Pleural space หมายถง พ3นท'ระหวางเย'อหมปอดท3ง 2 ช3น คอ visceral pleura and parietal pleuraโดย pleural space ดานขวาและซายก3นดวย mediastinum(1)ภายในประกอบดวย pleural fluid ท'ผลตจาก mesothelial cellsโดยปกตจะมความหนาประมาณ 10 µmเพ'อชวยหลอล'นในการขยบของปอดระหวางการหายใจ(2)

รปท' 1: pleural space

Pleural space disease เปนความผดปกตท'เกดข3นบรเวณ pleural space ประกอบดวย(1,3)

1. Pleural effusionเปนภาวะท'พบไดบอยของpleural diseaseถาจาแนกจากลกษณะของ pleural fluid โดย Light’s criteria จะแบงเปน Transudate effusions และ Exudate effusions ซ' งถามลกษณะดงตอไปน3 1 ขอจะจาแนกในกลม Exudate effusionsคอ อตราสวนของโปรตนใน Pleural fluid ตอโปรตนในเลอดมากกวา 0.5หรออตราสวนของ lactate dehydrogenase (LDH)ใน Pleural fluid ตอLDHในเลอดมากกวา 0.6 หรอ LDH ในPleural fluid มากกวาคาปกตของ LDH ในเลอด 1.67 เทา

1.1 Transudate effusionsมสาเหตจากหวใจลมเหลว, ตบแขง, โรคไต (nephrotic syndrome) ภาวะระดบ albuminในเลอดต'า, Fluid retention/overload, Pulmonary embolism, Lobar collapse และMeigs’ syndrome

1.2 Exudate effusionsมสาเหตจากมะเรง การตดเช3อ โรคหลอดเลอดและเน3อเย'อเก'ยวพน (Collagen-Vascular Disease related),abdominal and Gastrointestinal Disease–related, Chylothorax, uremia, sarcoidosis, หลงการผาตดหลอดเลอดหวใจ ฉายแสง และ อบตเหต

2. Pneumothoraxเปนภาวะท'มลมอยในชองเย'อหมปอด โดยแบงตามสาเหตไดดงน3 2.1 Spontaneous แบงเปน

- Primaryspontaneous pneumothorax(ไมมสาเหต) - Secondary spontaneous pneumothorax ลมร'วในชองปอดท'เกดข3นเองโดยมสาเหตได

จากโรคถงลมโปรงพอง, Bullous disease, Cystic fibrosis,Pneumocystis−related, Congenital cysts, idiopathic pulmonary fibrosis, Pulmonary embolism

- Catamenial - Neonatal

2.2 Traumatic 2.3 Iatrogenic 2.4 Other

3 Hemothoraxเปนภาวะท'มเลอดในชองหมปอด โดยสวนมากมกเกดในผปวยอบตเหตตอบรเวณทรวงอก หรออาจเกดตามหลงการผาตด

4 Pleurisy เปนภาวะการอกเสบของเย'อหมปอด

ในท'น3จะกลาวถงภาวะท'พบบอยและศลยแพทยท'วไปมบทบาทในการรวมรกษา คอ Paraneumonic pleural effusion และ Primaryspontaneous pneumothorax

Parapneumonic pleural effusions

Parapneumonic effusionsมการกลาวถงคร3 งแรกในสมยอยปต3000 ปกอนครสตกาลเปนสาเหตท'พบไดบอยถง 1ใน 3 ของpleural disease มอบตการณเพ'มมากข3นเร'อยๆ โดยพบวาผปวยท'มปอดตดเช3อรอยละ 57 จะมpleural effusion เกดข3น โดย pleural effusion บางสวนสามารถหายไดเอง แตบางสวนกลายเปน empyema thoracisซ' งทาใหมอตราตายรอยละ 1-40 ซ' งข3นกบหลายปจจย เชน โรครวมของผปวย (comorbid disease), immunocompromised patients วธการรกษา และ ความลาชาในการเปล'ยนแผนการรกษา(3–7)

