Border Police (ITBP) Force. The Football players were from the top two Ladakh teams – Oasis and Mahabodhi of the 1st High Altitude Border Village Friendship Football Tournament organised by ITBP in the months of October to November, 2017 in Ladakh region. My best experience was travelling by Delhi Metro. I have experienced that life in Delhi is very fast in comparison to our state, Arunachal Pradesh. The 12 day tour from 21 November 2017 to 2 December, 2017 was organized by 25th Battalion of ITBP. The group also visited to various historical and important places in the Capital and also travelled to historical places of Agra and Mathura. Sh. Our journey started on 21st October from Tezu to New Delhi by train. It was very amazing moment for me. Firstly, we traveled in ITBP bus. After returning to New Delhi, we visited President’s House and had a meeting with Hon’ble MoS (Home) Shri Kiren Rijiju at North Block. Our journey was amazing, we enjoyed a lot. It was our 1st ever visit to Delhi. We visited many historical places including Museum, birth place of Lord Krishna which is unforgettable and knowledgeable. ITBP personnel are amazing and they treated us like their own children. ITBP provided us all possible assistance at all the stages. They also Top up our mobile phones when needed. I have learned amazing things. Monuments like Taj Mahal, Qutub Minar, India Gate were beautiful and life long experience for the group.

ITBP- The Best Marching Contingent

New Delhi. The Indo-Tibetan Border Police (ITBP) has been adjudged as the best marching contingent among paramilitary and other auxiliary forces marched at Republic Day Parade, 2018 at Rajpath.

Smt Nirmala Sitharaman, Hon’ble Defence Minister presented the prestigious trophy to Sh. R K Pachnanda, Director General ITBP and Assistant Commandant Akshay Deshmukh, the parade commander of the ITBP contingent in an official function held at the South Block. 148

member contingent represented the ITBP which has participated in the parade after two year’s gap. The Contingent represented all formations of the Force comprising members from almost all Units of the Force. It was also a historic moment when the ITBP Tableau was featured at the Rajpath after 20 years. ITBP has won the best contingent trophy in the category for the record sixth time. Earlier, the Force had been adjudged the best contingent in the category in the years 1998, 1999, 2000, 2004 and 2011.

12th ITBP Contingent flagged off toDR Congo for UN Mission

New Delhi, 16 February, 2018:- 12th batch of ITBP commando contingent comprising of 140 members including 04 women personnel flagged off by Shri R. K. Pachnanda, Director General at Force HQ. The contingent departed to Bunia, Democratic Republic of Congo on Formed Police Unit duties for the United Nations Peace Keeping Mission.

The DG addressed the contingent and conveyed best wishes to the members. The contingent comprises of experienced personnel drawn from different units of the Force. All personnel have been specially trained for this UN deployment at UNCIVPOL School at ITBP campus Tigri, New Delhi. The Contingent is being commanded by Sh. Pawan Malik, Commandant. As per UN mandate, the

following task has been assigned to ITBP Contingent at D.R. Congo:– (i) Patrolling of mission compound to ensure security of UN staff and assets and to generate sense of security among locals, (ii) Crowd Control Management duty, (iii) Stand by Quick Reaction Team, (iv) Joint patrolling with local police, (v) Security of designated Compound, and (vi) Any other operational duty assigned by MONUC Hqrs.

Over the years, ITBP contingents have been doing commendable job in Bunia, D.R. Congo. Besides this, ITBP is also providing free medical advice and medicines to local population. After induction of 12th batch, the 11th batch returned to India. The 11th contingent served in UN Mission at D.R. Congo for over one year.

Sh. R K Pachnanda DG ITBP, distinguished guests, senior officers and personnel of the CAPFs and their families witnessed the Concert organised on the eve of Holi, 2018.

16 tunes of various martial and Indian music were presented by the Force Band. The one hour Concert was conducted by Major Nazir Hussain, VSM (Retd).

The Concert was a classical mix of traditional military tunes and Indian and Bollywood evergreen numbers. Sh Rijiju appreciated the Concert and said that this type of patriotic programmes should be organised by other Forces as well in various places the country.

He also congratulated the ITBP for 'Best Marching Contingent Trophy' on Republic Day Parade, 2018 and lauded the contribution of ITBP Band contingent for inspiring tunes for the best show by the marching contingent on Rajpath.

