Rome Rehabilitation 2011 XX Congresso Nazionale SIDC
Dott.ssa FT-OMT Sara Brunetin
Roma 10, 11, 12 Ottobre 2011
Meccanismi di lesione: • Rotazione esterna del piede• Iperdorsiflessione• Carico assiale• Pronazione (Molinari 2009)
Incidenza : 1-11% di tutte le distorsioni di caviglia
Meccanismi di lesione: • Rotazione esterna del piede• Iperdorsiflessione• Carico assiale• Pronazione (Molinari 2009)
Sports:• calcio, rugby, football• Sci, hockey (Wright 2004)
• Running o jumping sports
Incidenza : 1-11% di tutte le distorsioni di caviglia
Diagnostica per immagini
Rx RM
Fratture e diastasi Lesioni associate (Brown 2004, Mansour 2011)
tecnica invasiva↑ Falsi negativi
costi sanitarileg. sottili e poco definiti
(Hermans 2011)
SP 100%SN 43%
accuracy 72%(Oae 2003)
AITFL (discontinuità/detensione)
SP 70 / 93%SN 100%
accuracy 95 / 100%(Oae 2003)
Anamnesi
• meccanismo del trauma
• dolore localizzato AITFL > PITFL
• dolore malleolo mediale (leg.deltoideo)
• movimenti di rotazione esterna del piede e dorsiflessione dolorosi
• deambulazione dolorosa nella fase di avanzamento della tibia
(Lin 2006, Mulligan 2011)
Esame clinico
• Non edema significativo
• Tenderness AITFL
• ↓ ROM flessione dorsale (E.F. vuoto e doloroso) e plantare
• Deambulazione: evita la dorsiflessione
• Tests specifici
Tests clinici1. Palpation *2. External-rotation test *3. Dorsiflexion maneuver4. Dorsiflexion-compression test *5. Squeeze test *6. Crossed-leg test7. Heel thump test8. One-legged hop test9. Manual stability tests
* Interrater reliability (Alonso 1998)
Criterio di positività : il DOLORE (Beumer 2003)
Clinical tests for ankle syndesmosis injury: reliability and prediction of return to function.
Alonso et al., J Orthop Sports Phys Ther,1998
• Clinical tests: Palpation test (Pt)
External-Rotation test (ERt)
Dorsiflexion- Compression test (DCt)
Squeeze test (St)
• 53 pz con distorsione di caviglia (>trauma sportivo)
• Esaminatori 2 (1 blinded)
• Misure di outcomes: camminare 10 m senza dolore
ritorno allenamento
ritorno competizione sportiva
Clinical tests for ankle syndesmosis injury: reliability and prediction of return to function.
Alonso et al., J Orthop Sports Phys Ther,1998
1°Obt: Riproducibilità inter-esaminatore
ERt k = 0,75 St k = 0,50
DCt k = 0,36 Pt k = 0,36
2°Obt: Correlazione tra i tests
debole ma significativa tra ERt-Pt e tra ERt- DCt
3°Obt: Prognosi (St n.v.)
sogg P = sogg N
ERt-DCt: se P a 1 o entrambi → tempo di recupero
Trattamento
INTERVENTO CHIRURGICO syndesmotic screwindicato per: dislocamento lat. perone, eccessiva apertura mortaio, rottura leg. Deltoideo, fratture malleolari
TRATTAMENTO CONSERVATIVOReview 2006 Jones & Amendola: studi non confrontabiliPrognosi: 4-6 settimane (grado I°-II°)
FADI sport
NESSUN TRATTAMENTOossificazioni eterotopiche sindrome da impingement anteriore
Trattamento conservativo
1ª FASE ↓ dolorePRICE
TM (tecniche I°-II°)
2ª FASE↑ ROM e funzionalità
TM (tecniche III°-IV°)
es. rinforzo muscolare, allungamento, propriocettivi
3ª FASE ritorno sport gesto atletico
Evitare es. che richiedano eversione completa e piede abdotto in carico.
Introdurre gradualmente es. che inducano la dorsiflessione completa.
? Rinforzo mm supinatori e flessori plantari e mm medio e grande gluteo
(Bullock-Saxton 1994, Beckman & Buchanan 1995)
(Lin 2006, Mulligan 2011)
Conclusioni
distorsioni sindesmosi distorsioni laterali Esame clinico:
meccanismo del trauma + tests manuali RM per conferma diagnosi e lesioni associate prognosi prolungata trattamento per evitare sindromi dolorose
croniche RCT accuratezza diagnostica dei tests clinici
efficacia del trattamento riabilitativo
