• Monday-Friday: 8 a.m. - 4 p.m. SUNY Potsdam Student Health Services (SHS) must ensure all new, re-admitted and transfer and non-matriculated students meet state and local health requirements. The submission of these forms is tracked for each student. Failing to comply with the health requirements will result in progressive consequences that include being dis-enrolled from SUNY Potsdam, as NY State Law demands. To avoid being dis-enrolled, all students must please follows these steps:

1. Have your medical provider send a SIGNED copy of your measles, mumps, and rubella vaccines to Student Health Services (fax to: 315-267-3260).

a. Set your web browser to accept “pop-ups” from shsweb.potsdam.edu b. Log in to BearPAWS using your campus computer account username and password. c. Open the “New Student Services” link. d. Click on “Student Health Services Portal.” e. Complete all forms in “Required Health Information” and fill out the “Insurance” link.

Special Notice: insurance requirement: All matriculated students taking 12 or more credit hours are required to have health insurance coverage. See the “Student Health Insurance Plan” webpage for further details. Send in your health information right now, while you’re thinking about it. Students who arrive on campus without sending it are subject to progressive consequences and may have to be removed from campus to comply with NY State law. Therefore, each student is monitored carefully to ensure they’ve met the requirements. The information you submit is kept in your confidential medical chart and is only accessible to the staff at Student Health Services. More information regarding the health requirements, including what constitutes “proof” of immunization, is available on the “Health Requirements for Attending” page of the Student Health Services website. If you have questions, or if you experience any technical difficulties, please call Student Health Services.

Vehicle Registration: Parking Permits 4

University Police Van Housen Hall Extension, First Floor (315) 267-2222 www.potsdam.edu/police For each new academic year, any student who has a vehicle on campus will be required to have a current campus parking decal displayed on their vehicle. Any vehicle found parked on the campus without a current parking decal will be issued a campus parking ticket, which carries a $15.00 fine. Temporary permits are available at the University Police Department, 24-hours per day. Student Vehicle Registration: Must be done online through BearPAWS. To register your vehicle:

• Log into BearPAWS • Select the “College Life and Auxiliary Services” tab • Select “Student Vehicle Registration” - you will need information about your vehicle, including

plate number, make, model and color. If you live off campus, you must provide your local off-campus address. All students are asked to please provide their cell phone numbers, if applicable.

There is a vehicle registration and parking fee, based on your student status (part time/full time etc.) and the area for parking that you select. The fee will be applied to your student account. If you want to register more than one vehicle, the second vehicle will cost $10 and must be done in person at University Police with proper identification showing that the second vehicle is owned by you or a family member. The parking decal for your vehicle will be mailed to your campus box or local address (if you do not have a campus box) or held at University Police for pick-up, depending upon your selection. For parking information, including the parking lots designated for your type of permit, please visit our website. Student Vehicle Registration Fees (subject to change): Full Time: Fee: $105.00/Academic Year* Part Time: Fee: $57.50/Academic Year* Student Teachers: Fee: $80.00/Academic Year* Summer Students Fee: $30.00/Summer** Lot 31 ONLY: Fee: $70.00/Academic Year* Summer Lot 31 ONLY: Fee: $20.00/Summer** Motorcycle permits are also available (See University Police for details) *Academic year - September through May (Decals expire on Commencement Day in May) **Summer - May through August (Decals expire on the first day of the Fall Semester) All parking fees will be deferred to your college bill, unless you choose to pay by credit card or in-person at University Police. They accept Visa, MasterCard, American Express & Discover. Remember that ALL vehicles parked on the SUNY Potsdam campus MUST have a valid parking permit displayed. If you permanently change vehicles during the year, return your original decal and University Police will replace it for free. If you temporarily drive a different vehicle than what you have registered, you must obtain a temporary parking permit for the borrowed vehicle.

