Summary Bite.Cranial nerves C5, C6, C7, C8, and thoracic nerve T1 are, for the
most part, trunks forming the brachial plexus (a very large nerve bundle) that serves the entire arm and some of the muscles of the posterior triangle. The cervical plexus is formed by cranial nerves of C1, C2, C3, and C4 and is also present in the posterior cervical triangle.
Brachial PlexusThe ventral primary rami of some spinal nerves appear not to retain
their metameric (segmental) identities. Instead, they unite shortly after their origins to form bundles of nerve fibers that become intermixed to form plexuses for redistribution. In this form, the resultant fibers can reach their intended fields, whose original locations became altered.The brachial plexus is one such intermixing of ventral primary rami of spinal nerves C5 through T1 (and occasionally T2). Although the plexus is primarily responsible for innervation of the upper limb and associated structures, it also supplies some muscles of the neck (Fig. 7-8). A thorough discussion of this important plexus is found in any general textbook of anatomy.Cervical Plexus
Ventral primary rami of the first four cervical spinal nerves participate in the formation of the cervical plexus (Table 7-5 and Fig. 7-9). The ventral primary rami of all but the first cervical spinal nerve bifurcate into ascending and descending branches. These branches then join each other and become united, forming simple loops that lie deep to the prevertebral fascia. Branches arising from the cervical plexus are arranged in a superficial and deep division.The superficial division is completely sensory and consists of ascending and descending branches. The lesser occipital (C2), great auricular (C2 and C3), and transverse cervical (C2 and C3) comprise the ascending branch, whereas the supraclaviculars (C3 and C4) comprise the descending branches.The deep division of the cervical plexus is mostly motor, although it does contain sensory components. This division supplies motor innervation to the muscle masses deep to the floor of the posterior triangle, as well as proprioceptive fibers to the sternocleidomastoid and trapezius muscles. Furthermore, the deep division supplies the anterior vertebral muscles. A summary of the specific innervations is in Table 7-5. Two additional
important components of this division must be given special consideration: the ansa cervicalis and the phrenic nerve.Ansa Cervicalis
The ansa cervicalis (Figs. 7-9 and 7-13) and its two roots are derived from C1 through C3. The superior root, sometimes incorrectly referred to as the descending hypoglossal, receives fibers from C1 and C2 and the inferior root receives fibers from C2 and C3. Although the superior root of the ansa cervicalis travels for a short distance with the hypoglossal nerve (cranial nerve XII), it does not become a functional part of that nerve.
The superior root joins and travels with the hypoglossal nerve opposite the atlas and descends from that nerve as it passes superficial to the external carotid artery. As the superior root leaves the hypoglossal nerve, two filaments (both derived from C1) continue with the hypoglossal nerve for a few millimeters, then branch to supply motor innervation to the thyrohyoid and geniohyoid muscles. The superior root continues to travel superficial to, embedded within, or just inside the carotid sheath.
At the level just below the middle of the neck, the superior root turns posteriorly to form the ansa (loop) in joining with the inferior root, which has descended in the neck alongside the carotid sheath. Four branches arise from the ansa cervicalis to supply motor innervation to the sternohyoid, sternothyroid, and both bellies of the omohyoid muscles.Phrenic and Accessory Phrenic Nerves
The phrenic nerve (C3, C4, and C5), the only motor nerve of the diaphragm, also carries sensory fibers (Figs. 7-8, 7-9, and 7-13). The phrenic nerve passes deep to the prevertebral fascia as it lies on the anterior surface of the anterior scalene muscle.
On its way to distribute to the diaphragm, the phrenic nerve passes between the subclavian artery and vein and then enters the thorax, where it lies anterior to the root of the lung, contacting the fibrous pericardium. The sensory fibers carried by the phrenic nerve serve the mediastinal pleura and the pericardium of the heart.The accessory phrenic nerve (C5) is occasionally present. It descends into the thorax lateral to the phrenic nerve and posterior to the subclavian vein to join the phrenic nerve below the first rib. It also supplies motor fibers to the diaphragm.
