Chief complaint : ถกูรถสิบแปดลอ้ชนทา้ยขณะท่ีขบัรถยนต ์โดยไม่ไดค้าดเขม็ขดันิรภยั 3 วนัก่อนมาโรงพยาบาล

Present illness 3 วนัก่อนมาโรงพยาบาล ผูป่้วยถกูรถสิบแปดลอ้ชนทา้ย เวลา 01.00 น.โดยท่ีผูป่้วยไม่ไดค้าดเขม็ขดันิรภยั ผูป่้วยสลบ หลงัจากท่ีฟ้ืน ไม่สามารถจาํเหตุการณ์ได ้ร่วมกบัมีอาการปวดตน้คอ แขนและขาอ่อนแรงทั้งสองขา้ง ไม่สามารถกลั้นอุจจาระได ้

Present illness ตรวจร่างกายแรกรับท่ีโรงพยาบาลมหาชยั พบวา่ E4M0V5 ร่วมกบัส่งทาํ x-ray สงสัย C-spine fracture with spinal cord injury จึงส่งทาํ MRI หลงัจากนั้นไดย้า้ยมารักษาท่ีโรงพยาบาลศูนยก์ารแพทยฯ์

Physical examination ► HEENT : not pale conjunctiva, anicteric sclera, no dry lip, no lymphadenopathy ► Cardiovascular system : normal S1S2 , no murmurs ► Respiratory system : good air entry, no tachypnea, normal breath sound, no adventitious sound

Physical examination ► Abdominal : normoactive bowel sound, no abdominal distension, no tenderness, no guarding, no rebound tenderness, hypertympanic on percussion, no sign of chronic liver disease

Physical examination ►Neurological system

Speech: no dysarthria, no aphasia Cerebellar sign: can’t be evaluated Cranial nerve : can’t be evaluated

Sensory : decrease sensation at C4 and C5 Deep tendon reflex : 3+ at left upper extremities, and 1+ at the others

Babinski's sign : absent (plantar flexion) Stiff neck : can’t be evaluated

Physical examination ► Extremities : capillary refill < 2sec, no pitting edema ► PR exam : positive bulbocavernous reflex, positive perianal sensation, loss of sphincter tone

Problem lists 1. Quadriplegia 2. Decreasing sensation at C4 and C5 level 3. Abnormal deep tendon reflex : 3+ at left upper

extremities, and 1+ at the others 4. Loss of anal sphincter tone

Final diagnosis - Fracture at C3 – C4 spine - Anterior subluxation of C4 above C5 with traumatic disc herniation at C4/C5 level - Incomplete spinal cord injury at C4 vertebral level - Right vertebral artery injury

Conclusion Sign and symptoms - Quadriplegia - Decreased sensation at C4 and C5 level Positive pertinent finding - Abnormal deep tendon reflex : 3+ at left upper extremities, and 1+ at the others - Loss of anal sphincter tone

Definite Diagnosis : C3-C4 spine fracture with spinal cord injury at C4 vertebral level

Treatment Anterior Cervical Discectomy and Fusion [ACDF] C4-C5 with plate and screw : surgical procedure to treat nerve root or spinal cord compression.

Knowledge

Symptoms • Neck pain or tenderness with touch • Neck pain with movement • Headaches • Stiff neck • Difficulty moving your neck • Neck muscle spasms

Symptoms With spinal cord or nerve root injury in the neck • Arm or leg weakness and numbness • Weak hand grasp. This may cause you to drop objects • Poor handwriting • Difficulty with balance or falls • Loss of bowel or bladder control

Diagnosis • Physical exam and history • Imaging scans that take pictures of your neck - X-rays - Computed tomography (CT) scan - Magnetic resonance imaging (MRI)

Treatment Nonsurgical treatments • Wearing a neck brace or collar to support the neck • Medicine to reduce pain and swelling • Closed reduction : physical manipulation to realign the cervical vertebrae

Treatment Surgical treatment : Skeletal traction • used before surgery • uses weights, pulleys and ropes to exert a pulling force to help realign the cervical vertebrae.

Treatment Surgery of the cervical vertebrae is needed if: • Unstable cervical vertebrae • A lot of nerve damage in the cervical vertebrae • Spinal cord injury • Pocket of blood over the spinal cord • Cervical disc pushing on the spinal cord

ความไม่มัน่คงของกระดูกสันหลงัส่วนคอ 1. ท่า lateral view กระดูกสันหลงัอนับนเล่ือนตวัไปอยูห่นา้ต่อ

กระดูกสันหลงัอนัถดัไปมากกวา่ 3.5 mm

2. Plane of angulation โดยวดัความแตกต่างของมุมของกระดูกสันหลงัระดบัท่ีสงสัยเปรียบเทียบกบักระดูกสันหลงัท่ีปกติไม่วา่อยูต่ ํ่ากวา่หรือเหนือกวา่กต็ามถา้มากกวา่ 11º ถือวา่มีภาวะไม่มัน่คงของกระดูกสันหลงั

แตไ่มมี่อาการของระบบประสาท

สญูเสียความมัน่คง จะต้องทําให้กระดกูสนัหลงัส่วนท่ีเคล่ือนหลดุเข้าสู่

สภาพเดมิไมว่า่จากการดงึถ่วง ใสเ่ฝือกศรีษะ, ลําตวั, ปลอกคอหรือการผา่ตดั

ไมส่ญูเสียความมัน่คง ภายหลงัอบุตัเิหต ุอาจจะใช้แค่

ปลอกคอใสช่ว่ยในการรักษาก็

เพียงพอ
