****Treatment is viewed in 3 phases:The acute phase: relief of symptomsThe continuation phase: to allow more complete resolution and to prevent relapseThe long-term maintenance phase: dependent on prior episodes, risk factors, etc.
*Once depressive disorders are diagnosed, the initial objectives of treatment, in order of priority, are to:Reduce and ultimately remove all signs and symptoms of the depressive disorder.Restore occupational and psychosocial roles/functions to the asymptomatic state.Minimize the risk of relapse and recurrence.
*The efficacy of fluoxetine in relapse prevention has been encouraging. Reimherr et al conducted a study in which depressed patients were treated with open-label fluoxetine for 12 or 14 weeks. Responders (395 patients) were randomized to double-blind continuation treatment with fluoxetine or placebo for up to 50 additional weeks. After 24 total weeks of treatment, only 26% of fluoxetine-treated patients and 49% of placebo-treated patients had relapsed.