จากการรายงานของ Nick A. Maskellacquiredpleural infection) เช3อท'เปนสาเหตStreptococcus spp. รอยละ 52, Staphylococcus aureusและ anaerobes (รอยละ 20) พบไดนอยกวาสวนผปวยท'ตดเช3อจากในโรงพยาบาลinfection)พบเช3อด3อยามากข3น โดยพบaerobes รอยละ 17 และ Anaerobes

Nick A. Maskell ในป 2006 พบวาผปวยท'ตดเช3อจากบาน เช3อท'เปนสาเหตสวนใหญคอ Gram-positive aerobic organismsStaphylococcus aureusรอยละ 25สวนเช3อ Gram-negative organisms

พบไดนอยกวาสวนผปวยท'ตดเช3อจากในโรงพยาบาล (Hospพบเช3อด3อยามากข3น โดยพบ Staphylococci MRSA รอยละ 25MSSA รอยละ

Anaerobes รอยละ8 ดงแสดงในตารางท' 1(8)

สามารถแบงได 3 ระยะโดยแยกกนจากระยะเวลา (รปท' pleural fluid (ตารางท' 2) คอ

อาการ อาการแสดงและผลตรวจเพ'มเตม ท'สงสย pleural infection คอ ผปวยท'มปอดอกเสบตดเช3อ รวมกบ อตราเตนหวใจ (HR) ความดนโลหต (BP) ไมดข3นใน 2 วน และ ไข อตราการหายใจ (RR) ความเขมขนของออกซเจนในเลอด (SpO2) ไมดข3นใน 3 วน hemoculture found organism หรอ มผลเลอดดงตอไปน3 2 ใน 3 ขอ

- Albumin < 30 g/L - CRP > 100 mg/L - Platelet count > 400 000 /L - Na < 130 mmol/L - Intravenous drug abuse(8–10)

Investigation

หากสงสยวาผปวย pleural infection ใหสง Chest X-ray (CXR) PA view เพ'อด pleural effusion แตหากไมชดเจนอาจสง lateral view หรอทาultrasoundซ' งสามารถเหน pleural fluid ปรมาณนอยๆไดแลว ยงสามารถดloculated pleural effusionไดอกดวยโดยสงเกตจากAnechoicpleural collections(รอยละ19),Hyperechoic without septae(รอยละ14), Hyperechoic with septae (รอยละ67) และ Diaphragm immobilization(11–13)สวน CT scanพจารณาสงในกรณท'สงสยวามพยาธสภาพของอวยวะอ'นรวมดวย เชน หลอดอาหารทะล หรอ เน3องอกหลอดลม เปนตน โดย CT scan ใน empyema มลกษณะดงน3 (11,14,15)

- collectionเปนรปเลนสไมมขอบแยกจากเน3อปอด กดเบยดเน3อปอด - Split pleura’ sign : visceral and parietal pleural enhanmentบงบอกถงการอกเสบของ

pleura พบในผปวยทกราย - Pleural thickness ซ' งพบใน empyema รอยละ 92 แตจะไมพบใน simple

parapneumonic effusion - Thickening of extrapleural fat พบไดรอยละ 76 - Mediastinal lymph nodes ขนาด<2 cm

MRI ไมไดมประโยชนเหนอกวา CT แตอาจสงตรวจ MRI ในกรณท'ผปวยแพสารทบรงส ผปวยอายนอย หรอ ผปวยต3งครรภ(9)

Pleural fluid examinationพจารณาสงในกรณตอไปน3

- Pleuraleffusionหนา > 10 mm รวมกบผปวยมsepsis และมประวตปอดอกเสบตดเช3อ มการบาดเจบตอทรวงอก (Chest trauma)หรอเคยผาตด

