N17 Acute renal failure
In ICD-10 system,
‘Etiology' is the “First Priority”.
7
Acute Renal Failure
A clinical syndrome characterized by rapid reduction in renal
excretory function underpinned by variety of caused
ARF is defined by the
✴ glomerular filtration rate is reduced….decrease urine
✴ nitrogenous wasted accumulates in the body….uremia
✴ metabolic and electrolytes disturbances
Reserved for patients who have lost renal function
to the point that life can no longer be sustained
without intervention and need renal replacement
therapy.
8
Acute Kidney Injury
A clinical syndrome characterized by rapid reduction in renal
excretory function underpinned by variety of caused
AKI causes are generally divided into
✴ Pre-renal
✴ Renal (intrinsic)
✴ Post-renal
Used to describe the milder as well as severe
forms of acute renal dysfunction in patients.
9
AKI vs ARF
Acute Kidney Injury Acute Renal Failure
Defined as an abrupt change in
serum creatinine and/or urine output
and classified according the RIFLE
criteria or AKIN criteria
Defined as renal function inadequate
to clear the waste products of
metabolism despite the absence of
correction of hemodynamic or
mechanical causes
Intended to describe the entire
spectrum of disease from being
relatively mild to severe
Clinical manifestations of renal
failure include the following
- Uremic symptoms
- Hyperkalemia
- Hyponatremia
- Metabolic acidosis
AKI = ARF ?
10
RIFLE Criteria & AKIN Criteria
11
Acute Kidney Injury
A clinical syndrome characterized by rapid reduction in renal
excretory function underpinned by variety of caused
Acute Kidney Injury Criteria
Any of the following
- Increase in serum creatinine >0.3 mg/dL within 48 h
- Increase in serum creatinine >1.5x baseline within 7 d
- Urine volume <0.5 mL/kg/h x 6 h
KDIGO 2012
12
Acute Renal Failure

glomerulonephritis, Vascular occlusions
Postrenal ARF: Obstruction

13
Acute Tubular Necrosis (ATN)
Ischemic type
Prolonged renal ischemia
Toxic type
Endogenous toxins
Rhabdomyolysis
Hemolysis
Exogenous toxins
Antibiotics e.g. aminoglycosides, amphotericin B
Radiocontrast agents
Heavy metals e.g. cis-platinum, mercury
Poisons e.g. ethylene glycol
14
Nephrotoxic Renal Failure
N14 Drug- and heavy-metal-induced tubulo-interstitial
and tubular conditions
Use additional external cause code (Chapter XX), if desired, to
identify toxic agent.
N14.0 Analgesic nephropathy
N14.1 Nephropathy induced by other drugs, medicaments and
biological substances
N14.2 Nephropathy induced by unspecified drug, medicament or
