APOKYNTM (アポモルヒネ塩酸塩注射液)
10 mg/mL 皮下投与専用 静脈内注射不可 処方箋医薬品 製品説明:
APOKYNTM(アポモルヒネ塩酸塩, USP)は非麦角系ドパミン受容体作動薬である。化学名は
6aβ-Aporphine-10,11-diol hydrochloride hemihydrate で、C17H17NO2・HCl・1/2H2O の分子式を有
する。分子構造式並びに分子量は下記に示すとおりである。 (図省略) 分子量:312.79 アポモルヒネ塩酸塩は白~灰白色の微小結晶あるいは白色粉末で、80℃のお湯に溶解する。 APOKYNTM 10 mg/mL は皮下注射用の無色澄明の無菌注射液で、3 mL カートリッジ入りであ
る。溶液 1 mL中には、アポモルヒネ塩酸塩 10 mg とピロ亜硫酸ナトリウム 1 mg が注射用水中
に含有されている。また、溶液 1 mL中には、pH 調整剤として水酸化ナトリウム、塩酸のいず
れかまたはその両方と、防腐剤としてベンジルアルコール 5 mg/mLが含まれる。 臨床薬理:
作用機序: 本剤は非麦角系ドパミン受容体作動薬で、in vitro での親和性はドパミン D4 受容体に対しては
高く(Ki=4.4 nM)、ドパミン D2、D3、D5 受容体(Ki=35-83、26、15 nM)並びにアドレナ
リン受容体 α1D、α2B、α2C(K1=65、66、36 nM)には中等度、ドパミン D1、セロトニン
5HT1A、5HT2A、5HT2B、5HT2C 受容体に対しては低い(Ki=370、120、120、130、100 nM)。
アドレナリン β1、β2 受容体やヒスタミン H1 受容体に対しては親和性を示さない(K1>10,000 nM)。 パーキンソン病治療薬としての本剤の作用は、脳の中の尾状核被殻内でシナプス後膜ドパミン
D2 様受容体が刺激されることによると考えられているが、正確な作用機序は不明である。ア
ポモルヒネはパーキンソン病の動物モデルで運動機能を改善させることが明らかにされており、
特に霊長類で、神経毒、1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine(MPTP)による上行性黒質線
条体ドパミン経路の病変によっておきた運動障害を軽減させる。 薬物動態:
吸収: アポモルヒネ塩酸塩は親油性化合物で、腹壁への皮下投与後速やかに吸収される( 高濃度到
達時間は 10~60 分)。皮下投与したアポモルヒネのバイオアベイラビリティーは静脈内注射

と同等である。特発性パーキンソン病患者の腹壁へ本剤 2~8 mg を 1 回皮下注射すると血中濃
度は直線的に推移する。 分布: アポモルヒネの全血中濃度に対する血漿中濃度の比率は 1 である。見かけの分布容積の平均は
218L(123~404 L)である。髄液中の 高濃度は 大血漿中濃度の 10%以下で、10 分から 20分後に 高濃度に達する。 代謝と排泄: 見かけのクリアランスの平均は 223L/hr(125-401 L/hr)で、平均消失半減期は約 40 分(約 30~60 分)である。 ヒトでの代謝経路については明らかでないが、可能性としては硫酸化、N-脱メチル化、グルク
ロン酸抱合、酸化等が考えられる。In vitro ではアポモルヒネは急速に自動酸化される。 特殊患者:
アポモルヒネのクリアランスは年齢、性別、体重、パーキンソン病の罹病期間、レボドパの投
与量や治療期間によって影響されないと考えられる。
肝障害: 肝障害(Child-Pugh 分類による中等度の障害)のある患者と健康成人とを比較した試験で、本
剤を腹壁に単回皮下投与した場合、肝障害の患者では AUC0-∞が 10%、Cmax が 25%高かった。
重度の肝障害患者での試験は行われていない(「使用上の注意」並びに「用法・用量」の項参
照)。 腎障害: 腎障害(クレアチニンクリアランス推定値に基づく中等度の障害)のある患者と健康成人とを
比較した試験で、本剤を腹壁に単回皮下投与した場合、腎障害の患者では AUC0-∞が 16%、
Cmaxが 50%高かった。アポモルヒネの 高血中濃度到達平均時間、平均消失半減期には腎臓の
状態による影響はなかった。重度の腎障害患者での試験は行われていない。軽度、中等度の腎
障害のある患者では開始用量を減量すること(「使用上の注意」並びに「用法・用量」の項参
照)。 薬物相互作用: カルビドパ/レボドパ: 本剤とレボドパを併用したとき、レボドパの血中薬物動態に変化はなかった。しかし、運動機
能に対する改善効果に差異が認められた。運動機能の改善に必要なレボドパ濃度の閾値は有意
に低下し、その結果作用持続時間が長くなったが、レボドパ治療に対する 大の反応性に変化
はなかった。
肝代謝により消失する他の薬剤: In vitro 試験の結果、アポモルヒネの代謝における cytochrome P450 の寄与は小さかった。また
別の in vitro 試験の結果、アポモルヒネが cytochrome P450 酵素の基質、阻害剤、又は誘導剤と

して作用することによる薬物相互作用が起きる可能性は低いことが示された。
COMT 阻害剤および COMT により代謝される他の薬剤: アポモルヒネは catechol-O-methyl transferase(COMT)による代謝を受けないので、アポモル
ヒネと COMT 阻害剤又はこの経路によって代謝される薬剤との間で薬物相互作用が起きるこ
とはないと思われる。 臨床試験: 進行期パーキンソン病における運動合併症(ウェアリングオフ現象及びオンオフ現象)に対す
る本剤の有効性は、3 つの無作為化比較試験で検証されている。 これらの試験に参加した患者のパーキンソン病の平均罹病期間は 11.3 年で、レボドパと他の 1剤もしくはそれ以上の薬剤(通常、経口ドパミン受容体作動薬)との併用による治療を受けて
いた。これら 3 試験のうち、1 試験はアポモルヒネの使用経験がない患者で、残り 2 試験は試
験参加直前まで 3 ヶ月間以上アポモルヒネを使用していた患者で行われている。未使用患者の
大部分には、本剤投与開始 3 日前から制吐剤(trimethobenzamide)が投与された。患者の半数
は本剤投与開始から平均 2 ヵ月で制吐剤の併用を中止することができた。 各試験とも主要評価項目としてパーキンソン病統一スケールである UPDRS partⅢスコア(運
動機能検査)の変化量を用いた。UPDRS partⅢスコアは 14 項目からなり、パーキンソン病患
者の基本的な運動症状(振戦、固縮、運動緩慢、姿勢反射障害等)の重症度を評価できるよう
になっている。 初の試験は並行群間比較試験で、無作為割付された進行期パーキンソン病患者 29 名に、2 対
1 の比率で割り付けた治験薬(本剤もしくはプラセボ)を皮下投与した。いずれの患者も過去
にアポモルヒネの使用経験はない。実施医療機関において、それまで投与されていた抗パーキ
ンソン病薬の投与を一晩中止し、翌朝、盲検下で、オフ状態にある患者に治験薬(プラセボも
しくはアポモルヒネ 2 mg)を投与し、2 時間以上の間隔をおいて、個々の患者が通常量のレボ
ドパ服用時に得られる効果とほぼ同等の有効性が認められるまで(またはアポモルヒネが 10 mg もしくはプラセボが相当量になるまで)増量した。治験薬は 2 mg ずつ増量した(アポモル
ヒネは 4 mg、6 mg、8 mg、10 mg、プラセボはその相当量)。本剤投与群 20 名のうち 18 名で
は、レボドパ通常量の効果にほぼ匹敵する効果が約 20 分で得られた。本剤の平均投与量は 5.4 mg(2 mg が 3 名、4 mg が 7 名、6 mg が 5 名、8 mg が 3 名、10 mg が 2 名)。これに対し、プ
ラセボ投与群 9 名ではそのような効果が得られた患者は 1 名もなかった。至適用量における
UPDRS partⅢスコアのベースラインからの平均変化量は、本剤群の 23.9 に対しプラセボ群で
0.1 であった(p<0.0001)。 2 番目の試験はクロスオーバー試験で、本剤を 3 ヶ月以上使用している患者 17 名を無作為に割
り付けた。各患者が朝服用する抗パーキンソン病薬を通常通り服用し、その後オフ症状が発現
した時点で本剤(患者それぞれの維持用量)もしくはプラセボを単回皮下投与し、UPDRS partⅢスコアを経時的に評価した。本剤の平均投与量は 4 mg(2 mg が 2 名、3 mg が 9 名、4 mg が 2 名、4.5 mg、5 mg、8 mg、10 mg が各 1 名)であった。投与 20 分後の UPDRS partⅢス
コアのベースラインからの平均変化量は本剤投与時の 20.0 に対しプラセボ投与時で 3.0 であっ
た(p<0.0001)。

3 番目の試験はアポモルヒネを 3 ヶ月以上使用している患者 62 名での並行群間比較試験で、単
回投与であり、患者は次の 4 群、すなわち、①アポモルヒネ維持用量、②アポモルヒネ維持用
量に相当する量のプラセボ、③アポモルヒネ維持用量+2 mg(0.2 mL)、④アポモルヒネ維持用量
+2 mg に相当する量のプラセボ、のいずれかに、2 対 1(実薬:プラセボ)の比率で無作為に
割付けられた。各患者が朝服用する抗パーキンソン病薬を通常通り服用し、その後オフ症状が
発現した時点で治験薬を投与した。投与 20 分後の UPDRS partⅢスコアのベースラインからの
平均変化量は、本剤投与群の 24.2 に対しプラセボ投与時で 7.4 であった(p<0.0001)。 下図はアポモルヒネ及びプラセボ投与後の UPDRS partⅢスコアの平均変化量の経時的推移を
示す。 (図省略) この 3 番目の試験で、本剤投与量群(平均投与量約 4.5 mg)とアポモルヒネ維持用量+2 mg 群
(平均投与量約 6 mg)を比較すると、投与 20 分後の UPDRS パートⅢスコアのベースライン
からの平均変化量は、前者が 24、後者が 25 であった。このことから、アポモルヒネ 4 mg を通
常投与している患者では、投与量を 2 mg 増量しても、得られる臨床効果はほとんど変わらな
い可能性が示唆された。アポモルヒネ維持用量+2 mg 群では有害事象の発現率が高かった。 適応症:
APOKYN®(apomorphine hydrochloride injection)は進行期パーキンソン病における運動合併症
(ウェアリングオフ及びオンオフ現象)の急性期、間歇投与を適応とする。本剤の臨床試験は
他の薬剤との併用下で実施された。(「臨床薬理:臨床試験」の項参照) 禁忌:
本剤とオンダンセトロンとの併用で著しい血圧低下及び意識消失が報告されているので、
5HT3 拮抗剤(オンダンセトロン、グラニセトロン、ドラセトロン、パロノセトロン、アロセ
トロン等)とは併用しないこと。 本剤は、」アポモルヒネ及び添加物(特にピロ亜硫酸ナトリウム)に対する過敏症の既往があ
る患者には投与しないこと。 注意:
静脈内注射は行わないこと。 本剤の静脈内注射により、アポモルヒネが静脈内で結晶化したことによる血栓形成や肺塞栓等
の重篤な有害事象が発現している。したがって本剤の静脈内注射を行ってはならない。
全般:
本剤の皮下投与に関連して、以下に記載する重要な有害事象が報告されているが、そのほとん
どは非盲検試験で発現したものであった。開発段階における比較試験は比較的少数の患者を対
象に実施され、主に単回投与の作用について検討したものであった。これら有害事象の多くは、
進行期パーキンソン病の患者集団における発現率が不明であるため、本剤との因果関係を評価
することは困難である。

悪心・嘔吐:
推奨用量の範囲において、重篤な悪心、嘔吐が発現することがある。そのため米国の臨床試験
では、98%の患者が本剤投与開始 3 日前から制吐剤(trimethobenzamide)を服用し、その後も
低 6 週間は継続服用することとしたが、522 名中 262 名(50%)において、本剤投与継続中
に trimethobenzamide を中止した。trimethobenzamide の併用期間は平均約 2 ヵ月(1 日~33 ヵ
月)であった。その後も平均 1 年(0~3 年)のフォローアップ期間中、262 名中 249 名で
trimethobenzamide を服用することなく本剤の投与が継続された。trimethobenzamide を使用した
場合でも、悪心(31%)、嘔吐(11%)が発現し、悪心による中止(3%)や嘔吐による中止
(2%)が認められた。 本剤の米国での開発では、trimethobenzamide 以外の制吐剤の使用経験はない。ドパミン拮抗作
用のある制吐剤の中にはパーキンソン病の病状を悪化させるおそれがあるものもあるので、そ
れらの使用は避けること。
失神:
臨床試験において、失神が約 2%に発現した。
QT 延長及び催不整脈作用の可能性: 本剤(忍容性があれば 2 mg から 10 mg まで増量)とプラセボの単回皮下投与を比較した試験
で、ホルター心電図を用いたアポモルヒネとプラセボとの QTc 間隔の差の平均は、アポモル
ヒネ 4 mg で 0 msec、6 mg で 1 msec、8 mg で 7 msec であった。10 mg については、症例数が少
なかったため、QTc 間隔の変化を十分に評価することはできなかった。 本剤又はプラセボを単回投与(平均投与量 5.2 mg、投与量の幅は 2~10 mg、35 名中 30 名は 6 mg 以下)した比較試験では、本剤投与時とプラセボ投与時との QTc 間隔の変化の差は、投与
20 分後及び 90 分後で平均約 3 msec であった。全症例のうち 2 名(1 名が 2 mg と 6 mg、1 名が
6 mg)で顕著な QTc 延長(投与前に比較して、>60 msec)が認められ、QTc 間隔は投与直後に
500 msec を超えた。本剤の投与量が 6 mg 以下では QTc 延長はごくわずかであった。6 mg を超
える投与量を皮下投与してもさらなる臨床効果は得られないので、6 mg より高い投与量は推
奨されない。 QT/QTc 間隔を延長させる薬剤の中には、torsades de pointes の発現や突然死を引き起こすもの
がある。torsades de pointes と QT 延長の関連性は、延長作用が大きい場合(20 msec 以上)は非
常に明白であるが、延長作用が小さくてもリスクが高い場合もあり、また低カリウム血症、低
マグネシウム血症、徐脈のある患者や QTc 間隔を延長させる薬剤を併用している患者、先天
性 QT 間隔延長等の遺伝的素因のある患者でもその発現リスクが高まることが考えられる。 市販前臨床試験では推奨用量範囲内の本剤の使用では torsades de pointes は認められなかったが、
使用経験が少ないためその発現リスクの上昇を否定することはできない。また、動悸や失神が
torsades de pointes の前兆である可能性も考えられている。 上記のようなリスクファクターをもつ患者に本剤を投与する場合は十分注意することが望まし
い。
症候性低血圧:
ドパミン受容体作動薬は、特に投与量の増量中に起立性低血圧を発現することがある。更に、
パーキンソン病患者では起立動作に対する反応性が低下している場合がある。これらのことか