- Pleural effusion หนา< 10 mm หลงตดตามมความหนาเพ'มข3น หรอ มภาวะsepsis

การสงตรวจแนะนาใหใช ultrasound guide aspiration นาpleural fluid ท'ไดสง Gram stain, Culture, WBC, LDH, Protein, pH, Glucose ทกราย สวน Cytology และ AFB จะพจารณาสงในกรณท'สงสย โดย Pleural fluid profile สามารถแยกระยะของ pleural infection ไดดงแสดงในตารางท' 2

Treatment

เร'มมการกลาวถงการรกษาในศตวรรษท' 19 ดวยการเปดชองอก แตพบวามอตราการตายสงถงรอยละ 70 ตอมาพฒนาการรกษาดวยการใสสายระบาย รวมกบการใหยาปฎชวนะ กระท3งปจจบนมการใสยาเขาไปในชองหมปอดและการผาตดแบบสองกลองทาใหอตราตายลดลงเหลอรอยละ4.3 (17–20)

Richard W. Light นาเสนอแนวทางการรกษา Pleural infection ตามลกษณะของ Pleural fluid ในป 1995 ดงตารางท' 3(21) ตารางท' 3 :แนวทางการรกษา pleural infection โดยแบงตามลกษณะ Pleural fluid

Antibiotics

ผปวยทกคนควรไดรบ Antibiotics โดยชวงแรกใหทางหลอดเลอดดา เม'ออาการดข3นใหเปล'ยนเปนยากนระยะเวลาอยางนอย 3 สปดาหและมขอมลบงช3 ถงประโยชนในการใหยาเขาไปใน Pleural space โดยใหยาฆาเช3อตามการตอบสนองตอยาของเช3อท'ตรวจพบและใหครอบคมเช3อanaerobeหากเปนpneumococcal infection โดยยาท'ผานเขา Pleural space ไดด เชน B-lactamase inhibitors, metronidazole, cephalosporins หรอหากตรวจไมพบเช3อ แนะนาใหตามเช3อกอโรคจาก Community acquired และ anaerobe organisms แตหากเปนการตดเช3อในโรงพยาบาล ควรใหยาฆาเช3อท'ครอบคมเช3อ MRSA และ Anaerobe

ไมแนะนาใหยาฆาเช3อกลม Macrolide ถาไมไดสงสย atypical infection

Chest tube drainage

ขอบงช3 ในการใสสายระบาย(22)

1. Frank pus or turbid/cloudy pleural effusion 2. Non purulent pleural fluid with Gram stain or culture found organisms แตอาจยกเวนในกรณท'เช3อเปน

S. pneumonia 3. Suspected pleural infection with pleural fluid pH<7.2 4. After Treat with antibiotic alone but clinical not improved or worst 5. Loculated pleural effusion 6. Large non purulent pleural effusion

ไมมความแตกตางของ Outcome ในเร'องขนาดของ chest tube drainage ใสขนาด>28 Fr แตถาขนาด10-14 Fr สายมขนาดเลกควรflush สายเพ'อปองกนอดตนโดยflush NSS 20-30 mL q 6 hrทาง three-way และ suction -20 cmH2O(23)และพจารณานาสายออกเม'ออาการของผปวยดข3น ปรมาณ pleural effusion < 100 mL/day

Intrapleural fibrinolysis

Intrapleural fibrinolysis มประโยชนในการทาลาย loculationและชวยลดความหนดของ pleural effusionในชวง empyema phase โดยใชรวมกบ chest tube drainageหรอใชตามหลง fail chest tube drainage เน'องจากม loculationปจจบนมหลายการศกษาท3งท'เปนRandomised trials และ Meta-analysis โดยใช Streptokinase(STK)

