Summary

The Community Health Center of Branch County (CHCBC) conducted the Community Health Needs Assessment (CHNA) to further its commitment to the community and better determine the needs of whom they serve. For the purposes of this CHNA, the definition of community is all the residents of Branch County, Michigan. This CHNA began in January 2014 and was completed and approved by CHCBC’s Board of Trustees on April 28, 2014. CHCBC staff in collaboration with the Branch, Hillsdale, and St. Joseph Community Health Agency conducted the CHNA.

Data on health needs were derived using four methods:

Data gathered and analyzed from secondary sources such as the US Census, County Health Rankings, Michigan Department of Community Health, and the Branch-Hillsdale-St. Joseph Community Monitor.

A internet and paper-based survey, made available at 11 locations, generating 290 responses

A community forum on April 2, 2014 where members were selected based on their extensive knowledge and expertise on public health issues or experience with subpopulations impacted by most healthcare issues

Key informant interviews with representatives of the community with unique knowledge of health issues affecting the community

These data were analyzed to identify the top health needs of residents of Branch County:

Addiction (smoking, drugs, alcohol) Unwed and teen mothers Suicide prevention Parenting skills & family planning services Obesity Senior citizen adult day care Childhood mental health programs Health literacy & prevention education

CHCBC leaders and board members using consensus criteria including availability of evidence-based interventions and measures of impact, magnitude of the need, and fit within CHCBC’s strategy and capabilities prioritized these top health needs.

As a result of this analysis, the CHCBC identified areas of needed improvement and access of health education and programs focusing on the needs of addiction, parenting/pregnancy, mental health, and senior citizens as its focus for community health needs. This decision reflects a coming together of the health needs expressed by residents of Branch County and opportunities presented by the Affordable Care Act.

Adequate programming and access to these services is dependent not only on the supply of providers, but also on such factors as affordability, physical accessibility, acceptability of services, availability of hours, and coordination of care. To address these concerns, the CHCBC will serve as a community health catalyst and engage community partners in taking action to improve these programs and their access.

The Community Health Assessment Data Collection and Survey

The Community Health Center of Branch County collaborated with the Branch-Hillsdale-St. Joseph County Health Agency to create a comprehensive community-based health needs assessment.

Part of the health assessment plan was to distribute an assessment survey to the general population, gather secondary supportive data, and engage county community leaders in an open discussion on how best to meet the needs revealed by the process.

Secondary Data Collection

Secondary data for this assessment was collected from a variety of county, state and federal sources to create a more complete profile of the population and health needs. This report strived to use the most recent, complete, yearly data available.

The Branch-Hillsdale-St. Joseph Community Health Agency was instrumental in providing reliable secondary data. A recently published document, “The Monitor, 2013,” containing 100 health indicators for the county was a vital resource.

Secondary data was analyzed to determine needs and trends in Branch County. The data was presented to the community forum to help prioritize the needs of the community.

Primary Data Collection – County Resident Survey Results

Primary data is data collected directly from first-hand experience or opinions. The primary data for this assessment consisted of the county resident survey, the community forum, and key informant interviews.

A community health survey was created and distributed through various clinics and the internet from March 3, 2014 to March 24, 2014.

Paper forms of the survey were available at the following locations. Locations were selected to help receive feedback from various types of demographic segments in Branch County. The locations are:

Surveys were emailed to civic groups and community leaders and encouraged to participate and share the survey. The survey was also posted on various local group Face Book sites and shared. Flyers describing the survey were posted at various clinics and local businesses referring residents to the CHCBC website were the survey link was posted.

As a result, 290 individuals completed the health survey questionnaire.

Primary Data Collection - Community Forum

The Community Forum met on April 2, 2014. Members were selected from different industries and positions that have influence in the community. A complete list of attendees/invitees can be found in Appendix A. The forum consisted of a presentation of the county demographics and the survey results. After the presentation, members were asked to select areas they felt were needs for Branch County. Team members then prioritized the needs. The top 9 needs decided on by the forum in order of importance are:

Smoking Prevention Unwed/Teen Mothers Suicide Prevention Parenting Skills Obesity Family Planning Services for Low Income Senior Citizen Adult Daycare Childhood Mental Health Drug Abuse

The team was then able to combine a few needs to come up with seven needs that should be addressed in the community, these needs are as follows:

1. Work towards education and prevention of smoking and drug abuse

2. Programs that help reduce the number of teen and unwed mothers and assist current teen and unwed mothers

3. More community involvement in suicide prevention

4. Parenting skills and family planning services for low income families

5. Address obesity issues with adults and minors, especially by educating residents of importance of healthy eating and exercising.

6. Establish a Senior Citizen Day Care

7. Establish a program for childhood mental health issues

After prioritizing needs the forum worked in small teams to help develop ideas that could be used in finding ways to meet the needs and determine what current services Branch County has that could be used to meet the needs listed.

Primary Data Collection - Key Informant Interviews

Key informant interviews were used in situations where a professional in a certain field was not able to attend the Community Forum. This professional was explained the prioritized list of needs and asked to give input on how to meet the needs in their area of expertise. The key informant interviews were chosen to help gain more understanding in areas the Community Forum felt needed a more professional opinion than could have been offered at the forum meeting. The professionals interviewed and their comments are below.

