ISO 9001 is based on the plan-do-check-act methodology and provides a process-oriented approach to documenting and re-viewing the structure, responsibilities, and procedures required to achieve effective quality management in an organization. Changes introduced in the 2015 revision are intended to ensure that ISO 9001 continues to adapt to the changing environments in which organizations operate. Some of the key updates in ISO 9001:2015 include the introduction of new terminology, restruc-turing some of the information, an emphasis on risk-based thinking to enhance the application of the process approach, improved applicability for services, and increased leadership requirements.

This course provides a general overview of ISO 9001:2015. The course will be beneficial to all levels of experi-ence with the ISO 9001 standard. For those already familiar with the previous standard, the differences between the revi-sions will be highlighted. Some topics that will be covered dur-ing the training include establishing a quality policy and objec-tives, documented information control, corrective actions, risk based thinking, understanding the process approach, and a brief overview of internal auditing.

Human error also constitutes one of the direct causes of some of the most shocking industrial accidents which have occurred around the world. Although new safety regulations and even the industry‘s own risk assessments on oil and gas operations, he said, aren‘t enough if oil companies don‘t take action on what they learn. Root cause analysis (RCA) is a method of problem solving used for identifying the root causes of faults or problems. A factor is considered a root cause if removal thereof from the problem-fault-sequence prevents the final undesirable event from recurring; whereas a causal factor is one that affects an event's outcome, but is not a root cause. Though removing a causal factor can benefit an outcome, it does not prevent its recurrence with certainty.

This course provides the role of a staff who contribute in admin-istration, operation and inspection in industry. It is designed for staff who is responsible for analyzing problem, complaint, non-conformance. RCA is applied to methodically identify and cor-rect the root causes of events, rather than to simply address the symptomatic result. Focusing correction on root causes has the goal of entirely preventing problem recurrence. Conversely, RCFA (Root Cause Failure Analysis) recognizes that complete prevention of recurrence by one corrective action is not always possible.

Objective

To provide knowledge and awareness about the root cause analysis in industry

To improve efficiency by managing problem within your organizations

To apply the concept of root cause analysis management

To determine potential strategies and actions to ensure effective implementation

A SMS is intended to act as a framework to allow an organisa-tion, as a minimum, to meet its legal obligations under occupa-tional safety and health law. There is an implied moral obliga-tion placed on an employer to ensure that work activities and the place of work to be safe, there are legislative requirements defined in just about every jurisdiction on how this is to be achieved and there is a substantial body of research which shows that effective safety management (which is the reduction of risk in the workplace) can reduce the financial exposure of an organisation by reducing direct and indirect costs associated with accident and incidents.

This course will help participants and organizations understand the function, role and importance of developing and implement-ing a Safety Management System (SMS). The course has been designed to provide an overview of the Safety Management Systems concepts, specifically in light of the requirements of SMS. This course meets the awareness requirement of safety management in establishing, maintaining, and improving an organization‘s safety management system.