ら、ドパミン受容体作動薬を服用しているパーキンソン病患者は、特に増量中は、起立性低血
圧の徴候、症状の観察を十分に行うことが必要であり、また患者にはそのリスクがあることを
説明しておく必要がある。 本剤は収縮期血圧(SBP)及び拡張期血圧(DBP)を用量依存的に低下させる。この血圧低下
は用量依存的であり、SBP では 2 mg で平均 5 mmHg 低下と 10 mg で平均 16 mmHg 低下が、
DBP では 2 mg で平均 3 mmHg 低下と 10 mg で平均 8 mmHg 低下が認められた。投与 10 分後か
ら血圧変化が認められ、投与約 20 分後に 大に達した後、少なくとも投与 90 分後まで持続し
た。医療機関において本剤の増量中に経時的に血圧を測定したところ、収縮期起立性低血圧
(20 mmHg 以上低下)発現頻度の上昇が認められた(投与前の 4%に対し投与後では 18%)。
少数名ではあるが、本剤皮下投与後に重度の収縮期起立性低血圧(30 mmHg 以上低下かつ
SBP≦90 mmHg)が発現した。 進行期パーキンソン病患者における本剤の臨床試験で、550 名中 59 名(11%)に起立性低血圧、
低血圧及び失神が発現した。これらの有害事象のうち、重篤なものは 4 名(1%未満)で、10名(2%)が投与中止に至った。投与開始時やその後の長期投与期間中にも、時期を問わず発
現している。血圧低下が、その他に認められた重要な有害事象(転倒等)に関与しているのか
どうかは不明である。 アルコール、降圧剤、血管拡張剤(特に硝酸塩)との併用によって、本剤が血圧に及ぼす影響
が増大する可能性がある。本剤投与中はアルコール摂取を避け、降圧剤や血管拡張剤と併用す
る必要のある場合は特に注意すること。(「使用上の注意:薬物相互作用」及び「患者への注
意事項」の項参照)
転倒:
パーキンソン病患者はもともと姿勢反射障害がある上に、患者によっては自律神経症状もあり、
また、抗パーキンソン病薬の降圧作用に起因する失神により転倒するリスクが高い。本剤皮下
投与によって血圧低下と運動量の変化が同時に起きることにより転倒のリスクが高まるおそれ
がある。(注意:症候性低血圧及び使用上の注意:ジスキネジー) 臨床試験では 30%の患者で転倒と考えられる事象が報告され、約 5%が重篤な有害事象と判断
された。しかしながら、これらのデータは対照群を置かない非盲検試験で得られたものであり、
また進行期パーキンソン病患者での転倒の発現率が不明であることから、本剤との関連性を正
確に評価することはできなかった。
幻覚/精神病様行動: 臨床試験中に、幻覚が 14%の患者に発現した。あるプラセボ対象二重盲検比較試験において、
幻覚あるいは錯乱が本剤投与群の 10%に発現し、プラセボ投与群では 0%であった。幻覚によ
り本剤の投与が 1%の患者で中止された。 市販後からの報告において、本剤の投与開始あるいは増量後に重度の精神病様行動を含む精神
症状や行動異常が新規発現する、あるいは悪化する可能性が示された。他の抗パーキンソン病
薬も、思考や行動に関して似たような作用を持ちうる。この異常な思考や行動は様々な徴候
(妄想念慮、妄想、幻覚、錯乱、失見当識、攻撃的行動、激越、譫妄)として認められる。 精神病性障害を有する患者は精神病が悪化するリスクがあるので、通常は本剤を使用すべきで
はない。また、一部の抗精神病薬はパーキンソン病の症状を悪化させる、あるいは本剤の効果

を減弱させる可能性がある。(「使用上の注意:薬物相互作用:ドパミンアゴニスト」の項参
照)
日常生活動作中の突発性睡眠:
本剤を皮下注射している患者が、日常生活において眠気の前兆なしに突然眠り込んだという文
献報告がある。本剤による傾眠が多いのは明らかであるが、日常生活動作中の突発性睡眠が起
きる場合には、患者から特に報告がなくても必ず先行する眠気があると専門医の多くが考えて
いる。本剤の使用開始後かなり経過してから発現することもあるので、眠気や傾眠状態につい
て継続的に患者を観察すること。また、患者は特定の動作中における傾眠状態や眠気について、
直接質問を受けるまで気がついていない場合がある、ということに留意すること。 本剤の投与を開始する前に、傾眠が発現することがあるという可能性について患者に注意し、
特に併用している鎮静剤、睡眠障害の有無など、本剤投与によりリスクが増大する可能性があ
るファクターについて確認すること。著しい日中の眠気、あるいは能動的な動作(会話、食事
等)中に突発性睡眠が発現した患者については、通常は本剤の使用を中止すること。本剤を継
続する場合には、車の運転や、その他の危険性がある活動を避けるよう、患者に指導する。減
量により日常生活動作中の突発性睡眠が改善できるかどうかについては、十分な情報はない。
冠動脈系有害事象:
臨床試験中、本剤を投与した患者の 4%に狭心症、心筋梗塞、心停止及び突然死が発現した。
狭心症、心筋梗塞は本剤投与後短時間(2 時間以内)で発現したのに対し、心停止及び突然死
は、本剤投与との時間的関連性は認められなかった。本剤は安静時の SBP、DBP を低下させる
が、それ自体、冠状動脈(及び脳)の虚血状態を悪化させるおそれがある。心臓血管系及び脳
血管系疾患の患者に本剤を投与する場合は特に注意が必要で、冠状動脈虚血や脳虚血の徴候、
症状が認められた場合には、投与継続するか否か慎重に再検討すること。
硫酸塩によるアレルギー:
本剤は、一部の患者においてアナフィラキシー症状や、生命にかかわる、又はそれに準じて重
篤な喘息発作等のアレルギー反応を引き起こす可能性がある亜硫酸塩(ピロ亜硫酸ナトリウ
ム)を含有している。一般に、硫酸塩に対する過敏性の有病率は不明であるが、低いと推察さ
れる。硫酸塩に対する過敏性反応の頻度は、喘息患者で高い傾向がある。
注射部位反応:
臨床試験で本剤の皮下注射を受けた 550 名のうち 26%の患者が、挫傷(16%)、肉芽腫
(4%)、そう痒症(2%)等の注射部位反応を訴えた。比較試験での本剤注射の例数は少ない
(患者総数、患者一人あたりの総投与数とも)ものの、比較試験中の本剤投与による注射部位
反応の報告数は、プラセボと同程度であった。
薬物乱用の危険性:
他の国々でパーキンソン病患者による本剤乱用の報告がまれにある。投与頻度が高くなってい
き、幻覚、ジスキネジー、異常行動に至るのが特徴である。これらの患者にはリビドー亢進を
伴う精神性的刺激がもともとあるものと考えられる。運動兆候に対して多すぎる使用といった
本剤乱用の徴候が認められないかどうか、注意して観察すること。

使用上の注意:
ジスキネジー:
本剤によりジスキネジー又はジスキネジーの悪化が起きることがある。臨床試験ではジスキネ
ジー又はジスキネジーの悪化が 24%で報告された。2%の患者がジスキネジーのために試験を
中止した。
ドパミン受容体作動薬投与により報告されている事象:
本剤投与により下記の事象が発現したという報告はないが、これらは他のドパミン受容体作動
薬投与との因果関係があると報告されている。
退薬による異常高熱及び錯乱 本剤の投与により発現したとの報告はないが、原因不明の悪性症候群に似た症候(体温上昇、
筋固縮、意識低下、自律神経障害等)が抗パーキンソン病薬の急激な減量、中止、変更で報告
されている。
線維化性の合併症 後腹膜線維症、肺浸潤、胸水、胸膜肥厚及び心弁膜症が麦角系ドパミン受容体作動薬を投与し
た患者で報告されている。薬剤の投与を中止すれば、これらの有害事象は軽快することがある
が必ずしも完治はしない。これらの有害事象の発現には、投与薬剤の麦角構造が関係している
と考えられるが、非麦角系ドパミン受容体作動薬でも発現するかどうかは不明である。
悪性黒色腫 疫学研究によると、パーキンソン病患者の悪性黒色腫発症リスクは、一般人口に比べて高い
(約 2~6 倍)。この増加がパーキンソン病自体によるものか、治療薬物等、他の因子が原因
であるのかは不明である。 上記の理由で、本剤の投与にあたって、患者及び医師は、適応疾患を問わず、頻繁にかつ定期
的に黒色腫について確認することが望ましい。しかるべき資格を有する者(皮膚科医等)によ
る定期的皮膚検査を実施するのが理想的である。
持続勃起症:
本剤投与で持続性の有痛性勃起が発現することがある。臨床試験では 361 名中 3 名(1%未
満)に有痛性勃起が認められ、1 名は投与を中止した。臨床試験では外科処置を要した患者は
認められなかったが、重症の場合には外科処置が必要となることもある。
肝障害:
軽度及び中等度の肝障害のある患者に本剤を投与する場合、Cmax 及び AUC が上昇するおそれ
があるので注意すること。重度の肝障害を有する患者を対象とした臨床試験は行われていない。

軽度及び中等度の腎障害のある患者に本剤を投与する場合、Cmax 及び AUC が上昇するおそれ
があるので開始用量を 1 mg にすること。重度の腎障害を有する患者を対象とした臨床試験は
行われていない。(「臨床薬理」及び「用法・用量」の項参照)
アルビノラットでの網膜病変:
ドパミン受容体作動薬を長期間投与したアルビノラットに網膜変性が認められている(主に 2年間の癌原性試験期間中)。この病変はアルビノラットを用いたより短期間の強照度下の試験
でも認められている。アルビノマウスでの 2 年間の発癌性試験や、ラット及びサルでの 1 年間
の試験ではこのような変化は認められていない。本剤に関して発癌性試験は行われていないが、
作用機序を考慮すると同様の毒性が発現する可能性があるかもしれない。ヒトへの影響は確認
されていないが、脊椎動物に一般的にみられる網膜障害のメカニズムの崩壊(例えば光照射に
よる桿体細胞の離脱)が関わっている可能性もあることから、無視することはできない。 患者への注意事項:
本剤は皮下注射だけを目的としたものであり、静脈内注射してはならない。患者並びに介護者
には添付の患者用説明書並びにペン型インジェクターの使用説明書を読んでもらうこと。患者
には本剤を処方通りに正しく使用するよう指導すること。医療監督者のいない状況で本剤を使
用することになる患者や介護者には、医師その他適切な資格を有する医療従事者が製品の適切
な使用に関する指導を行い、初回投与は観察下で行うこと。 中でもペン型インジェクターの使い方については次の 2 点に特に注意して詳細な指導を行うこ
と:1)薬剤の投与量は mg ではなく mL で示されることを患者が理解する必要がある。投与
量 1 mg の時の注射器の表示は 1.0 mL ではなく、0.1 mL であることに特に注意喚起すること
(1.0 mL は投与量が 10 mg となる)。処方量が 1 mg である場合、生命にかかわる過量投与が
起きるのを防ぐために、患者並びに介護者にはこの区別を理解させることが重要である。2)カートリッジに入っている薬液量が不足している場合でも、目盛を維持用量に合わせることは
可能であることを、患者並びに介護者に説明すること。この場合、その回の注射では維持用量
の一部が投与されることになり、不足分の用量が注射器の窓に表示される。正しい維持用量全
量を投与するためには、カートリッジを新しいものに交換し、不足分の用量に目盛を正確に合
わせる必要がある。このような状況は可能な限り避けるべきで、患者並びに介護者には、カー
トリッジには 1 回分の維持用量を投与するだけの薬液が残っていない可能性があることを説明
しておくこと(例えば、残量が不足していたらカートリッジを取り替えられるように、患者や
介護者にカートリッジ 1 本で何回注射したか記録をとっておくよう勧めるなど)。 注射部位を変えながら適切に消毒を行って投与するよう患者に指導すること。
患者には、幻覚その他の精神病様行動の徴候が起こる可能性があることを説明すること。また、
患者が精神病を有している場合には、悪化のリスクがあるので通常本剤を使用すべきではない
こと、多くの抗精神病薬は本剤の作用を減弱させる可能性があることも説明すること。 起立性低血圧が起こる可能性があり、浮動性めまい、嘔気、失神、ときに発汗等の症状が伴う
場合もあることを、患者に説明すること。低血圧や起立性症状は投与開始時もしくは時期を問

わず用量増量時の発現が多い(数ヶ月間の投与後に発現した複数の症例が認められている)。
従って、(特に長時間)座ったり横になったりしていた後や、特に本剤使用開始直後には、急
に立ち上がらないよう患者に注意喚起すること。アルコール、降圧剤、血管拡張剤は本剤の降
圧作用を増強させることがある(「注意:症候性血圧低下」、「注意:薬物相互作用」の項参
照)。 本剤には傾眠や日常生活動作中の突発性睡眠の可能性などの鎮静作用があることを患者に注意
喚起すること。傾眠はしばしば見られる有害事象で、重大な結果をもたらすことがあるので、
本剤の使用経験を十分に積んで精神/運動能力に影響するかどうか正しく判断できるまでは、
患者は車の運転その他危険を伴う作業は行わないこと。本剤による治療中のどの時点において
も、傾眠が強くなったり、日常生活動作中(テレビを観ているとき、車に同乗しているとき
等)に眠り込んでしまったりした場合には、医師に相談するまでは車の運転その他危険を伴う
作業はしないよう、患者に説明すること。相加作用が起きる可能性あることから、他の鎮静剤
を服用していたり飲酒習慣のある患者には注意を促すこと。 本剤の生殖能及び胚・胎児発生への影響については試験が行われていないので、妊娠した場合
や妊娠を考えている場合は、その旨を医師に伝えるよう患者に指導すること(「使用上の注
意:妊婦への投与」の項参照)。 本剤は乳汁中に排泄される可能性があるので、授乳を考える場合には医師に伝えるよう指導す
ること。 パーキンソン病治療に使用される中枢性ドパミン作動薬(本剤を含む)を1剤もしくは複数使
用中に、賭博への強い衝動、過剰な性的衝動やその他の強い衝動が起こり、その衝動をコント
ロールできない例が報告されている。薬剤がこれらの事象を引き起こすのかどうかは証明され
ていないが、これらの衝動は、薬剤の減量または中止により、いくつかの症例で回復したと報
告されている。医師は、本剤使用中に、賭博の衝動、性的衝動の増強、あるいはその他の衝動
が新たに発現したか、または増悪したかについて、患者に確認する必要がある。患者は、本剤
使用中に上記のような衝動が新たに発現または増悪した場合は、医師に知らせる必要がある。
患者が本剤使用中に、このような衝動を発現した場合には、医師は、薬剤の減量または投与中
止を考慮する必要がある。 まれに乱用(処方された投与回数を著しく上回る本剤の使用)の報告がある。本剤の乱用によ
り不適切な性行動が起きることがある。 薬物相互作用:
5HT3拮抗剤: 本剤とオンダンセトロンとの併用で著しい血圧低下及び意識消失が報告されているので、5HT3
拮抗剤(オンダンセトロン、グラニセトロン、ドラセトロン、パロノセトロン、アロセトロン
等)とは併用しないこと。(「禁忌」の項参照)
降圧剤と血管拡張剤: 降圧剤又は血管拡張剤を併用している患者(n=94)のほうが併用していない患者(n=456)より
も、以下に記載する事象の発現率が高かった:「低血圧」10% vs 4%、「心筋梗塞」3% vs 1%、

