How to Create a New User

Click on “New User Registration” to create your user id.

Fill in the appropriate information.

How to Build a Consent

Click the Search Tab which allows you to type in the name of the procedure to find the consent. Once you review the

results, select the procedure you want and click “Start”

OR Go to the “Browse” tab. Specialties are listed in alphabetical order. Select a specialty, then “Consents-Basic”

A list of procedure will show on the right side of the screen. Select a procedure then click “Start” in the bottom right.

Any fields with a red asterisk * are required (in this case, Anatomical Location is required) – NOTE: you will NOT be able

to print without completing the required fields.

If this is a SINGLE procedure consent, choose Site or Side from list.

If this is a Multiple procedure consent, choose OTHER and type in the correct SITE/SIDE beside the procedure name in

the Multiple box.

The date will appear on the consent exactly as it is typed.

PLEASE VERIFY the “REASON FOR PROCEDURE” contains the DIAGNOSIS on the PROVIDERS’ ORDERS.

You may scroll down page to review procedure specific language and required risks to disclose.

Click Generate Document, review your consent, click PRINT.

Creating Possible Procedures

When you select multiple procedures, if you check the box “Is Possible” in the tray below, this will mark that procedure

as a possible procedure.

The “possible” procedure is listed after the primary procedure and it also lists all the information needed for the possible

so the patient can review and agree.

How to Create Favorites

Favorites are specific by user. Each user has their own set of favorites which cannot be shared with other users.

Select a Procedure and click “Add Favorite” in the bottom right corner. This will add to your favorites whatever

procedure is selected in the tray below.

Click on the “Favorites” tab and you can see the list you created. This applies to all categories (Education, Pictures and

Combos)

Creating a Combo Favorite

You can create a combination favorite which includes more than one procedure.

First, select all the procedures you would like to include so they show in the tray below, then click “Add Favorite”

The “New Combination Favorite” box will appear. Enter the name you want to use to identify this favorite.

Go to the “Favorites” Tab, then select “Combos”. This will list all your combo favorites.

Blood Consents: Use “Blood Consent – Good Shepherd”

1. Good Shepherd Blood consent should be completed prior to the following procedures: Type and

Crossmatch, hysterectomy, prostatectomy, coronary artery bypass graft heart/valve replacement,

abdominal aortic aneurysm repair, nephrectomy, and total hip/knee/joint replacement except in cases of

emergency.

2. The Good Shepherd Blood Consent is completed in addition to the "Disclosure and Consent Medical and

Surgical Procedures".

3. The Good Shepherd Blood Consent is to be printed separately from the procedure consent, if necessary.

Anesthesia Consents: Use “Anesthesia Consent – Good Shepherd”

1. Anesthesia consents are required prior to any anesthetizing procedure.

2. Anesthesia consents are available within the iMed system, but will not print automatically with surgical consents.

3. Print the Anesthesia consent separately from the procedure consent.
