Dear Sir,Application for Stay Put within JKR Sibu, Sibu DivisionRefer to the letter PWD/C/A2/01/39/pt.13(5) dated 29 NOV 2012 and letter PWD/C/A2/01/11/12/PT.16(99) as attached,

1. I, Kong Chwe Ming, firstly grateful and appreciate for the promotion from PenolongJurutera Awam Gred 29 to Penolong Jurutera Awam Gred 36.

2. I crave to ask for your kindness to approve my application to stay put within JKR

Sibu due to following factors stated below, A. I just transfered back to Sibu from Kapit dated 16/02/2011 letter PWC/C/A2/01/39/pt.11(3). As a Sibu citizen, I have worked for Kapit Divisionfrom 14/12/2001 until 16/02/2011 (10 years). During this period,I travelled toevery corner in Kapit or rural areas around include Song, Long Busan, Long Ulu,Belaga and other places almost once a week. Due to my health problem thatdeteriorate yearly, I succeeded in transfer back to Sibu in 2012 after 10 years.But unfortunately, I have to transfer away from my family to Bintulu once againdue to the promotion rules.

B. Secondly, my wife, Wong Ing Kiew, as an independent housewife, are now

suffering from Major Depressive Disorder (Major Mental Illness) which worsenedafter the death of her mother in 2009. There is a letter attached from PsychiatryDepartment of Hospital Sibu , following up treatments in Hospital Sibu from timeto time by appointment.

C. Moreover, 3 out of 4 of my sons/daughters are far away from home to further their studies. There is only 1 youngest son left at home who will be sit for STPM

exam by next year. That s why my wife needs my monitor and attentive care fromtime to time in her living routines.

D. Next, I, as the station head of Rejang Park Recycle Station Sibu held by Tzu Chiorganization which is newly opened n functioned as collection of all used butrecycle materials, need my full attention during my non-working hours.

E. Last but not least, I as the supervisor of Projek Bantuan Miskin(PBRT), is stillhelping District Office continuing in efforts and responsibilities of supervising thecontractor accomplishments according to the scope of works.

With that all the problem stated above, I am afraid that will affect my work performanceand also my beloved family. Lastly, I begged again and thank you from the bottom of myheart in advance for your consideration and kind arrangement. Thank you.
