Indications of Basti

• Mainly Basti is useful in the diseases created due to vitiated Vata ,problems of muscles & bones .

• Diseases affecting the whole body (sarvangaroga) eg. Rheumatoid arthritis, localized diseases (ekangaroga) e.g. Sciatica, Spondylosis.• Digestive tract problems like abdominal pain due to gas ,Constipation, Ulcerative colitis etc, retention of flatus, urine or stool.



• Muscle atrophy ,Muscle dystrophy • Nervous disorders or Neurological problems (vatavyadhi) like Hemiplegia, Parkinsonism ,Sciatica ,Motor Neuron disease • Heart problems like Palpitation, Angina pectoris

• Psychosis



• Disorders of udana vayu like Dry Bronchial Asthama ,Dry cough ,Hiccough .

• Diseases of Apana vayu like Menstrual disorders ,Infertility ,Oligozoospermia

• Basti is recommended for all types of Vata dominant conditions where pain & stiffness are the main features


Anuvasan Basti

• Generally oils like Bala, Balaguduchyadi, Bala Ashwagandhadi, Lakshadi are used for degenerative pathology.

• Sahacharadi, Triphaladi, Chinchalavan are used for obstructive pathology or in obesity associated morbidities


Indications of Anuvasan Basti

• All indications of niruhabasti are indicated for anuvasan also.• Persons with extreme dryness of vata dosha.• Pregnancy ---- Matrabasti with dose of 40 to 60 ml of Lakshadi tail or Shatavari tail is given in the 7th month of pregnancy


Contra indications of Anuvasan Basti

• Condition contraindicated to Asthapan or Niruha Basti

• Empty stomach• Acute fever• GIT – anemia, jaundice, anorexia, spleenomegaly, ascitis, liver disorder, hunger, diarrhea


Contra indications of Anuvasan Basti

• Respiratory – cold, productive cough(kaphaja kasa)• Kushtha – skin eruption with discharge• Urustambha• ENT:-nasal inflammation, eye inflammation.• Hormonal:- Diabetes, thyroid disorder• The abdominal pain due to Kapha, ama conditions, extreme obesity,• Intestinal parasites, poisoning, gout


Anuvasana - Samyakyog

Best Vata – Anuloman • Timely removal of waste products (mala) with oily substances• No adverse side effects• Provides nutrition for Dhatus• Lightness in the body• Strength of the body increases


Anuvasana - Asamyakyog

• Body ache

• Dryness in the body

• Nausea

• Drowsiness


Niruha basti

• In this type herbal decoction is used • e.g. herbs like Dashamool, Rasna, Erand,Triphala are used for decoction.

• Decoction is mixed with honey, rock salt & oil systematically and make ready to administer in the body.

• It is a cleansing process


Niruhabasti – Indications

• GIT- Flatulence, Abdominal discomfort, constipation, worms etc.• Respiratory- Rhinitis, Cough • CVS- Congestive Heart Disease, Hypertension. • Neurological- Hemiplegia, Sensory and motor disturbance like tingling & numbness of the extremities, Facial paralysis


Niruhabasti – Indications

• Endocrinal - Hypothyroidism • Locomotors - Joint pain, Muscular Pain, Regional pain like backache, stiffness, lock jaw etc. • Excretory - Dysuria, Constipation, anal pain, Urinary calculi etc. • Reproductive- Oligospermia, Azospermia, Scrotal swelling Vaginal pain, labor pain, Breast pain, Menstrual irregularity like in PCOD • ENT &Ophthamic- Headache, Earache & Glaucoma 8/28/2016 26Prof.Dr.R.R.Deshpande

Niruhabasti – Contra Indications

• GIT- Indigestion, low metabolic fire, post internal Oleation, Ascites, piles, Acute Diarrhea• Respiratory- Dyspnoea, Acute cough, hiccough etc• Endocrinal-DM• Excretory- Anal or perianal swelling, anal strictures• Reproductive-In pregnancy• ENT-Dryness in throat,• Others- Obesity, Skin diseases, edema


Preparation of basti dravyas

• First mix honey and saindhav homogenously.• Add oil and again mix drug homogenously.• Now add the kalka i.e. paste of drug and make the mixture homogenous• After kalka gomutra (if indicated) should be added and at the last warm decoction should be added.• Final prepared basti dravya should be homogenous and warm (around 40 to 45 degree C).• Once the mixture is prepared it cannot be heated.


Uttar Basti

• The medicated decoctions and oils are passed in the urinary bladder through urethra or in uterus through vagina.

• In male it is given though urethra , while In female it is given through vagina.


Uttar Basti - Indications

• It is indicated in menstrual problems like amenorrhea, dysmenorrhea, menorrhegia, leucorrhea.

• It is used in primary and secondary infertility.• It is very much useful to remove Tubal blocks• It is used in patient of recurrent abortions.• PCOD• It is used for urethral stricture and dysuria.


Uttar Basti

• Specific Time-• After the menses on the 5,6,& 7th day for the 3 months.

