禽流感HA及NA蛋白以鑲嵌的方式表現在桿狀病毒外套膜,以製備基因重組桿狀病毒。我们研究亦發現若以原本屬於桿狀病毒包膜蛋白gp64之胞質區段(cytoplasmic domain,CTD) 來取代HA蛋白本身之CTD可更有效率地將此HA蛋白鑲嵌並表現在桿狀病毒的外套膜上。我們以Baculovirus display表現平台構築了兩種禽流感之重組桿狀病毒(Bac-HA64 & BacSC-NA),經由免疫螢光/共軛焦顯微鏡的觀察,兩種病毒在感染昆蟲細胞後,皆可以有效地表現重組HA及NA蛋白於細胞膜上。而由免疫金電子顯微鏡觀察,兩種重組蛋白也都可以展現在病毒的套膜上。為進一步探討研發之基因重組桿狀病毒是否適合開發成有效的次單位疫苗,因此進行雞隻疫苗效力及攻毒實驗免疫實驗,結果顯示雞隻經Bac-HA64重組桿狀病毒次單位疫苗免疫兩次後,HI抗体可達64-128倍以上並完全保護雞隻攻毒後免於致病。另HI抗体力價達16倍以上者之雞隻使用108.5 ELD50之H5N1病毒攻毒可得完全保護效果。這些結果顯示HA抗原適合開發成有效的次單位疫苗,用以防備禽流感。

O4 乳羊分離之Staphylococcus aureus與Staphylococcus lentus對於

Methicillin與Vancomycin之抗藥性情形

摘 要

葡萄球菌在人及動物為普遍造成食物中毒、傷口及醫源性的感染源,由於金黃色葡萄球菌具強烈致病性與對抗生素具有抗藥性,相關的研究也很多。金黃色葡萄球菌引起之牛羊乳房炎亦為一重要的問題,為瞭解乳羊場分離的葡萄球菌之抗藥性的分佈情形,首先將臨床分離菌株(2006~2008 分離之S. aureus113株及S. lentus 31株)進行16種抗生素敏感性試驗,將所獲得之結果進行分析後,發現普遍地存在著多重抗藥性,其中2006~2007分離之MRSA為0%(0/86),VRSA為64.0%(55/86),而2008年分離之MRSA則為44.4%(12/27), VRSA為29.6%(8/27);另外2008年分離之MRSL為100%(31/31),VRSL為96.8%(30/31);將此菌株再輔以最小抑菌濃度(MIC)試驗,將兩者結果進行分析與比較發現MRSA MIC50 為16, MIC90 為>256,VRSA MIC50 為 16~24,MIC90 為48,另MRSL MIC50和 MIC90 分別為>256,VRSL 的MIC50 和MIC90分別為32及64 均呈現抗藥性質。我們繼續探討乳羊與人源性MRSA的MecA與VacA,B,C,D基因檢測及脈衝電泳分析其同源關係。

O5 Methicillin-resistant Staphylococcus aureus associated necrotizing fasciitis and mastitis

Background: Methicillin-resistant Staphylococcus aureus (MRSA) is one of the most predominant microorganisms responsible for skin and soft tissue infection. MRSA have emerged as a significant pathogen in the microbiology of necrotizing fasciitis (NF), a fulminant soft tissue infection, and postpartum mastitis, a process of subcutaneous cellulitis. Our goals were to (1) determine the correlations of clinical, epidemiological, and microbiological features of MRSA with NF and mastitis, and (2) examine the choice of empiric antibiotic therapy.

Methods: Staphylococcal chromosomal cassette mec (SCCmec) types, pulse-field gel electrophoresis (PFGE) patterns, and virulence factors of 16 MRSA isolated from NF patients and 11 MRSA isolated from mastitis patients in 2008 were determined. Medical records were reviewed, including demographic characteristics, comorbid underlying conditions, and clinical features.

Results: Most MRSA NF infections were usually monomicrobial. Over 90% of MRSA isolates were resistant to erythromycin and clindamycin. Resistance to trimethoprim-sulfamethoxazole (SXT) significantly differed between HA-MRSA isolates and CA-MRSA isolates. Molecular analysis of 16 available MRSA isolates indicated that the SCCmec III and nontypeable type were only identified in HA-MRSA, whereas SCCmec IV and V were present in the 6 CA-MRSA isolates. All isolates possessed γ-hemolysin and only CA-MRSA isolates encoded Panton-Valentine leukocidin (PVL) genes. Virulence factors TSST-1, ETA, and ETB of MRSA were rarely detected among NF patients.

