Effective April 1, 2014, Mercy Maricopa Integrated Care began operations as the Regional Behavioral Health Authority for Maricopa County. Funds for services are provided through a contract with the Arizona Department of Health Services/Division of Behavioral Health Services (ADHS/DBHS) and the Arizona Health Care Cost Containment System (AHCCCS).

Mercy Maricopa Integrated Care 2015 Provider Directory Welcome to Mercy Maricopa Integrated Care (Mercy Maricopa) your Regional Behavioral Health Authority (RBHA). Our goal is to provide you with access to highquality physical and behavioral health care. We have a large network of health care providers. These providers join us in supporting our mission to empower you on your path to recovery, to make good choices and live a healthy life.

About our network of providers We have built our provider network to treat behavioral and physical health together. We choose our network of contracted providers carefully. The providers included in this directory offer a broad range of culturally sensitive, individual and specialty services for adults and their families. Our service area is Maricopa County and a small portion of Pinal County.

How to use this directory This directory provides a list of the physical and behavioral health providers in Mercy Maricopas network in Maricopa and Pinal Counties. It is organized alphabetically, by location and provider type. All providers in this directory accept Medicaid. The directory includes services that may be available to adults who are: Determined to have a serious mental illness (SMI) Eligible for Medicaid (also called the Arizona Health

Care Cost Containment System or AHCCCS) and determined to have general mental health and/or substance abuse issues

Integrated careYou are part of an integrated health care program. That means your physical and behavioral health providers can work together to consider your whole health behavioral health, physical health, medication and case management needs.

As a member of this program, you have a choice of providers. Your Primary Care Provider (PCP) helps coordinate your physical health care. You can choose any PCP in this directory. You may change your PCP at any time, for any reason. You can also choose to allow your PCP and your behavioral health team to share information so they can work together to help you get the right care at the right time in the right setting. You are the most important part of your health care team. You have the choice and right to decide where you go for services.

The providers listed in this directory have agreed to provide you with health care, vision and pharmacy coverage. You may go to any of the providers listed in this directory. However, some services and specialists may require a referral. That means you have to get approval in advance.

Getting specialty careIf your PCP thinks you need specialized treatment, he or she will give you a referral to see a specialist. A specialist is a doctor who provides health care services for a specific disease or part of the body. Some examples of specialists include: oncologists, who treat cancer; cardiologists, who treat heart conditions; and orthopedists, who treat bone, joint or muscle conditions.

It is very important to get a referral from your PCP before you see a specialist. If you dont have a referral before you get services from a specialist, you may have to pay for these services yourself. If the specialist wants you to come back for followup visits, be sure to check with your PCP to find out if these visits are included under your original referral. If they are not included, your PCP can help you get a new referral for the additional visits. If there are specific hospitals you wish to use, find out whether your PCP uses these hospitals.

Certain health care services, such as hospitalization or outpatient surgery, require prior authorization from Mercy Maricopa. This means the service must be approved by Mercy Maricopa before it will be covered. Check your Member Handbook for a complete list of services that require this type of approval. Your provider is responsible for getting prior authorization before you receive the service. What services dont require approval in advance from a PCP?You may get the following services on your own, without a referral from your PCP. You must get these services from providers in the Mercy Maricopa network. Routine womens health care, which includes

breast exams, mammograms (Xrays of the breast), Pap tests and pelvic exams.

Flu and pneumonia vaccinations (as long as you get them from a Mercy Maricopa provider).

What if you have a medical emergency?Get medical help as quickly as possible. Call 911 for help or go to the nearest emergency room, hospital or urgent care center. You dont need to get prior approval or a referral first from your PCP or other network provider. You may get covered emergency medical care whenever you need it, anywhere in the United States.

Ambulance services are covered in situations where other means of transportation in the United States would endanger your health.

Getting care when traveling outside of the Mercy Maricopa service areaIf you need care when you are outside the service area, your health care coverage is limited. The only services we cover when you are outside our service area are care for a medical emergency, including poststabilization care, urgently needed care, renal dialysis (kidney) and care that Mercy Maricopa or a provider has approved in advance.

Your rights to servicesYou and your family have the right to receive services in a timely manner. You and your family have the right to receive services that meet your needs. If you experience any problems getting timely services, or if you have any concerns about the services you received (or didnt receive), please contact Mercy Maricopa or the Arizona Department of Health Services/Division of Behavioral Health Services (ADHS/DBHS). What if you use noncontracted providers to get covered services?Noncontracted providers are providers that are not part of the Mercy Maricopa provider network. If you use noncontracted providers without getting prior approval from your PCP or Mercy Maricopa, you could be responsible for the full cost of the services. You must use network providers, except in emergency or urgent care situations or for outofarea renal dialysis or other services. If you get routine care from outofnetwork providers, neither AHCCCS nor Mercy Maricopa will be responsible for the costs.

What should you do if you receive a bill from a noncontracted provider that you think should be paid by Mercy Maricopa?We cover certain health care services that you get from noncontracted providers. These include care for a medical emergency, urgently needed care, renal dialysis (kidney) that you get when you are outside the Mercy Maricopa service area and care that has been approved in advance by Mercy Maricopa.

If you get a bill for the services, do not pay it. Send the bill to the address listed below for processing.

If the claim is payable, we will pay the provider. If you have already paid for the services, submit a copy of the bill and proof of payment to us for processing. If the claim is payable, you will be reimbursed for our share of the cost. If we deny the claim, you will be notified in writing and provided with your appeal rights.