PACES Office First Floor, Merritt Hall (315) 267-2658 http://www.potsdam.edu/offices/paces/index.cfm SUNYCard The campus ID card, also known as the SUNYCard, provides access to meal plans, campus buildings, library materials, Bear Express, and other campus services. Students can check meal account balances, sign up for low balance notifications and upload and submit ID photos online at sunycard.potsdam.edu. You must show a valid government issued photo ID (passport, driver’s license) to pick up or be issued a SUNYCard. To deactivate a lost or stolen card, or to reactivate a card, log into BearPAWS. Bear Express Bear Express is an optional spending account accessed through the SUNYCard. It may be used on campus for services, in vending machines, and at participating off campus restaurants and businesses. Bear Express accounts are automatically opened for most residential students and appear on the college bill. When an account is opened, the amount selected is billed in both the fall and spring semesters. Changes or cancellation to the account must be made prior to the first Thursday of the first week of classes, via BearPAWS. Money may be added to the account at the PACES Business Office, at Dining Service locations or online through BearPAWS. Unused account balances roll over from semester to semester and remaining balances (over $20) may be credited to accounts at the end of the academic calendar year. Banking PACES, in partnership with North Country Savings Bank, provides two ATM’s on campus. One is located in the Barrington Student Union and the other is located in Carson Hall, adjacent to Becky’s Place. Eating on Campus and Meal Plans PACES operates eight restaurants on campus. Meal plans can be used at every dining operation on campus. Every restaurant accepts debit and credit cards. PACES operates one all you care to eat facility (Lehman Dining Hall), a food court with eight kiosks (Student Union Dining Court), five cafes (Crane Snack Bar, Minerva’s Café, Dexter’s Café, Becky’s Place, Performing Arts Café), a franchise operation (Tim Hortons). PACES manages a catering division, a concession stand at Maxcy Hall (The Stand), and a campus bakery/commissary. Kitchen Meal Plans are available to residential, non-residential and commuter students. Plans offer the best value, convenience, and cost savings for students who dine or buy grab and go items while on campus. Meal Plans are valid for one semester; they do not rollover and are non refundable. When an account is opened, the plan you select is billed in both the fall and spring semesters. Changes or cancellation to the account must be made prior to Thursday of the first week of classes. Student Mailroom Services All mail generated by the College, along with all mail delivered to the College by the United States Postal Service or express delivery services, will be distributed through the Student Mailroom. Mailbox Requirements Graduate students (if they live on campus). Other Graduate Students are able to request a mailbox in-person at the Barrington Mailroom window. For non-campus residents, mailbox issuance will be based solely on box availability. Mailing Address For on-campus residents, prior to your entry semester, the Student Mailroom will assign all qualified a campus mailbox. This will then create your official SUNY Potsdam mailing address. For example, if one is assigned to mailbox number #, then the mailing address will be as follows: Name SUNY Potsdam # Barrington Drive Potsdam, New York 13676 Upon the completion of your course registration, you will be assigned a campus mailbox. In August, the Student Mailroom will release your specific mailbox assignment to you. You will receive an e-mail notification, sent to your SUNY Potsdam e-mail address, which will share the specific address details. Your mailing address will remain the same for the duration of your studies at SUNY Potsdam. This mailing address will be used by others (both on campus and off campus) to contact you. When you receive your mailing address, be sure to share it with your family, friends and any company, group or agency whom you anticipate receiving mail or packages. Mailbox Activation When you arrive on campus, you will need to go to the Student Mailroom, located on the first floor of the Student Union to sign out your mailbox key. Please note the mailroom service hours above to complete this process. Your mailbox should be checked regularly to ensure that you receive any important correspondence from the College in a timely manner.

Any graduate students wanting to live on campus must submit a housing request through an on-line process found below. Prior to making your housing preferences, please refer to our website at potsdam.edu/studentlife/reshalls for more information about your options. Since there is no designated graduate student housing, room assignments are made on a first come, first served basis. Due to limited space, we advise you to submit housing requests as soon as possible (recommended deadlines of July 15th for the fall, January 1st for the spring, and April 15th for the summer).

