Ⅰ 統計の範囲

この統計は、「Ⅱ 用語」で定義するアイソトープ、放射性医薬品及びこれらの使用で発生

した RI 廃棄物を対象としている。供給量等は公益社団法人日本アイソトープ協会が供給・

集荷した数量である。

Ⅱ 用語

1.アイソトープ:

放射性同位元素等の規制に関する法律(昭和32年6月10日法律第167号)(以下、

「放射性同位元素等規制法」と表す)に規定された放射性同位元素

1)非密封アイソトープ:密封されていない放射性同位元素

2)密封アイソトープ:密封された放射性同位元素

2.医療機器:

1.2)の密封アイソトープのうち、医薬品、医療機器等の品質、有効性及び安全性の確

保等に関する法律(昭和35年8月10日法律第145号)で規定されている医療機器

3.放射性医薬品:

放射性医薬品の製造及び取扱規則(昭和36年2月1日厚生省令第4号)に規定された放

射性医薬品

1)in vivo :in vivo 放射性医薬品(体内診断薬、治療薬)

2)in vitro:in vitro 放射性医薬品(体外診断薬)

4.RI 廃棄物: アイソトープ、放射性医薬品または放射性同位元素等規制法に規定された放射線発生装

置の使用により発生した廃棄物

1)集荷:医療機関、研究機関等から RI 廃棄物を集め保管すること

2)処理:RI 廃棄物の安定化及び減容のため焼却等すること

5.医療機関:

医療法(昭和23年7月30日法律第205号)に基づく病院及び診療所。教育機関及び

民間企業の付属病院並びに診療所を含む

6.衛生検査所:

臨床検査技師等に関する法律(昭和33年4月23日法律第76号)に基づく衛生検査所

7.教育機関:

学校教育法(昭和22年3月31日法律第26号)に基づく学校(大学にあってはその学

部)。大学の付属病院及び付属研究所・試験所・研究施設等を除く

8.研究機関:

国立、公立、特殊法人、公益法人等の研究所及び試験所並びに教育機関または民間企業の

付属研究所、試験所、研究施設

9.民間企業:

民間の工場及び作業場。付属研究所、試験所及び付属病院を除く

10.その他の機関: 上記5、6、7、8及び9の分類に属さない事業所

11.集計期間: 2019年4月1日から2020年3月31日の期間

Explanatory Notes Ⅰ Coverage of Statistics

This booklet presents the statistical data on the distribution of radioisotopes, radiopharmaceuticals, and radioactive waste, defined in the Glossary below. The data in the booklet are the amount of those that were supplied, collected or treated through Japan Radioisotope Association.

Ⅱ Glossary 1. Radioisotope:

Radioisotope prescribed by Act on the Regulation of Radioisotopes, etc.(Act No. 167 of June 10, 1957) There are two types of radioisotope: sealed and unsealed radioisotopes. Unsealed radioisotope consists of radionuclides/labeled compounds and radiopharmaceuticals but excluding clinical use. Sealed radioisotope means radioactive source for various applications. Sealed radioisotope for radiation therapy is described as “Medical Device” (see below).

2. Medical Device: Sealed radioisotope prescribed by Act on Ensuring Quality, Effectiveness and Safety of Pharmaceuticals and Medical Devices, etc(Act No.145 of August 10,1960)

3. Radiopharmaceutical: Radiopharmaceutical prescribed by basically Act on Ensuring Quality, Effectiveness and Safety of Pharmaceuticals and Medical Devices, etc(Act No.145 of August 10,1960) Radiopharmaceuticals are used mainly for injections, namely “in vivo use”. The assay kits containing radiolabeled compounds are also described as radiopharmaceuticals, namely “in vitro use” as long as they are used for clinical examinations. Radiopharmaceuticals are, here, described as “in vivo” for clinical diagnosis and therapy, and “in vitro” for diagnostic assay.