重篤な「肺炎」5% vs 3%、重篤な「転倒」9% vs 3%、「骨及び関節損傷」6% vs 2%。多くの事
象について発症機序は不明であるが、低血圧の悪化によるものかもしれない。(「注意:症候
性低血圧」の項参照)
ドパミン拮抗薬: アポモルヒネはドパミン受容体作動薬であり、抗精神病薬(フェノチアジン、ブチロフェノン、
チオキサンチン)、メトクロプラミド等のドパミン拮抗薬は本剤の有効性を減弱させることが
ある。抗精神病薬の投与を行っている精神疾患患者には、有益性がリスクを上回ると判断され
る場合のみドパミン受容体作動薬を投与すること。
QT/QTc 間隔を延長させる薬剤: 本剤と QT/QTc 間隔を延長させる薬剤とを併用する場合には、十分注意すること。(「QT注意:延長及び催不整脈作用の可能性」の項参照)
薬剤/臨床検査との相互作用: 本剤と臨床検査との間で相互作用は認められていない。
癌原性、変異原性、遺伝毒性:
本剤の癌原性試験は行われていない。 細菌を用いた in vitro Ames 試験並びに in vitro マウスリンフォーマ試験において、本剤には変異
原性があることが確認されている。また、ヒトリンパ球細胞を用いた in vitro 染色体異常試験
及び in vitro マウスリンフォーマ試験において、染色体異常が認められた。マウスでの小核試
験では異常は認められなかった。 雄ラットでの受胎能試験において、2 mg/kg(mg/m2換算で 大推奨用量の 0.6 倍)の皮下投与
で授精能への影響が認められたという論文報告がある。カニクイザルでの 39 週の試験では、
1.0 mg/kg 及び 1.5 mg/kg(mg/m2換算で 大推奨用量の 0.6 倍及び 1 倍)の皮下投与で睾丸重量
の有意な低下が認められた。 妊婦への投与:
プレグナンシーカテゴリーC: 本剤について生殖発生毒性試験は行われていない。また、本剤を妊婦に投与した場合の胎児へ
の影響や生殖能力に対する影響は不明である。妊婦への投与は、本剤投与の必要性が明白な場
合に限る。 授乳婦への投与:
アポモルヒネのヒト乳汁中への移行は不明である。薬剤の多くは乳汁中に移行すること、また
乳児では本剤により重篤な副作用が発現するおそれがあることから、母親に対する本剤の重要
性を考慮して、授乳を止めるか投薬を中止するかを決めること。 小児への投与:
小児に対する安全性及び有効性は確立されていない。

高齢者への投与:
臨床試験において、65 歳未満の患者は 239 名、65 歳以上は 311 名であった。有害事象の発現
率は高齢者と非高齢者でほぼ同様(90% vs 87%)であったが、高齢者において錯乱及び幻覚の
発現が多かった。また、重篤な有害事象(生命にかかわる事象、入院又は後遺症が残るような
重篤な事象)も高齢者に多く認められた(27% vs 17%)。転倒(骨及び関節の損傷)、心血管
系・呼吸器系・消化器系の有害事象も高齢者に多く認められた。高齢者のほうが、有害事象が
原因で本剤投与を中止することが多かった(29% vs 21%)。 有害事象:
比較試験における有害事象発現率: レボドパと他の抗パーキンソン薬とを併用しているパーキンソン病患者 550 名に本剤が投与さ
れた。患者の 86%がドパミン受容体作動薬を併用していた。すべての患者で、本剤投与開始前
に運動合併症(オフ現象)が認められた。有害事象は治験担当医が各自の用語で記録した。推
定発現率を意味あるものにするため、類似した事象は MedDRA の標準分類を用いてまとめた。 比較試験で も多く見られた有害事象はあくび、ジスキネジー、嘔気・嘔吐、傾眠、浮動性め
まい、鼻漏、幻覚、浮腫、胸痛、発汗増加、潮紅、蒼白であった。 無作為化比較試験のうち本剤での有害事象が も多かったのは、アポモルヒネ未使用のパーキ
ンソン病患者に 長 4 週間投与した多施設共同プラセボ対照無作為化並行群間比較試験であっ
た(表 1)。この試験における本剤の投与量は 2~10 mg で、個々の患者のレボドパ通常量で
の効果と同程度に症状のコントロールができる量とした。処方に当たっては、この表に示す数
値は患者背景その他要因が治験とは異なる医療現場での有害事象発現を予測するために用いる
ことは出来ないということを認識しておくこと。同様に、ここに示した発現頻度は、異なる治
療法、用法、別の治験医による別の臨床試験での数値と比較することは出来ない。しかし、こ
こに示す数値は試験参加集団における有害事象発現率への薬剤性及び非薬剤性要因の相対的な
関与を推定する上での何らかの基準となることは確かである。

全フェーズ II/III 臨床試験中に認められたその他の有害事象: 本剤は 550 名に投与され、うち 89%が少なくともひとつの有害事象を経験した。表 1 に示した
ものの他に も多かった有害事象(少なくとも 5%以上の患者に認められ治験薬と関連がある

と考えられる有害事象)は、発現頻度の高い順に、注射部位反応、転倒、関節痛、不眠、頭痛、
うつ病、尿路感染、不安、うっ血性心不全、四肢疼痛、背部痛、パーキンソン病悪化、肺炎、
錯乱、発汗増加、呼吸困難、疲労、斑状出血、便秘、下痢、脱力、脱水であった。 市販後に認められた事象
臨床試験で報告されている有害事象に加えて、パーキンソン病患者における本剤の市販後使用
成績調査において以下の有害事象が確認されたが、不確定な対象集団による自発報告であるた
め、発現率や本剤との因果関係を正確に把握することは困難である。次の精神障害が報告され
ている:衝動制御障害、病的賭博、リビドー亢進(性欲過剰を含む)。 薬物乱用及び依存:
薬物乱用の危険性:
オフ現象が頻繁に起きる場合に、すべてのオフ現象についてすべての症状を回避しようとする
意図での本剤使用が、本剤乱用(処方された頻度を超える増量)の動機としてまれに報告され
ている。本剤乱用のもうひとつの動機として、陰茎勃起刺激及びリビドー亢進に関連する精神
性的反応がまれに報告されている。過剰使用の男性で報告されている有害事象は、頻繁な陰茎
勃起、非定型的性行動、リビドー亢進、ジスキネジー、激越、錯乱、うつ病等である。進行期
パーキンソン病と関連するオン-オフ現象又はウェアリングオフ現象がみられる患者で、オフ
状態の急性期(レスキュー)治療として本剤を投与時の依存性発現の可能性について評価する
ための試験は実施されていない。 過量投与:
間歇投与: 62 歳の男性に誤って 25 mg を皮下投与した過量投与例が報告されている(Journal of Neurology, Neurosurgery, and Psychiatry (1990), Vol. 53, pp. 96-102)。この患者は投与 3 分後に悪心を催し、
20 分間意識が消失した。その後意識は回復し、心拍数 40 回/分、仰臥位血圧 90/50 であった。1時間以内に症状は完全に回復した。 用法・用量:
混乱を防ぐため、本剤の処方は常に mL 表示で行うこと。0.6 mL(6 mg)以上の用量は推奨し
ない。患者及び介護者には投与準備並びに注射の方法を詳しく指導し、特にペン型インジェク
ターを正しく使用するよう注意すること(「使用上の注意:患者への注意事項」の項参照)。 本剤は皮下注射専用である。本剤投与の開始に際しては必ず制吐剤を併用すること(「注意:
嘔気・嘔吐」の項参照)。試験では殆どが制吐剤として trimethobenzamide を使用しており、原
則として同剤を使用すること。本剤の投与開始 3 日前から trimethobenzamide(900 mg/日)を
投与し、少なくとも 2 ヶ月間は続けること。 本剤とオンダンセトロンとの併用で著しい低血圧並びに意識消失が報告されていることから、
本剤と 5HT3 拮抗薬(オンダンセトロン、グラニセトロン、ドラセトロン、パロノセトロン、
アロセトロン等)とは併用しないこと(「禁忌」の項参照)。 本剤の投与は 0.2 mL(2 mg)から開始し、有効性、忍容性に基づいて、以下の手順で 大推

奨用量である 0.6 mL(6 mg)まで漸増すること。 医療従事者が血圧の推移をチェックできる環境で、オフ状態にある患者に 0.2 mL(2 mg)の試
験投与を行う。投与前、投与 20 分後、40 分後、60 分後に臥位血圧及び立位血圧を測定する。
この試験投与量で臨床的に重大な起立性低血圧がおきた患者は本剤による治療に適していない
とみなされる。0.2 mL(2 mg)で忍容性があり、有効性が認められた場合は、開始用量は 0.2 mL(2 mg)とし、オフ現象がおきたときに頓用する。投与量は必要に応じて数日おきに 0.1 mL(1 mg)ずつ、外来で増量可能である。 このほかには、忍容性が認められた用量(0.3 mL もしくは 0.4 mL)を観察下での試験用量と
して定め、次にその試験用量より 0.1 mL(1 mg)少ない用量を用いて外来での投与試験(定期
的に有効性、忍容性の両方を評価する)を開始するというのが用量決定の一般指針である(下
記に詳しく述べる)。 0.2 mL(2 mg)で忍容性には問題ないが有効性が認められなかった患者については、次に発現
したオフ症状に対し 0.4 mL(4 mg)を投与してもよい。但し、初回用量 0.2 mL(2 mg)投与
から 2 時間以上経過してから実施する。投与前、投与 20 分後、40 分後、60 分後に臥位血圧及
び立位血圧を測定する。0.4 mL(4 mg)で忍容性がみられた場合、開始用量は 0.3 mL(3 mg)とし、オフ現象がおきたときに頓用する。投与量は必要に応じて数日おきに 0.1 mL(1 mg)ず
つ、外来治療で増量可能である。0.4 mL(4 mg)で忍容性に問題がある場合は、次に発現した
オフ現象に対し 0.3 mL(3 mg)を投与してもよいが、0.4 mL(4 mg)の投与から 2 時間以上経
過してからでなければならない。投与前、投与 20 分後、40 分後、60 分後に臥位血圧及び立位
血圧を測定する。0.3 mL(3 mg)で忍容性がみられた場合、開始用量は 0.2 mL(2 mg)とし、
オフ現象がおきたときに頓用する。必要に応じ、また 0.2 mL(2 mg)で忍容性に問題ない場合
には、投与量は数日経過した後 0.3 mL(3 mg)にまで増量出来る。そのような患者の場合、外
来治療では通常 0.4 mL(4 mg)まで増量してはならない。 本剤の開発プログラムで試験を行った患者の殆どは 0.3~0.6 mL(3~6 mg)で効果があった、
比較試験では 0.6 mL(6 mg)を超える用量で有効性が増大したというエビデンスはないので、
0.6 mL(6 mg)を超える用量での投与は推奨できない。開発段階での平均投与回数は 1 日 3 回
で、1 回に 0.6 mL(6 mg)を超える用量の投与、1 日 6 回以上の投与、1 日合計投与量で 2.0 mL(20 mg)を超える投与の経験は、非常に少ない。 特定のオフ現象に対して本剤を 1 回投与して効果がなかった場合、そのオフ現象にもう一度投
与してはならない。1 回のオフ現象に対する再投与の効果についての系統的試験は行われてお
らず、再投与の安全性は明らかにされていない。 本剤による治療が一定期間(1 週間)以上中断された場合は、0.2 mL(2 mg)から投与を再開
し、有効用量まで漸増していくこと。 軽度もしくは中等度の肝障害のある患者に投与する場合は Cmax及び AUC の上昇に注意するこ
と(「臨床薬理」並びに「使用上の注意」の項参照)。 軽度もしくは中等度の腎障害のある患者に投与する場合は、試験用量、開始用量を 0.1 mL(1

APO-go Pen 10 mg/ml 注射用溶液
1. 医薬品の名称 APO-go PEN 10 mg/ml 注射液* * 本文中での略称は APO-go® Pen とする。 2. 定性的及び定量的組成
1 ml 中にアポモルヒネ塩酸塩 10 mg を含有する。 添加物の全リストは、6.1 項を参照のこと。
3. 剤型
本剤は、無色澄明の注射液である。 4. 臨床成績 4.1 効能・効果
レボドパ及び/又は他の経口ドパミン受容体作動薬を服用しているにもかかわらず持続し
ているパーキンソン病の患者における運動合併症(オンオフ現象)の治療 4.2 用法・用量
APO-go Pen 10 mg/ml 注射液は、間歇注射による皮下投与用である。 用量 成人 用法 対象患者について: 本剤を使用する患者は、自らのオフ症状発現を認識して、必要時に自分で注射可能、又は
介護者が注射可能な患者とする。 投与開始前に、ドンペリドン投与(通常 60 mg/日)を 2 日以上行っていることが必須で
ある。 本剤の投与は、専門の医療機関で開始すること。投与は、パーキンソン病の治療経験があ
る医師(神経科医等)による監督下で行うこと。ドパミン受容体作動薬との併用の有無に
かかわらず、APO-go 投与開始前に患者へのレボドパ投与を 適化する必要がある。 閾値用量の決定: 患者ごとの至適用量は、以下のような漸増法により決定する。 アポモルヒネ塩酸塩 1 mg(0.1 ml:約 15~20 μg/kg)を患者のオフ時に皮下注射し、運動
機能改善効果を 30 分間観察する。 効果がみられないか不十分な場合には、2 回目の用量としてアポモルヒネ塩酸塩 2 mg(0.2 ml)を皮下注射し、更に 30 分間観察を行う。