For those who plan to live on campus:

• Log into your BearPAWS account. • Go to the “New Student” link. (Please note: if you start here with Summer Session, you must go to

“College Life and Auxiliary Services” to request housing for the fall semester, as you are no longer considered “new”.)

• Go to the “New Student Housing Registration” link. • Indicate in the space provided that you want on campus housing. • Read & agree to “Housing License”. • Submit your preference for where you want your room. • If you have a roommate request, enter your desired roommate’s Potsdam ID number in the field

provided. Your desired roommate must enter your Potsdam ID number on their form as well. • Most on-campus living options mandate that you must have a meal plan. • If you have any physical conditions or concerns that impact your housing needs, please see the

Medical Housing Process and Forms on our web page. • Room and roommate assignments are viewable in BearPAWS beginning August 1.

Office of Student Accounts 412 Raymond Hall (315) 267-2137 [email protected] www.potsdam.edu/offices/studentaccounts The Student Accounts Office coordinates the billing process, handles all payments made toward students' accounts, manages the monthly payment plan, processes financial aid payments, disburses all refunds from over-payments, calculates refunds due to withdrawals and performs other miscellaneous financial functions. Estimated Cost of Attendance http://www.potsdam.edu/admissions/financial/graduatecoa.cfm Please note that this form is for general estimation purposes only and all expenses are subject to change. Aid appears as a credit on the bill only after it has been certified/confirmed by Financial Aid. Bills The Student Accounts Office issues three types of bills – semester bills, monthly bills and special semester bills are issued prior to the start of the semester. Students who have fulfilled all their obligations, such as filing the FAFSA, accepting an aid package, etc., will show credits reflecting their financial aid. Students who make changes to their account (such as meal plan changes, accept/decline optional fees and have changes to financial aid amounts) after the semester bill is issued will not receive notification that the balance due has changed. Students should review their on-line billing statement weekly in BearPAWS to monitor changes to their account. Student Billing Statements are available online 24/7 through your BearPAWS account. To view the updated and most current bill, students must sign in to BearPAWS:

Web Registration: Once you have activated your Campus Computer Account (CCA), follow these simple instructions to web register: Log-in to BearPAWS

1. http://BearPAWS.potsdam.edu 2. Click "Current Students". 3. Click "BearPAWS".

Register for Classes 1. Click "Student Services and Financial Aid". 2. Click "Registration". 3. Click "Add/Drop Classes". 4. Check to see that you have selected the correct semester. 5. Click "Submit Term".

Add Classes to Your Schedule 1. Use the Class Search option to identify the courses you wish to add. 2. Enter the CRN's (5 digit code number) for each class in the "Add Class" box (at bottom of page). 3. Click "Submit Changes". 4. Scroll down to see your schedule and check for Registration Errors.

BearPAWS Web Registration Error Messages Remember, courses with Web Registration Errors are not added to your schedule. You must request an override from the instructor or Department Chair if "Staff" is listed as Instructor.

• Instructor Approval - You need written permission from the instructor to register for this section. Submit the permission form to Graduate Studies.

• Prerequisite Required - You need the prerequisite for the class. • Closed Section - The section you are requesting is filled. • Link Error Required - You need to be registered for a related section of the class. Select the two

sections at the same time. • Time Conflict with ____ - You have registered for another class at this time. • Major Restriction - You need to be a major in the department to take this class. • Class Restriction - You need to be in the appropriate class (senior, graduate, etc.) to take this

M T W R F S Please note that all days are represented with a single capital letter. Since Tuesday is represented with a capital “T”, it is important to note that Thursday is represented with a capital “R”.

Print your Schedule 1. Click "Student Schedule by Day and Time” (at bottom of page) - for Bookstore Orders, click

"Schedule Details". 2. Click "Print" from the web browser. Pick up schedule from printer!