4. Radioactive Waste: Waste generated from unsealed radioisotopes or radiopharmaceuticals that were supplied through Japan Radioisotope Association, or generated by radiation generators defined by Act on the Regulation of Radioisotopes, etc. 1) Collection of radioactive waste : To collect radioactive waste from organizations, and store them. 2) Treatment of radioactive waste : To treat radioactive waste by incineration etc. , for volume reduction and

stabilization. 5. Hospital and Clinic:

Hospital and Clinic defined by Medical Care Act (Act No. 205 of July 30, 1948), including those attached to educational institutions and companies.

6. Clinical Laboratory: Clinical Laboratory prescribed by Act on Clinical Laboratory Technicians (Act No.76 of April 23,1958)

7. Educational Institution: Schools, Faculties of Universities and Colleges as specified by School Education Act (Act No. 26 of March 31, 1947), excluding research institutions, laboratories and hospitals attached to educational institutions.

8. Research Institute: Clinical laboratories are not included.

9. Private Company: Private factories and workshop, excluding research institutes, experiment station, laboratories, and hospitals attached to those institutions.

10. Others: Institutions excluded those defined above in 5, 6, 7, 8 and 9.

11. Survey Period, Fiscal Year: Data was surveyed in the period of fiscal year, namely, from the first of April to the end of March in the following year.

1 概 要 1.1 アイソトープ 1.1.1 非密封アイソトープ 供給量

非密封アイソトープの出荷個数は引き続き減少傾向だが、2019 年度は 2017 年度と同様

に 3H ガスの供給量(MBq)が著しく増加したため、全体の供給量が大きく増加した。ま

た、分子イメージングや医薬品開発等に利用が見込まれる 18F、67Ga、90Y の供給が増える

と共に、新たに短半減期核種である 124I の利用が見られた。 1.1.2(1) 密封アイソトープ 供給量

密封アイソトープの供給量全体(MBq)は 60Co の供給量に依存しており、60Co の供給

量のほとんどを占める放射線滅菌用線源の供給量の増加に伴って昨年度より増加した。 192Ir は主に非破壊検査用線源、アフターローディング用線源として使用されており、例

年どおりの供給量であった。 1.1.2(2) 医療機器 供給量

医療機器の供給量全体(MBq)は、60Co ガンマナイフ用線源に依存する。60Co ガンマナ

イフ用線源は、例年 10 個弱で推移している。 前立腺がん治療のための 125I 永久挿入用線源の供給量は、数年間減少傾向が続いている。 192Ir アフターローディング用線源は例年どおりの供給量であった。

1.1.3 輸送物の個数

輸送物の個数は主に非密封アイソトープの供給量の減少に伴い、減少傾向が続いている。

1.2 放射性医薬品 1.2.1 in vivo 供給量

2019 年度の供給量(MBq)の合計は前年度とあまり変わらず、2015 年度からほぼ横ば

い状態である。 PET 検査用核種である 18F は、過去 5 年間において毎年増加の傾向にあったが、2019

年度は前年度とほぼ同量である。 in vivo 全供給量の約 8 割を占めている 99Mo-99mTc ジェネレータ及び 99mTc 注射剤につ

いては、供給量の合計が前年度より 2.7%の減少となった。これは海外原子炉のトラブル

によりモリブデン原料の供給が不安定となったこと、また、台風による停電が発生し一時

的な供給への影響があったことにも起因する。 その他の検査薬用核種としては、18F や 99mTc への利用に移行していることによって、

67Ga、201Tl の減少傾向が続いている。 223Ra を用いた治療薬については、前年度より 11.3%減少した。同じく治療薬に用いら

れている 90Y は前年度より 37.0%減少した。 1.2.2 in vitro 供給量

125I の供給量(MBq)の合計は、他の検査方法への移行により減少傾向が続いていたが、

2016 年度以降は横ばいである。前年度より変動があった検査の内訳は、下垂体機能検査

(31.0%減)、膵・消化管機能検査(16.3%減)、性腺・胎盤機能検査(8.3%増)及び腫瘍

マーカー検査(5.9%増)である。 1.2.3 使用施設数

2019 年度における放射性医薬品の使用施設数は 1,228 施設であり、過去 5 年間におい

て大きく増減していない。 1.3 RI 廃棄物

2019 年度の集荷数量は 7,497 本(200L ドラム缶換算)であった。例年に比して放射

化した放射線発生装置の廃棄が多く、非圧縮性不燃物の集荷数量は前年比 47.9%増であ

った。 処理数量については、前年度に比べ 37.2%増加した。

1 Overview 1.1 Radioisotopes

1.1.1 Supply of Unsealed Radioisotopes Though the number of shipments of unsealed radioisotopes is ongoingly decreasing, the total amount