และ Urokinase (UK) ซ' งปรมาณยา และวธแตกตางกน เชน Steven A Sahn ใช STK 250,000 IU in 50 to 100 mL of saline daily หรอ UK 50,000- 100,000 IU ประสบความสาเรจในการรกษา รอยละ 67-100 ซ' งยาท3ง2 ชนดน3 ใหผลการรกษาไมแตกตางกน(24)หรอการศกษาของ Davies RJ ใส STK 250,000 lU/day ในชองหมปอด เทยบกบ saline flushes พบวาผปวยท'ใส STK ชวยระบาย pleural fluid ไดปรมาณมากกวา และ ตดตามภาพเอกซเรยพบวาpleural fluid ลดลง (25) และ สวนอกการศกษาพบวาชวยลดปรมาณผปวยท'ตองผาตด รวมถงลดอตราตายดวย(26) หรอเม'อเปรยบเทยบ UK กบ saline UK สามารถระบาย pleural fluid ไดดกวา saline เชนกน(27)อยางไรกตามม meta-analysis ท'ตพมพในป 2006 ไมแนะนาใหใส Intrapleural fibrinolysis ในผปวย Empyema thoracisท'ใสChest tube ทกราย (28)

ปรมาณยาท' British Thoracic Society pleural disease guideline 2010(BTS)แนะนา คอ STK 250,000 IU หรอ 250,000 IU ทก 12 ช'วโมง หรอ urokinase 100,000 IU วนละคร3 ง แตละคร3 งใสใน pleural space นาน 2-4 ช'วโมง(9)

ภาวะแทรกซอนท'พบได เชน ไข, เจบหนาอก (pleuritic chest pain)และรอยแดงบรเวณหนาอก(24)

Surgery

BTS แนะนาใหผาตดเม'อ Persistent sepsis (5-7 days) and a residual pleural collection after chest tube drainage and proper antibiotic โดยการผาตดในผปวย Empyema thoracis fibrinopurulent stage และ Organising stage ม 2 แบบ คอVideo-assisted thoracoscopic surgery (VATS) และThoracotomy with decortication

Video-assisted thoracoscopic surgery (VATS)เปนการผาตดแบบสองกลอง ท'มประสทธภาพใกลเคยงกบ open decorticationในการรกษา Empyema thoracisและเหนอกวา chest tube drainage ใน fibrinopurulent stageและ organizing state มหลายการศกษาท'เปรยบเทยบประสทธภาพระหวาง VATS กบ การใสสายระบาย หรอ การผาตดแบบเปด เชน การศกษาของ Wait MAและคณะ ศกษาในป 1997 เปรยบเทยบการทา Chest tube drainage plus streptokinase เทยบกบ VATS ในผปวย empyema thoracisfibrinopurulent phase พบวา VATS ใหผลสาเรจในการรกษารอยละ 91 ซ' งมากกวาการทา Chest tube drainage plus streptokinase(สาเรจรอยละ 44) อยางมนยสาคญทางสถตอกท3งระยะเวลานอนโรงพยาบาลนอยกวา แตคารกษาพยาบาลไมตางกน(19)ตอมาในป 2003 Anstadt MP และคณะ เปรยบเทยบการทา Chest tube drainage กบการผาตด thoracotomy, decortication, and/or open window thoracostomy พบวา การผาตด อตราตายไมตางกน แตระยะเวลานอนโรงพยาบาลนอยกวากลม Chest tube drainage อยางมนยสาคญทางสถต จงแนะนาวาหากใส Chest tube drainage แลวยงม sepsis ตดตามอาการและ CXR ไมดข3นใหผาตดและ Lackner RP รายงานในป 2000 ในผปวย empyema

thoracisfibrinopurulent phase รกษาดวยการทา VATS พบวารกษาสาเรจ รอยละ 76 แตบางสวนยงตองการทา open decortication การศกษาอ'นท'ศกษาในผปวย empyema thoracis stage 2 และ 3 พบวา การรกษาดวย VATS สาเรจรอยละ 92-100 สวน การรกษาดวย open surgery สาเรจ รอยละ 100(29)

