What the 5-year results showed?
• START A : direct estimates of α/β suggested breast cancer is a sensitive to fraction size as late-reacting normal tissues; so no benefit from continued use of 2Gy fractions
• START B : suggested 40 Gy in 15 fractions is as safe and non-inferior in terms of tumor control to 50 Gy in fractions

Conclusion• Breast cancer & the dose-limiting normal tissues respond
similarly to fraction size, no advantage for 2Gy fractions
• Patients can be safely and effectively treated to a lower total dose with fewer fractions than the historical standard 50Gy/25F
• No detrimental effects of hypofractionation are indentified in the subgroups studied
• Results support 40 Gy in 15 fractions as UK standard for all patients with invasive breast cancer (NICE Guidance 2009)

Essai TARGIT-A• Essai de phase III international, multicentrique, prospectif, randomisé
• Comparaison d’une radiothérapie per opératoire ciblée au contact du lit tumoral, en une séance unique de 20 Gy, avec une irradiation classique du sein en totalité sur 5 semaines
• Population :• Femmes à partir de 45 ans• CCI unifocal (imagerie classique)• Éligible à une chirurgie conservatrice
• Objectif principal : récidive locorégionale
• Objectifs secondaires : évaluation de la morbidité et toxicités locales
• Évaluation / 3 mois pendant 6 mois, puis tous les 6 mois jusqu’à 5 ans, puis annuellement jusqu’à 10 ans.

• Deux stratégies ont été réalisées• Pre-Pathologie, c’est-à-dire IORT
pendant la première intervention
• Post-pathologie, c’est-à-dire une fois le résultat histo définitif obtenu et que la patiente, étant une bonne indication d’IORT, est réopérée

A San Antonio 2012• Dans la population traitée par TARGIT avant la tumorectomie
• La différence de récidive locale est de 1%
• Dans la population traitée par TARGIT en POST-tumorectomie• La différence de récidive locale est de 3,7%
• La mortalité semble inférieure dans le bras IORT 3,9% vs 5,3% mais non significativement différente
• La positivité des récepteurs à la progestérone est un paramètre de meilleur pronostic
• En conclusion, la procédure POST-PATHOLOGIE n’est plus à proposer dans cette technique
• La sélection des patientes est primordiale (> 60 ans + de 40 dans l’étude TARGIT), RH+, tumeur < 2 cm, ganglion sentinelle négatif
