Metodi:
• 10 pazienti Non-ischemici (69±9, 6 maschi, NYHA class II o III, QRS 173±20 ms, LVFE 27±5%)
• In tutte le configurazioni Biventricolari ed MPP testate è stato valutato il pattern di attivazione
Conclusioni
MPP, rispetto alla stimolazione BIV, aumenta il LV dP/dtmax (30±13 vs. 25±11%; p=0.041), riduce ilQRS (22%±11 vs. 11%±11; p=0.01) e diminuisce il tempo di attivazione totale (25±15 vs. 10±20%; p=0.01). MPP fa contrarre una parte maggiore del Ventricolo sinistro nei primi 25 ms (35±22 vs. 16±8%; p=0.005) e nei 50 ms dopo la stimolazione (78±27 vs. 60±23%, p=0.03), suggerendo un fronte d’onda di propagazione più veloce

Conclusions:• CRT with MPP can significantly improve acute LV hemodynamic parameters assessed with pressure-volume loop
measurements as compared with CONV.• Importantly, we observed a non significant difference between systolic improvement in ischemic patients, who
typically have poor CRT response, and in non ischemic patients.
Results:
• Relative to BASELINE, the best MPP intervention as compared with the best CONV SSP intervention significantly increased LV:
• dP/dtmax: 15.9% vs 13.5%; P<.001• stroke work: 27.2% vs 19.4% P=.018• stroke volume: 10.4% vs 4.1% P=.003• ejection fraction: 10.5% vs 5.3% P=.003
• Moreover, the best MPP intervention improved acute diastolic function.

Risultati:• Compared with BiV pacing at any LV site,MPP yielded a small but consistent increase in hemodynamic response. A
correlation between the increase in hemodynamics and Q-LV on MPP was observed for all measurements,includingthose taken at the best and worst sites.The MPP-induced improvement in contractility was associated with significantly greater narrowing of the QRS complex than conventional BiV pacing.

• Objective
• To evaluate the effect of multipoint pacing (MPP) on acute haemodynamics, cardiac contractility, and left ventricle (LV) dyssynchrony, in comparison with conventional cardiac resynchronization therapy (CRT).
• Results
• MPP showed a further reduction in LV dyssynchrony compared with conventional biventricular pacing. Moreover, MPP resulted in an additional improvement in LVEF and in CI, and this was translated into a higher number of acute responders to CRT.

On the basis of the CCS, patients were classified into 1 of 3 response groups: 1. Worsened2. Improved3. Unchanged according to a predefined scheme.Patients were classified as worsened if they died or were hospitalized for worsening HF or had an increase in NYHA functional class. Patients were classified as improved if they had not worsened and had a decrease in NYHA functional class. Patients who had neither worsened nor improved were classified as unchanged.

Invasive hemodynamic and electroanatomic assessment was performed at:
- baseline (non-CRT);
- BIV pacing delivered via the implanted CRTsystem (BIV implanted)
- BIV pacing delivered via analternative temporary LV lead (BIV altern)
- MultiPoint Pacing delivered via 2 vectors of the quadripolar LV lead