投与量の増量は、次の注射との間隔を 40 分以上あけて、十分な運動機能改善効果が得ら
れるまで行う。 投与法の確立: 至適用量が決まったら、オフ症状発現時に、下腹部又は大腿部外側に単回皮下注射により
投与する。同一人でも注射部位により吸収に差が生ずることがあるので、本剤に対する反
応性を評価するため、投与後 1 時間患者の観察を行うこと。患者の反応性により、投与量
の変更も可能である。 本剤の至適用量は個人間で差があるが、各患者について、いったん確立したら比較的一定
量で保つ。 投与継続にあたっての注意: 本剤の 1 日量は患者間で大きな差があるが、通常は、3~30 mg の範囲内で、1 日に 1~10回、時には 12 回頓用する。 本剤の 1 日量は 100 mg を超えず、1 回の注射では 10 mg を超えないことが望ましい。 臨床試験では、レボドパの用量をある程度減量することが可能な症例が多かった。但し、
これには患者間で大きな差があるので、経験のある医師の指導下で注意深く実施していく
必要がある。 投与法をいったん確立したら、患者によってはドンペリドンの投与を徐々に減らしていく
ことが可能であるが、嘔吐も低血圧もなく投与を中止することができる患者は少数である。 小児等への投与: 本剤の 18 歳未満の患者への投与は禁忌である(4.3 項参照)。 高齢者への投与: パーキンソン病患者集団のうち高齢者の数は非常に多く、本剤の臨床試験の対象患者でも
高齢者は高い割合を占めている。本剤を投与する高齢患者の管理は、若年患者の場合と同
様である。 腎機能障害: 腎機能障害患者では、成人及び高齢者で推奨されている投与方法に準じる(4.4 項参照)。
4.3 禁忌
呼吸抑制、認知症、精神病、肝機能不全患者に本剤を投与してはならない。 レボドパ服用後のオン状態で重篤なジスキネジー又はジストニーが発現する患者への本剤
の投与は避けること。 アポモルヒネ又は本剤の添加物に対して過敏症を示すことが明らかな患者に本剤を投与し
てはならない。 本剤の 18 歳未満の患者への投与は禁忌である。
4.4 警告・使用上の注意
腎疾患、肺疾患、又は循環器系疾患がある患者及び悪心・嘔吐を発現しやすい患者に本剤

を投与する場合は十分注意すること。 高齢や体力の低下した患者では、投与開始時には特に注意することが望ましい。 ドンペリドンによる前処置を行った場合でも、本剤投与が低血圧を誘発することがあるの
で、心疾患を合併している患者、又は降圧剤等の血管作動性薬剤を投与している患者、特
に起立性低血圧を合併している患者では、慎重に投与すること。 本剤は、特に高用量で投与した場合、QT 間隔を延長させる可能性があるので、torsades de pointes 型不整脈のリスクがある患者には、慎重に投与すること。 本剤投与により、注射部位反応が発現する。これは、注射部位を変えていくことにより、
又は結節及び硬結部分に(使用可能ならば)超音波をあてることによって軽減できること
がある。 本剤は、まれに重篤なアレルギー反応及び気管支痙攣を起こすことがある重亜硫酸ナトリ
ウムを含有している。 本剤を投与した患者で、溶血性貧血及び血小板減少症が報告されている。レボドパと本剤
を併用する場合には、定期的に血液検査を行うこと。 本剤と他の薬剤、特に治療域が狭い薬剤との併用は注意することが望ましい(4.5項参
照)。 多くの進行期パーキンソン病患者で精神障害がみられる。一部の患者では本剤投与により
神経精神障害が悪化することが示されている。 これらの患者への本剤投与は、特に慎重に行うこと。 特にパーキンソン病患者では、本剤投与により傾眠がみられており、また他のドパミン受
容体作動薬により突発性睡眠が発現することがある。患者にこのことを伝え、本剤投与中
の車の運転、機械の操作には注意するよう説明すること。傾眠の既往がある患者は車の運
転、機械の操作をしないようにすること。更に、減量又は投与中止が検討される場合もあ
る。 パーキンソン病治療のため本剤等のドパミン受容体作動薬を投与した患者で、病的賭博、
リビドー亢進、性欲過剰が報告されている。 4.5 他の薬剤との相互作用及びその他の相互作用
本剤を投与する患者はほとんど、他のパーキンソン病治療薬を併用している。本剤投与開
始時には、異常な副作用又は作用増強の徴候がみられないか患者をモニターする必要があ
る。 神経遮断剤を本剤と併用した場合に、拮抗作用を示すことがある。クロザピンと本剤との
間で相互作用が起きる可能性があるが、クロザピンは精神症状の合併を軽減する目的で投
与されることもある。 本剤が他剤の血漿中濃度に及ぼしうる影響については、確認されていない。したがって、
本剤と他の薬剤、特に治療域が狭い薬剤とは、注意して併用することが望ましい。
降圧・強心剤 本剤はドンペリドンとの併用の場合でも、これらの薬剤の降圧作用を増強することがある
(4.4 項参照)。 本剤と QT 間隔を延長させることが明らかな薬剤との併用は避けるのが望ましい。

4.6 妊婦、授乳婦への投与 妊婦での本剤使用経験はない。 動物での生殖発生毒性試験で催奇形性は認められていないが、ラットに投与した場合、母
体に対して毒性がある用量で出生児に呼吸障害が発現することがある。ヒトでのリスクの
可能性については不明である。5.3 項参照。 投与が必要であることが明らかな場合を除いて、本剤を妊娠中に投与してはならない。 本剤の母乳への移行については不明である。授乳の継続/中止又は本剤の継続/中止につ
いては、小児への授乳の有益性と女性患者に対する本剤の有益性とを考慮に入れて決定す
ること。 4.7 運転能力及び機械操作能力への影響 本剤使用中に傾眠が認められる患者には、傾眠症状が出なくなるまで、車の運転や、覚醒
低下が自分自身や他の人々を重篤な損傷、死亡のリスクにさらすことになるような活動
(機械の操作等)に従事することを控えるように注意すること(4.4項参照)。 4.8 副作用
非常に高頻度:Very common(>1/10) 高頻度:Common(>1/100~<1/10) ときに:Uncommon(>1/1,000~<1/100) 非常にまれに:Very rare(<1/10,000) 不明(現在までのデータからでは推定できない) 血液及びリンパ系障害 ときに: 本剤を投与した患者で、溶血性貧血及び血小板減少症が報告されている。 まれに: 本剤投与中に好酸球増加症がまれに発現している。 免疫系障害 まれに: ピロ亜硫酸ナトリウムを含有しているため、アレルギー反応(アナフィラキシー、気管支
痙攣等)が発現することがある。 精神障害 高頻度: パーキンソン病患者では精神障害の発現頻度が高いため、本剤投与にあたっては特に注意
すること。本剤投与中に精神障害(一過性で軽度の錯乱、幻視等)が発現している。 不明: パーキンソン病治療のため本剤等のドパミン受容体作動薬を、特に高用量で投与した患者
は、病的賭博、リビドー亢進、性欲過剰の徴候を示すことが報告されているが、それらは
通常、減量又は投与中止によって改善される。

神経系障害 高頻度:本剤の投与開始時に、投与毎に一過性の鎮静が発現することがあるが、通常 2、3 週後に消失する。 本剤投与と傾眠の間には関連性がある。 浮動性めまい/頭部ふらふら感も報告されている。 ときに: 本剤は「オン」時にジスキネジーを誘発することがあるが、患者によっては重篤な場合
があり、また一部の患者では投与中止に至ることがある。 血管障害 ときに: 体位性低血圧がみられることがあるが低頻度で、通常は一過性である(4.4 項参照)。 呼吸・胸郭・縦隔障害 高頻度: 本剤投与中にあくびが報告されている。 ときに: 呼吸困難が報告されている。 胃腸障害 高頻度: 特に本剤の導入時に悪心、嘔吐が発現することがあるが、それは通常、ドンペリドンを投
与しなかったことによる(4.2 項参照)。 皮膚及び皮下組織障害 ときに: 限局性及び全身性皮疹が報告されている。 一般・全身障害および投与部位の状態 非常に高頻度: ほとんどの患者で、特に継続使用により、皮下結節、硬結、紅斑、圧痛、脂肪織炎等の注
射部位反応がみられる。その他いろいろな局所反応(刺激、そう痒、挫傷、疼痛)が発現
することもある。 ときに: 注射部位壊死と潰瘍形成が報告されている。 不明: 末梢性浮腫が報告されている。 臨床検査 ときに: 本剤投与患者でクームス試験陽性が報告されている。

4.9 過量投与 この投与経路での本剤の過量投与については、ほとんど臨床経験がない。過量投与の症状
の治療は経験的に、以下に記載の方法で行われる。 激しい嘔吐はドンペリドンによる処置が有効な可能性がある。 呼吸抑制はナロキソンによる処置が有効な可能性がある。 低血圧:ベッドの足を高くする等の適切な処置をすること。 徐脈はアトロピンによる処置が有効な可能性がある。
5. 薬効薬理 5.1 薬力学的特性
薬効分類:ドパミン受容体作動薬 ATC 分類:N04B C07 アポモルヒネはドパミン受容体を直接刺激する物質で、D1・D2 受容体両方の作動薬の特
性を持っており、レボドパとは輸送経路も代謝経路も異なる。 無処置実験動物において、アポモルヒネの投与は黒質線条体細胞の発火率を抑制し、低用
量で自発運動(内因性ドパミン遊離のシナプス前抑制を表していると考えられる)を減少
させることがわかったが、パーキンソン病の運動障害に対する作用は、シナプス後受容体
を介すると考えられる。ヒトにおいてもこの二相性作用がみられる。 5.2 薬物動態学的特性
アポモルヒネの皮下投与後の動態は 2 コンパートメントモデルで表され、代謝半減期は 5(±1.1)分、消失半減期は 33(±3.9)分である。臨床反応は、髄液中のアポモルヒネ濃度
との相関性が高く、本剤の分布は 2 コンパートメントモデルで も良く表すことができる。
アポモルヒネは皮下組織から速やかにかつ完全に吸収される。このことは、臨床作用の速
やかな発現(4~12 分)と相関しており、そして本剤の臨床作用の持続時間が短い(約 1時間)のはクリアランスが速いことによって説明される。アポモルヒネは、全体の 10%以

の一端はブロモブチルゴム製ピストンで、もう一端はブロモブチルゴム/アルミニウムキ

agonist. Apomorphine hydrochloride is chemically designated as 6aβ-Aporphine-10,11-diol
hydrochloride hemihydrate with a molecular formula of C17H17NO2 • HCl • 1/2H2O. Its structural
formula and molecular weight are:
Apomorphine hydrochloride appears as minute, white or grayish-white glistening crystals or as
white powder that is soluble in water at 80°C.

APOKYN® 10 mg/mL is a clear, colorless, sterile solution for subcutaneous injection and is
available in 3 mL cartridges. Each mL of solution contains 10 mg of apomorphine hydrochloride,
USP as apomorphine hydrochloride hemihydrate and 1 mg of sodium metabisulfite, NF in water
for injection, USP. In addition, each mL of solution may contain sodium hydroxide, NF and/or
hydrochloric acid, NF to adjust the pH of the solution and 5 mg/mL of benzyl alcohol, NF as a
preservative.
CLINICAL PHARMACOLOGY: Mechanism of Action: APOKYN is a non-ergoline dopamine
agonist with high in vitro binding affinity for the dopamine D4 receptor (Ki = 4.4 nM), moderate
affinity for the dopamine D2, D3, and D5 (Ki = 35-83, 26, and 15 nM, respectively), and adrenergic
α1D, α2B, α2C (Ki = 65, 66, and 36 nM, respectively) receptors, and low affinity for the dopamine
D1, serotonin 5HT1A, 5HT2A, 5HT2B, and 5HT2C (Ki = 370, 120, 120, 130, and 100 nM,
respectively) receptors. Apomorphine exhibits no affinity for the adrenergic β1 and β2 or histamine
H1 receptors (Ki > 10,000 nM).
The precise mechanism of action of APOKYN as a treatment for Parkinson’s disease is unknown,
although it is believed to be due to stimulation of post-synaptic dopamine D2-type receptors within
the caudate-putamen in the brain. Apomorphine has been shown to improve motor function in an
animal model of Parkinson’s disease. In particular, apomorphine attenuates the motor deficits
induced by lesions in the ascending nigrostriatal dopaminergic pathway with the neurotoxin
1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) in primates.

Pharmacokinetics: Absorption: Apomorphine hydrochloride is a lipophilic compound that is
rapidly absorbed (time to peak concentration ranges from 10 to 60 minutes) following
subcutaneous administration into the abdominal wall. After subcutaneous administration,
apomorphine appears to have bioavailability equal to that of an intravenous administration.
Apomorphine exhibits linear pharmacokinetics over a dose range of 2 to 8 mg following a single
subcutaneous injection of apomorphine into the abdominal wall in patients with idiopathic
Parkinson’s disease.
Distribution: The plasma-to-whole blood apomorphine concentration ratio is equal to one. Mean
(range) apparent volume of distribution was 218 L (123 – 404 L). Maximum concentrations in
cerebrospinal fluid (CSF) are less than 10% of maximum plasma concentrations and occur 10 to
20 minutes later.
Metabolism and Elimination: The mean apparent clearance (range) is 223 L/hr (125 – 401 L/hr)
and the mean terminal elimination half-life is about 40 minutes (range about 30 to 60 minutes).
The route of metabolism in humans is not known. Potential routes of metabolism in humans
include sulfation, N-demethylation, glucuronidation and oxidation. In vitro, apomorphine
undergoes rapid autooxidation.
Special Populations: The clearance of apomorphine does not appear to be influenced by age,

gender, weight, duration of Parkinson’s disease, levodopa dose or duration of therapy.
Hepatic Impairment: In a study comparing subjects with hepatic impairment (moderately
impaired as determined by the Child-Pugh classification method) to healthy matched volunteers,
the AUC0-∞ and Cmax values were increased by approximately 10% and 25%, respectively,
following a single subcutaneous administration of apomorphine into the abdominal wall. Studies
in subjects with severe hepatic impairment have not been conducted (see PRECAUTIONS and
DOSAGE AND ADMINISTRATION).
Renal Impairment: In a study comparing renally-impaired subjects (moderately impaired as
determined by estimated creatinine clearance) to healthy matched volunteers, the AUC0-∞ and
Cmax values were increased by approximately 16% and 50%, respectively, following a single
subcutaneous administration of apomorphine into the abdominal wall. The mean time to peak
concentrations and the mean terminal half-life of apomorphine were unaffected by the renal status
of the individual. Studies in subjects with severe renal impairment have not been conducted. The
starting dose for patients with mild or moderate renal impairment should be reduced (see
PRECAUTIONS and DOSAGE AND ADMINISTRATION).
Drug-Drug Interactions: Carbidopa/levodopa: Levodopa pharmacokinetics were unchanged
when subcutaneous apomorphine and levodopa were co-administrated in patients. However, motor
response differences were significant. The threshold levodopa concentration necessary for an
improved motor response was reduced significantly, leading to an increased duration of effect

without a change in the maximal response to levodopa therapy.
Other Drugs Eliminated Via Hepatic Metabolism: Based upon an in vitro study, cytochrome
P450 enzymes play a minor role in the metabolism of apomorphine. In vitro studies have also
demonstrated that drug interactions are unlikely due to apomorphine acting as a substrate, an
inhibitor, or an inducer of cytochrome P450 enzymes.
COMT Interactions: A pharmacokinetic interaction of apomorphine with catechol-O-methyl
transferase (COMT) inhibitors or drugs metabolized by this route is unlikely since apomorphine
appears not to be metabolized by COMT.
Clinical Studies: The effectiveness of APOKYN in the acute symptomatic treatment of the
recurring episodes of hypomobility, “off” episodes (“end-of-dose wearing off” and unpredictable
“on/off” episodes), associated with advanced Parkinson’s disease was established in three
randomized, controlled trials. On average, patients participating in these trials had Parkinson’s
disease for 11.3 years and were being treated with L-dopa and at least one other agent, usually an
oral dopamine agonist. One of the three studies was conducted in patients who did not have prior
exposure to apomorphine and two were conducted in patients with at least 3 months of
apomorphine use immediately prior to study enrollment. Almost all patients without prior
exposure to apomorphine began taking an antiemetic (trimethobenzamide) three days prior to
starting apomorphine. After exposure to apomorphine, 50% of patients were able to discontinue
use of a concomitant antiemetic, on average 2 months after initiating apomorphine.