ITBP hosted the 18th All India Police Shooting competition, as a part of All India Police Duty Meet (AIPDM) at Basic Training Centre, Bhanu, Panchkula, Haryana from 2nd December to 7th December 2017. His Excellency, Sh. V. P. Singh Badnore, Hon’ble Governor Punjab and administrator of Chandigarh (UT) was the Chief Guest at the grand opening ceremony, in which 843 participants including 16 female participants from 34 state police and Central Armed Police Force participated in the march past which was followed by spectacular demonstrations by ITBP ‘Janbaaz’ team on motor cycles, K-9 unit, exhibition drill and band display by ITBP mahila troops. In this fiercely contested competition, the events include use of service weapons of police forces from all over the country. A total of 13 events including 5 events of rifle shooting and 8 events of pistol and Carbine shooting were being conducted at the ITBP long and short firing ranges. Sh. R.K. Pachnanda, DG, ITBP welcomed the Hon’ble Governor, distinguis hed guests and participants. He said that the ITBP was assigned the responsibility to conduct AIPSC in the years 2006 and 2008 also. ITBP has also successfully conducted

the AIPDM-15. He further added that the meet initially started in 1953 and included pistol and rifle shooting only. Sh. Badnore, in his address to the gathering, said that the shooting competitions have become more complicated these days due to introduction of various types of targets, timing, positions and distance in firing.

The State Police and Central Armed Police Forces have produced players of international repute, who have brought laurels to the nation with their achievements. Our countrymen should not only strive to work hard to hone their skills in sports, but should also inspire the next generation to participate actively in sports. The opening shot was fired by Sh. T. S. Dhillon, former I.G.P. after which the shooting ranges were thrown open to the participants to display their professional skills over the next five days.

The closing ceremony was held on 7th December, 2017 in which H. E. Prof Kaptan Singh Solanki, Hon’ble Governor Haryana was the Chief Guest. BSF won the overall championship while CRPF achieved the second position.

A Joint Search Operations launched at Company Operating Base (COB) of 29th Bn ITBP & Chhattisgarh Police in village named Bagjhar of Narayanpur District in Chhattisgarh on 10th July 2017. During Search, four suspects were apprehended with 02 Bharmar Rifles by the party.

Area Domination Patrol (ADP) cum search, joint operations was launched by 44th Bn ITBP from Kohka COB on 15th August, 2017. The party encountered an IED blast near Jameri village.

However, no loss of men and material was reported in the said operation.

Three suspects were apprehended by the ITBP and an IED was recovered.

Joint Operations Area Domination Patrol (ADP) cum search was conducted by Khadgaon Company Operating Base 44th Bn ITBP in general area Bhatgaon, Rajnandgaon District of Chhattisgarh.

During thorough search, party found a temporary hideout, tell tale sign and waste food.

Later, the hideout of naxals was destroyed by the party.

Encounter with Naxals29th Bn ITBP launched a joint search operations

from Urendabeda COB in Badai in Kondagaon, district, Chhattisgarh. While one group was being placed in Badgai village naxals observed the movement of the party and fired upon it.

The advancing party took cover and retaliated immediately. Exchange of fire started at about 1450 hrs, which lasted for 15 minutes upto 1505 hrs.

Naxals managed to escape taking advantage of rear nullah and thick undergrowth. During encounter, total 112 Rds were fired by own troops. After stoppage of fire, the area was searched thoroughly and the party recovered IEDs, naxal literature and other miscellaneous items from the site.

Patrol (ADP) reached Amdhai Ghati, NICCO mining site, two procklein (JCB) were put on fire and two bikes thrown into ditch of 50 Meters by Naxalites on 4th September, 2017.

The ADP retreived the bikes with the help of locals from the ditches and brought it to Amdhai Ghati Camp. Naxal had also displayed banners at the same location which were removed and brought to the Police Station Chhotte Donger.

On 7th November, 2017 a CASO was planned by 46th Bn ITBP and CGP from Kurusnar COB in general area Ghodagaon, District–Narayanpur Chhattisgarh. During the operation at 0500 hrs, operation party cordoned off the village and started search.

On 2nd November, 2017 an operation was planned by 45th Bn ITBP from Chhote Donger COB in Amdhai Ghati Area District- Narayanpur Chhattisgarh. During the operation at around 0400 hrs, suddenly a blast sound was heard. The party immediately took tactical position and started the search in the area. Meanwhile, one more blast occurred at the same site.

The search party succeeded to recover one Bharmar Rifle from a suspected house. House owner Binjururam Dugga, Age-32 years was nabbed on the spot and handed over the CGP for further questioning. CGP questioned him and released after recording his identity.