Of the mastitis-associated MRSA isolates, 10/11 (90.9%) was HA-MRSA (i.e., a history of hospitalization, surgery in the past year). However, antimicrobial resistant patterns and PCR results indicate that most isolates may be CA-MRSA. Mastitis caused by MRSA isolates was significantly less resistant to SXT (0), fucidin (0), clindamycin (72.7%), and erythromycin (72.7%) than those of other MRSA clinical syndromes. SCCmec IV and V types accounted for 81.9% of mastitis-associated MRSA. PVL genes were frequently identified. Mastitis had progressed to breast abscess and required surgical drainage in 1 MRSA infection, carrying PVL gene. None of the MRSA strains isolated from patients with mastitis carried the TSST-1 genes.

The SmaI-digested PFGE analysis of the 16 MRSA isolates associated with NF identified 8 PFGE patterns, 11 MRSA isolates associated with mastitis identified 6 PFGE patterns. Pulsotypes A, C, and D accounted for 68.8% of NF isolates and 72.7% of mastitis isolates.

Conclusions: MRSA is a significant emerging cause of NF and mastitis in Taiwan. In endemic areas, treatment of NF and mastitis patients should target MRSA with initial empiric antibiotics, exclusive erythromycin and clindamycin.

O6 犬隻沙氏桿菌分子流行病學之調查

摘要

沙氏桿菌為革蘭氏陰性菌,是人畜共通傳染病,造成人類嚴重的胃腸炎、菌血症及傷寒等疾病與經濟損失。其感染途徑,主要是吃到污染病原的食物,感染後病原亦可從糞便排出;但某些無臨床症狀的感染動物成為潛在性的帶菌者,由糞便中排出沙氏桿菌而污染環境造成動物及人類的感染。

隨著生活型態的改變,圈養寵物的人口逐年攀升,由家庭寵物來源感染的沙氏桿菌症成為新的關注焦點;本調查係針對南部地區隨機採檢犬隻糞便,並參照世界動物衛生組織(International Office of Epizootics;OIE)分離步驟,進行沙氏桿菌的分離鑑定。共採集2,000件糞便檢體進行鑑定,共分離出295隻沙氏桿菌,其中自動物醫院分離出的陽性株64件(21.7%;64/295)、寵物店21件(7.1%;21/295),動物收容中心210(71.2%;210/295)件;分離率以動物收容中心為最高。

進一步以脈衝式膠體電泳(pulsed-field gel electrophoresis, PFGE)基因圖譜與傳統血清型試驗鑑定分析沙氏桿菌分離株,共鑑定出12種血清型,其中以S. Albany(75%)及S. Newport(13.2%)居多;藥物敏感性試驗中顯示,藥物抗藥性比例以tetracycline(86.8%)、ampicillin(78.5%)及sulfamethoxazole- trimethoprim(75.2%)三種藥物最高;從上述結果顯示與人們一起生活的寵物,有可能成為人類沙氏桿菌症的潛在感染來源,藉由以上資料,可提供民眾於飼養寵物的衛教資訊及臨床獸醫師對犬隻施用抗生素之參考依據,並提供未來進行沙氏桿菌感染之風險評估資料之依據。

O7 High mobility group 1 (HMGB1) protein as the therapeutic target for severe sepsis and septic shock

Severe sepsis and septic shock represent the major cause of the overall mortality among infected patients. The pathogenesis of sepsis is mediated in part by bacterial endotoxin like lipopolysaccharide (LPS), which stimulates macrophages/monocytes to sequentially release early (e.g., TNF-α, IL-1β) and late pro-inflammatory cytokines. Recently, it has been found that levels of high mobility group 1 (HMGB1) protein remain high in the majority of patients suffering severe sepsis and septic shock, indicating that the cytokine indeed is a downstream and late mediator of inflammation and thus sepsis. HMGB1 is actually a nuclear factor and simultaneous a secreted protein. It was originally identified as an architectural chromatin-binding factor that bends DNA, promotes protein assembly on specific DNA targets and thus facilitating chromosome construction in the nucleus. After secretion from activated monocytes and macrophages, it binds with high affinity to RAGE (the receptor for advanced glycation end products) and TLR (Toll-like receptors), becoming a potent mediator of inflammation extracellularly. In humans, serum HMGB1 concentrations were elevated in adult patients with severe sepsis and septic shock compared with healthy controls. Furthermore, anti-HMGB1 antibodies were capable of rescuing mice from lethal experimental sepsis, even when the first doses are given 24 h after the onset of sepsis, indicating a wider window for HMGB1-targeted therapeutic strategies. Therefore, agents capable of attenuating released HMGB1 may have potential in the prevention and treatment of inflammatory diseases. As a platform technology for induction of complete human monoclonal antibodies has been established, additional studies and collaboration are required to fully define the relationship of HMGB1 to therapeutics of septic disease.

