1 SCOPE AND PURPOSE

This document is the Emergency Procedure and Incident Response Plan applicable for the Construction project (associated to the HEALTH & SAFETY PLAN).

This document identifies the most common hazards that can occur on the

construction site, the likelihood of their occurrence, the possible impact on

the property and the estimated impact on health and safety.

It defines the general provisions taken to cope with emergencies and it

provides also detailed guidelines designed to better cope with natural,

industrial, or medical emergencies that can take place during the

construction, erection and commissioning phases.

The guidelines describe the actions required by the personnel of the

Contractor, Contractors, Sub-Contractors, and visitors in the event of a listed

emergency.

2 DEFINITIONS

A relatively insignificant event that might have serious consequences.

2 Accident:

An unforeseen event or one without an apparent cause, anything that

occurs unintentionally, a misfortune or mishap, especially one causing

injury or death.

3 Hazard:

Source or situation with a potential for harm in terms of injury or ill health,

damage to property, damage to the workplace, the environment, or a

combination of these.

4 Hazard Identification:

Process of recognizing that a hazard exists and defining its

characteristics.

5 Emergency Situation:

A sudden occurrence, a combination of circumstances, a danger

demanding immediate remedy or action.

6 Medical Emergency:

Critical: Patient has symptoms consistent with a life-threatening illness or

injury with a high probability of death if immediate action is not taken

7 Nominated Person: A person is nominated and entitled by the

_____________________________ company Construction Manager to carry

out emergency responses and ensure the safety of the personnel during

an Emergency situation. The Nominated Persons are designated with

reference to their knowledge and training (First-aid, fire fighting, etc). A

list of the Nominated Persons with their assigned mission in case of

emergency situations is established and updated when necessary.

3 RESPONSIBILITIES

3.1 General organization

The Emergency Procedure and Response Plan are based on separate

organizations, medical facilities and structures, with a cooperation and

coordination unit: of the Emergency Response Team (ERT).

Each Construction Contractor shall define and maintain its own organization

and coordinate each other, especially in case of large emergency.

3.2 The Construction Manager

The Head Contractor Construction Manager serves as the “Emergency

Director” and is responsible for:

a) Approving the list of nominated persons - E.R.T. - (Emergency Response

Team)

b) Ensuring that the Emergency Procedures is issued and updated if

necessary,

c) Protecting the health and safety of all personnel, employees during

emergency and accounting for all by conducting an assembly roll

call,

d) Authorizing the site-wide evacuation of personnel, as needed.

e) Authorizing assistance from off-site emergency service organizations

(e.g., ambulance companies, hospitals, health departments, fire

brigade, law enforcement agencies

- Ensuring that a written report about the emergency situation has been

drawn up.

3.3 Safety In charge

The Safety Incharge shall:

a) Identify, evaluate, natural and industrial hazards and determine the

likelihood of their occurrence, their impact on health and safety and

on property,

b) Ensuring that all personnel, employees, contractors and

subcontractors have been provided with an initial site safety briefing

and to communicate the nature, level and degree of hazards

expected on site,

c) Ensuring that first-aid training and qualification for E.R.T., H.C.

Supervisors and Contractors & Sub-Contractors foremen involved on

site activities have been provided,

d) Ensuring that site personnel are aware and acquainted with all

applicable emergency response guidelines and receive regular

briefings when significant changes are made in the emergency

procedures, safety plan(s) or the guidelines,

3.4 Contractors Construction Managers

The Contractors Project and/or Construction Managers are responsible for the

Safety at work areas where they are executing the works, according to their

own Health & Safety Plan and/or Emergency Response Plans which will be

issued before the beginning of works and updated as required.

They shall keep an injury and accident logbook for statistics.

3.5 First aid team

The project requires the establishment of first aid centre :

3.6 The Emergency Response Team (E.R.T.)

The Emergency Response Team includes people listed in Appendix 1.

In case of Emergency, the E.R.T. personnel are notified via communication

devices (mobile and/or fixed telephone, Walki/Talkie, etc., see appendix 1)

and shall immediately go to the scene of an emergency in order to assess the

situation and direct necessary immediate protective actions.

If the E.R.T. is unable to safely access the scene, they shall assemble at any

other suitable location to determine where they will establish the “command

post”.

3.7 Duties of Safety Personnel Not Engaged in ERT

Safety personnel not attached to the ERT will, depending on the place and

the nature of emergency, take the following action:

a) Ensure that all personnel in the respective areas have started to

evacuate,

b) If required, direct personnel in the safe direction up to the assigned

Evacuation Assembly Area,

c) Check for potentially dangerous situations directly or indirectly incurred

by the evacuation process. Report these incidents immediately to

Safety Officers so that necessary corrective actions shall be taken to

safe the situation prior to return to work.

