รักษาอาการขางเคียงจากรังสีรักษาในเบื้องตนไดอยางถูกตอง
• มีเจตคติที่ด ีและเชื่อวาอาการขางเคียงที่เกิดจากรังสีรักษานั้นบางอยางสามารถหลีกเลี่ยงได หรือถาหลีกเลี่ยงไมไดก็สามารถใหการรักษาแบบประคับประคองไดในภายหลัง

Concept of Radiation Complication
• Acute radiation complication:– Less than 3 months– Recover to normal Recover to normal
• Subacute radiation complication:– 3 – 6 months– Often temporary and can be recovered
• Chronic radiation complication:– More than 6 months– Usually permanent
Risk factor of radiation complication
• Radiotherapy:– Total dose– Area of treatment– Overall treatment time
• Others:– Age: pediatric > adult, old > adult– Underlying disease– Previous treatment at radiation field– Combine with other treatment
Normal tissue tolerance
• TD = Tolerance dose• TD 5/5:
– โอกาสการเกิดภาวะแทรกซอนในอวยัวะตางๆ ในสัดสวนรอยละ 5 ในระยะเวลา 5 ป
• TD 50/5:– โอกาสการเกิดภาวะแทรกซอนในอวยัวะตางๆ ในสัดสวนรอย
ละ 50 ในระยะเวลา 5 ป

Treatment: During RT• สามารถอาบน้ําได• หามใชครมีทุกชนิดทาบริเวณฉายรังสี• ระวังไมใหเกิดการเสียดสีกับบริเวณผิวหนังที่ฉายรังสี• หามใชกระเปาน้ํารอนหรือน้ําแข็งวางบนผิวหนัง• ระวังไมใหผิวหนังสวนที่ฉายรังสีถูกแดดจัดโดยตรง• หามโกนหนวด โกนขน หรือผม ในบริเวณที่ฉายรังสี• ถาคันมาก หรือมีการติดเชือ้รวมดวย อาจทาดวย gential

• Cold cream or moisture cream ทา และนวด บริเวณที่ฉายรังสี โดยเริ่มหลงัจากฉายรังสีครบแลวประมาณ 4-6 สัปดาห
• ระวังไมใหเกิดบาดแผลบริเวณที่ฉายรังสี• Radiation fibrosis, Hypopigmentation and
telangiectasia: ไมมีวิธีรักษา ตองอธิบายใหผูปวยเขาใจ• Ulceration: hyperbaric oxygen

• Late:– Brain necrosis: > 50 Gy– Mental retardation: age < 3 yr– Leuko-encephalopathy– Neurocognitive deficit– Sensorineural hearing loss: > 60 Gy– Impairment of vision: > 30-65 Gy– Cataract– Lhermitte’s symptom: electric shocklike
sensation, > 45 Gy

• Acute:– Brain edema – Increase intracranial pressure– Otitis media– Red eye syndrome: > 50 Gy– Moist desquamation– Alopecia– Demyelination of spinal cord

• Skin and subcutaneous tissue• Oral cavity:
– Dental caries– Mucositis– Xerostomia– การรับรสของ taste bud เสียไป
• Bone: Osteoradionecrosis, trismus
• Spinal cord
Dental caries• กอนเริ่มฉายรังสี: ควรตองไดรับการตรวจรักษาชองปาก
และฟนจากทันตแพทยกอน– ถอนฟน ควรทํากอนเริ่มฉายรังสี 1-2 สัปดาห– อุด ขูด หนิปูน สามารถทําไดขณะรับการฉายรงัสี
• ระหวางการฉายรังสี: หามการถอนฟน– สามารถแปรงฟนได แตควรใชแปรงสีฟนที่ออนนุม– ยาสีฟนควรมีสวนผสมของฟลูออไรด
• หลังการฉายรังสี: ตรวจรักษากับทนัตแพทยอยางสม่ําเสมอ– การถอนฟนสามารถถอนได แตตองระมดัระวัง เพราะแผล
อาจจะหายชา และอาจเกิดการติดเชื้อ แลวนําไปสู osteomyelitis ได