• Instruments: Cuscos speculum, Infant feeding tube no. 8 or simple rubber catheter no. 3, syringe of 5ml, betadin, cotton, gloves, swab holder, IUI canula ( Intra Uterine Canula) ,Tampoon, Drepping clothes

• All instruments should be sterilized in Autoclave


Uttar Basti - Procedure

• Pre therapeutic procedure --- Two Nirhuha with Dashamool &Triphalabharad are given prior to Utter Basti.

• Lithotomy position is given to the patient.• Painting and drepping is done• Sims speculum is applied.


Uttar Basti - Procedure

• Infant feeding tube or catheter or Intra uterine canula is inserted in the uterus through the cervix very gently and carefully upto 3cm.

• Dose of sneha :-- 3 cc to 5 cc• Sneha is taken in syringe and administer in uterus ,with Intra Uterine canula

• Post therapeutic position should be ‘Head low position’ given for about 1 hour.

Applied uttar basti

• In strictures or obstructive pathology- Kshar, Kasis ,Sahachar oils are used.

• In infertility Narayan oil or Phala Ghrut are used.

• For improving the strength of reproductive organs the Phala Ghrut or Bala Oil, Yashtimadhu oil is used.


Madhutailik Basti

• Madhutailik basti is a sub-type of Niruh basti

• In which madhu i.e. honey and taila i.e. oils are used in equal or much more quantity


Uttar Basti– In Male

• Instruments: --- Simple Rubber Catheter 3 Number, Gloves, Xylocaine gel 2% & Syringe of 50 ml are needed.

• These all instruments to be used should be sterile.

• Sneha needed in a dose of 20-50 ml.


Uttar Basti– In Male

• After stool passing in the morning it is given in between 6 to 9 a.m. or in the evening in between 5-7

• It is given either every day or an alternate day for 7-14 days.


Uttar Basti– In Male -Procedure

• The catheter should be inserted in the urethra up to 3-5 cms.

• The sneha should be administered very slowly.• After the treatment Green Gram soup or meat soup should be given to patient.

• Patients should be asked to avoid the sexual act during the course of the therapy.

• Cleaning of the genitals is advised specifically.


Yapan basti

• It is used for children, women, old age persons, for removal of vitiated doshas and also for the improvement of complexion and voice

• This Basti provides longevity of human being. Substances used are

• Honey and oil , approximately 90 g each + Erand root decoction 180ml + Shatpushpa Churna 20mg + Saindhav (Rock salt) 10 Gm + Madanphal Churna 10 Gm.


Siddha basti

• Increases strength, muscle power, improves complexion.

• Substances used – Yava, Kulattha, Pippali, Yashtimadhu, Saindhav, Honey.

Generally each substance is taken in 40 gm dose and water is added 4 times and after boiling till it remains 1/4th it is given in 250 ml dose.


Rajyapan Basti -Indications

• Cleansing & tonifying in nature• Indicated in Urinary tract infections, glomerulo nephritis, avascular necrosis of hip joints, Inguinal & Scrotal pain, Musculoskeletal diseases like muscular dystrophy, neurological cases like Multiple sclerosis, Parkinson's disease etc.


Rajyapan Basti-Indications

• It is recommended in dry allergic cough or in COPDs like emphysema, bronchiectasis, chronic fever, gouty arthritis, irritable bowel syndrome, Knee joint osteoarthritis etc.

• It is used in Auto immune conditions like Ankylosing spondylosis & RA.

Rajyapan Basti increases the strength, it increases the sexual drive, improves the digestion and Vital sap.


Rajyapan Basti-Ingrediants

• Decoction of Ashwagandha, Shatavari, Erandamool, Bala in a dose of 240 ml.

• Honey - 20 ml.• Rock salt - 5 Gm• Sesame oil - 40 ml• Ghee - 40 ml.• Meat soup – 160 ml & Cow Milk – 160 ml.


Vaitaran Basti-Indications

• In Ama associated conditions to relieve the pain e.g. in Amavata (RA)

• It reduces the acute inflammation especially relieves the pain & burning.


Vaitaran Basti- Ingrediants

• Chincha leaves ( Tamarind) --- 40 Gm + • Jaggery- 20 Gm + Rock Salt- 10 Gm +• Cow Urine or Gomutrasav 160 ml + Sesame oil- 30 ml

• It is used for 1 – 3 weeks.


Lekhan Basti

• Herbs like Triphala, Mustha, Kulatha, Sahachar are used for decocotion along with Cow urine or Gomutrasava. Rock salt is also added in it with honey

• It is given in 500-900 ml dose alternate day. Triphaldi or Sahacharadi oil 60-100 ml are used for Anuvasan Basti.

• It is used in obesity, Hypercholesterolemia & in PCOD with obesity


Tiktak sheer Basti

• Indicated in osteoporosis, osteopenia • cervical spondylosis, lumber disks problem like compression, prolapsed disks

• Pathological fracture• Hair graying, hair fall• Dental caries • For the height gaining


Tiktak sheer Basti

• Guduchi, Patol, Vasa, Nimb, Kirattikta Bharad total 50 Gm +

• 800 ml water, boiled till it remains 100 ml • followed by 100 ml milk addition and• Again boild till water gets evaporated+• PanchaTikta Ghee 10 ml to 40 ml.