4 COMMON HAZARDS

A full analyse of all common hazards that may occur on sites or camps of this

project during the work phase has been conducted in order to define the

main and most important of them, and to classify them in terms of likelihood

of occurrence, estimation of the impacts that they could bring on health and

safety or on property.

For those of these events which have been classified with a most likely

occurrence and which present major impacts on both health & safety and

on property, the issuing of Emergency Response Guidelines prior to their

potential arising will facilitate the organization of the tasks and duties of the

ERT. Common Emergencies are:

5 EMERGENCY RESPONSE AND GUIDELINES

5.1 Reporting and first steps procedure

Any person witnessing an emergency situation or an unusual occurrence

shall immediately call the E.R.T., i.e. :

The E.R.T. shall immediately report to the area / building identified to assess

the situation and direct necessary protective actions.

Upon evacuation, the nominated person or next senior employee of each

Evacuation Assembly Area defined by the Construction Contractor Manager

shall perform accountability. This shall be performed quickly and the results

shall be reported to the E.R.T.

5.2 Emergency response guidelines

The purpose of these guidelines is to establish standard procedures to follow in

the event of the most likely and hazardous situations determined by the

above analysis:

A. Medical emergency

B. Fire / Major structural fire

A.Medical Emergency

The purpose of this guideline is to establish a standard procedure to follow in

the event a medical emergency occurs on the construction site. In the event

of such an accident, the following procedures will be enacted:

1. A person witnessing a medical emergency on site will immediately call the E.R.T. :

The witness will not attempt to move seriously injured personnel, but, after

calling for Help:

a. Check if the accident scene is safe

b. Provide first aid, only if trained so

c. Wait for instructions from Medical Team (ambulance to be activated

only on Doctor’s instructions;

d. Later on, interview all other witnesses, prepare appropriate reports.

2. Upon notification, the E.R.T. will :

a. Call immediately the corresponding Doctor,

b. Dispatch a nominated person to the location, to help the witness and

assess the situation if necessary.

5.3 The following list covers basic actions to take in an emergency. These steps

apply to almost any emergency and should be followed in sequence:

Ø Stay calm

Ø Assess the situation

Ø Take command

Ø Provide protection

Ø Aid and manage

Ø Maintain contacts

Ø Guide emergency services

Stay calm- your example can influence others and thereby aid the

emergency response.

Assess the situation- Determine what happened and what the emergency is.

Look at the big picture. What has happened to whom and what will continue

to happen if no action is taken? Try to identify the cause that must be

controlled to eliminate immediate, ongoing, or further danger.

Take command- the most senior person on the seen should take charge and

call, or delegate someone to call, emergency services. And explain the

situation. Assign tasks for controlling the emergency. This action also helps to

maintain order and prevent panic.

Provide protection- Eliminate further losses and safeguard the area. Control

the energy source causing the emergency. Protect victims, equipment,

materials, environment and accident scene from continuing damage or

further hazards. Divert traffic, suppress fire, prevent objects from falling, shut

down equipment or utilizes and take other necessary measure. Preserve the

accident scene; only disturb what is essential to maintain life or relive human

suffering and prevent immediate or further losses.

Aid and manage- provide first aid or help those already doing so. Manage at

the scene. Organize the workforce for both a headcount and emergency

assignments. Direct all workers to a safe location. This makes it easier to

identify the missing, control panic, and assign people to emergency duties.

Maintain contact- Keep emergency services informed of situation. Contact

utilities such as gas where required. Alert management and keep them

informed. Exercise increasing control over the emergency until immediate

hazards are controlled or eliminated and cause can be identified.

Guide emergency services- Meet services on site. Lead them to emergency

scene. Explain ongoing and potential hazards and cause(s), if known.

COMMUNICATION OF PROCEDURE :

To be effective, an emergency Response Procedure must be clearly

communicated to all site personnel. The following activities should be

considered.

Ø Review the procedure with new site subcontractors and new workers to

ensure that it covers their activities adequately.

Ø Review new work areas in operating plants to ensure that new hazards are

identified and covered in the procedure.

Ø Review the procedure with the Environment Health and Safety

representatives on a regular basis to address new hazards or significant

changes in site conditions.

I. COLLAPSE OF EARTH:

In case collapse of Earth the following action to be taken immediately.

1. Asses the situation quickly. Arrange to collect all the persons at the

Central Assembly Area and take head count.