– Xylocaine viscus– ยาระงับปวด
Other complications• การรับรสนอยลงของ Taste bud
– เริ่มจาก หวาน เปรี้ยว ขม และเค็ม– > 20 Gy
• Xerostomia: ภาวะน้ําลายแหง– น้ําลายสวนใหญสรางจาก Parotid gland: 65%
• Osteoradionecrosis– เมื่อน้ําลายเหนียวจะทําใหฟนผุ และกอใหเกิดภาวะนี้ตามมา– > 70 Gy

• Pneumonitis:– Acute pneumonitic phase: 1-3 mo
• Injury to type II pneumocyte (release of surfactant)• Proliferation of type II pneumocyte at 1-3 mo and compensatory
hypertrophy of lamellar bodies– Late pneumonitic phase: 3-6 mo
• Sclerosis of alveolar wall and extensive endothelial damage• Fibrosis with loss of function

• Symptoms: dyspnea, nonproductive cough, pleuritic chest pain, fever, rales, and abnormal radiographic finding

• Dose/Time/Volume: factors are important
• Mean lung doses: > 10-20 Gy• Treatment:
– high dose steroids (dexa 16-20mg/day)

• Coronary artery disease– 10-15 yr after RT
• Pericarditis– 6 mo – 1 yr after RT
• Cardiomyopathy• Valvular damage• Dose:
– < 40 Gy for < 25% of cardiac volume
– < 30 Gy for larger volume

Management
• ยกแขนดานผาตัดใหสามารถยดืตรงชิดกับใบห ูหรือเอื้อมไปจับไหลดานตรงขามทางดานหลังได ใหทําทุกๆวัน วันละหลายๆครั้ง และสม่ําเสมอ
• ระวังไมใหแขนดานตัวโรคเกิดแผล และไมควรใหมีการเจาะเลือดดานนี้
• ระวังไมใชยก หรือรับน้าํหนักมากเกินไป• สวมหรอืพัน elastic bandage เพื่อประคับประคองแขน
ได• ทํากายภาพฟนฟู หากเปนอยางรุนแรง

GI: Acute side effects
Nausea/vomiting• Diarrhea • Abdominal cramping
• Rectal dyscomfort• Rectal bleeding from
proctitis
Mx : Imodium, Lomotil, or Kaolin and pectin
Mx : small enema with hydrocortisone, low-residue diet with no grease or spices and
↑fiber in stool (Metamucil, Fibercon)
GI: Late side effects
Management
• Supportive care• Surgery
• Small bowel– Obstruction– Perforation– Fistula: enterocolic,
enterocutaneous, enterovaginal fistula
– Chronic diarrhea, enteritis, cachexia

Mx of proctitis
• Prednisolone 12 tab mixed liquid paraffin 30-50 ml, per rectal, OD
• Severe case …Electrocauterizationor colostomy
Rectosigmoid
• Proctitis• Rectal ulcer and
rectal stricture• Rectovaginal fistula• Sigmoid perforation
GU: Acute side effects
Management :
•Antispasmodics (Pyridium, Urispas)
•Fluid intake at least 2-2.5 L daily
•UTI : sulfonamides and antibiotics
• Genitourinary system
– Dysuria– Frequency urine and
nocturia
GU: Late side effects
• Cystitis• Vesicovaginal fistula• Ureteral stricture• Urethral stricture• Bladder ulcer
Mx of cystitis• Antibiotic• Cystoscopic,laser, or
cautery treatment• Clot evacuation and
continuous bladder irrigation (for acute)
• Urinary diversion• Hyperbaric oxygen

Management
• Supportive care• Use of vaginal dilators
several times daily, started during course of treatment
• Skin massage
Other sequelae• Liver or renal toxicity• Vaginal stenosis• Vault necrosis• Subcutaneous fibrosis• Leg edema• Pelvic infection• Hemorrhage• Neuritis