嚴重的敗血症及敗血性休克是目前感染病患中最主要的死因,敗血症之致病機轉部分肇因於如脂多醣的細菌性內毒素,其會刺激巨噬細胞或單核球狀、使其繼而釋出如TNF-α與IL-1β等早期細胞激素及晚期發炎反應性細胞激素。近年來之研究指出第一高游動性組蛋白質(high mobility group 1; HMGB1)在絕大多數嚴重敗血症及敗血性休克患者體內濃度皆有升高,暗示了HMGB1即為位於下游負責敗血症發生的晚期發炎反應性細胞激素。HMGB1因此事實上不但是一種細胞核因子、也同時是種分泌性蛋白質,HMGB1最早是因其具有染色質結合和扭曲DNA之能力而被鑑別出來,在細胞核中促使蛋白質在特定的DNA上組合的結構蛋白,故與染色體的組裝有關。一旦當它被活化的單核細胞或巨噬細胞釋放,則會與RAGE (the receptor for advanced glycation end products)以及 TLR (Toll-like receptors)等受體以高親合力結合、在細胞外也能因此成為強力的發炎反應媒介物。嚴重的敗血症及敗血性休克病患其血清HMGB1濃度遠超過健康正常人,此外在動物試驗中HMGB1抗體甚至還能於實驗動物發展出敗血症徵候24小時後再施打第一劑來拯救動物、使其不致發生致死性敗血症;這些結果給HMGB1作為治療標的開了一扇窗,因此能夠中和釋出HMGB1的藥物對預防及治療全身性的發炎疾病將極有潛力;由於吾人已建立了完全人類單株抗體之載台技術。因此,進一步的研究與合作對於成功地治療敗血病相當重要。

ABSTRACT Salmonella enterica causes a number of significant poultry diseases and is also a major pathogen in humans. Most poultry infected by Salmonella become carriers; infection may also be fatal, depending on the particular serovar and the age of the bird at infection. Younger birds are more susceptible to infection by Salmonella, so it is critical that hatcheries monitor birds. We used cloacal swabs and hatched eggshell membranes (HEMs) to evaluate Salmonella prevalence in waterfowl hatcheries and young geese. Six serogroups (A, B, C1, C2, D, and E) of Salmonella were detected in the hatcheries. S. Typhimurium was dominant in one goose hatchery and also in the geese from this hatchery that had been transferred to a farm. However, Salmonella serogroup C1 was the major serogroup found in goose and duck hatcheries. Salmonella Potsdam and Montevideo belonged to C1 were the major serotypes in hatcheries and young goslings. Genetic study by plasmid and PFGE analysis showed that isolates from duck hatchery were more diverse than those from goose. In S. Potsdam, isolates with genotype 3, 4 and 5 were isolated from goose hatcheries; in contrast, isolates with genotype 7, 8 and 9 were only identified in duck’s. We suggest that S. Potsdam may evolve into genotypic isolates to infect goose and duck separately.

O9 The change of Salmonella genomic island and R plasmid-mediating survival in THP-1 cells for Salmonella enterica serovar Virchow

Salmonella genomic island (SGI1) is a 43-kb DNA mobile element region found firstly in S. Typhimurium and consist of genes blaPSE, floR, aadA2, sulI, and tetG responsible for resistance to ACSSuT resistance. Complex class 1 integron In104 of most SGI1 types is inserted between res (or tnpR) and ORF44. However, In104-related InSGI1-J is located within ORF S023. Previously, we reported SGI1-J in human S. Virchow isolates from 1994. We investigated the evolution of SGI1-J and genetic variations of 98 S. Virchow human isolates from 2004 to 2006 by PCR amplification and PFGE as well as plasmid analysis. Antimicrobial resistance was associated mostly with the deletions in the InSGI1-J. Genetic adaptation of S. Virchow adaptation occurs in reduction of PFGE types and increase of plasmid complexity. Further, the deletion results in dramatic change in InSGI1-J. Two ampicillin resistance plasmids of two strains confirmed the resistance to bacteriocidal effect of THP-1 cells.