Drop Classes from Your Schedule 1. In your current schedule, click the "Web Dropped" option under the "Action" column for the class. 2. Click "Submit Changes". 3. When finished, close the browser window!

Adding Courses through Graduate Studies (Sattlerlee Hall 238) Complete an add/drop form, available on the CGS “Find a Form” website, or by stopping at the Center for Graduate Studies (238 Satterlee Hall). Please note that adding a class after classes begin is possible during the add/drop period with a signature from the instructor on an add/drop form. Bring the add/drop form to the Center for Graduate Studies for processing. The deadline for adding classes is 4:30 p.m. on the last day of the add/drop period. Please refer to the Academic Calendar for dates. Dropping Courses Students may drop a course in two ways, either online through BearPAWS or directly through Graduate Studies by completing an add/drop form. Students can only drop courses, online or in-person, during the add/drop period. The online drop option on BearPAWS will be shut off at midnight on the last day of the add/drop period. Please refer to the Academic Calendar for dates. Sample Schedule of Classes

Viewing the required textbook list for your classes Step 1: Go to http://bookstore.potsdam.edu. Step 2: Mouse over the “textbooks” box at the top of the page. Step 3: Click on “textbooks" in the dropdown menu. Step 4: Click on “Continue to Textbook Selection” at the bottom of the page. Step 5: Use the drop down menus to create your course list. You will need to select the term, the department, the course and the section number. Click on “add course” and your class schedule will be displayed in the box under “your current course list”. Step 6: Click on “Get Course Materials”. Step 7: Click on the books you want to view or purchase and follow the directions on the screen. If you submit an order, you will receive two emails. The first email is an order confirmation that will list what you requested. It will calculate your bill using all NEW prices. Once the order is filled, you will receive another email and it will give you a final price determined by the condition of the books we have in stock (New, Used, Rental, or e-book) and what you received. Please be aware that orders are not filled until two weeks prior to the start of the semester to account for changes in the book list. If you place your order significantly prior to the beginning of the semester, there will be a delay between emails. Helpful Tips:

Graduate courses are offered at the Extended Learning Building (Building E) of the JCC campus, but there are other services available to all students throughout the JCC campus such as: “smart” classrooms, copying services, conference/meeting spaces, videoconferencing facilities, JCC’s Fitness Center, student access to open computer labs, food service and vending machines, free parking & student lounges. Academic Integrity, Procedures, Due Process & Student Rights

Basic Standards of Academic Integrity Specific acts that are considered to be academic dishonesty and that are prohibited by this Code include, but are not limited to:

1. Cheating: using unauthorized notes, study aids or information on an examination; altering graded work after it has been returned, then submitting the work for re-grading; allowing another person to do one's work and submitting that work under one's own name.

2. Plagiarism: presenting as one's own the distinctive ideas, facts or words of another (in part or in whole) without appropriate acknowledgement of the source. Issues of plagiarism apply to any type of work including, but not limited to exams, papers, any writing or printed text, computer programs, websites, art, music, photography or video.

3. Fabrication: falsifying or inventing any data, citation, or information. a) Citation: Any attribution to, or use of, a source (real or invented) from which the referenced material was not obtained, including use of a quoted reference from a non-original source while implying reference to the original source. b) Data: Presenting data that were not gathered in accordance with standard deadlines defining appropriate methods for data collecting; generating data and failing to include an accurate account of the method by which the data were gathered or collected; presenting data that have been altered or contrived in such a way as to be deliberately misleading. c) Information: Providing false information in connection with any inquiry regarding academic dishonesty.

4. Multiple submission: submitting identical or substantial portions of similar work for credit more than once, without prior explicit consent of the course instructor(s) to whom the material is being or has been submitted.