Open surgery ประกอบดวยการทา Thoracotomy เพ'อเปดเขาสบรเวณท'มโพรงหนอง การทา decortication เปนการเอาfibrin ท'หมรอบปอดออก เพ'อชวยใหปอดขยายตวไดดข3น และ อาจทา Thoracoplastyเปนการจดเรยงกระดกและกลามเน3อของ chest wall ใหมในกรณท'การอกเสบลกลามไป แตปจจบนทานอย

Colanceski R เสนอขอบงช3 ในการทา VATS กบ open surgery ดงน3 (30)

Indications for VATS decortications

- Nonloculated pleural effusion - Noncapsulated pleural effusion - low pleural LDH grade - Thin pleural peel and pleural empyema in the fibrinopurulent stage

Contraindications for VATS decortications

- obliterated pleural space

Indications for open decortication

- Multilocular - Encapsulated - Thick pleural peel - Interlobar or diaphragmatic adhesions pleural empyema in the fibrinopurulent stage.

Primaryspontaneous pneumothorax

Introduction

Primary spontaneous pneumothorax (PSP)เปนภาวะท'มลมในชองเย'อหมปอด หรอหมายถงมลมอย

ระหวางปอดและผนงทรวงอก ในคนท'แขงแรง ไมไดมโรคท'เน3อปอด(31) อบตการณการเกด PSP 7.4/100,000/

ปในผชาย และ 1.2/100,000/ป ในผหญง อตราสวนชายตอหญง 6.2:1(32)มปจจยเส'ยงคอ ผชาย ผอม สง และ

สบบหร' (33,34) โดยเช'อวาเกดจากการแตกของ subpleural bleb(31)และมโอกาสเกดซ3 าเฉล'ยรอยละ 30 (รอยละ

16-52) ในชวง 6 เดอนถง 2 ปแรก ปจจยเส'ยงท'ทาใหเกดซ3 าคอ การสบบหร' และอายท'มากกวา 60 ป (33,35)

Sign and symptom

อาการมกเกดในชวงท'ไมไดออกแรง อาการท'พบไดเชน เจบแนนหนาอกดานท'มลมร'วแบบแปลบๆ

ทนททนใดจากน3นจะปวดคงท'และหายไปหลง 24 ช'วโมงแมวายงมลมคางอยในเย'อหมปอด เหน'อยเฉยบพลน

ซ' งใน PSP มอาการเพยงเลกนอยหรออาจไมมอาการ โดยอาการท'เกดข3นนอยกวา Secondary spontaneous

pneumothorax อาจตรวจรางกายไมพบความผดปกตหากลมร'วนอยกวารอยละ 15 ของคร' งทรวงอก

(hemithorax) อาการท'ตรวจพบสวนมากมหวใจเตนเรว (Tachycardia) (31,33)นอกจากน3นสามารถตรวจพบ

ทรวงอกเคล'อนไหวลดลง เคาะโปรง ฟงเสยงหายใจไดเบาลง และรสกเสยงหายใจไกล (diminished fremitus)

อาการท'ไมรนแรงไดแก หายใจ<24 คร3 ง/นาท หวใจเตน 60-120 คร3 ง/นาท ความดนปกต ออกซเจนในเลอด >

90% และสามารถพดไดจบประโยคโดยไมตองหยดหายใจ และอาการท'บงช3 ถง Tension pneumothorax คอ

หายใจขด หายใจเรว หวใจเตนเรวมากกวา 135 คร3 งตอนาท ความดนตก เหง'อออก cyanosis และหวใจหยดเตน

(31,33,36)

Investigation

Arterial blood gas (ABG) พจารณาสงในรายท' SpO2< 92% room air ซ' งจะพบ hypoxemia (PaO2<10.9

kPa) increase alveolar–arterial oxygen gradient and acute respiratory alkalosis (31,33)ซ' งยนยนการวนจฉยโดย

ดจากภาพเอกเรย โดยท'วไปจะสง Standard PAChest X-ray (CXR) สามาถทาไดงายและเรวมความคลาดเคล'อน