O10 Immunity of Chicken to Salmonella enterica serovar Enteritidis infection

S. Enteritidis is major pathogen to cause outbreak of foodborne salmonellosis through contaminated poultry eggs, meats and their products. The amounts and functions of peritoneal macrophages were investigated. Maximal number of peritoneal macrophages was found at age of 31-33 days. There were no difference in phagocytic ability for peritoneal macrophages collected from 18-42-days-old chicks, However, intracellular bacteria were completely removed for peritoneal macrophages from 29-days-old chicks. To enhance of immunity of chicks younger than 29-day-old, (-1,3-1,6-glucan of Schizophyllum commune and peritoneal macrophages from 22-day-old chicks were used. Oral feeding of 0.1% of (-1,3-1,6-glucan was enough to protect the S. Enteritidis infection in liver and spleen. The protected mechanism of (-1,3-1,6-glucan was directly interaction with peritoneal macrophages to enhance phagocytic and bactericidal effect. Further, we investigated the route of S. Enteritidis infection on eggs. 2-year-old female chickens were challenged with S. Enteritidis by the following methods: cloacal, oral, intraperitoneal (IP) and intravenous (IV) injection. Salmonella contamination was examined in egg membrane, albumen, yolk membrane, and yolk. Salmonella was found on eggs treated with all methods. However, IP injection showed maximal infection in all egg parts. Major infection was found on egg membrane, suggesting that Salmonella infects mainly oviduct. Although highest antibody titer to S. Enteritidis was found in chicks treated by IP method, Salmonella was still found in eggs of IP groups. It implicate that antibody might protect not enough for S. Enteritidis infection through macrophage transmission.

Salmonella enterica serotype Typhimurium produces a variety of fimbrial appendages, among which the type 1 fimbriae is the most common type. In vitro, static broth culture favours S. Typhimurium to produce type 1 fimbriae, while solid agar inhibits its expression. Previous study indicated that a transposon inserted in the stbC gene, which would encode an usher protein for Stb fimbriae of a non-flagellar S. Typhimurium LB5010 strain, conferred it to agglutinate yeast cells on both static broth and solid agar media constitutively, and was mannose-sensitive. Reverse transcriptase polymerase chain reaction (RT-PCR) revealed that the expression of fimA, a major fimbrial subunit gene for type 1 fimbriae, and fimZ, a positive regulator gene of fimA, were increased in the stbC mutant strain more than in the parental strain when grown on LB agar; fimW, a repressor gene of fimA, exhibited lower expression. Nonetheless, fimbrial appendages were not observed on the stbC mutant with electron microscopy; instead, flagella structures were expressed. Flagella formation was correlated with the motile phenotype detected by MSRV agar medium and reaction with the flagellar antiserum. Microarray data and RT-PCR also indicated that the expression of three genes involved in chemotaxis/flagella mechanism, motA, motB, and cheM, was enhanced in the stbC mutant. In addition, the S. Typhimurium stbC mutant was resistant to a variety of antibiotics, consistent with the finding that expression of yhcQ and ramA, two genes involved in multidrug resistance, was enhanced. A complementation test revealed that transforming a recombinant plasmid possessing the coding sequence of stbC gene restored a stbC mutant to exhibit the mannose-sensitive agglutination phenotypes as the parental S. Typhimurium LB5010 strain, however, the flagellar formation and multidrug resistance phenotypes were not affected in the complemented strain. The present study indicates that a transposon inserted in the stbC gene of S. Typhimurium enables it to exhibit mannose-sensitive agglutination, flagellar structures, and resist to a variety of antibiotics. Complementing the stbC strain with the coding sequence of stbC restores the mannose-sensitive agglutination phenotypes as the parental strain, indicating the possibility of interplay of different fimbrial system to coordinate their expression.