2. Find out the number of persons missing.

3. Call for First Aid personnel available at the site along with First Aid Box.

4. Ascertain if any workmen are completely trapped under the collapsed

earth or inside or in between scaffolding materials.

5. Place barricading and caution tape around the accident site.

6. If any workman is completely trapped inside, take the following steps.

Ø Carefully remove the material around the trapped person and if

necessary use hand shovel with utmost care.

Ø Try to guide the trapped to come out. Never try to pull out the trapped

person by forceful means.

Ø Give First Aid first.

Ø Arrange for shifting the injured personnel to the First Aid clinic.

Ø Contact nearest Hospital and shift the injured for further treatment.

II COLLAPSE OF SCAFFOLD/SHUTTERING:

In case collapse of Scaffold/Shuttering the following action to be taken

immediately.

1. Assess the situation quickly. Arrange to collect all the persons at the

Central Assembly Area and take head count.

2. Find out the number of persons missing.

3. Call for First Aid personnel available at the site along with First Aid Box.

4. Ascertain if any workmen are completely trapped under the

scaffolding materials.

5. Place barricading and caution tape around the accident site.

6. If any workman is completely trapped inside, take the following steps.

Ø Carefully remove the material around the trapped person.

Ø Try to guide the trapped to come out. Never try to pull out the trapped

person by forceful means.

Ø Give First Aid first.

Ø Arrange for shifting the injured personnel to the First Aid clinic.

Ø Contact nearest Hospital and shift the injured for further treatment.

III. FALL OF PERSON/MATERIAL FROM HEIGHT:

In the event of fall of a person/Material from height, the following actions

shall be taken by the Site Engineer/Site Supervisor.

1. Comfort him and assure confidence in him that he will be alright.

2. Remove the material around the person if any.

3. Enquire if he has any internal injury.

4. If any visible swelling or fractures is suspected, do not shift or move the

person.

5. Arrange for medical stretcher to move the person to the First Aid Clinic.

6. If bleeding injury is noticed, keep the injured part elevated from ground

and tie a bandage firmly.

7. Shift the person to First Aid Clinic and administer First Aid.

* If the injured is unconscious the following actions shall be taken

immediately by the First Aider:

Comfort him and assure confidence in him that he will be alright.

Check for any visible injuries.

Give him cardio pulmonary resuscitations by trained personnel.

After restoring normally, place the injured in recovery position.

Shift the person to First Aid Clinic and administer First Aid.

Arrange to shift the injured to the nearest Hospital at the earliest.

IV. OUTBREAK OF FIRE:

In the event of any fire being noticed at the site, the following actions shall

be taken.

a) The person noticing the fire shall try to put out if it is small and

controllable.

b) Shout Fire, Fire and alert others.

c) Rush to Fire Alarm point and ring the Bell/Alarm/Switch.

d) Switch off main electric power supply.

e) Collect all the personnel at the central Assembly Area and take a head

count.

f) If the fire is uncontrollable, inform all persons listed in the Emergency

Contact Telephone Numbers and tell them about the type of

emergency, place/exact location/contact person and telephone

number.

g) On hearing the Fire alarm, the Fire Fighting Team shall reach the

location immediately and try to put out the fire with the help of fire

fighting equipment which includes Fire Extinguisher, Sand bucket, The

members of the Fire Fighting Team shall wear suitable PPE like Helmet,

Goggles, Nose/Face mask, Leather Gloves and Safety Shoes.

h) The personnel shall be trained in proper and safe handling of fire

fighting equipment.

i) The area under fire shall be barricaded and no unauthorized personnel

shall be allowed to enter the area.

j) The First Aid Team shall be ready with the First Aid box to give necessary

First Aid to the casualties, if any.

k) The Male Nurse or one First Aider shall remain in First Aid Clinic to give

further treatment and to send the casualty to nearby Hospital for further

treatment.

l) Contact the Project Incharge/Fire Fighting Incharge to strengthening of

Fire Fighting Team, if required.

m) Make a clear way for Fire Fighting vehicle to approach the fire- zone.

n) Take directions from Safety Manager/Officer/Supervisors at the

Assembly Point, to put out fire.

The different types of fires and first aid fire fighting equipments suitable for use

on them are as under.

Major structural fire

The purpose of this guideline is to establish standard procedures for

responding to a fire affecting the construction site.

In the event of a fire, the following procedures will be taken :

1. Upon detection, the person discovering the fire is to communicate

immediately to the E.R.T.

2. Construction area supervisor may apply hand-held fire extinguishers and if

trained to do so and contacting the relevant managers, if there is

minimum risk to the personnel involved.

