What is MedMar?
MedMar, Inc. is an Illinois Corporation that has applied to own & operate a medical
cannabis dispensary in the 44th ward located at 2843 N. Halsted.
Our mission is to provide patients with safe, reliable and affordable legal access to
medical cannabis.
Our vision is to be the most reputable and trusted medical cannabis dispensary pro-
viding our patients with a unprecedented level of care.
Our active owners consist of medical professionals including doctors, pharmacists,
and nurses, as well as former law enforcement officials.

Background Information & State Law
The Compassionate Use of Medical Cannabis Pilot Program Actlegislation was passed bythe Illinois General Assembly and was signed by Governor Quinn on August 1, 2013
4 year pilot program began January 1, 2014
60 dispensary licenses are allocated statewide, including 2 for Lake View Township
Eligible patients must be granted a state license, by having a documented diagnosis of one of
a specific set of ailments as specified by the Illinois Department of Public Health, including:
HIV
Glaucoma
Multiple Sclerosis
Alzheimers Disease
ALS
Epilepsy

Boundaries are North Avenue on the south,

August 8th Dispensary Application issued by the state

Patient Product Offering
Our dispensary will offer cannabis, edibles, as well as related products that are routinely
used by patients in the everyday storage & consumption of cannabis.
Our goal is to provide our patients with the highest quality and most appropriate product
available for their specific medical needs.
All cannabis products whether edibles, oils, inhalants, ointments or other forms, will be dis-
pensed safely and in child-proof packaging.
Our team will secure relationships with the very best cultivation centers in Illinois ensuring
that our Lake View facility will satisfy the discerning needs of area residents.
Our staff will be engaged in ongoing training and education as it relates to the types of
products that will be dispensed and how they can best serve patients and their symptoms.

Security
Our facility will provide a heightened level of security that will offer employees and patientsa safe working environment.
The interior and exterior of the property will be under surveillance by CCTV and monitored
by security staff ensuring oversight of the entire facility
Product deliveries will take place in a secured delivery bay that is accessible from the alley
and will minimize any risk of criminal activity.
The layout of the store interior will prevent anyone who is not a patient or employee from
entering any area where cannabis will be present
MedMar will utilize an industry leading software called BioTrack to monitor inventory and
track all product sales.
Our inventory records will be available for instant electronic audit by Illinois State Police asrequired by law.

Patient Drop-offAt the dispensary, the general publicwill not have access to any area
where cannabis is dispensed or
handled. All persons will enter the
facility and walk into a general waiting
area where all persons will be greeted
by a security officer.
Initial Security Check PointThe security officer will not permit
anyone to remain on the premises that
is not engaging in activity permitted by
the act. The area will be monitored by
video surveillance. The officer charged
with greeting each person that enters
the waiting area will verify patient and/
or caregiver identification.
Primary Security Check PointOnce verification is complete, the securityofficer will buzz the patient or caregiver
through the initial security door and into
the secure holding area. Inside the secure
holding area, a video camera will record
the face of the patient or caregiver. After
the initial security door closes, a second
security door will be opened by a patient
counselor who will greet the patient and
bring them into the limited access area.
Limited Access AreaPatients will only be allowed into thelimited access area when a patient
counselor is available to assist them
and only after approval by the security
officer. At a maximum, four patients
will be allowed into the limited access
area with four company patient
counselors. After the patient interaction
is complete, the patient counselor will
escort the patient back to the security
door where they will be allowed to
leave under the surveillance of thesecurity o fficer.
Product DispensingPatients will walk up to a counter where they
will consult with the staff regarding the type of
product to be purchased and quantity. If there
is the need for more in-depth consultation,
the patient will have the option of meeting
privately in one of the two consultation offices.
Waiting AreaThis area will be used by patient caregivers
and those who are not licensed patients.
This area will also provide information for
prospective patients and their caregivers and
allow a space for them to interact and consult
with the dispensary staff.
Consultation OfficesThere are two private offices
that can be used for private
consultation with a registerednurse or licensed pharmacist for

Caregiver Pathway

Secure Delivery BayUpon arrival at the cultivation center, the
cultivation delivery team will be met by the Agent-
In-Charge or patient consultant and the security
officer. No delivery shall take place unless two
employees are present to accept the delivery. The
security officer and patient consultant or Agent-
In-Charge shall escort the delivery team into the
secure delivery area. The entire process will be
recorded on video surveillance.
Restricted AccessThe restricted access area will be limited
to the Companys employees that are either
a licensed pharmacist, licensed pharmacy
technician, registered nurse, or security
officer. Any employee that attempts to enter
the restricted access area will need to scan
their security badge and their entrance to
the restricted access area will be monitored
and overseen by security personnel.

Secure Delivery Room. Inside the secure delivery room, the
dispensary employee will verify that
each item from the cultivation center has
been delivered per the shipping manifest
sent the prior day from the cultivation
center as verified by the BioTrack
THC dispensary order manifest. The
dispensary employee will check each
item off on the shipping manifest.
