INTRODUCTION

Idiopathic membranous nephropathy is an

autoimmune disease and a common cause of the nephrotic syndrome in

adults

The phospholipase A2 receptor 1 (PLA2R1) was discovered as the major antigen involved in

the pathogenesis of adult idiopathic membranous

nephropathy.

30% of the patients with this disease have no obvious secondary cause of the disease

is thought that other endogenous

glomerular antigens may be involved in

these cases.

They analyzed serum samples from patients with :

idiopathic membranous nephropathy patients

other glomerular diseases

healthy controls

to identify circulating antibodies against

glomerular antigens other than PLA2R1

THROMBOSPONDIN TYPE 1

• It is a protein that in humans is encoded by the THBS1 gene.• This protein is an adhesive glycoprotein that mediates cell-to-cell and cell-

to-matrix interactions. It can bind to fibrinogen, fibronectin, laminin and type V collagen

•  Natural inhibitor of neovascularization and tumorigenesis (apoptosis) in healthy tissue.

•  Positive and negative modulation of endothelial cell adhesion, motility, and growth.

• Affecting neuronal migration in the rostral migratory stream.• Interacts with numerous proteases involved in angiogenesis• Inmune system regulation

IDIOPATHIC MEMBRANOUS NEPHROPATHY

• Is one of the principal causes of nephropathy. It is caused by the thickening of part of the glomerular basement membrane which helps filter the blood.

• The thicker glomerular membrane does not work normally. As a result, Large amounts of protein are lost in the urine.

• 70% of the times is associated with PLA2R1, the other 30% is still unknown .

Symptoms :• Edema • Fatigue• Poor appetite• Urination, excessive at night• Weight gain

Find circulating antibodies against glomerular antigens other than PLA2R1, that can explain the other 30% of the patents that are still an unknown process

Se estudiaron muestras de Pacientes con NMI y muestras control para analizar total de proteínas en extractos glomerulares humanos

RESULTADOS

• En pacientes negativos para anticuerpos PLA2R1 se encontró un antígeno de 250 KD presente en el glomérulo humano.

• Se encontró que es una proteína N-glicosilada y que se expresa exclusivamente en la membrana celular, lo que ayuda a concluir que es diferente al PLA2R1.

• Tras todos los estudios realizados se detectó que la proteína que se estaba buscando era la THSD7A

• También se vio que tanto la forma nativa como la recombinante de la proteína mostraban la misma masa tras la glicosilacion lo que se puede interpretar como una pequeña diferencia en modificaciones post transcripcionales.

• Todas las muestras presentaron inmunoprecipitación

anticuerpos contra IgG1 hasta IgG4

Personal conclussions

• I think is very important to know a new antigen for the IMN, because it is a way to create effective treatments for these people. This discovery is very interesting because the authors are motivating us to investigate. Now is clear that PLA2R1 and THSD7A are implicated in IMN, but now the objective is to search which are the others antigens implicated in this disease and why the antibodies are expressing in some people but not in others.

Personal conclussions

• Been able to investigate this kind of diseases that don't have a known reason is a really good thing, because lots of them can have a better treatment if only the scientist and doctors could understand what is really happening, because some of this are not well treat not because lack of possibilities but because lack of knowledge.