3. These persons will:

a. Contact the in-site and off-site Fire Brigade, if necessary,

b. Dispatch a patrol / nominated person to the location of the fire,

c. Notify the Medical Support Team (if any medical problem),

d. Notify all personnel when the fire response is terminated,

e. Issue report / log of events, as required.

4. The area supervisor will:

a. Conduct quick walkthrough of the area / facility to determine if any

assistance is necessary for evacuation of injured personnel,

b. Inspect all areas, offices and restrooms to ensure they have been

vacated,

c. Secure all sensitive documents (without threat for his life),

d. Upon their arrival, advice the Fire Brigade of location, nature and

identification of the hazardous materials on site,

e. Proceed to the designated Emergency Assembly Area,

f. Conduct roll call for each area of responsibility,

g. Await instructions by / maintain close communications with the

Emergency Response Team.

h. Inform personnel when it is safe to return to work.

V. ELECTRIC SHOCK:

On noticing a person affected by electric shock, the following actions shall

be taken by the personnel available nearby.

a) Isolate the source of supply.

b) Free the person from source of contact by the help of non-conducting

object. Do not try to pull the person with bear hands.

c) If it is high voltage, do not go too close to the injured person before

isolating source of power supply.

d) Check for airway, breathing and circulation of the affected person.

e) If needed, cardio pulmonary resuscitation shall be administered by

trained person.

f) If required, send a person for informing the authorities listed in the

Emergency Contact Numbers and ask for further medical assistance.

g) Do not leave the person, resuscitate till the victim is restored to normal

breathing.

h) Place him in a recovery position.

i) Shift him to the First Aid Clinic for further treatment.

VI. GENERAL EMERGENCY:

Following procedure shall be followed in case of general emergency

arising due to collapse of structure, lifting equipment Overturning of

vehicles or equipment and other natural calamities floods, earthquake

etc.

a) The person noticing the emergency situation shall ring the alarm and

announce on the Public address system, if available or else shout and

alert others.

b) Switch off main electric power supply.

c) Arrange to collect all the personnel at the central Assembly Area and

make a head count.

d) Inform all the personnel listed in the emergency contact Telephone

Numbers and tell them about type of emergency, place/exact

location/contact person and the telephone number.

e) On hearing the alarm, the Emergency Control Team shall wear PPE like

Helmets, Goggles, Face mask, Leather Gloves & Safety Shoes.

f) The area shall be barricaded and no unauthorized personnel shall be

allowed to enter the area.

g) Search and rescue team shall check all the work spots in the near

vicinity and ensure no person is trapped inside. If any person is trapped,

he shall be rescued or guided to come out to a safer area.

h) The First Aid Team shall be ready with the First Aid box to give further

treatment and to send the casualty to nearby Hospital for further

treatment.

i) Contact the Project Incharge/Fire Fighting Incharge to strengthening of

Fire Fighting Team, if required.

j) Make a clear way for rescue vehicles to approach the area for

evacuation of causalities.

Outbreak of fuel barrel while loading/unloading ( Heavy spillage of oil)

STEPS:

a) Asses the situation quickly and cordon off the area around Sillage.

b) If barrel is rolling try to stop at one place

c) Check any hot works are in progress, immedaitely stop the work

d) Try to place the barrel in vertical position

e) Keep flammable/Spark generating equipment's away from this area.

f) If possible use dust to clean the spillage of oil.

g) Keep fire fighting equipment's ready

h) Collect all the contaminaed earth if any.

i) Transfere the oil to the container to reduce further impact

j) Incase of any fire occurances, follow the procedure as laid in the out

break of fire.

k) Call ERT leader for Help.

3. objective:

Objective of this Project EHS Plan is to highlight the steps to be taken to minimize

environmental damage, occupational health risks and prevent incidents at project site.

Brigade Enterprises Ltd. strongly believes that all incidents are preventable and always

tries to achieve Zero accident rates.

4. Scope:

Scope of this Project EHS Plan is to cover all the activities pertaining to Environmental

protection, Occupational Health and Safety.

5. Authorities & Responsibilities:

EHS is a Line Responsibility. This means that everyone from the senior-most level to the

junior level from both BEL and contractors, are responsible for protection of Environment,

Occupational Health and Safety.

5.1 Project Incharge/Project Managers:

Project In charge/Project Engineers who direct and control the personnel at sites are

primarily responsible for the Safety and Health of all the personnel under their command.

They shall;

5.1. 1 Ensure effective implementation of EHS Management Systems in their respective

Site.

