17
B. Course and Statistics
• Prevalence is 13% in U.S. – differs by culture–Similar rates among whites and
African-Americans–Higher rates among Latino males–Higher rates among Native
Americans–Lower rates among many Asian
populations
18
B. Course and statistics (cont.)
• Sex difference in rates –As much as 5:1 males:females
(depends on age group)• Alcohol use is most common
between ages 18 and 24• Disorder onset peaks in the 20’s and
mid-30’s–Women tend to onset later than
men
19
C. Related Problems• High comorbidity
–Among most frequent of comorbid conditions
–Comorbidity cuts across classes of disorders (depression, anxiety, ASPD, psychotic, bulimia)
–Contribution to crime
20
C. Related Problems (cont.)
• Effects on a fetus– Fetal alcohol syndrome (FAS)
• Constellation of physical, cognitive and behavioral problems in children exposed to alcohol in the womb
• Not clear how much alcohol is needed to produce the syndrome; most cases are in women with chronic alcohol use disorder
21
C. Related Problems (cont.)• Effects later in life
–Wernicke-Korsakoff Syndrome• two-stage disease resulting from severe and chronic alcohol use or severe nutritional deficits in thiamine (vitamin B1)
–First stage = sudden onset of global confusion, affected muscle movements, and problems with eyes (double vision or paralysis)
–Second stage = sudden development of profound amnesia for recent memory with intact remote and immediate memory cannot learn new things
• Mean onset is in the 40’s
22
D. Causal Influences1. Biological factors
• Alcoholism runs in families• Twins data: MZ > HZ (genetic
influence)• MZ-identical twins that share 100% of the same
DNA.• HZ twins share only 50% of their DNA (fraternal
twins).
Strains of inbred mice differ in level of preference for alcohol
• Strains of inbred mice differ in level of sensitivity to alcohol
• Children of alcoholics show low sensitivity to alcohol, which predicts later alcoholism
23
D. Causal influences (cont.)
2. Neurobiological factors• GABA system – alcohol may serve its
anxiolytic (anti-anxiety) function• Serotonin system – alcohol appears to
dampen effects• Dopamine system – alcohol has its
rewarding/pleasurable effects• One version of the D2 receptor gene
more common among alcoholics than controls
24
D. Causal influences (cont.)
3. Psychological and environmental factors
• Need access to alcohol (env.)• What psychological factors are
related to alcohol use?• Positive reinforcement (social
acceptance, social competence)• Self-medication• Expectancies (beliefs about
alcohol’s effect)• Personality (impulsivity, negative
emotionality)

93
Alcohol dependence
1. Aversion treatments• Disulfiram (Antabuse) – causes build up of
acetaldehyde (byproduct in metabolism of alcohol) high levels cause sickness (nausea, vomiting, palpitation, hypotension, flushing)
• Problem: only works when taken; doesn’t cure dependence, just prevents use
• Start 250 mg/day orally 12-24 hr after abstinence (งด้แอลุ่กอฮอลุ่ก*อนิ 12 ชมิ หลุ่ง 14 วินิ) or (blood /breath alcohol level is zero).
• Increase to 500 mg/day if no disulfiram-ethanol reaction
94
Alcohol dependence
1. Disulfiram• Monitoring : liver function• ADRs: drowsiness, metalic or
garlic taste, sexual dysfunction, psychosis
• Education of dangers of consuming even small amount of alcohol: food, OTC med, mouthwashes, topical lotions
95
G. Alcohol dependence
2. Inpatient hospital• Usually 30-day stay
3. Alcoholics Anonymous (AA)• Common “12-step” program how to
stay sober (not a cure)• Based on medical model alcoholism
is a powerful disease that requires a “higher power” to control
• Little well-controlled research on its effectiveness
96
G. Alcohol dependence
4. Motivational interviewing (MI)• Effective method of engaging people
in treatment• Contrast to traditional “blame”
approach5. Relapse prevention
• Cognitive behavioral type treatment confront beliefs; change behavior (avoid cues for use)
97

