VORAPAXAR - ANTAGONISTA RECEPTOR TROMBINA
• Blocatge del PAR-1 anul·la la resposta de les plaquetes a la trombina i és una diana terapèutica atractiva de la inhibició de l’activació plaquetaria i per tant de l’agregació
• Vorapaxar és un derivat de la himbacina i antagonista oral del PAR-1 (bloca l’acció de la trombina en el PAR-1)
• Els antagonistes del PAR-1, com el Vorapaxar no actuen sobre la trombina i per tant no interfereixen amb l’acció de la trombina en la cascada de la coagulació.
• Els antagonistes del PAR-1 poden tenir un perfil més adequat d’eficàcia/seguretat

• Ticagrelor va reduir l’ús de recursos (dies estada, proves complementàries, intervencions, tractament hemorràgies), especialment durant el primer mes, i va ser globalment més econòmic (estalvi entre 678$ [Suècia] i 977$ [US])
• Aquesta anàlisi no va tenir en compte el preu dels medicaments

CONCLUSIONS
• S’estan estudiants nous fàrmacs antiplaquetars que bloquejen elsreceptors del ADP o de la trombina
• El prasugrel ha demostrat ser més eficaç que les dosis habituals de clopidogrel en malalts tractats amb ICP, però amb increment de sangrat als malalts > 75 anys, amb baix pes o AVC previ
• El ticagrelor, un fàrmac directe, es més eficaç que les dosis habituals de clopidogrel, independent de l’estrategia terapèutica. No augmenta l’hemorragia del CABG
• L’ús d’aquests fàrmacs a la pràctica diària requereix una medicina personalitzada, avaluant el risc de sangrat i de trombosi en el contextde cada malalt.