A joint search operation name 'Sharad-XIII' was planned by 40th Bn ITBP, HAWK (MP) and Chhattisgarh Police from Malaida COB in area Bhujari on 22nd December 2017. During search operation, party recovered following items from village Bhujari-

On 20th December 2017, troops of 40th Bn while on road construction security duty, succeeded to recover one IED weighing approx 10 KG from the area approx 3.5 km ahead from Gattapar, Distt Rajnandgaon, Chhattisgarh.

The IED was placed in a steel container. The IED was destroyed on the spot by BDDS teams.

45th Bn ITBP had launched a joint search operation from Pharasgaon COB on 24th December, 2017.

During search Operation, the party found a 25 feet naxal memorial near village Chhote Pharasgaon. Subsequently, the memorial was destroyed by ITBP.

On 23rd November, 2017 a search Ops was conducted by 45th Bn from Chhote Dongar COB towards Badgaon village area, District Narayanpur Chhattisgarh. The Ops party, while returning, got an input to cordon an area at Chhote Dongar weekly market.

On observing ITBP troops at weekly market, one suspected person tried to escape, but search party nabbed him on the spot and handed over to CGP for further interrogation.

Joint search operation code name ‘Kartik-XII’ was planned by 44th Bn ITBP and CGP from Kohka COB in general area Dungeli, Kondal, Korcha, Teregaon, and Jamdi on 13th November 2017.

The search party succeeded to recover one Bharmar Rifle hidden in blue polythene sheet. The houses of suspected villagers were also searched and naxal literature ‘Prabhat Jan – March edition’ was recovered from the house of a villager namely Shyam Singh.

Apprehension of two naxals by 41st Bn

On the basis of specific intelligence, a search operation was conducted by Mardapal COB of 41st Bn & Chhattisgarh Police on 27th December, 2017. During the course of the operation, two naxals were apprehended by ITBP from the jungle area of Nedwal. Both the naxals were handed over to Chhattisgarh Police. They have confessed their involvement in the firing incident upon newly opened Hadeli Camp on 6th December, 2017.

During the course of operation, the party succeeded to apprehend 08 suspected naxals alongwith 6 Bharmar rifles, 2 Knives, 2 Jerry Cans, 1 Bundle of electric wire & 1 tarpaulin.

On 6 January, 2018, search Operation was conducted by 41st Bn ITBP from Ranapal, COB in Nawagaon area, Distt -Kondagaon Chhattisgarh. During the operation, the party succeeded to apprehend 3 suspected naxals from the jungle area between village Ranapal and Nawagaon. One Tiffin Bomb weighing 10 KG, one dry battery & one bundle of electric wire was also recovered from them. Apprehended naxals along with seized items were handed over to local police.

Ministry of Health and Family welfare has issued a office memorandum dated 9th November, 2017 for the simplification of procedure for treatment at private hospitals empaneled under CGHS/CS (MA) rules 1944. With reference to the treatment in private empaneled hospital under CGHS ministry has been receiving representation for simplification of procedure for undergoing treatment at private hospitals empaneled under CGHS for specific treatment procedures listed under CGHS rate list are advised by a Specialist in a Central Government/State Govt. Specialist hospital or a CGHS Medical officer without any requirement. It has now been decided that CGHS beneficiaries are allowed to undergo treatment at private hospitals empaneled under CGHS of specific treatment procedures listed under CGHS rate list are advised by a Specialist in a Central Government/State Govt. specialist hospital or a CGHS Medical Officer without any requirement of any other referral (permission) letter. Private empaneled hospitals shall perform the

treatment on cashless basis in respect of pensioners, ex-MPs, Freedom Fighters, Regular employees (both CGHS and CS (MA) beneficiaries) of this Ministry & other categories of CGHS beneficiaries, who are presently eligible for credit facility and shall enclose the prescription issued by Government Specialist or a CGHS Medical Officer, in original (or a self attested photocopy) along with the hospital bill submitted to the competent authorities. Serving government employees shall enclose the prescription issued by a Government Specialist or CGHS Medical Officer in original (or a self-attested photocopy), while submitting the medical claim to the concerned Ministry/department/office for reimbursement. CGHS Medical Officer/Government Specialist shall not refer the beneficiaries to any particular empaneled hospital by name but, shall specify the treatment procedure and mention ‘referred to any CGHS empanelled centre’. 5th Bn ITBP organized a high altitude Ice hockey tournament in Shyok area of Eastern Ladakh from 28th December to 31st December, 2017 under the civic action program. This outreach programme was in consonance with the Government’s initiative of ‘Khelo India’ for talent hunt. Definition:- The term bad Obsteric history (BOH) is applied to a pregnant mother where her present obstetric outcome is likely to be affected adversely by the nature of previous obstetric disaster.