Alcohol use disorders
A. BackgroundsB. Course and statisticsC. Related problemsD. Causal influencesE. Acute alcohol
intoxicationF. Alcohol withdrawal
syndromeG. Alcohol dependence
112
A. Background Early research – Jellinek’s alcoholic stages
Stage 1 = “prealcoholic” – social drinker who begins drinking to reduce stress
Stage 2 = “prodromal” – becomes obsessed with alcohol; drinks in secret; blackouts
Stage 3 = “crucial” – can’t stop once they start; denial; nutritional problems; social problems; life falls apart
Stage 4 = “chronic” – like the “wino” on the street; sobriety followed by binges, benders
113
A. Background (cont.)
Jellinek’s two alcoholic types1. Gamma alcoholic = person who
loses control of use– Typical in U.S.
2. Delta alcoholic = person cannot abstain, but can control use– Typical in Europe
114
B. Course and Statistics
Prevalence is 13% in U.S. – differs by culture Similar rates among whites and
African-Americans Higher rates among Latino males Higher rates among Native
Americans Lower rates among many Asian
populations
115
B. Course and statistics (cont.) Sex difference in rates
As much as 5:1 males:females (depends on age group)
Alcohol use is most common between ages 18 and 24
Disorder onset peaks in the 20’s and mid-30’s Women tend to onset later than
men
116
B. Course and statistics (cont.)
Course tends to be chronic with waxing and waning Stress exacerbates; tends to wane later
in life
Effects of alcohol differ throughout course Tolerance usually develops (alcohol has
less effect) If tolerance is low, high blood alcohol
levels can be lethal
117
C. Related Problems
High comorbidity Among most frequent of comorbid
conditions Comorbidity cuts across classes of
disorders (depression, anxiety, ASPD, psychotic, bulimia)
Contribution to crime
118
C. Related Problems (cont.)
Effects on a fetus Fetal alcohol syndrome (FAS)
▪ Constellation of physical, cognitive and behavioral problems in children exposed to alcohol in the womb
▪ Not clear how much alcohol is needed to produce the syndrome; most cases are in women with chronic alcohol use disorder
119
C. Related Problems (cont.)
Effects later in life Wernicke-Korsakoff Syndrome
▪ two-stage disease resulting from severe and chronic alcohol use or severe nutritional deficits in thiamine (vitamin B1)▪ First stage = sudden onset of global confusion, affected muscle movements, and problems with eyes (double vision or paralysis)
▪ Second stage = sudden development of profound amnesia for recent memory with intact remote and immediate memory cannot learn new things
▪ Mean onset is in the 40’s
120
D. Causal Influences
1. Biological factors• Alcoholism runs in families• Twins data: MZ > DZ (genetic
influence)• MZ-identical twins that share 100% of the same
DNA.• DZ twins share only 50% of their DNA (fraternal
twins).
• Strains of inbred mice differ in level of preference for alcohol
• Strains of inbred mice differ in level of sensitivity to alcohol
• Children of alcoholics show low sensitivity to alcohol, which predicts later alcoholism
121
D. Causal influences (cont.)
2. Neurobiological factors• GABA system – alcohol may serve its
anxiolytic (anti-anxiety) function• Serotonin system – alcohol appears to
dampen effects• Dopamine system – alcohol has its
rewarding/pleasurable effects▪ One version of the D2 receptor gene
more common among alcoholics than controls
122
D. Causal influences (cont.)
3. Psychological and environmental factors
• Need access to alcohol (env.)• What psychological factors are
related to alcohol use?▪ Positive reinforcement (social
acceptance, social competence)▪ Self-medication▪ Expectancies (beliefs about
alcohol’s effect)▪ Personality (impulsivity, negative
emotionality)
123
E. Acute alcohol intoxication
Signs and symptoms Maladaptive mental state (increased
aggression) Neurologic signs: inco-ordination,
unsteady gait, slurred speech Impaired attention and memory High alcohol level 400-800 mg /dL:
coma stupor cardiovascular collapse
124
E. Acute alcohol intoxication
Treatment Prefer nonpharmacological
treatment: ▪ Alcohol level decline 15 mg/dL/hr▪ Supportive and preventive
Pharmacological treatment: ▪ Thiamine replacement: 100 mg IM then
100 mg 3 times/wk 3-4 wk
▪ In violent and agitated patient:▪ Short acting benzodiazepine (BZD): Lorazepam 1-2 mg po
▪ Or 2-5 mg IM or IV haloperidol
125
F. Alcohol withdrawal syndrome (AWS)
Occur 6-12 hr after end of heavy drinking
Sign and symptom Autonomic
hyperactivity Increased
muscle tremor Insomnia Nausea/
vomiting Transient
hallucination of illusions
Agitation Anxiety
Generalized seizures may occur 12-48 hr after end of heavy drinking
Delirium tremens (DT) may occur 3-5 days after: Disturbance of
consciousness Change in
cognition Perception

elevation V/S
126
F. Alcohol withdrawal syndrome (AWS)
AWS depend on Amount and frequency of drinking Wt, gender, age, year of alc use, PD,
liver function, previous medical history, tolerance