ในการประเมนลมร'ว ไมสามารถด bullous lung disease ได ขนาดของ pneumothorax ประเมนไดหลายรปแบบ

เชน BTS guideline มกไดคานอยกวาความเปนจรง โดยวดจากขอบปอดถงผนงทรวงอกดานในท'ระดบข3วปอด

แบงเปน small size คอ <2 cm (pneumothorax< 50% by volume) และ large size ≥2cm สวน American

guideline วดระหางจากข3วปอดกบผนงทรวงอกดานบน โดย large size ≥3cm ซ' งถาเปน Apical pneumothorax

จะวดปรมาณลมไดมากกวาความเปนจรง(37,38)ในกรณท'ลมร'วมปรมาณนอยไมเหนจาก Standard PA CXR

สามารถสง CXR ทา lateral หรอ Ultrasound ได สวน CT scan เปนgold standard ในการวนจฉย pneumothorax

สามารถเหนลมร'วปรมาณนอยๆได ประเมนขนาดของลมร'วไดแมนยาข3น และดพยาธสภาพเน3อปอดไดดวย

Treatment

BTS guidelines2010รปท' 8ในผปวยท'มลมร'วปรมาณเลกนอย (Small PSP) และไมมอาการ แนะนาใหกลบบาน

นดตดตามอาการ และ CXR 2-4 สปดาหแบบผปวยนอกกบ respiratory physicians กระท'งหาย และแนะนาอาจม

ลมร'วมากข3น หรอกลบเปนซ3 าได ดงน3นหากมอาการหายใจขดใหรบมาหองฉกเฉน หากสบบหร'ใหหยดสบบหร'

เน'องจากเปนปจจยเส'ยงใหเกดการเปนซ3 า เม'ออาการหายดสามารถกลบไปทางานและกจกรรมตามปกตได งด

เลนกฬาปะทะกระท'งfull resolution งดเดนทางทางอากาศ ถงแมวาไมมขอมลวาการเดนทางทางอากาศจะทาให

เกดลมร'วซ3 า และไมมระยะเวลากาหนดวาเม'อไรถงเดนทางทางอากาศได ใน BTS guidelines แนะนะใหเดนทาง

ทางอากาศไดหลง full resolution 1 ป และงดดาน3าตลอดชวตในกรณท'ไมไดทา surgical pleurodesis

- Small PSP และ หายใจขดไมมาก (non significant breathlessness) เน'องจากผปวย PSP รอยละ 80 มลม

ร'วนอยกวารอยละ 15 โดยพบวาไมมการกลบเปนซ3 า และ ไมมpersistent air leak จงแนะนาใหสงเกต

อาการในโรงพยาบาล ให high flow oxygen (39)ตดตาม CXR กระท'งปอดขยายเตมท'จงใหกลบบาน

และตดตามกบ respiratory physicians กระท'ง full resolution

- Large PSP แตไมมอาการ แนะนาใหสงเกตอาการและ conservative treamntในโรงพยาบาล

- Large PSP และมอาการหายใจขด พจารณา needle aspiration (NA) หรอ Chest tube drainage โดย NA

ใชเขม 14-16 G เจาะดดผลการรกษาไมตางจาก Chest tube (success rate 69% VS 50.5%)(40)แต

ระยะเวลานอนโรงพยาบาลส3นกวา แตหากทายาก หรอคร3 งแรกไมสาเรจยงมลมร'ว แนะนาใหใส Chest

tube

- มการศกษาของ Hsuan-Chang Kuo ป2010 เทยบการใสสายระบายขนาดเลก 8-12 Fr เทยบกบสายระบายขนาดใหญ พบวาระยะเวลานอนโรงพยาบาลไมตางกน แตการใสสายขนาดเลกใสระยะเวลาส3น มการกลบเปนซ3 าต'ากวา สายขนาดเลก รอยละ 20 เทยบกบสายขนาดใหญกลบเปนซ3 ารอยละ 56

