"17. Lai nodrošinātu kvalitatīvus un pacientiem drošus ārstniecības
pakalpojumus, ārstniecības iestāde veic šādus pasākumus:
17.1. ievieš un nodrošina nepārprotamu pacientu identifikāciju visā
ārstniecības procesa laikā, izmantojot vismaz divus identifikatorus (piemēram,
pacienta vārds, uzvārds un personas kods);
17.2. nodrošina efektīvu komunikāciju starp pacienta ārstniecības procesā
iesaistītajām ārstniecības personām un pacientu, tai skaitā nodrošina:
17.2.1. savlaicīgu kritisko izmeklējumu (tādas diagnostisko izmeklējumu
novirzes no normas, kas visticamāk liecina par nopietnu risku pacienta veselībai)
rezultātu paziņošanu attiecīgā pacienta ārstniecības procesā iesaistītajām
ārstniecības personām vai pacientam;
17.2.2. nepieciešamos medicīniskos dokumentus saskaņā ar medicīnisko
dokumentu lietvedības kārtību regulējošiem normatīvajiem aktiem attiecīgā
pacienta ārstniecībā iesaistītām ārstniecības personām atsevišķos ārstēšanas
posmos vai pacientu nosūtot vai pārvedot starp struktūrvienībām vienas
ārstniecības iestādes ietvaros, vai pacientu nosūtot vai pārvedot uz citu
ārstniecības iestādi;

LABĀ PRAKSE*
1. Slimnīcā pacienta datus ievada vienā vietā un vienu reizi - tiek lietota viena
centrālā pacientu datu bāze. Pacienti tiek reģistrēti, izmantojot centralizētu vienotu
datubāzi un standartizētu procedūru.
2. Klīnicistiem ir iespējams atlasīt pacientu datus pēc specifiskiem kritērijiem
(piem., 'mani pacienti', 27.februārī ārstējušies pacienti, ar dg. E20.0, antibiotikas
XYZ saņēmušie utt.)
3. Pilnīga informācija, kas nepieciešama korektai pacientu identifikācijai, ir
skaidri atspoguļota visos IS ekrānos, uz aprocēm, uz izdrukām.
4. Lai mazinātu pacientu sajaukšanu, programmu ekrānos, atspoguļojot vairāku
pacientu datus, tie ir skaidri nodalīti / izcelti.
5. Medicīniskā numerācija (pacientu ID nr., slimības vēstures nr. u.c.) satur
pārbaudes ciparu ( "check digit" ), kas palīdz izvairīties no datu ievades kļūdām.

6. SIS lietotāji tiek brīdināti, ja vēlas izveidot jaunu ierakstu pacientam, kura
vārds, uzvārds ir jau kādam pacientam sistēmā.
7. Programmu lietotāju saskarnes (programmu logi) testa, treniņa un back-up
versijām ir ar skaidri pamanāmu atšķirīgu marķējumu, lai izvairītos no sajaukšanas
ar darba ('īsto') programmas versiju.
8. Organizācijai ir izveidots process, kā piešķirams pagaidu unikāls pacienta ID
numurs sistēmas atteices gadījumos vai nezināma pacienta gadījumos.
9. Pacienta identitāte tiek verificēta visos 'atslēgas' punktos un nododot pacientu
no viena posma otrā (piem., ievietojot palātā, ņemot analīžu paraugus, atzīmējot
vitālos rādītājus, ievadot ordinācijas un to izpildi, izpildot manipulācijas utt.)
10. Programmas limitē vienlaicīgi atveramo dažādu pacientu datu logus, lai
izvairītos no vienam pacientam domātās informācijas ievadīšanas / interpretēšanas
otram pacientam.
11. Mirušie pacienti tiek skaidri identificēti kā miruši.

Positive patient identification is central to the provision of safe and effective
care. Reducing, and where possible, eliminating patient identification errors, is
fundamental to improving patient safety. The hospital shall demonstrate that the

recognition in order to reduce incidents of patient misidentification in all care
settings.

Interpretive Guidelines:
The hospital shall evidence that it provides for interpretation for individuals
who speak languages other than the predominant language of the
organization. Wherever possible, interpreters should be inde-pendent (i.e. not
family members). In addition the hospital shall evidence that it also provides
alternative communication techniques or aides for those who are hearing
impaired, vision impaired or have other specific needs, or take other steps as
needed to effectively communicate with the patient.

PC.10 – Discharge Planning Policies
Interpretive Guidelines:
Poor planning and communication with ongoing care providers or patients
themselves at discharge can often be the cause of patient safety incidents including
a lack of continuity of care, medication miss management or a lack of social care
support.
The hospital must define the discharge planning process and communicate it to all
appropriate areas of the hospital. This process is imperative in the patient care
delivery system to ensure that patients’ needs are being met and to minimize the
likelihood of having any patient re-hospitalized for reasons that could have been
prevented.

The documentation associated with the discharge planning process will be
included as a part of the patient’s medical record as a means of
coordinating communication with other providers involved in the patient’s
care throughout the hospital. The patient’s physician, a registered nurse,
social worker, and/or other qualified staff member will be responsible for
the development of information and materials to implement the discharge
plan for the patient.