5.1.2 Propagate EHS Management system awareness to all the personnel in the site.

5.1 3 Co-ordinate with the MR in preparation of EHS Procedure Manual and other

related documents for the department/sites.

5.1.4 Carry out hazard identification and risk assessment; ensure control measures on

the significant risks, work spot inspection on day to day basis.

5.1.5 Identify the Aspect/Impact and ensure control measures on the identified

significant impacts.

5.1.6 Set Objectives and Targets, management programme for achieving the set

objectives.

5.1.7 Review the progress of EHS implementation periodically in the Sites.

8. Incident Recording and Investigation:

All incidents associated with the work involving personal injury, medical treatment,

near miss should be recorded and investigated. The First Information shall be sent to

the MR immediately on occurrence and informed by telephone (within 4 hr.) and

later detail incident investigation report in the prescribed format.

Subsequently a detailed investigation shall be carriedout. This shall include the following.

8.3 Sketches of the incident site.

8.4 Root cause for the incident.

8.5 Lessons learnt.

8.6 Recommendations.

The Investigation Report as per the format Form No BG/FORMAT/EOHS/05(Rev 1)

The reasons for the incidents shall be analyzed and preventive/corrective actions

shall be implemented at site and monitor on the corrective actions.

9. Safety Statistics:

Monthly Safety Statistics shall be forwarded to Safety Incharge as per the format

enclosed. BG/FORMAT/EOHS/04(Rev 0)

10. Emergency Procedures:

There is the potential for a range of emergency situations to occur on-site in relation

to works. These situations need to be identified and specific emergency procedures

developed and made known.

The following information should be documented:

10.1 Overall emergency plan and structure.

10.2 Register of emergency equipment and locations, such as first aid equipment,

fire extinguishers,

10.3 Register of staff with current first aid qualifications

10.4 Arrangements and co-ordination with other work site occupants in the

event of an emergency

10.5 Register of external contacts such as Police, Fire Brigade, Emergency Rescue,

Mock drills for the emergencies shall be conducted quarterly once.

Responsibilities for the various personnel at site are practiced during mock drill.

11 Waste Management:

The waste generated in the construction sites as a result of the activities is identified in

the Aspect/Impact evaluation. Bearing in mind the legal requirements, an

Operational Control Procedure on Waste Management has been prepared. The

Operational Control Procedure on Waste Management shall be strictly followed. All

the waste generated like used or waste oil, used cotton waste, old batteries, plastics,

bio-medical waste, empty paint drums /brushes, used printer cartridges/ribbons etc.,

and shall be collected separately.

Waste oil, used cotton waste, bio-medical, plastic scrap, batteries, cartridges etc.

shall be disposed through authorized recyclers.

Types of fire extinguishers

Water extinguishers are suitable for class A (paper, wood etc.) fires, but not for

Class B, C and D fires such as burning liquids, electrical fires or reactive metal fires.

In these cases, the flames will be spread or the hazard made greater! Water mist

extinguishers are suitable for class A and C; see below.

Foam Extinguisher: suitable for Class A and Class B fires, with limited effectiveness

for class F fires. Not considered effective for class C fires, and dangerous if used

for electrically energized equipment.

Powder Extinguisher; These extinguishers are suitable for Class A, Class B Fires.

Carbon-Di-Oxide Extinguisher; For E Class Fires

18. Environment, Health And Safety Rules:

18.1 All new staff and workers to undergo induction before taking the work.

18.2 Always follow the Company’s EHS Policy and procedures.

18.3 Always use adequate and appropriate Personal Protective Equipments for the

task being executed.

18.4 Get permits before starting any job which requires Work permit.

18.5 No child labour shall be employed and no children are permitted to enter the

construction site.

18.6 All employees must take adequate safety measures to ensure that there are no

incidents at the work place.

18.7 No one shall run within the construction site unless in an emergency.

18.8 Defective equipments shall not be used.

18.9 Always report any incident regardless of any injury/damage.

18.10 Correct the unsafe situations at once, if it is not within your capability, report

the same to your supervisor/safety personnel.

18.11 No shortcuts shall be entertained in the execution works.

18.12 Do not use power tools and equipment until you have been properly

instructed in the safe work methods and become authorized to use them.

18.13 Be sure that all guards are in place. Do not remove, displace, damage, or

destroy any safety device or safeguard furnished or provided for use on the

job, nor interfere with the use thereof.

18.14 Always barricade all the cutouts, excavated pits and dangerous areas.

18.15 A good job is a clean job, and a clean job is the start of a safe job. So keep

your working area free from rubbish and debris.