of supply (MBq) in fiscal 2019 shows significant increase since the amount of 3H gas supply increased remarkably just like in fiscal 2017. Regarding the radioisotopes used for molecular imaging and drug development such as 18F, 67Ga, 90Y, the supply amount increased and short-half-life radionuclides such as 124I were newly supplied.

1.1.2 (1) Supply of Sealed Radioisotopes

The total amount of supply (in MBq) of the sealed radioisotopes depends on that of 60Co. The increase of sources used for irradiation sterilization which mostly account for 60Co led to increase in the 2019’s total amount.

The supply of 192Ir, which has been mainly used as NDT (Non Destructive Testing) and RALS (Remote After Loading System) source, was almost the same as last few years.

1.1.2 (2) Supply of the Medical Device Sources

The total supply volume (MBq) of medical device sources depends on the 60Co gamma-knife source. The average replacement of the 60Co gamma-knife source is a little less than 10 per year.

The amount of 125I for permanent implant source continued to decrease for the past few years. The amount of 192Ir for RALS was almost the same as the past few years.

1.1.3 Number of Transport Packages for Radioisotope

The decreasing trend in the number of transport packages for radioisotope is continuing, mainly because the amount of supply of unsealed radioisotope is decreasing.

1.2 Radiopharmaceuticals

1.2.1 Supply of in vivo Radiopharmaceuticals (Diagnosis and Therapy) The total amount of supply (MBq) of in vivo radiopharmaceuticals in fiscal 2019 shows little change

from last year and has been almost flat since fiscal 2015. The supply amount of 18F for PET diagnosis, which had been on the increase for five years, is almost

the same as that in last year. About 99Mo-99mTc generators and 99mTc injections (99mTc-labeled pharmaceuticals) which account for

approximately 80% of the whole in vivo supply, the total of the supply resulted in a 2.7% decrease compared to last year. This is caused by the instability of 99Mo supply due to the unexpected troubles in foreign reactors. The temporary supply shortage due to power outage caused by the typhoon also contributes to this decline.

As for the rest of diagnostic radiopharmaceuticals, 67Ga and 201Tl, which are being replaced by 18F and 99mTc, continue to decrease.

Regarding 223Ra therapeutic agent, the supply amount decreased by 11.3% compared to last year. 90Y, which is used as a therapeutic agent as well, decreased by 37.0% compared to last year.

1.2.2 Supply of in vitro Radiopharmaceuticals

The total supplied amount (MBq) of 125I remains almost the same level since fiscal 2016 after continuing to decrease owing to a shift to other test procedures without using isotopes. Looking at the breakdown by examination category, there are some changes in figures from last year: the pituitary function tests (-31.0%), pancreas & gastrointestinal function tests (-16.3%), gonad & placenta function tests (+8.3%) and tumor marker tests (+5.9%).

1.2.3 Number of Hospitals and Clinical Laboratories Using Radiopharmaceuticals The number of hospitals and clinical laboratories using radiopharmaceuticals for clinical purposes

was 1,228 in fiscal 2019, which is almost the same over the past 5 years. 1.3 Radioactive Waste

The total amount of collected radioisotope wastes in 2019 was equivalent to 7,497 of 200-liter containers. Compared to the average year, a lot of activated radiation generating apparatuses was requested for disposal, and the amount of collected incompressible incombustibles increased by 47.9% compared to previous year.

The total amount of treating radioactive waste increased by 37.2% compared to the previous year.
