Dr. Chestnut holds a Bachelor of Physical Education degree, a Master

Part 1: A Summary and Critique of the Clinical Eviden-
ce Regarding Spinal Healthcare Interventions: To Pra-
ctice Evidence-Based Healthcare You MUST Know and
Understand the Evidence.
Course Description:The prevalence of low back pain and other neuro-
musculoskeletal health problems is extremely high and
the financial and human burdens associated with these
health issues enormous, about this there is no deba-
te. The treatment and/or prevention of these health
problems over the past several decades represents a
dismal failure and a financial disaster; about this there
is no debate. Clearly it is imperative that the most evi-
dence-based interventions with respect to effective-
ness, cost-effectiveness, and safety are identified and
promoted as we move into the future; about this there
is no rational debate.
The questions being begged are obvious. Of the avai-
lable interventions which, if any, represents the most
evidence-based option with respect to effectiveness,
cost-effectiveness, and safety. What represents eviden-
ce-based intervention for low back pain and other neu-
romusculoskeletal health problems?
It is not possible to practice evidence-based care if you
are not aware of the evidence. Further, it is not suffice
to just be aware of the evidence, you must understand
how to determine what represents valid evidence and
how to implement the most valid evidence into clinical
practice.
Course Outline:A summary, review, and critique of the literature regar-
ding interventions for low back pain and other neuro-
musculoskeletal health issues will be provided.
Learning Objectives:
1. Be able to identify and understand the landmark
clinical studies regarding chiropractic adjustment/SMT.
2. Be able to identify and understand the landmark
clinical studies regarding other available interventions.
3. Develop a basic understanding of methodological
validity.
4. Develop an understanding of what repre sents evi-
dence-based spinal healthcare interventions
Part 2: The Neurophysiological Importance of Seg-
mental Motion: Understanding the Science Behind
Chiropractic.
Course Description:A review of literature regarding the consequences
of deficient segmental motion will be provided with
an emphasis on scar tissue adhesions, inflammation,
increased nociceptive input, deficient proprioceptive
input, altered sensori-motor integration, altered motor
control, and altered cerebellar function. A review of the
consequences of sufficient proprioception in both neu-
romusculoskeletal and overall health will be provided.
A review of the literature regarding the consequences
of chronic increased nociception and chronic decrea-
sed proprioception on both neuromusculoskeletal and
overall health will be provided. An introduction to the
physiological models of allostasis, epigenetics, auto-
nomic balance, and health and illness will be provided
in context of understanding the evidence-based relati-
onship spinal health and illness and overall health and
illness.

Course Outline:A review of the clinical literature reveals that chiropra-
ctic is the most evidence-based and most effective
intervention to restore and maintain segmental spinal
motion and spinal health. A review of the neurophysi-
ological and neuromusculoskeletal literature explains
why. Chronic reduced motion in synovial joints results
in chronic adverse adaptive changes such as increased
connective tissue adhesions, increased inflammatory
mediators, increased nociception, and decreased pro-
prioception. Sufficient motion in synovial joints is an
essential requirement to maintain health and prevent
adverse adaptive changes. Recent neurophysiologi-
cal research links deficient spinal segmental motion
with deficient proprioception, increased nociception,
altered sensori-motor cortex structure and function,
altered motor control, altered cerebellar structure
and function, and to adaptive changes in autonomic
nervous system function. Allostasis and allostatic
load are models of stressors and associated adaptive
physiology that can help elucidate the global neuro-
physiological benefits of motion and the global health
consequences of reduced segmental motion and as-
sociated sequelae. When discussing the relationship
between spinal health and overall health, Segmental
Neuromusculoskeletal Dysfunction is most accurately
interpreted as an allostatic load, as a stressor, as a con-
tributing factor to states of adaptive allostasis that con-
tribute to a decline in health not as a singular cause of
disease. Similarly, overall spinal health is most accura-
tely interpreted as an essential contributing factor to
overall health, not as the single source of health or as
the most significant variable determining health. The
scope of practice and the clinical focus of chiropractic
interventions is neuromusculoskeletal system health
and function. However, both spinal health and loss of
spinal health clearly effect the entire human organism
and the entire human experience. Chiropractic exper-
tise should include knowledge and understanding of
the indissoluble connection among segmental motion,
neuromusculoskeletal health and function, and global
health, function, and quality of life.
Learning Objectives:1. Understand the scar tissue, inflammatory, and affe-
rent neurological consequences of deficient segmental
motion.
2. Understand the importance of sufficient segmental
motion to spinal and overall health.
3. Understand the role of chiropractic in restoring and
maintaining segmental motion and neuromusculoske-
letal health and function, and how this contributes to
overall health and quality of life.
Part 3 – A Critique of Systematic Reviews: Understan-
ding Study Selection Criteria and Quality Rating Scales
and How This Effects Validity
Many recent systematic reviews assessing and rating
the level of evidence regarding the effectiveness of
interventions for low back pain, and the clinical guide-
lines based upon these reviews, have concluded that
SMT is of no benefit at all or is no better than any other
interventions for LBP.
Is this because the data from studies shows that chiro-
practic adjustment/SMT is simply no more effective
than usual medical care, physical therapy, surgery,
acupuncture, massage, or any other available interven-
tion?
Or, is there some biases with respect to study selection
and study quality ratings that negatively affect the
strength of evidence ratings for chiropractic adjust-
ment/SMT? Is it valid to completely ignore care fre-
quency and duration with respect to study validity and
selection criteria? Is it fair to define studies that include
only mobilizations from physical therapists as SMT stu-
dies and pool the results from these studies with those
that include thrust manipulations from chiropractors?
What happens to the level of evidence for chiropractic
SMT/adjustment if we select studies with a reasonable,
clinically representative frequency and duration of care
that includes thrust manipulation/adjustment? Are
there any systematic reviews that have done this?
Course Outline:This presentation critiques, compares, and contrasts
the different conclusions of the most recent systematic
reviews of SMT in order to expose some documentable,
significant bias against SMT when it comes to ratings of
strength of evidence. The issues of study inclusion and
exclusion criteria and methodology of rating study qu-
ality and strength of evidence will be specifically exa-
mined as will the methodological issues of individual
studies of SMT.
Learning Objectives:1. Understand how to evaluate the validity of syste-
matic review conclusions and clinical guidelines based
on selection criteria, study quality ratings, and strength
of evidence ratings.
2. Understand the importance study validity with
respect to defining SMT and differentiating thrust ma-
nipulation from mobilization, frequency and duration
of care, and fair comparisons of SMT to other interven-
tions
3. Understand the methodological explanations for
both the positive and negative findings regarding SMT,
the apparent heterogeneity of results, and the bias
with which the evidence for SMT is often either down-
graded or outright dismissed while the evidence for
other interventions is often artificially exaggerated.