Change in Part III (Motor Examination) of the Unified Parkinson’s Disease Rating Scale (UPDRS)
served as the primary outcome assessment measure in each study. Part III of the UPDRS contains
14 items designed to assess the severity of the cardinal motor findings (e.g., tremor, rigidity,
bradykinesia, postural instability, etc.) in patients with Parkinson’s disease.
The first trial used a parallel design, randomizing 29 patients with advanced Parkinson’s disease to
subcutaneous apomorphine or placebo in a 2:1 ratio. Patients had no prior exposure to
apomorphine. In an office setting, hypomobility was allowed to occur by withholding the patients’
Parkinson’s disease medications overnight. The following morning, patients (in a hypomobile
state) were started in a blinded fashion on study treatment (placebo or 2 mg of apomorphine) and
redosed at increasing doses, after at least 2 hours, until a therapeutic response approximately
equivalent to the individual patient’s response to their usual dose of levodopa was observed (or
until 10 mg apomorphine or placebo equivalent was given). At each redosing, study drug was
increased by 2 mg or 0.2 mL (to 4 mg, 6 mg, 8 mg, or 10 mg of apomorphine) or placebo
equivalent. Of the 20 patients assigned to apomorphine, 18 achieved a therapeutic response at
about 20 minutes that was approximately equivalent to the therapeutic response to a usual dose of
levodopa. The average apomorphine dose was 5.4 mg (3 patients on 2 mg, 7 on 4 mg, 5 on 6 mg, 3
on 8 mg, and 2 on 10 mg). In contrast, of the 9 patients assigned to placebo, none reached such a
therapeutic response. The mean changes-from-baseline for UPDRS Part III scores at the best dose
were 23.9 and 0.1 for the apomorphine and placebo respectively (p < 0.0001).

The second trial used a crossover design, randomizing 17 patients who had been using
apomorphine for at least 3 months. Patients received their usual morning doses of Parkinson’s
disease medications and were followed until hypomobility occurred, at which time they received
either a single dose of subcutaneous apomorphine (at their usual dose) or placebo. Their UPDRS
Part III scores were then evaluated over time. The average dose of apomorphine was 4 mg (2
patients on 2 mg, 9 on 3 mg, 2 on 4 mg, and 1 each on 4.5 mg, 5 mg, 8 mg, and 10 mg). On
average, the mean changes-from-baseline UPDRS Part III scores at 20 minutes were 20.0 and 3.0
points for the apomorphine and placebo groups respectively (p < 0.0001).
The third trial used a parallel design, randomizing 62 patients who had been using apomorphine
for at least 3 months. Patients were randomized in a 2:1 (active:placebo) ratio to one of four
groups and were dosed once. The groups were: apomorphine at the usual dose, placebo at a
volume matching the usual apomorphine dose, apomorphine at the usual dose + 2 mg (0.2 mL), or
placebo at a volume matching the usual apomorphine dose + 0.2 mL. Patients received their usual
morning doses of Parkinson’s disease medications and were followed until hypomobility occurred,
at which time they received the randomized treatment. The mean changes-from-baseline for
UPDRS Part III scores at 20 minutes post dosing were 24.2 and 7.4 points for the pooled
apomorphine groups and the pooled placebo groups, respectively (p < 0.0001). The figure below
describes the mean change in UPDRS Motor Scores over time after pooled apomorphine and
pooled placebo administration.

In this third trial, comparing patients randomized to apomorphine at the usual dose (mean dose
about 4.5 mg) and patients randomized to apomorphine + 2 mg (mean dose about 6 mg), the mean
changes-from-baseline for UPDRS Part III scores at 20 minutes post dosing were 24 and 25,
respectively. This suggests that patients chronically treated at a dose of 4 mg might derive little
additional benefit from a dose increment of 2 mg. There was an increased incidence of adverse
events in patients randomized to apomorphine + 2 mg.
INDICATIONS AND USAGE: APOKYN (apomorphine hydrochloride injection) is indicated for
the acute, intermittent treatment of hypomobility, “off” episodes (“end-of-dose wearing off” and
unpredictable “on/off” episodes) associated with advanced Parkinson’s disease. APOKYN has
been studied as an adjunct to other medications (see CLINICAL PHARMACOLOGY: Clinical
Studies).

CONTRAINDICATIONS:
Based on reports of profound hypotension and loss of consciousness when apomorphine was
administered with ondansetron, the concomitant use of apomorphine with drugs of the 5HT3
antagonist class (including, for example, ondansetron, granisetron, dolasetron, palonosetron,
and alosetron) is contraindicated.
APOKYN is contraindicated in patients who have demonstrated hypersensitivity to the drug
or its ingredients (notably sodium metabisulfite).
WARNINGS:
Avoid Intravenous Administration: Serious adverse events (such as intravenous
crystallization of apomorphine, leading to thrombus formation and pulmonary embolism)
have followed the intravenous administration of apomorphine. Consequently, apomorphine
should not be administered intravenously.
General: The significant adverse events described below have been reported in association
with the use of subcutaneous apomorphine, but almost all of them occurred during
open-label, uncontrolled studies. In the development program, the controlled trial data
involved relatively few patients, and examined primarily the effects of single doses. Because
the background rate of many of these events in a population of patients with advanced

Parkinson’s disease is unknown, it is difficult to assess the role of apomorphine in their
causation.
Nausea and Vomiting: At the recommended doses of apomorphine, severe nausea and vomiting
can be expected. Because of this, in domestic clinical studies, 98% of all patients were treated
with the antiemetic trimethobenzamide for three days prior to beginning apomorphine and were
then encouraged to continue trimethobenzamide for at least 6 weeks. Among 522 patients treated,
262 (50%) discontinued trimethobenzamide while continuing apomorphine. The average time to
discontinuation of trimethobenzamide was about 2 months (range: 1 day to 33 months). For the
262 patients who discontinued trimethobenzamide, 249 patients continued apomorphine without
trimethobenzamide for a duration of follow-up that averaged 1 year (range: 0-3 years). Even with
the use of trimethobenzamide in clinical trials, 31% of the patients experienced nausea and 11% of
the patients experienced vomiting. In clinical trials, 3% of the patients discontinued apomorphine
due to nausea and 2% discontinued due to vomiting.
In the domestic development of apomorphine, there was no experience with antiemetics other than
trimethobenzamide. Some antiemetics with anti-dopaminergic actions have the potential to worsen
the clinical state of patients with Parkinson’s disease and should be avoided.
Syncope: In clinical studies, about 2% of patients experienced syncope.
QT Prolongation and Potential for Proarrhythymic Effects: In a study in which patients

received increasing single doses of apomorphine from 2 to 10 mg (if tolerated) as well as placebo,
the mean difference in QTc between apomorphine and placebo, as measured by Holter monitor,
was 0 msec at 4 mg, 1 msec at 6 mg, and 7 msec at 8 mg. Too few patients received a 10 mg dose
to be able to adequately characterize the change in QTc interval at that dose. In a controlled trial in
which patients were administered placebo or a single dose of apomorphine (mean dose of 5.2 mg;
range of 2-10 mg, with 30 of 35 patients receiving a dose of 6 mg or less), the mean difference
between apomorphine and placebo in the change in QTc was about 3 msec at 20 and 90 minutes.
In the entire database, 2 patients (one at 2 and 6 mg, one at 6 mg) exhibited large QTc increments
(> 60 msecs from pre-dose) and had QTc intervals greater than 500 msecs acutely after dosing.
Doses of 6 mg or less thus are associated with minimal increases in QTc. Doses greater than 6 mg
do not provide additional clinical benefit and are not recommended.
Some drugs that prolong the QT/QTc interval have been associated with the occurrence of
torsades de pointes and with sudden unexplained death. The relationship of QT prolongation to
torsades de pointes is clearest for larger increases (20 msec and greater), but it is possible that
smaller QT/QTc prolongations may also increase risk, or increase it in susceptible individuals,
such as those with hypokalemia, hypomagnesemia, bradycardia, concomitant use of other drugs
that prolong the QTc interval, or genetic predisposition (e.g., congenital prolongation of the QT
interval). Although torsades de pointes has not been observed in association with the use of
apomorphine at recommended doses in premarketing studies, experience is too limited to rule out
an increased risk. Palpitations and syncope may signal the occurrence of an episode of torsades de
pointes.

Caution is recommended when administering apomorphine to patients with the risk factors
described above.
Symptomatic Hypotension: Dopamine agonists may cause orthostatic hypotension at any time,
especially during dose escalation. Parkinson’s disease patients, in addition, may have an impaired
capacity to respond to an orthostatic challenge. For these reasons, Parkinson’s disease patients
being treated with dopaminergic agonists ordinarily require careful monitoring for signs and
symptoms of orthostatic hypotension, especially during dose escalation, and should be informed of
this risk.
Apomorphine causes dose-related decreases in systolic (SBP) and diastolic blood pressure (DBP).
Dose-dependent mean decrements in SBP ranged from 5 mmHg after 2 mg to 16 mmHg after 10
mg. Dose-dependent mean decrements in DBP ranged from 3 mmHg after 2 mg to 8 mmHg after
10 mg. These changes were observed at 10 minutes, appeared to peak at about 20 minutes after
dosing, and persisted up to at least 90 minutes post-dosing. Patients undergoing titration of
apomorphine showed an increased incidence (from 4% pre-dose to 18% post-dose) of systolic
orthostatic hypotension (≥ 20 mmHg decrease) when evaluated at various times after in-office
dosing. A small number of patients developed severe systolic orthostatic hypotension (≥ 30 mmHg
decrease and systolic BP ≤ 90 mmHg) after subcutaneous apomorphine injection.
In clinical trials of apomorphine in patients with advanced Parkinson’s disease, 59 of 550 patients
(11%) had orthostatic hypotension, hypotension, and/or syncope. These events were considered

serious in 4 patients (< 1%) and resulted in withdrawal of apomorphine in 10 patients (2%). These
events occurred both with initial dosing and during long-term treatment. Whether or not
hypotension contributed to other significant adverse events seen (e.g., falls), is unknown.
The effects of apomorphine on blood pressure may be increased by the concomitant use of alcohol,
antihypertensive medications, and vasodilators (especially nitrates). Alcohol should be avoided
when using APOKYN and extra caution should be exercised if APOKYN must be administered
with concomitant antihypertensive medications and/or vasodilators (see PRECAUTIONS: Drug
Interactions and Information for Patients).
Falls: Patients with Parkinson’s disease (PD) are at risk of falling due to the underlying postural
instability and concomitant autonomic instability seen in some patients with PD, and from
syncope caused by the blood pressure lowering effects of the drugs used to treat PD. Subcutaneous
apomorphine might increase the risk of falling by simultaneously lowering blood pressure and
altering mobility (see WARNINGS: Symptomatic Hypotension; PRECAUTIONS: Dyskinesias).
In clinical trials, 30% of patients had events that could reasonably be considered falls and about
5% of patients had falls that were considered serious. Because these data were obtained in open,
uncontrolled studies, and given the unknown background rate of falls in a population of patients
with advanced Parkinson’s disease, it is impossible to definitively assess the contribution of
apomorphine to these events.
Hallucinations / Psychotic-Like Behavior: During clinical development, hallucinations were

reported by 14% of the patients. In one randomized, double-blind, placebo-controlled study,
hallucinations or confusion occurred in 10 % of patients treated with APOKYN and 0 % of
patients treated with placebo. Hallucinations resulted in discontinuation of apomorphine in 1% of
patients.
Post marketing reports indicate that patients may experience new or worsening mental status and
behavioral changes, which may be severe, including psychotic-like behavior after starting or
increasing the dose of APOKYN. Other drugs prescribed to improve the symptoms of
Parkinson’s disease can have similar effects on thinking and behavior. This abnormal thinking
and behavior can consist of one or more of a variety of manifestations, including paranoid ideation,
delusions, hallucinations, confusion, disorientation, aggressive behavior, agitation, and delirium.
Patients with a major psychotic disorder should ordinarily not be treated with APOKYN because
of the risk of exacerbating psychosis. In addition, certain medications used to treat psychosis
may exacerbate the symptoms of Parkinson's disease and may decrease the effectiveness of
APOKYN. (see PRECAUTIONS: Drug Interactions: Dopamine Antagonists)
Falling Asleep During Activities of Daily Living: There have been reports in the literature of
patients treated with apomorphine subcutaneous injections who suddenly fell asleep without prior
warning of sleepiness while engaged in activities of daily living. It is clear that somnolence is
commonly associated with APOKYN and many clinical experts believe that falling asleep while
engaged in activities of daily living always occurs in a setting of pre-existing somnolence even if

patients do not give such a history. Prescribers should therefore continually reassess patients for
drowsiness or sleepiness, especially since some of the events occur well after the start of treatment.
Prescribers should also be aware that patients may not acknowledge drowsiness or sleepiness until
directly questioned about drowsiness or sleepiness during specific activities.
Before initiating treatment with APOKYN, patients should be advised of the possibility that they
may develop drowsiness and specifically asked about factors that could increase the risk with
APOKYN, such as concomitant sedating medications and the presence of sleep disorders. If a
patient develops significant daytime sleepiness or episodes of falling asleep during activities that
require active participation (e.g., conversations, eating, etc.), APOKYN should ordinarily be
discontinued. If a decision is made to continue APOKYN, patients should be advised not to drive
and to avoid other potentially dangerous activities. There is insufficient information to determine
whether dose reduction will eliminate episodes of falling asleep while engaged in activities of
daily living.
Coronary Events: During clinical development, 4% of patients treated with apomorphine
experienced angina, myocardial infarction, cardiac arrest and/or sudden death; some cases of
angina and myocardial infarction occurred in close proximity to apomorphine dosing (within 2
hours), while other cases of cardiac arrest and sudden death were observed at times unrelated to
dosing. Apomorphine has been shown to reduce resting systolic and diastolic blood pressure and,
as such, it has the potential to exacerbate coronary (and cerebral) ischemia. Extra caution should
be used in prescribing apomorphine for patients with known cardiovascular and cerebrovascular

disease. If patients develop signs and symptoms of coronary or cerebral ischemia, the continued
use of apomorphine should be carefully re-evaluated.
Contains Sulfite: APOKYN contains sodium metabisulfite, a sulfite that may cause allergic-type
reactions, including anaphylactic symptoms and life-threatening or less severe asthmatic episodes
in certain susceptible people. The overall prevalence of sulfite sensitivity in the general population
is unknown and probably low. Sulfite sensitivity is seen more frequently in asthmatic than in
non-asthmatic people.
Injection Site Reactions: Among the 550 patients treated with apomorphine subcutaneous
injections during development, 26% of patients complained of injection site reactions, including
bruising (16%), granuloma (4%), and pruritus (2%). There was a limited experience (both for
overall numbers of patients as well as the total number of injections per patient) with apomorphine
injections in controlled trials. In this limited controlled experience, the number of injection site
reactions reported by patients receiving apomorphine was similar to that reported by patients
receiving placebo.
Potential for Abuse: There are rare reports of apomorphine abuse by patients with Parkinson’s
disease in other countries. These cases are characterized by increasingly frequent dosing leading to
hallucinations, dyskinesia, and abnormal behavior. Psychosexual stimulation with increased libido
is believed to underlie these cases. Prescribers should be vigilant for evidence that patients are
abusing apomorphine, such as use out of proportion to motor signs (see DRUG ABUSE AND

