Méthodologie et Objectifs
• Etude internationale, de phase III, randomisée, contrôlée, menée en double-aveugle, versus Placebo
• Objectifs : – Evaluer la tolérance et l’efficacité du Rituximab utilisé en
association avec le méthotrexate (MTX) chez des patients atteints de PR et n’ayant pas répondu aux traitements par au moins un anti-TNFα.
– Etudier la pharmacocinétique et la pharmacodynamie du Rituximab (étendue de la durée de déplétion en cellules B et effets sur les immunoglobines et le facteur rhumatoïde)

• Age de 18 à 80 ans atteints de PR depuis au moins 6 mois
• Réponse inadéquate aux anti-TNFα (intolérance ou absence d’efficacité) :
– etanercept pdt au moins 3 mois (25 mg 2 x sem), – infliximab en 4 perfusions ou plus (au moins 3 mg/kg), – adalimumab pdt au moins 3 mois (40 mg 1 sem/2)

• Il n'y a actuellement pas de consensus soit en 2 perfusions (1gx2 à 15j d’intervalle). soit 4 perfusions (375g/m2/semaine, 4 semaines
consécutives). Prévention des rechutes…
• Dans les formes rénales réfractaires, l'association rituximab et cyclophosphamide pourrait être utile.
• En cas de contre-indication au cyclophosphamide, d'autres immunosuppresseurs peuvent se discuter (azathioprine et mycophénolate mofétil).

• What is the best combination of IS to use with RTX in severe MG? – Depend of the past experience of each patients – The risk of PML increase with the number of IS
• What is the best regimen of RTX for MG? – 1g x2 become the standard regimen for autoimmune diseases

• How can we reduce the risk of relapse?
– If a flare-up will not be life-threatening: treat “à la carte” based on clinical signs, as Rheumatologist did for RA
– If you should avoid a relapse → preventive treatment: 1g every 6 months (for 2 to 3 years…)

• Check for pregnancy, serologies HIV, hepatitis B and C • Dose hemoglobin, white blood cell count, gammaglobulins • Measure B cell count (flow cytometry)

Conclusions, perspectives for MG
RTX is effective in refractory MG (2/3 of cases)
- Recommended for severe MG resistant to steroids + 2 IS
- From 1 to 4 month after the first injection - The effects persist over 6 – 12 months - Patients can be retreated
• Waiting for relapse? • Systematical preventive retreatment? • Which dose: complete cycle or 1g only?