Dixie Koenemann – Coldwater Community Schools Assistant Superintendent

Ms. Koenemann explained that Coldwater Community Schools do currently have projects in place to help with childhood/teenage mental health and substance abuse. Ms. Koenemann also mentioned that Coldwater Community Schools are in the process of reprioritizing some programs and adding others. Some of the things that Coldwater Community Schools are doing include:

A social worker at the elementary schools to help determine any mental health needs in children and work with them and their parents.

Working with Pines Behavioral Health to work with students at Franklin Alternative High School who may need counseling.

Smoking, drug, and alcohol education and programs to help students understand the risk of being involved with these issues.

Work with Branch Intermediate School District and their early childhood programs to recognize any severe mental health issues at an early age.

Julie Santure – City of Coldwater Community Enrichment Director

Ms. Santure explained that the City of Coldwater currently has programs that focus on decreasing obesity and improving healthy living in both Coldwater and Branch County. Ms. Santure also mentioned that they would be able to increase the number of programs and nutritional classes offered when the construction of the Dr. Robert W. Browne Aquatic Center & Recreation Campus is completed. The city of Coldwater has released its spring and summer recreation guide providing a list of all programs this spring and summer for all ages. Ms. Santure mentioned that on Monday April 14, 2014 a Recreation Guide was made available to the public listing all the recreation options and parks residents of Branch County have available to them. The City of Coldwater is very active in the long-term health of their residents and has many programs that prove that.

Community Health Needs Assessment

Branch County Implementation Plans for 2014-2016

Given the findings of the 2014 Branch County Community Health Center (CHCBC) Community Health Needs Assessment, and hospital resources available to address identified issues, CHCBC prioritizes the following needs as those to be included in its implementation strategy.

Priority Issue 1: Work towards education and prevention of smoking and drug abuse

Goal: Decrease the number of residents who smoke and use drugs.

Recommendations: The community has programs in place for residents to get treatment for addiction services.

CHCBC will continue to work with Pines Behavioral Health with these programs as well as try to help in development of other programs focusing on smoking and drug abuse.

CHCBC will develop a smoking cessation program for residents that cannot afford other methods to quit smoking.

CHCBC will work with area high schools in education of the dangers of smoking and drug abuse for youth attempting to reach the youth before smoking and drug use starts

Priority Issue 2: Programs that help reduce the number of teen and unwed mothers and assist current teen and unwed mothers

Goal: Reduce the number of teen and unwed mothers in Branch County and to offer services that can help guide teen and single mothers through parenthood.

Recommendations: The community currently has some programs in place offered through the Branch-Hillsdale-St. Joseph Health Agency (BHSJHA) however, the community needs more programing in this area due to the high numbers of teen and unwed mothers.

Improve reproductive health education within the communities Work with the (BHSJHA) and school boards to reinstitute the Life Skill program within

the schools Develop a peer mentoring program with older teen and younger teens regarding

experiences

Priority Issue 3: More community involvement in suicide prevention

Goal: To increase recognition of signs of suicide and reduce the number of suicides

Recommendations: Programs are in place for residents seeking help for issues that may lead to suicide. CHCBC along with other community agencies want to help educate the community in the signs of a suicidal person and to make them aware that it is ok to seek help.

Work with schools and employers to develop a strategy that will help the community understand the services that are available to them and that it is ok to ask for help

Develop a suicide awareness campaign that will help residents understand the signs of a person who may consider suicide and what they can do to help

Priority Issue 4: Parenting skills and family planning services for low income families

Goal: To increase access to family planning services and improve parenting skills for Branch County parents.

Recommendations: Currently there are parenting and family planning services through various community groups. The challenge for the community currently is to know what programs are available and where they are.

To assess what parenting skills and family planning services are currently available and develop a master list available to the public.

Promote existing resources and programs within the community. Develop a mentoring program or community health worker to assist new young moms

through pregnancy and the child’s first year. Increase the availability of evidence based teen self-esteem programming within the

schools, church and/or community settings that will foster positive teen self-image and future goal setting mind-sets.

Priority Issue 5: Address obesity issues with adults and minors, especially by educating residents of the importance of healthy eating and exercising.

Goal: To help prevent long term healthcare issues by helping reduce obesity among residents and help encourage and educate them in healthy eating and exercise.

Recommendations: The family doctors in the county do a good job of watching the weight and health of their patients. Branch County also has many sports programs, 5-k runs and walking trails that all encourage exercise in our community. Branch County also has many health clubs and recreation facilities including the construction of the Browne Aquatic and Recreational Center.

To help make residents aware of the all the long-term health complications involved in living an unhealthy life.

Continue the education already in the schools and in the health clinics. Work with the Branch Area Career Center and the City of Coldwater Recreation

Department in offering classes focused on nutrition. Train members of the public to teach health and nutrition classes free of charge to the

public at CHCBC. Help the Branch Area Food Pantry improve food quality with more fruits and vegetables

and less unhealthy snack foods.

Priority Issue 6: Establish a Senior Citizen Day Care

Goal: To establish a place that senior citizens that are alone all day can go to be taken care of by staff and to participate in activities.

Recommendations: Funding and capacity is not available at this time for a program at CHCBC. This issue will be forwarded to the Burnside Senior Center for potential action.

Priority Issue 7: Establish a program for childhood mental health issues

Goal: for children to have a mental health counselor available to them that specializes in mental health for children.

Recommendations: The community schools do have social workers in the elementary schools to help address any mental health needs early on. Coldwater is also changing their Alternative education system to focus more on mental health needs of their teenage students.

Funding and capacity is not available for a full-time mental health counselor in the county. This issue will be forwarded to Pines Behavioral Health for potential action.