DEPENDENCE).
PRECAUTIONS: Dyskinesias: Apomorphine may cause dyskinesia or exacerbate pre-existing
dyskinesia. During clinical development, dyskinesia or worsening of dyskinesia was reported in
24% of patients. Overall, 2% of patients withdrew from studies due to dyskinesias.
Events Reported with Dopaminergic Therapy: Although the events enumerated below have not
been reported in association with the use of apomorphine, they are associated with the use of other
dopaminergic drugs.
Withdrawal-emergent Hyperpyrexia and Confusion: Although not reported with apomorphine, a
symptom complex resembling the neuroleptic malignant syndrome (characterized by elevated
temperature, muscular rigidity, altered consciousness, and autonomic instability), with no other
obvious etiology, has been reported in association with rapid dose reduction, withdrawal of, or
changes in antiparkinsonian therapy.
Fibrotic Complications: Cases of retroperitoneal fibrosis, pulmonary infiltrates, pleural effusion,
pleural thickening, and cardiac valvulopathy have been reported in some patients treated with
ergot-derived dopaminergic agents. While these complications may resolve when the drug is
discontinued, complete resolution does not always occur. Although these adverse events are
believed to be related to the ergoline structure of these compounds, whether other, nonergot
derived dopamine agonists can cause them is unknown.

Melanoma: Epidemiological studies have shown that patients with Parkinson’s disease have a
higher risk (2- to approximately 6-fold higher) of developing melanoma than the general
population. Whether the increased risk observed was due to Parkinson’s disease or other factors,
such as drugs used to treat Parkinson’s disease, is unclear.
For the reasons stated above, patients and providers are advised to monitor for melanomas
frequently and on a regular basis when using APOKYN for any indication. Ideally, periodic skin
examinations should be performed by appropriately qualified individuals (e.g., dermatologists).
Priapism: Apomorphine may cause prolonged painful erections in some patients. During clinical
development, painful erections were reported by 3 of 361 males (< 1%), and one patient withdrew
from apomorphine therapy because of priapism. Although no patients in the clinical development
program required surgical intervention, severe priapism may require surgical intervention.
Hepatic Impairment: Caution should be exercised when administrating apomorphine to patients
with mild and moderate hepatic impairment due to the increased Cmax and AUC in these patients.
Studies of subjects with severe hepatic impairment have not been conducted (see CLINICAL
PHARMACOLOGY and DOSAGE AND ADMINISTRATION).
Renal Impairment: The starting dose should be reduced to 1 mg when administrating
apomorphine to patients with mild or moderate renal impairment because the Cmax and AUC are

increased in these patients. Studies in subjects with severe renal impairment have not been
conducted (see CLINICAL PHARMACOLOGY and DOSAGE AND ADMINISTRATION).
Retinal Pathology in Albino Rats: Retinal degeneration has been observed in albino rats treated
with dopamine agonists for prolonged periods (generally during 2-year carcinogenicity studies).
This lesion has also been observed when albino rats were exposed to these agents for shorter
periods under higher intensity light exposures. Similar changes have not been observed in 2-year
carcinogenicity studies in albino mice or in rats or monkeys treated for 1 year. APOKYN has not
been tested in carcinogenicity studies, but based on its mechanism of action it would be expected
to cause similar toxicity. The significance of this effect in humans has not been established, but
cannot be disregarded because disruption of a mechanism that is universally present in vertebrates
(e.g., disk shedding) may be involved.
Information for Patients: APOKYN is intended only for subcutaneous injection and must not be
given intravenously. Patients and caregivers should be urged to read the attached Patient Package
Insert and Directions for Use for the dosing pen. Patients should be instructed to use APOKYN
only as prescribed. Patients and/or caregivers who are advised to administer APOKYN in
medically unsupervised situations should receive instruction on the proper use of the product from
the physician or other suitably qualified health care professional and then observed during the
initial dosing.
In particular, patients and caregivers must receive detailed instruction in the use of the
dosing pen, with particular attention paid to two issues: 1) Patients need to be aware that the

drug is dosed in milliliters, not milligrams. Patients should be particularly cautioned that a
dose of 1 mg is represented on the dosing pen as 0.1 mL, and not as 1.0 (the latter
representing a dose of 10 mg). It is critical that patients and caregivers be made to
understand this distinction to prevent potentially life-threatening overdose if a dose of 1 mg
is prescribed. 2) Patients and caregivers must be informed that it is possible to dial in their
usual dose of apomorphine even though the cartridge may contain less than that amount of
drug. In this case, they will receive only a partial dose with the injection, and the amount left
to inject will appear in the dosing window. To complete the correct dose, patients/caregivers
will need to “re-arm” the device and dial in the correct amount of the remaining dose. If at
all possible, this situation should be avoided, and patients and caregivers should be alerted to
the fact that there may be insufficient drug left in the cartridge to deliver a complete dose
(for example, patients and caregivers should be urged to keep records of how many doses
they have delivered for each cartridge, so that they can replace any cartridge that has an
inadequate amount of drug remaining).
Patients should be instructed to rotate the injection site and to observe proper aseptic technique.
Patients should be informed that hallucinations or other manifestations of psychotic-like behavior
can occur. Patients should also be advised that, if they have a major psychotic disorder, that
APOKYN should not ordinarily be used because of the risk of exacerbating the psychosis. Patients
with a major psychotic disorder should also be aware that many treatments for psychosis may
decrease the effectiveness of APOKYN. (see PRECAUTIONS: Drug Interactions: Dopamine

Antagonists).
Patients should be advised that they may develop postural (orthostatic) hypotension with or
without symptoms such as dizziness, nausea, syncope, and sometimes sweating. Hypotension
and/or orthostatic symptoms may occur more frequently during initial therapy or with an increase
in dose at any time (cases have been seen after months of treatment). Accordingly, patients should
be cautioned against rising rapidly after sitting or lying down, especially if they have been sitting
or lying for prolonged periods, and especially at the initiation of treatment with APOKYN.
Alcohol, antihypertensive medications, and vasodilating medications may potentiate the
hypotensive effect of apomorphine (see WARNINGS: Symptomatic Hypotension;
PRECAUTIONS: Drug Interactions).
Patients should be alerted to the potential sedating effects of APOKYN, including somnolence and
the possibility of falling asleep while engaged in activities of daily living. Since somnolence is a
frequent adverse event with potentially serious consequences, patients should neither drive a car
nor engage in other potentially dangerous activities until they have gained sufficient experience
with APOKYN to gauge whether or not it affects their mental and/or motor performance adversely.
Patients should be advised that if increased somnolence or episodes of falling asleep during
activities of daily living (e.g., watching television, passenger in a car, etc.) are experienced at any
time during treatment, they should not drive or participate in potentially dangerous activities until
they have contacted their physician. Because of possible additive effects, caution should be
advised when patients are taking other sedating medications or alcohol in combination with

APOKYN.
Because apomorphine has not been evaluated for effects on reproduction and embryo-fetal
development, patients should be advised to notify their physicians if they become pregnant or
intend to become pregnant (see PRECAUTIONS: Pregnancy).
Because of the possibility that apomorphine may be excreted in breast milk, patients should be
advised to notify their physicians if they intend to breast-feed.
There have been reports of patients experiencing intense urges to gamble, increased sexual urges,
other intense urges and the inability to control these urges while taking one or more of the
medications that increases central dopaminergic tone and that are generally used for the treatment
of Parkinson’s disease, including APOKYN. Although it is not proven that the medications caused
these events, these urges were reported to have stopped in some cases when the dose was reduced
or the medication was stopped. Prescribers should ask patients about the development of new or
increased gambling urges, sexual urges or other urges while being treated with APOKYN. Patients
should inform their physician if they experience new or increased gambling urges, increased
sexual urges or other intense urges while taking APOKYN. Physicians should consider dose
reduction or stopping the medication if a patient develops such urges while taking APOKYN.
Rare cases of abuse (use of apomorphine significantly in excess of prescribed frequency) have
been reported. Apomorphine abuse may be associated with inappropriate sexual behavior.

Drug Interactions:
5HT3 Antagonists: Based on reports of profound hypotension and loss of consciousness when
apomorphine was administered with ondansetron, the concomitant use of apomorphine with drugs
of the 5HT3 antagonist class (including, for example, ondansetron, granisetron, dolasetron,
palonosetron, and alosetron) is contraindicated (see CONTRAINDICATIONS).
Antihypertensive Medications and Vasodilators: The following adverse events were experienced
more commonly in patients receiving concomitant antihypertensive medications or vasodilators (n
= 94) compared to patients not receiving these concomitant drugs (n = 456): hypotension 10% vs
4%, myocardial infarction 3% vs 1%, serious pneumonia 5% vs 3%, serious falls 9% vs 3%, and
bone and joint injuries 6% vs 2%. The mechanism underlying many of these events is unknown,
but may represent increased hypotension (see WARNINGS: Symptomatic Hypotension).
Dopamine Antagonists: Since apomorphine is a dopamine agonist, it is possible that dopamine
antagonists, such as the neuroleptics (phenothiazines, butyrophenones, thioxanthenes) or
metoclopramide, may diminish the effectiveness of APOKYN. Patients with major psychotic
disorders, treated with neuroleptics, should be treated with dopamine agonists only if the potential
benefits outweigh the risks.
Drugs Prolonging the QT/QTc Interval: Caution should be exercised when prescribing

apomorphine concomitantly with drugs that prolong the QT/QTc interval (see WARNINGS: QT
Prolongation and Potential for Proarrhythmic Effects).
Drug/Laboratory Test Interactions: There are no known interactions between APOKYN and
laboratory tests.
Carcinogenesis, Mutagenesis, Impairment of Fertility: Carcinogenicity studies have not been
conducted with APOKYN.
Apomorphine was mutagenic in the in vitro bacterial Ames test and the in vitro mammalian mouse
lymphoma assay. Apomorphine was also clastogenic in the in vitro chromosomal aberration assay
in human lymphocytes and the in vitro mouse lymphoma assay. Apomorphine was negative in the
in vivo micronucleus assay in mice.
In a published fertility study in male rats, an adverse effect on fertility was observed at a dose of 2
mg/kg administered subcutaneously (0.6 times the MRHD in a mg/m2 basis). A significant
decrease in testis weight was observed in a 39-week study in cynomolgus monkey at subcutaneous
doses of 1.0 and 1.5 mg/kg (0.6 and 1 times the MRHD on a mg/m2 basis).
Pregnancy: Pregnancy Category C: Reproduction studies have not been conducted with
apomorphine. It is also not known whether apomorphine can cause fetal harm when administered
to a pregnant woman or can affect reproductive capacity. Apomorphine should be given to a
pregnant woman only if clearly needed.

Nursing Mothers: It is not known whether apomorphine is excreted in human milk. Because
many drugs are excreted in human milk and because of the potential for serious adverse reactions
in nursing infants from apomorphine, a decision should be made as to whether to discontinue
nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
Pediatric Use: The safety and efficacy of APOKYN in pediatric patients has not been established.
Geriatric Use: In the apomorphine clinical development program, there were 239 patients less
than 65 years of age and 311 who were 65 years of age or older. Adverse events were about
equally common in older and younger patients (90 vs 87%), but with older patients more likely to
experience confusion and hallucinations. Serious adverse events (life-threatening events or events
resulting in hospitalization and/or increased disability) were also more common in older patients
(27 vs 17%), with older patients more likely to fall (experiencing bone and joint injuries), have
cardiovascular events, develop respiratory disorders, and have gastrointestinal events. Older
patients were more likely to discontinue apomorphine treatment as a result of adverse events (29
vs 21%).
ADVERSE REACTIONS
Clinical Trial Experience
Adverse Events Incidence in Controlled Clinical Studies: APOKYN® has been administered to
550 Parkinson’s disease patients who were taking some form of L-Dopa along with other

Parkinson’s disease medications. Eighty-six percent of patients were taking a concomitant
dopamine agonist. All patients had some degree of spontaneously occurring hypomobility (“off
episodes”) at baseline. Adverse events were recorded by the clinical investigators using
terminology of their own choosing. To provide a meaningful estimate of the proportion of
individuals having adverse events, similar types of events were grouped into a smaller number of
standardized categories using MEDDRA dictionary terminology.
The most common adverse events seen in controlled trials were yawning, dyskinesias, nausea
and/or vomiting, somnolence, dizziness, rhinorrhea, hallucinations, edema, chest pain, increased
sweating, flushing, and pallor.
The most extensive experience with apomorphine in randomized, controlled trials comes from a
multicenter randomized placebo-controlled parallel group trial conducted in apomorphine-naïve
PD patients treated for up to 4 weeks (Table 1). Individual apomorphine doses in this trial ranged
from 2-10 mg, optimized to achieve control of symptoms comparable to each patient’s response to
his or her usual dose of L-dopa. The prescriber should be aware that these figures cannot be used
to predict the incidence of adverse events in the course of usual medical practice where patient
characteristics and other factors differ from those that prevailed in the clinical studies. Similarly,
the cited frequencies cannot be compared with figures obtained from other clinical investigations
involving different treatments, uses, and investigators. However, the cited figures do provide the
prescribing physician with some basis for estimating the relative contribution of drug and nondrug
factors to the adverse-event incidence rate in the population studied.

