Nhận định nhu cầu chăm sóc cho gia đình và cộng đồng ta vừa dựa vào phương pháp nhận định cá nhân vừa dựa vào kỹ năng của YTCC. Đó là:
Xác định vấn đề sức khoẻ và chọn vấn đề sức khoẻ ưu tiên
Lựa chọn giải pháp chăm sóc và kỹ thuật chăm sóc.

Thu thập và xác định các chỉ số
Các chỉ số sức khoẻ cần thiết như số sinh, số chết, số kết hôn, số có thai, số trẻ mới đẻ dưới 2500g, số trẻ suy dinh dưỡng …
Các chỉ số phục vụ y tế như số trẻ dưới 1 tuổi được tiêm chủng, số phụ nữ khám thai, số lần truyền thông giáo dục sức khoẻ, số cặp vợ chồng chấp nhận biện pháp tránh thai.
Các chỉ số về kinh tế-xã hội cần thiết như số hộ đói nghèo, số nghiện hút, nhiễm HIV …

Xác định vấn đề sức khoẻ:
Để xác định vấn đề sức khoẻ ta có thể dựa vào các tiêu chuẩn sau:
Các chỉ số của vấn đề ấy đã vượt quá mức bình thường chưa?
Cộng đồng đã biết rõ vấn đề ấy chưa?Vấn đề có khả năng giải quyết được
không?(Tự giải quyết hoặc đề nghị trên giúp giải
quyết)

Lựa chọn vấn đề và chăm sóc ưu tiên
Sau khi xác định các vấn đề sức khoẻ, bạn có thể thấy trong cộng đồng có nhiều vấn đề sức khoẻ. Lúc này bạn phải lựa chọn ưu tiên, vì không thể coi các vấn đề như nhau và không thể giải quyết ngay mọi vấn đề được.
Để lựa chọn ưu tiên, người ta sử dụng 1 bảng điểm để cân nhắc từng tiêu chuẩn sau:

1. Mức độ phổ biến của vấn đề (nhiều người mắc hoặc liên quan)

Nhận định kết quả: cộng dồn điểm từng vấn đề, xét giải quyết ưu tiên từ vấn đề có điểm cao đến vấn đề có điểm thấp.

Dựa vào kiến thức và kinh nghiệm. Trước tiên phải xem vấn đề cần được quan tâm, rồi quyết định vấn đề khó khăn nào của người bệnh phải được giải quyết ngay.

Phương tiện, thiết bị, nguồn lực sẵn có, cũng như khả năng nhân viên, thời gian, tình hình thực tế của bệnh nhân, gia đình và cộng đồng
Mỗi hoạt động chăm sóc có thể thực hiện một lần hoặc phải thực hiện nhiều lần.

đo được kết quả.Quyết định giải pháp: Là những biện pháp (con đường thích hợp để giải
