The initial step is to search for the proper patient. You will know if you are selecting the correct patient by the blue star. The star confirms that a patient belongs to your practice.

Next, on each patient you MUST assign a provider. This will ensure the proper physician gets credited for the work being done. Scroll to the Providers section on the main page. Once there, select Add New.

While in the screen, select the proper clinician from the list on the left and the appropriate role on the right.

To begin data entry on your patient’s chart, start in the Online VP Entry Screen. Here is where you will find all of the measures. Note: you may have to scroll up and down the page to find these different measures.

For patients with Diabetes, you will need to add the condition. Under Online VP Entry, select the Conditions tab and Add Diabetes.

The final Diabetic Measure is for Aspirin Usage. This will need to be added to the patient’s medications list by selecting the Medications tab within the Online VP Entry screen. Here, simply select Add to add the medication.

After addition of medication, you need to notate this within the measures. First, go to the Single Entry screen. Here you will need to notate the provider, then select Aspirin/Antiplatelets from the Measure and fill out accordingly below.

For patients with Hypertension (HTN), you will need to add the condition. Under Online VP Entry, select the Conditions tab and Add Hypertension (HTN).

For your hypertensive patients, be sure to document their Blood Pressures. Go to the Online VP Entry and enter the appropriate data.

For patients with CAD, you will need to add the condition. Under Online VP Entry, select the Conditions tab and Add CAD.

First, you need to notate Aspirin/Antiplatelete therapy within the measures. First, go to the Single Entry screen. Here you will need to notate the physician, then select Aspirin/Antiplatelets from the Measures list and document accordingly below.

Also within the Single Entry screen, you will need to document ACE/ARB therapy. In the same manner as above, select ACE/ARB from the Measures list and document accordingly.

For patients with IVD, you will need to add the condition. Under Online VP Entry, select the Conditions tab and Add IVD.

The first IVD measure to be documented is LDL Control. Under the Online VP Entry, enter the appropriate data next to the LDL measure.

Lastly, document your patient’s Aspirin/Antiplatelet therapy. Within Single Entry, select the Aspirin/Antiplatelets measure from the Measures list. Fill out the data accordingly.Remember to save your

work!

For patients with Heart Failure, you will need to add the condition. Under Online VP Entry, select the Conditions tab and Add Heart Failure.

For your patients with Heart Failure, you need to document Beta-Blocker Therapy. This is done by going to the Single Entry screen, selecting Beta Blockers from the Measures list, and documenting accordingly.
