18/02/2000 (13 ans 3 mois)Diagnostic :Normodivergent, Classe II,1 subdivision droite, Classe II squelet-tique, déficit antérieur : 15 mm, 16 et 26 en occlusion inversée.Traitement :– Préparation de la denture : expansion 16/26, alignement, nivel-lement, redressement et mésialisation de 46 puis de 47, recul descanines ;– Correction de la denture : fermeture des espaces ;– Finition de la denture (« artistic bends »).

Observations cliniques

Amélioration du déficit antérieur, résolution de la Classe II. Lacorrection de la denture est satisfaisante et les objectifs prévusont été atteints. La demande d’extraction des dents de sagesse aété refusée. Les radiographies de fin de contention ont été per-dues et la patiente a refait les radiographies 6 mois après. Cesincidents expliquent la différence d’éruption des dents de sagesse(moulages de fin de contention en relation avec les radiographiesde fin de contention).

18/02/2000 (13y 3m)Diagnosis:Normodivergent, Classe II,1 subdivision right, skeletal Class II,15mm anterior deficit, crossbite of 16 and 26.Treatment:– Preparation of dentition: expansion 16/26, alignment, levelling,uprighting and mesialization of 46 then 47, retraction of canines.

– Correction of the dentition: space closure.– Finishing (“artistic bends”).

25/10/2002 (15y 11m).Retention:Maxillary Hawley plate to be worn at night and bonded lingual wirefrom 43 to 33.

End of retention documents

8/9/2003 ( 16y 9m).

Clinical observations

Improvement of anterior deficit, correction of Class II. Correctionof the dentition was satisfactory and the planned objectives wereachieved. Proposed extraction of wisdom teeth was declined. Endof retention radiographs were lost and the patient had more taken6 months later. These events explain the difference in the eruptionof the wisdom teeth (end of retention casts versus end of retentionradiographs).

DDM et succion du pouce jusqu’à 10 ans.

Vertical dimension:– facial type: normodivergent;– nose-lips -chin relationship: harmonious.Horizontal dimension:– position of the mid third relative to the cranial base: normal;– position of the lower third relative to the mid third: normal.

Skeletal diagnosis

Vertical dimension: normodivergent.Horizontal dimension: Class I.

Dental diagnosis

Vertical dimension: normal.Horizontal dimension: Class II,1 bi-maxillary protrusion.Transverse dimension: bilateral crossbite.

Etiological diagnosis

Jaw-tooth discrepancy and thumb sucking until 10 years of age.

Reprise des éléments diagnostiques

Normodivergent, Classe II,1 subdivision droite, Classe II sque-lettique, biproalvéolie ; déficit antérieur de 15 mm ; 16 et 26 enocclusion inversée.

Choix d’extractions éventuelles, explications

14-24-34 ont été extraites pour corriger le déficit antérieur, 45 l’aété pour corriger le déficit antérieur et participer à la réductionde la Classe II droite.Le devenir des dents de sagesse sera évalué en fin de traite-ment.

Stratégie/Objectifs de traitement

Correction de la Classe II,1 subdivision droite et de la bipro-alvéolie.

Type d’appareillage

Appareillage fixe multi-attaches Edgewise .022 x .028 sans angula-tion ni torque, force extraorale (High Pull Head Gear) et arc palatin.

Plan de traitement

– Extractions des prémolaires.– Préparation de la denture : alignement, nivellement, redresse-ment et mésialisation successives des 46 et 47 ; recul des cani-nes ; expansion transversale des 16-26.– Correction de la denture : fermeture d’espaces.– Finition de la denture (« artistic bends »).

Durée de traitement

27 mois.

Treatment

Recap of diagnosis

Normodivergent, Class II,1 subdivision right, skeletal class II, bi-maxillary protrusion; 15mm anterior deficit , crossbite of 16 and26.

Choice of possible extractions, explanations

14-24-34 were extracted to correct the anterior deficit; 45 wasextracted to correct the anterior deficit and help reduce the rightside Class II. The situation of the wisdom teeth will be evaluated at end of treat-ment.

Strategy/Treatment aims

Correction of the Class II,1 subdivision right and of the bi-maxil-lary protrusion.

Type of appliance

Fixed multiband Edgewise appliance .022x.028 with no angula-tion or torque, High Pull Head Gear and palatal bar.

Treatment plan

– Extraction of premolars.– Preparation of dentition: alignment, levelling, consecutiveuprighting and mesialization of 46 and 47; retraction of canines;transverse expansion of 16-26. – Correction of dentition: space closure.– Finishing (“artistic bends”).

Duration of treatment

27 months.

Night-time maxillary Hawley plate and bonded lingual wire from33 to 43.