Piccha Basti

• Indicated in IBS, Bacillary dysentery, Ulcerative colitis etc.

• Honey 200 ml + Saidhav 15 Gm + Nalpamaradi Keram ( Sneha) 100 ml + Yashtimaddhu Kalka 30 Gm + Shalamali & mustadi siddha kshir 200 ml

Usually this basti is given by drip method


Karma – Kala – yoga Basti

• 1) Yoga Basti -- 3 Niruha + 5 Anuvasana = 8 bastis -- Control to maintain Vata

• 2) Kala Basti -- 6 Niruha + 10 Anuvasana = 16 bastis -- Control of seasonal Vata

• 3) Karma Basti -- 12 Niruha + 18 Anuvasana = 30 bastis-- Control of extreme Vata vitiation


Economics of Basti

• Rubber catheter cost is about 6-7 Rupees per piece.• Plastic gloves are available at 80 Rupees per 100 pairs.

• Enema pot is about 150 to 175 Rupees.• Medicated oil cost varies from 350 to 450 Rupees per liter to 800 to 1200 rupees per liter

• Generally Basti costs about 150-200 Rupees per day.


Pre treatment ( Purva Karma )

• External Oleation i.e. massage (Abhyanga)- At least on stomach & back.On abdomen it should be more circular & soft .Make sure that there is no serious pathology inside the abdomen like ulcer, perforation, strangulation, Intussception

• Fomentation- Tube fomentation or fomentation with napkin.


Pre treatment ( Purva Karma )

• Anuvasana basti (if patient has excess dryness, then give 2-3 Anuvasana basti)

• Light meal before Anuvasan and empty stomach before Niruha.

• Excretion of the stool and passing of urine.


After treatment of Basti

• Take rest• Bath with lukewarm water• Have light diet. Sansarjanakrama is not necessary.

• Use warm water to drink.


After treatment of Basti

• Don’t • Sit for long time• Vehicle journey• Sleeping during day time• Coitus• Suppression of natural urges• Having cold water for drinking or even bath ( which can provoke vata)


Basti Therapy in Clinical Practice

• 1) Cervical Spondylosis --- Tikta Ksheer Basti + Shali Shastik Pinda Swedan 10 days. It relieves pain, spasm .It gives relief for sensory & motor symptoms like Tingling Numbness, pain during the movement of neck



• 2) Backache ---- Sahacharadi Oil Matrabasti 60 ml + Patra pottali Swedan relieves the pain ,stiffness & spasm

•



• 3) Ulcerative colitis , IBS, Bacillary dysentery --- Mocharas Piccha basti – 25 Gm Mocharas+ 100 ml cow milk+ 100 ml water boil till evaporation of total Water. Many times this is given by Enema pot & drip method. Internally Bilva Avaleha is useful

•



• 4) Dry Cough (Allergic cough),Asthma --- Sesame oil Matra Basti – 60 ml 7 Days give symptomatic relief.

•



• 5) In weight gaining- Balaguduchyadi Oil 50 ml Matrabasti 15 days. + Full body Gentle massage with Kshir Bala oil + Internally ShatavariKalpa or Chyavanprash



• 6) Oligospermia --- Kavach Beej Churna + Shatavari 10 Gm each in 200 ml milk plus 200 ml water and by boiling till water get evaporated+ Phala Ghrutum basti for 15 days improves the sperm count.



• 7) Chronic Constipation --- Matra Basti with Chincha Lavan oil is useful & used in a dose of 60 ml for 7days.+ advice about adequate drinking of water + regular walling exercise + Yogic exercises to improve tone of abdominal muscles like Naukasan + Enough roughage in diet

• Bala Guduchydi Basti ( A.H Kalpa 4/1,2)• Honey 200 ml + Saindhav 15 GM+ Sahacharadi oil 150 ml + Sukumar Ghrita 150 ml +Kalka of Yavani ,Madan Phala ,Bilva,vacha,Musta,Pippali 30 Gm + Decoction of Bala ,Guduchi,rasna,triphala,Dashamul 400ml + Mansa rasa 200 ml

• Uses – Vata pacifying, Nourishing to increase weight



• Bala Patoladi Basti ( A.H. Kalpa 4/5,6)• Honey 200 ml + Saindhav 15 Gm + Kshirbal tail 150 ml + Triphaladi Ghrutam 150 ml + Kalka of Priyangu ,Pippali,Musta 30 Gm + Decoction of Bala,Patola,Laghu Panchamul,Eranda,Yava 400 ml + Aja mansa ras 200 ml

• Uses – Eye Tonic, To increase muscular strength



• Erandamula Basti ( A.H.Kalpa 4/7 to 10)• Honey 200 ml + Saindhav 15 Gm + Murchit Til oil 200 ML + Kalka of Vacha, Shatavha, Priyangu, Yashtimadhu,Pippali,musta,indrayav 30 Gm + Decoction of Erandamul ,Rasna, Guduchi,Ashvagandha,Punarnava,Aragvadha,devdaru,madanphala 600 ml + Gomutra 75 ml
