A 52 year‐old obese woman presents with complaints of fatigue. She notes a decreased energy level, particularly in the afternoons sometime will get hungry and want to eat some snack. She has had difficulty concentrating on clerk work. She denies constipation or cold intolerance but she notice that she was thirsty and voided urine frequently for some nights. She also notices that her weight was lost for 5 kg during last 2 months.
In past medical records, she had high fasting glucose on her last pregnancy 20 years ago at 28th week gestational
Patient StoryPatient Story
p , g g g p g y y g gage but normal oral glucose tolerant test. She has no other prior medical problems. She has one older sister with diabetes, and her father died of heart disease at the age of 62. She does not smoke but drink some alcohol only in special occasions.
She has been employed as an office clerk for 10 years. She usually eats out for lunch and often “pick up sandwiches” for dinner. Her hobbies include reading and watching old movies. She played basketball in high school time but since then has made no time for exercise.
On review of the systems, she feels palpitations in the chest but no chest pain after brief hard exercise. She avoids walking upstairs, as she gets short of breath. At night she notices a burning sensation on the feet. She also mentions an abnormal discharge from vagina.
Her physical exam is remarkable for a blood pressure of 140/ 85, pulse of 80. She is 160 cm. tall and 80 kg inHer physical exam is remarkable for a blood pressure of 140/ 85, pulse of 80. She is 160 cm. tall and 80 kg in weight. The mucus membrane is moist, and there is no palpable thyroid nodule. The cardiovascular exam is normal, and the lungs are bilaterally clear. The extremities appear normal except for trace edema in the ankles.
Fasting laboratory values are as follows: CBC normal; serum glucose 255 mg/dl; BUN 10 mg/dl; creatinine 0.8 mg/dl; total cholesterol 300 mg/dl, LDL cholesterol 220 mg/dl, HDL cholesterol 35 mg/dl; triglycerides 712 mg/dl. Urinalysis presents positive sugar and few Candida yeast cells in urine. Vaginal discharge has numerous Candida yeast cells.
The doctor asks her to go to an exercise course in the evening which including weight watching and diet control. The doctor prescribes metformin to control her blood sugar, fluconazole vaginal suppository for yeast infection. The doctor makes an appointment again next month to check her body weight, blood pressure, blood sugar and, lipid level.
