must be included with this request form. The Business Reason should include descriptions of the campus

applications used when accessing HR private data, which private data elements are needed, how the data will be

secured, and who will be able to access the private data.

To submit this form, follow these instructions: 1. Log into the Minnesota State IT Service Desk portal: https:\\servicedesk.minnstate.edu

2. Select “Click to login” in the upper right corner of the screen

• Enter your “StarID/password”

• Press Enter or click Log in.

3. Under Service Catalog select “For Staff”

• Under Accounts & Passwords• Select RADS/Data Management (EPM11/REPL)• Select “Operational Data Security Form (Create/Change/Delete)”

• Click the “Paperclip” icon to attach the scanned/completed and signed pdf form to the ticket.

• In the field "Short Description" type "Operational Data Security," complete remaining fields and click “Submit.”