- ถายงมลมร'ว (Persistent air leak) หรอปอดยงขยายไมด (non expanding lung) โดยตาแหนงสายด สาย

ไมหกพบ ไมมการกดทบหรอclampสายแนะนาใหสงตอไปยงrespiratory physician ใน 24 ช'วโมง และ

หากยงม persistantairleak 3-5 วนใหสงตอไปยงศลยแพทยทรวงอก เพ'อผาตดโดยขอบงช3 ในการผาตด

คอ

1. Second ipsilateral pneumothorax 2. First contralateral pneumothorax 3. Synchronous bilateral spontaneous pneumothorax 4. Persistent air leak (despite 5–7 days of chest tube drainage) or failure of lung re-expansion 5. Spontaneous haemothorax 6. Professions at risk (eg, pilots, divers)

7. Pregnancy

Medical chemical pleurodesisสามรถใช Tetracycline 1500 mg รวมกบ 1% lidocaine 250 mg (25 mL) เพ'อลดอาการปวด หรอ Monocyclineหรอ Doxycycline หรอ Talc มการศกษาการใช Tetracycline ใสผาน Chest tube พบวาเกดการกลบเปนซ3 ารอยละ 25 เม'อเทยบกบกลมควบคม รอยละ 41(41)ซ' งผลการศกษาไมตางจากการศกษาของ Yubiao GUO(34)(34)และมPersistent airleakรอยละ 10 (42)สวนการใส Talc ในชองหมปอดเกดลมร'วซ3 า รอยละ 8 เทยบกบการใส chest tube อยางเดยว รอยละ 36 แตระยะเวลาการใส chest tube และระยะเวลานอนโรงพยาบาลไมตางกน(43)อยางไรกตาม Chemical pleurodesisเกดการกลบเปนซ3 าสงกวาเม'อเทยบกบ Video-assisted thoracoscopic surgery (VATS) with pleurectomy and pleural abrasion (รอยละ5)หรอ Open surgerywith pleurectomy(รอยละ 1)จงใชในกรณท'ผปวยไมสามารถผาตดได

- Tension pneumothorax เปนภาวะฉกเฉนท'ตองรบแกไข โดยทาemergency needle decompression ดวย

เขม 14 gauge ยาว 4.5 cm ท' Intercostal space ท'2 midclavicular line ถาไมสาเรจใหเล'อนมาท'

Intercostal space ท'4 หรอ 5 รวมกบ high concentratin oxygen

- Persistent air leak: เกดไดรอยละ 56 ในผปวยท'รกษาดวยการใสสายระบาย (44)SSP เกดไดบอยกวาในผปวย PSP การศกษาของ C.B.E. Cheeในป 1998 พบวาการใสสายระบายเพ'อรกษา PSP มลมร'วรอยละ 25 ท'7 วน แต 15 วนไมมลมร'วในผปวยทกคน (45)อาจพจารณาsuction ดวย high-volume low-pressure (-10 ถง -20 cmH2O) air flow 15-20 L/min ในผปวยบางรายท'ยงมลมร'วหลงใสสายระบายมากกวา 48 ช'วโมงไมควรตอhigh pressure suction เพราะจะทาใหเกด hypoxemia ไดการsuctionสามารถทาใหลมหยดร'วประมาณ 8 วน

- นอกจากน3 มรายงานในสตวทดลองเร'องการใช chemical pleudesisดวย tetracycline มประสทธภาพใน

การทาใหปอดขยายตวและชวยหยดลมร'วได(46) การรกษาดวยการผาตดเพ'อรกษาpersistent air leak และปองกนการเกดลมร'วซ3 าโดยการทาลาย visible bullae หรอ bleb และเช'อมเย'อหมปอดท3ง 2 ช3นใหตดกน ซ' งพจารณาผาตดเม'อยงมลมร'วอย 4-7 วน (47)โดยการรกษาม 2 แบบ คอ Open surgerywith pleurectomyและ VATS with pleurectomy and pleural abrasion