Other Adverse Events Observed During All Phase 2/3 Clinical Trials: APOKYN has been
administered to 550 patients; 89% had at least one adverse event (AE). The most common AEs in
addition to those in Table 1 (occurring in at least 5% of the patients and at least plausibly related to
treatment) in descending order were injection site complaint, fall, arthralgia, insomnia, headache,
depression, urinary tract infection, anxiety, congestive heart failure, limb pain, back pain,
Parkinson’s disease aggravated, pneumonia, confusion, sweating increased, dyspnea, fatigue,
ecchymosis, constipation, diarrhea, weakness, and dehydration.
Post Marketing Experience
In addition to the adverse events reported during clinical trials, the following adverse reactions
have been identified during post-approval use of APOKYN® in Parkinson’s disease patients.
Because these reactions are reported voluntarily from a population of uncertain size, it is not

always possible to reliably estimate their frequency or establish a causal relationship to drug
exposure. The following psychiatric disorders were reported: impulse control symptoms,
pathological gambling, and increased libido (including hypersexuality).
DRUG ABUSE AND DEPENDENCE: Potential for Abuse: A rarely reported motivation for
apomorphine abuse (escalation of dose beyond prescribed frequency) is the use of apomorphine to
attempt to avoid all symptoms of all “off” events when “off” events occur frequently. A second,
rarely reported, motivation for apomorphine abuse is a psychosexual reaction related to the
stimulation of penile erection and increase in libido. Adverse events that have been reported in
males with overuse include frequent penile erections, atypical sexual behavior, heightened libido,
dyskinesias, agitation, confusion, and depression. No studies have been conducted to evaluate the
potential for dependence when apomorphine is used as acute (rescue) treatment of “off” episodes
in the patients with “on/off” or “wearing-off” effects associated with late stage Parkinson’s
disease.
OVERDOSAGE: Intermittent Injection: A report of an accidental overdose of 25 mg injected
subcutaneously in a 62 year old man was published in Journal of Neurology, Neurosurgery, and
Psychiatry (1990), Vol. 53, pp. 96-102. After 3 minutes, the patient felt nauseated and lost
consciousness for 20 minutes. Afterwards, he was alert with a heart rate 40/minute and a supine
blood pressure of 90/50. He recovered completely within an hour.
DOSAGE AND ADMINISTRATION: The prescribed dose of APOKYN should always be

expressed in mL to avoid confusion and doses greater than 0.6 mL (6 mg) are not
recommended. Patients and caregivers must receive detailed instructions in the preparation
and injection of doses, with particular attention paid to the correct use of the dosing pen (see
PRECAUTIONS: Information for Patients).
APOKYN is indicated for subcutaneous administration only. APOKYN should not be initiated
without use of a concomitant antiemetic (see WARNINGS: Nausea and Vomiting). Most
antiemetic experience is with trimethobenzamide and this should generally be used.
Trimethobenzamide (300 mg tid orally) should be started 3 days prior to the initial dose of
apomorphine and continued at least during the first two months of therapy.
Based on reports of profound hypotension and loss of consciousness when apomorphine was
administered with ondansetron, the concomitant use of apomorphine with drugs of the 5HT3
antagonist class (including, for example, ondansetron, granisetron, dolasetron, palonosetron, and
alosetron) is contraindicated (see CONTRAINDICATIONS).
The dose of APOKYN must be titrated on the basis of effectiveness and tolerance, starting at
0.2 mL (2 mg) and up to a maximum recommended dose of 0.6 mL (6 mg) as follows:
Patients in an “off” state should be given a 0.2 mL (2 mg) test dose in a setting where blood
pressure can be closely monitored by medical personnel. Both supine and standing blood pressure
should be checked predose and at 20, 40, and 60 minutes post dose. Patients who develop
clinically significant orthostatic hypotension in response to this test dose of apomorphine should

not be considered candidates for treatment with APOKYN. If the patient tolerates the 0.2 mL (2
mg) dose, and responds, the starting dose should be 0.2 mL (2 mg) used on an as needed basis to
treat existing “off” episodes. If needed, the dose can be increased in 0.1 mL (1 mg) increments
every few days on an outpatient basis.
Beyond this, the general principle guiding dosing (described in detail below) is to determine a
dose (0.3 mL or 0.4 mL) that the patient will tolerate as a test dose under monitored conditions,
and then begin an outpatient dosing trial (periodically assessing both efficacy and tolerability)
using a dose 0.1 mL (1 mg) lower than the tolerated test dose.
For patients who tolerate the test dose of 0.2 mL (2 mg) but achieve no response, a dose of 0.4 mL
(4 mg) may be administered at the next observed “off” period, but no sooner than 2 hours after the
initial test dose of 0.2 mL (2 mg). Both supine and standing blood pressure should be checked
predose and at 20, 40, and 60 minutes post dose. If the patient tolerates a test dose of 0.4 mL (4
mg) the starting dose should be 0.3 mL (3 mg) used on an as needed basis to treat existing “off”
episodes. If needed, the dose can be increased in 0.1 mL (1 mg) increments every few days on an
outpatient basis. If a patient does not tolerate a test dose of 0.4 mL (4 mg), a test dose of 0.3 mL (3
mg) may be administered during a separate “off” period, no sooner than 2 hours after the test dose
of 0.4 mL (4 mg). Both supine and standing blood pressure should be checked predose and at 20,
40, and 60 minutes post dose. If the patient tolerates the 0.3 mL (3 mg) test dose, the starting dose
should be 0.2 mL (2 mg) used on an as needed basis to treat existing “off” episodes. If needed, and
the 0.2 mL (2 mg) dose is tolerated, the dose can be increased to 0.3 mL (3 mg) after a few days.

In such a patient, the dose should ordinarily not be increased to 0.4 mL (4 mg) on an out-patient
basis.
Most patients studied in the apomorphine development program responded to 0.3 mL to 0.6 mL (3
mg to 6 mg). There is no evidence from controlled trials that doses greater than 0.6 mL (6 mg)
give an increased effect and these doses are not recommended. The average frequency of dosing
was 3 times per day in the development program, and there is limited experience with single doses
greater than 0.6 mL (6 mg), dosing more than 5 times per day and with total daily doses greater
than 2.0 mL (20 mg).
If a single dose of apomorphine is ineffective for a particular “off” period, a second dose should
not be given for that “off” episode. The efficacy of a second dose for a single “off” episode has not
been systematically studied and the safety of redosing has not been characterized.
Patients who have a significant interruption in therapy (more than a week) should be restarted on a
0.2 mL (2 mg) dose and gradually titrated to effect.
When dosing patients with mild and moderate hepatic impairment, caution should be exercised
due to the increased Cmax and AUC in these patients (see CLINICAL PHARMACOLOGY and
PRECAUTIONS).

For patients with mild and moderate renal impairment, the testing dose and subsequently the
starting dose should be reduced to 0.1 mL (1 mg) (see CLINICAL PHARMACOLOGY and
PRECAUTIONS).
Patients should be instructed to administer apomorphine as described in the Patient Instruction
Leaflets.
HOW SUPPLIED: APOKYN® (10 mg/mL) containing apomorphine hydrochloride (as
apomorphine hydrochloride hemihydrate), USP is supplied as a clear, colorless, sterile, solution in
3 mL cartridges. The 3 mL glass cartridges are used with a manual reusable, multiple dose injector
pen. The pen can deliver doses up to 1.0 mL in 0.02 mL increments. The pen is provided in a
package with six needles and a carrying case.

APOKYN® (AY-po-kin)
(apomorphine hydrochloride injection)
Read the Patient Information that comes with APOKYN before you start taking it and each time
you get a refill. There may be new information. Share this information with your caregiver. This
leaflet does not take the place of talking with your healthcare provider about your medical
condition or treatment. If you or your caregiver do not understand the information, or have any
questions about APOKYN, talk with your healthcare provider or pharmacist.
What is APOKYN?
APOKYN is used by injection, as needed, only to treat loss of control of body movements in
people with advanced Parkinson’s disease (PD). This condition is also called hypomobility or
“off” episodes. An “off” episode may include symptoms such as muscle stiffness, slow
movements, and difficulty starting movements. APOKYN may improve your ability to control
your movements when it is used during an “off” episode. This may help you walk, talk, or move
around easier. APOKYN is not used to prevent “off” episodes. APOKYN does not take the place
of your other medicines for PD.

Who should not take APOKYN?
Do not take APOKYN if you are:
• allergic to APOKYN or to any of its ingredients. The active ingredient is apomorphine
hydrochloride. APOKYN also contains a sulfite called metabisulfite. Sulfites can cause
severe, life-threatening allergic reactions in some people, especially in people with asthma.
Tell your healthcare provider if you have had an allergic reaction to any “sulfite” containing
medicines. See the end of this leaflet for a complete list of ingredients in APOKYN.
• being treated with certain drugs to treat nausea and vomiting or irritable bowel
syndrome. These medications (including, for example, ondansetron, granisetron,
dolasetron, palonosetron, and alosetron) are called 5HT3 antagonists or blockers. People
taking this type of drug together with apomorphine have had severely low blood
pressure and lost consciousness or “blacked out.”
APOKYN has not been studied in children.

• about all your medical conditions including if you:

• are pregnant or planning to become pregnant. It is not known if APOKYN can
harm your unborn baby.
• are breastfeeding. It is not known if APOKYN passes into your milk and if it can
harm your baby. You should choose to either use APOKYN or breastfeed, but not
both.
• about all the medicines you take, including prescription and non-prescription medicines,
vitamins, and herbal supplements. APOKYN and certain other medicines interact with each
other, causing serious side effects. This happens especially when you take APOKYN with
certain medicines called “vasodilators” and some other medications that lower blood pressure,
or take medicines that make you sleepy. Keep a list of all the medicines you take. Your
healthcare provider or pharmacist will tell you if you can take APOKYN with your other
medicines.
How should I take APOKYN?
Read the APOKYN “Instructions for Use” for complete instructions on preparing and giving an
injection of APOKYN. Do not inject APOKYN unless you and your caregiver have been
taught the right way and both of you understand all the directions. Ask your healthcare
provider if you do not understand something.

Use APOKYN exactly as prescribed by your healthcare provider. APOKYN should be
injected just under the skin (i.e., subcutaneously), and not into a vein.
Your healthcare provider or other qualified health professional must show you and your caregiver
how to inject APOKYN before you start using it. Your healthcare provider will prescribe
APOKYN that comes in prefilled glass cartridges that are used with a special multiple-dose
injector pen.
• Your healthcare provider will tell you what dose of APOKYN to use and how often you
should take it. Your healthcare provider will also tell you how to change your dose of
APOKYN, if needed. Do not change your dose of APOKYN or use it more often unless your
healthcare provider has told you to.
• Choose an injection site on your stomach area, upper arm, or upper leg. Change your injection
site each time APOKYN is used. This will lower your chances of having a skin reaction at the
site where you inject APOKYN. Do not inject APOKYN into an area of skin that is sore, red,
infected or damaged.
• Never reuse needles with your APOKYN Injections. Use a new needle with each injection.
• Only use APOKYN that is clear and colorless. Do not use APOKYN that is cloudy, green, or

contains particles. Call your pharmacy for a replacement.
• Your healthcare provider will usually prescribe another medicine called an “antiemetic”, to
take when you are using APOKYN. Antiemetic medicines help to lessen the symptoms of
nausea and vomiting that can happen with APOKYN.
• If you take too much APOKYN, you may experience more side effects than usual and they
may be stronger than usual. If you are experiencing more side effects or stronger side effects
than you commonly have, you should contact your healthcare provider immediately. If you
are unable to contact your healthcare provider, you should have someone take you to the
Emergency Room. It is a good idea to discuss this potential problem with your healthcare
provider at the time that you start APOKYN.
What should I avoid while taking APOKYN?
• Do not take APOKYN with any of these drugs: ondansetron, dolasetron, granisetron,
palonosetron, and alosetron or any drug of the 5HT3 antagonist class or group.
• Do not drink alcohol while you are taking APOKYN. Alcohol used with APOKYN can
cause worse side effects.
• Do not take medicines that make you sleepy while you are taking APOKYN.

• Do not drive a car, operate machinery, or do anything that might put you at risk of
getting hurt until you know how APOKYN affects you. APOKYN may cause dizziness or
fainting. Tell your healthcare provider if you get dizzy or faint with APOKYN.
• Do not change your body position too fast. Get up slowly from sitting or lying. APOKYN
can lower your blood pressure and cause dizziness or fainting.
What are the possible side effects of APOKYN?
• heart problems. If you develop shortness of breath, fast heartbeat, or chest pain while taking
APOKYN, call your healthcare provider right away or get emergency help.
• severe nausea and vomiting. Severe nausea and vomiting can happen with APOKYN. Tell
your healthcare provider if this is a problem for you. Your healthcare provider may prescribe a
medicine called Tigan to help prevent nausea and vomiting. Some patients can stop taking
Tigan after using APOKYN for some time. Some patients may need to continue taking Tigan
to help prevent nausea and vomiting. Talk to your healthcare provider before you stop taking
Tigan.
• sleepiness or falling asleep during the day. Some patients treated with APOKYN may get
sleepy during the day or fall asleep without warning while doing everyday activities such as

talking, eating, or driving a car. Tell your healthcare provider if you (or your caregiver) see
either of these effects.
• falls. The changes that occur with PD, and the effects of some PD medicines, can increase the
risk of falling. APOKYN can also increase this risk.
• sudden uncontrolled movements (dyskinesias). Some people with PD may get sudden,
uncontrolled movements after treatment with some PD medicines used to treat PD. APOKYN
can cause or worsen this effect.
• dizziness. APOKYN can lower your blood pressure and cause dizziness. This effect usually
happens when APOKYN treatment is started or when the APOKYN dose is increased. With
dizziness, there may also be other symptoms such as nausea, fainting, and sometimes
sweating. Do not get up too fast from sitting or after lying down, especially if you have been
sitting or lying down for a long period of time. Tell your healthcare provider if dizziness is a
problem for you.
• Hallucinations / psychotic-like behavior. APOKYN can cause or worsen psychotic-like
behavior including hallucinations (seeing or hearing things that are not real), confusion,
excessive suspicion, aggressive behavior, agitation, delusional beliefs (believing things that
are not real), and disorganized thinking. Call your healthcare provider right away if you
experience any of these symptoms.

• depression. Some people get depression while taking APOKYN. Call your healthcare
provider right away if you get depression with APOKYN.
• headache. APOKYN can cause headaches. If these become severe or do not go away, call
your healthcare provider.
• injection site reactions. Soreness, redness, bruising and itching may happen at the injection
site. Changing the injection site with each injection, and putting ice on the injection site
before and after injections, may help lower these effects.
• intense urges. Some people with PD have reported new or increased gambling urges,
increased sexual urges, and other intense urges, while taking PD medicines, including
APOKYN. If you experience new or increased urges, tell your healthcare provider.
APOKYN can also cause yawning, a runny nose, and swelling of your hands, arms, legs, and feet.
Talk to your healthcare provider about any side effects that bother you or that don’t go away.
These are not all the side effects with APOKYN. For more information, ask your healthcare
provider or pharmacist.