5. Obtaining or providing an unfair advantage: a) Gaining or providing access to examination materials prior to the time authorized by the instructor. b) Stealing, destroying, defacing or concealing library materials or other shared-use materials. c) Providing materials, information or other assistance on an academic assignment without authorization from the instructor(s) d) Gaining or providing access to previously given examination materials, where those materials clearly indicate that they are to be returned to the instructor at the conclusion of the examination. e) Intentionally obstructing or interfering with another student's academic work. f) Assisting others in the violation of this Honor Code

6. Falsification of records and official documents: altering documents affecting academic records, forging signatures of authorization or falsifying information on an official academic document, grade report, letter of permission, petition, drop/add form, ID card, attendance list or any other official College document.

Procedures, Due Process and Student Rights SUNY Potsdam's procedure for the enforcement of the Academic Honor Code lies within the campus judicial system. The College judicial procedures and students' rights within these procedures are listed in the "Code of Student Rights, Responsibilities and Conduct". The Campus Judicial System in consultation with the course instructor(s) will assign appropriate sanctions should it be determined that a student is responsible for a violation of the Academic Honor Code. (Note: the information and definitions listed in this Honor Code were adapted from The SUNY Potsdam Academic integrity Policy, The Northwestern University Principles Regarding Academic Integrity and The Rice University Honor Council.

Self-Disclosing Statement SUNY Potsdam is an academic community committed to the principles of civility and fairness in pursuit of a purposeful academic experience. The provisions that follow shall govern the students of SUNY Potsdam as members of this institutional community.

• The SUNY Potsdam Code shall apply to conduct that occurs on College premises, at College sponsored activities, and to off-campus conduct that adversely affects the College community and/or the pursuit of its objectives.

• Each student shall be responsible for his or her conduct from the time of application for admission through the actual awarding of a degree, even though conduct may occur before classes begin or after classes end, as well as during the academic year and during periods between terms of actual enrollment, even if his/her conduct is not discovered until after a degree is awarded.

• The Code shall apply to a student’s conduct even if the student withdraws from school while a disciplinary matter is pending.

• If a student is involved in an incident outside of the Potsdam Community while representing SUNY Potsdam (ex. Student Teaching, conference attendee, etc…), the student is expected to contact the Department Chair or Program Coordinator of any charges, arrests and/or convictions immediately.

SUNY Potsdam Academic Honor Code Throughout their history, Institutions of higher learning have viewed themselves and have been viewed by society as a community of persons not only seeking the truth and knowledge, but seeking them in a truthful and ethical fashion. Indeed, the institution traditionally trusted by the public and the one to which individuals most often turn for unbiased factual information is the university. Thus, how a university behaves is as important as what it explores and teaches. SUNY College at Potsdam expects all members of its community to conduct themselves in a manner befitting this tradition of honor and integrity. Enrollment at the College requires a commitment to the principles of the SUNY Potsdam Academic Honor Code both in spirit and in adherence to rules and policies. The Academic Honor Code makes SUNY Potsdam a better and more enjoyable place as it affords to each member of the SUNY Potsdam community the trust and freedom that honesty promotes. Personal honor, integrity and respect for the word and work of another are the basis of the Academic Honor Code. Thus, it is the responsibility of every student enrolled in SUNY Potsdam to adhere to and uphold this Code in pursuit of academic integrity. The Academic Honor Code furthers this effort by prohibiting lying, cheating and stealing. This Code applies to all academic programs, faculties and departments at the College both graduate and undergraduate. More detailed standards of academic conduct may be set forth by each of the schools and individual faculty members. It is the responsibility of every member of the academic community to be familiar with and supportive of the Academic Honor Code. Academic Honesty Pledge On all academic work done by students at SUNY Potsdam, the below listed pledge is either required or implied. Furthermore, this statement is regarded as an indication that the student understands and has complied with the requirements and assignments as set forth by the course instructor and as stated in this Academic Honor Code. "On my honor: I will not give nor receive any inappropriate assistance on any academic work in accordance with the SUNY Potsdam Academic Honor Code and the directions given to me by each course instructor."
