Chirurgia: sempre, quando non deve essere eccessiva. Se deve essere troppo estesa significa che la malattia è troppo avanzata
Radioterapia (ricordare: i “tramiti” toranscutanei)
Chemioterapia sistemica e topica
Immunoterapia topica (fattore prognostico negativo: talcazione efficace mancanza di spazio per endopleurica)

BEST THERAPYBEST THERAPY
Multimodality therapy (Surgery, RT, CT) gave a 2-and 5-year survival rates of 38% and 15% respectively in 176 patients (out of 183, 7 of whom died postoperatively), with 51 months (4.2 y.rs) median survival, i.e. the longest of any reported series.Patients who should undergo such therapy have epithelial histology, negative nodes and completely
resecable disease (Sugarbaker DJ, J Thorac Cardiovasc Surg, 1999) (Zellos LS, Sem Oncol, 2002).

Gemcitabine (G), Platin (P), Ifosfamide (I) and VNB have effectiveness in single agent Chemotherapy (CT)
P and I are synergistic
G and P are synergistic
Tolerance and effectiveness of GIP was proven tolerated and useful in our previous experience in NSCLC
GIP acts in more than one phase of the cell cycle
Once the tumor burden (slowly growing cells) was reduced by front line GIP, the subsequent accelerating cell grow addressed by VNB.

IL2 has been proven to reduce pleural effusion by means a mechanism that remains unsettled (anti-Simian Virus 40? Activation of macrophages and T lymphocytes? Other?)
(The etiology of MM is: Asbestos exposure Simian Virus 40 infection)
Pleurodesis (slerotherapy) has failed because of the inability of the lung to expand and halts the intrapleural
therapy.