Other Information about APOKYN.
Studies of people with Parkinson’s disease show that they may be at an increased risk of
developing melanoma, a form of skin cancer, when compared to people without Parkinson’s
disease. It is not known if this problem is associated with Parkinson’s disease or the medicines
used to treat Parkinson’s disease. APOKYN is one of the medicines used to treat Parkinson’s
disease, therefore, patients being treated with APOKYN should have periodic skin examinations.
How should I store APOKYN?
• Store APOKYN cartridges at room temperature, 77°F (25°C).
• When traveling, keep the cartridges, at 59 to 86°F (15 to 30°C).
• Keep APOKYN and all medicines out of the reach of children.
General information about APOKYN.
Medicines are sometimes prescribed for conditions that are not mentioned in patient information
leaflets. Do not use APOKYN for a condition for which it was not prescribed. Do not give

4. Clinical Particulars
4.1 Therapeutic indications
The treatment of disabling motor fluctuations (‘on-off’ phenomena) in patientswith Parkinson’s disease which persist despite individually titrated treatmentwith levodopa (with a peripheral decarboxylase inhibitor) and/or otherdopamine agonists.
4.2 Posology and method of administration
APO-go Pen 10 mg/ml Solution for Injection is for subcutaneous use byintermittent bolus injection.

Selection of patients suitable for APO-go injections:Patients selected for treatment with APO-go should be able to recognise theonset of their ‘off’ symptoms and be capable of injecting themselves or elsehave a responsible carer able to inject for them when required.
It is essential that the patient is established on domperidone, usually 20mgthree times daily for at least two days prior to initiation of therapy.
Apomorphine should be initiated in the controlled environment of a specialistclinic. The treatment should be supervised by a physician experienced in thetreatment of Parkinson’s disease (e.g. Neurologist). The patient’s treatmentwith levodopa, with or without dopamine agonists, should be optimised beforestarting APO-go treatment.Determination of the threshold dose.

The appropriate dose for each patient is established by incremental dosingschedules. The following schedule is suggested:-
1mg of apomorphine HCl (0.1ml), that is approximately 15-20 micrograms/kg,may be injected subcutaneously during a hypokinetic, or ‘off’ period and thepatient is observed over 30 minutes for a motor response.
If no response, or an inadequate response, is obtained a second dose of 2 mg ofapomorphine HCl (0.2ml) is injected subcutaneously and the patient observedfor an adequate response for a further 30 minutes.
The dosage may be increased by incremental injections with at least a fortyminute interval between succeeding injections, until a satisfactory motorresponse is obtained.
Establishment of treatment.
Once the appropriate dose is determined, a single subcutaneous injection maybe given into the lower abdomen or outer thigh at the first signs of an ‘off’episode. It cannot be excluded that absorption may differ with differentinjection sites within a single individual. Accordingly, the patient should thenbe observed for the next hour to assess the quality of their response totreatment. Alterations in dosage may be made according to the patient’sresponse.
The optimal dosage of apomorphine hydrochloride varies between individualsbut, once established, remains relatively constant for each patient.
Precautions on continuing treatment.
The daily dose of APO-go varies widely between patients, typically within therange of 3-30mg, given as 1-10 injections and sometimes as many as 12separate injections per day.
It is recommended that the total daily dose of apomorphine HCl should notexceed 100mg and that individual bolus injections should not exceed 10mg.
In clinical studies it has usually been possible to make some reduction in thedose of levodopa; this effect varies considerably between patients and needs tobe carefully managed by an experienced physician.
Once treatment has been established domperidone therapy may be graduallyreduced in some patients but successfully eliminated only in a few, withoutany vomiting or hypotension.

Children and adolescents:
APO-go Pen 10mg/ml Solution for Injection is contra-indicated for childrenand adolescents under 18 years of age (see section 4.3).
Elderly:
The elderly are well represented in the population of patients with Parkinson’sdisease and constitute a high proportion of those studied in clinical trials ofAPO-go. The management of elderly patients treated with APO-go has notdiffered from that of younger patients.
Renal impairment:
A dose schedule similar to that recommended for adults, and the elderly, canbe followed for patients with renal impairment (see section 4.4).
4.3 Contraindications
In patients with respiratory depression, dementia, psychotic diseases or hepaticinsufficiency.
Intermittent apomorphine HCl treatment is not suitable for patients who havean ‘on’ response to levodopa, which is marred by severe dyskinesia ordystonia.
APO-go should not be administered to patients who have a knownhypersensitivity to apomorphine or any excipients of the medicinal product.
APO-go is contra-indicated for children and adolescents under 18 years of age.
4.4 Special warnings and precautions for use
Apomorphine HCl should be given with caution to patients with renal,pulmonary or cardiovascular disease and persons prone to nausea andvomiting.
Extra caution is recommended during initiation of therapy in elderly and/ordebilitated patients.
Since apomorphine may produce hypotension, even when given withdomperidone pretreatment, care should be exercised in patients with pre-existing cardiac disease or in patients taking vasoactive medicinal productssuch as antihypertensives, especially in patients with pre-existing posturalhypotension.
Since apomorphine, especially at high dose, may have the potential for QTprolongation, caution should be exercised when treating patients at risk fortorsades de pointes arrhythmia.

Apomorphine is associated with local subcutaneous effects. These cansometimes be reduced by the rotation of injection sites or possibly by the useof ultrasound (if available) to areas of nodularity and induration.
APO-go Pen 10mg/ml Solution for Injection contains sodium bisulphite whichmay rarely cause severe allergic reactions and bronchospasm.
Haemolytic anaemia and thrombocytopenia have been reported in patientstreated with apomorphine. Haematology tests should be undertaken at regularintervals, as with levodopa, when given concomitantly with apomorphine.
Caution is advised when combining apomorphine with other medicinalproducts, especially those with a narrow therapeutic range (see section 4.5).
Neuropsychiatric problems co-exist in many patients with advancedParkinson’s disease. There is evidence that for some patients neuropsychiatricdisturbances may be exacerbated by apomorphine.
Special care should be exercised when apomorphine is used in these patients.
Apomorphine has been associated with somnolence, and other dopamineagonists can be associated with sudden sleep onset episodes, particularly inpatients with Parkinson’s disease. Patients must be informed of this andadvised to exercise caution while driving or operating machines duringtreatment with apomorphine. Patients who have experienced somnolence mustrefrain from driving or operating machines. Furthermore, a reduction ofdosage or termination of therapy may be considered.
Pathological gambling, increased libido and hypersexuality have been reportedin patients treated with dopamine agonists for Parkinson’s disease, includingapomorphine.
4.5 Interactions with other medicinal products and other forms of interaction
Patients selected for treatment with apomorphine HCl are almost certain to betaking concomitant medications for their Parkinson’s disease. In the initialstages of apomorphine HCl therapy the patient should be monitored forunusual side-effects or signs of potentiation of effect.
Neuroleptic medicinal products may have an antagonistic effect if used withapomorphine. There is a potential interaction between clozapine andapomorphine, however clozapine may also be used to reduce the symptoms ofneuropsychiatric complications.
The possible effects of apomorphine on the plasma concentrations of otherdrugs have not been studied. Therefore caution is advised when combiningapomorphine with other medicinal products, especially those with a narrowtherapeutic range.

Even when co-administered with domperidone, apomorphine may potentiatethe antihypertensive effects of these drugs. (See section 4.4).
It is recommended to avoid the administration of apomorphine with otherdrugs known to prolong the QT interval.
4.6 Pregnancy and lactation
There is no experience of apomorphine usage in pregnant women.
Animal reproduction studies do not indicate any teratogenic effects, but dosesgiven to rats which are toxic to the mother can lead to failure to breathe in thenewborn. The potential risk for humans is unknown. See Section 5.3.
APO-go should not be used during pregnancy unless clearly necessary.
It is not know whether apomorphine is excreted in breast milk. A decision onwhether to continue/discontinue breastfeeding or to continue/discontinuetherapy with APO-go should be made taking into account the benefit of breast-feeding to the child and the benefit of APO-go to the woman.
4.7 Effects on ability to drive and use machines
Patients being treated with apomorphine and presenting with somnolence mustbe informed to refrain from driving or engaging in activities (e.g. operatingmachines) where impaired alertness may put themselves or others at risk ofserious injury or death unless patients have overcome such experiences ofsomnolence (see also Section 4.4).

Common:Neuropsychiatric disturbances are common in parkinsonian patients. APO-goshould be used with special caution in these patients. Neuropsychiatricdisturbances (including transient mild confusion and visual hallucinations)have occurred during apomorphine HCl therapy.
Not known:Patients treated with dopamine agonists for treatment of Parkinson's disease,including apomorphine, especially at high doses, have been reported asexhibiting signs of pathological gambling, increased libido and hypersexuality;generally reversible upon reduction of the dose or treatment discontinuation.
Nervous system disordersCommon:Transient sedation with each dose of apomorphine HCl at the start of therapymay occur; this usually resolves over the first few weeks.
Apomorphine is associated with somnolence.
Dizziness / light-headedness have also been reported.
Uncommon:Apomorphine may induce dyskinesias during ‘on’ periods, which can besevere in some cases, and in a few patients may result in cessation of therapy.
Vascular disordersUncommon:Postural hypotension is seen infrequently and is usually transient (See Section4.4).

These may include subcutaneous nodules, induration, erythema,tenderness and panniculitis. Various other local reactions (such as irritation,itching, bruising and pain) may also occur.

4.9 Overdose
There is little clinical experience of overdose with apomorphine by this routeof administration. Symptoms of overdose may be treated empirically assuggested below:-
Excessive emesis may be treated with domperidone.
Respiratory depression may be treated with naloxone.
Hypotension: appropriate measures should be taken, e.g. raising the foot of thebed.
Bradycardia may be treated with atropine.
5. Pharmacological Properties
5.1 Pharmacodynamic properties

Apomorphine is a direct stimulant of dopamine receptors and, whilepossessing both D1 and D2 receptor agonist properties, does not sharetransport or metabolic pathways with levodopa.
Although in intact experimental animals, administration of apomorphinesuppresses the rate of firing of nigro-striatal cells and in low dose has beenfound to produce a reduction in locomotor activity (thought to represent pre-synaptic inhibition of endogenous dopamine release) its actions onparkinsonian motor disability are likely to be mediated at post-synapticreceptor sites. This biphasic effect is also seen in humans.
5.2 Pharmacokinetic properties
After subcutaneous injection of apomorphine its fate can be described by atwo-compartment model, with a distribution half-life of 5 (± 1.1) minutes andan elimination half-life of 33 (± 3.9) minutes. Clinical response correlates wellwith levels of apomorphine in the cerebrospinal fluid; the drug distributionbeing best described by a two- compartment model. Apomorphine is rapidlyand completely absorbed from subcutaneous tissue, correlating with the rapid

onset of clinical effects (4-12 minutes), and that the brief duration of clinicalaction of the drug (about 1 hour) is explained by its rapid clearance. Themetabolism of apomorphine is by glucuronidation and sulphonation to at leastten per cent of the total; other pathways have not been described.
5.3 Preclinical safety data
Repeat dose subcutaneous toxicity studies reveal no special hazard forhumans, beyond the information included in other sections of the SmPC.
In vitro genotoxicity studies demonstrated mutagenic and clastogenic effects,most likely due to products formed by oxidation of apomorphine. However,apomorphine was not genotoxic in the in vivo studies performed.
The effect of apomorphine on reproduction has been investigated in rats.Apomorphine was not teratogenic in this species, but it was noted that doseswhich are toxic to the mother can cause loss of maternal care and failure tobreathe in the newborn.
No carcinogenicity studies have been performed.

6.5 Nature and contents of container
Cartridge.
APO-go Pen 10 mg/ml is a disposable multiple dose pen injector systemincorporating a clear glass (type I) cartridge containing a clear solution forinjection. The glass cartridge is sealed at one end with a bromobutyl rubberpiston, and at the other end with a bromobutyl rubber/aluminium membrane.
APO-go Pen is available in packs containing 1, 5, or 10 x 3ml pens in amoulded plastic tray in an outer cardboard carton. Each pen contains 3ml ofsolution for injection. Bundle Packs containing 25 Pens are available in someterritories. These bundle packs consist of 5 boxes each containing 5 pens.
Not all pack sizes are marketed.
6.6 Instruction for use, handling and disposal.

ATTACHING THE NEEDLE
(a) Before using your pen system you will need a surgical wipe and one needlein its protective cone (see 5). Take the pen out of its box and remove the outersleeve (see 3).

(b) Wipe the membrane (see 4) with the surgical wipe.
(c) Peel off the paper from the needle cone (see 2), and screw the coneclockwise onto the membrane. This will attach needle securely.
PLEASE NOTEIt is important to bring the needle to the Pen in a straight line, as shown. Ifthe needle is presented at an angle it may cause the Pen to leak. This willattach the needle securely.
(d) Remove the protective cone, but do not throw it away. Do not remove theneedle protector at this stage (see 6).
(e) Replace the pen’s outer sleeve.
HOW TO SELECT YOUR CORRECT DOSAGE
(see attached diagram)
(f) Press the red dosage dial (see 1) and turn the dial clockwise until the arrowpoints to your prescribed dosage (see 7,8). Then release downward pressureon the red dial. The dose is now set, and you do not need to redial forsubsequent injections.

Important; If you pass your prescribed dose while turning the dial, justcontinue pressing and turning in the same direction until you arrive at it again.Never pull and turn the red dosage dial at the same time.
If your prescribed dose is 2mg or less, it is necessary to “prime” the pen beforeinjecting the first dose. Do this by emptying the first 2mg dose onto a papertissue and discard. Then, set the dose you require for injection and inject in theusual way (see below under “INJECTING”). If the first dose required is morethan 2mg, then it is not necessary to “prime” the pen.
INJECTING
(see attached diagram)
(g) Pull out the red dosage dial as far as it will go. Check the white scale onthe plunger and inject only if the highest number visible corresponds to theintended dose.
(h) Using a surgical wipe, clean the area of skin around the proposed site ofinjection.
Remove the pen’s outer sleeve.
(i) Remove the needle protector (see 6).

(j) Insert the needle into the skin as directed by your doctor.
To inject, press the red dosage dial down as far as it will go, using your thumbif possible. Once the red dosage dial is fully depressed, count to three beforewithdrawing the needle.
(k) Remove and discard the needle, using the protective cone (see 5). This isdone by replacing the protective cone onto the used needle, and pushing itgently into place. Once secure, you can unscrew the needle anti-clockwise.Discard the needle in a safe place.
Important: Each needle can only be used once.
PREPARING FOR THE NEXT INJECTION:
Check that there is enough apomorphine left in the cartridge for the nextinjection (see 9). If there is, put a new needle in place, following the sameprocedure as before. (Remember not to throw away the protective cone).
If there is not enough apomorphine left for another injection, prepare anotherpen.
Finally, replace the outer sleeve of the pen.