Case – I:- A 26-year-old female, case of ANC with 5 previous second trimester pregnancy loss reported in OPD. On investigation found to be suffering from “Anti-phospholipid (APLA) Syndrome, she was immediately put on injectable heparin, progesterone support and all possible treatment along with bed rest.

In spite of all efforts patient reported with bleeding P/V and pain abdomen at around 22 weeks POG which generally happens in such cases due to placental thrombosis and finally went into preterm labour.

Case – II:- A 30-year-old antenatal case reported in OPD. She gave history of previous full term delivery at Punjab, baby died due to very low birth weight, autopsy not done. Also gave history of twin pregnancy followed by abortion due to foetal loss and H/O ruptured ectopic pregnancy with laparotomy done outside. Unfortunately she was detected to have congenitally malformed foetus (Trisomy-18) at twelve weeks period of gestation. There is a possibility of genetic defect in her previous pregnancies too.

These were two among few challenging cases where only little or no help can be offered. Other reasons of repeated pregnancy loss apart from APLA syndrome and congenital malformations are inherited or acquired thrombophilia, incompitent cervix, Rh isoimmunization, ectopic (tubal or ovarian) pregnancy, molar pregnancy, recurrent miscarriages due to uterine fibroid, parental chromosomal abnormalities causing balanced translocation. The patients suffering from medical ailments like poorly controlled diabetes, thyroid disordered, hypertensive disorders (PIH), heart diseases requiring warfarin therapy are also probable candidates of BOH. Congenital anatomic abnormalities of uterus should always be ruled out which are responsible for 10 – 15% of recurrent abortion. maternal infections like syphilis, toxoplasmosis, listeriosis, ureaplasma, M. hominis are the cause of repeated pregnancy loss.

Treatment starts with proper counseling of the couple so’ as to alleviate anxiety and improve the Psychology. Cases of chromosomal anomalies should go for a mandatory genetic counseling session. Defect may be in the abortus or in the couple itself. They should be provided with an option of preconception genetic screening, prenatal diagnosis and if required pregnancy with sperm or oocyte donor gametes. Preconception screening and counseling offer an opportunity to identify and mitigate maternal risk factors before pregnancy begins. A strategy is provided to reduce or eliminate the pathology revealed by her obstetric or medical history and by specific testing.

Cases of autoimmunity due to APLA syndrome, ANA antibodies and with inherited thrombophilia with history of several faetal wastage carry a poor outcome and remain a challenge for the treating Obstetrician. Such cases should be immediately shifted to heparin therapy and low dose aspirin tablets.

Newer therapies which are under trial are treatment with hydrochloroquinine (HCQ), low dose prednisolone, vitamin D, pentoxyphylline, TNF targeted therapies, plasma exchange and IV immunoglobins. Patient with incompetent Cervix requires prompt diagnosis and timely encirclage surgery. Case of persistent ectopic and molar pregnancies require early diagnosis and prompt action. Those suffering from anatomic abnormalities like bicornuate or septate uterus should be offered metroplasty operation.

Hypertension is one of the common complication met with pregnancy and contribute significantly to maternal and perinatal morbidity and mortality. Counseling of known diabetic patients planning pregnancy or those with gestational diabetes is a must. The fear of anatomic deformities in the offspring is powerful motivation to control blood glucose levels. Patient with uncontrolled diabetes mellites should be informed about greater incidence of pre-eclampsia, infections, post-partum hemorrhages, intrauterine death and increase risk of cesarean deliveries. Incidence of cardiac diseases in India is 9.7%. All patient with certain heart diseases should be assessed by a through and detailed history

of previous cardiac events, previous obstetric and associated medical complications, family history along with details of previous surgery, medications and investigation done. Complication of cardiac diseases on pregnancy like preterm delivery, IUGR, risk of foetal congenital heart diseases (4.5% versus 0.6% in overall papulation) should be explained to them. Patient with functional impairment (NYHA III / IV) carries high maternal and foetal mortality rate.

Conclusion:-All patient with Bad obstetric history should be treated as high risk pregnancy because of probable poor maternal or foetal outcome requiring sophisticated surveillance and follow-up.
