BIENVENIDOS A VALENCIA

Estimados colegas,

A lo largo de las dos últimas décadas la evaluación y manejo terapéutico de los tumores ver-tebrales han evolucionado ostensiblemente, manteniendo actualmente una rápida progre-sión. El mejor conocimiento de las enfermedades sistémicas, una valoración preoperatoria y estadiaje más adecuado, el manejo multidisciplinario –radiólogo, oncólogo y anestesió-logo–, el conveniente entrenamiento del cirujano de columna (ortopeda o neurocirujano) y su equipo, con un perfil y conocimientos oncológicos adecuados, junto con la introducción de nuevas técnicas quirúrgicas e instrumentaciones, permiten una mayor agresividad en el manejo de estos pacientes, obteniendo un pronóstico y unos resultados más beneficiosos, con el objetivo de mejorar su calidad de vida.

El incremento de la incidencia y prevalencia de cáncer en los diferentes órganos hace que el número de pacientes con enfermedad metastásica, con o sin sintomatología, sea cada vez más elevado. Pulmón, mama, linfoma y mieloma son los orígenes más comunes, siendo la columna torácica, la más frecuentemente afectada seguida de la lumbar y la cervical, cada una de ellas con sus particularidades anatómicas.

Los principios básicos para proporcionar la educación, investigación, consultoría y oportu-nidades de “networking” para los cirujanos de columna y especialistas implicados en el ma-nejo de estos pacientes, están bien demostrados por el sistema de cursos y reuniones que se organizan en todo el mundo. Pero en ocasiones, excesivamente compartimentalizados por especialidades.

Es por ello, que hay que realizar un esfuerzo importante durante los próximos años y estable-cer foros multidisciplinarios de conocimiento, para estructurar y dirigir las diferentes activi-dades en campos especiales, de forma particular la afectación tumoral de la columna verte-bral. Iniciar y organizar proyectos de investigación clínica y la publicación de sus resultados en la literatura internacional, serían algunos de los objetivos más deseados en esta reunión.

Es una oportunidad única organizar un evento educativo con la contribución de los diferen-tes especialistas del programa que dan un alcance especial sobre el estado del arte del ma-nejo de los tumores de la columna vertebral.

Estamos convencidos de que los participantes de este curso van a cosechar e incrementar un conocimiento más profundo e integral del campo de la patología oncológica.

El curso incluye desde la valoración inicial de los pacientes, diferentes opciones terapéuticas oncológicas, perspectiva general de las técnicas básicas de la reconstrucción de la resec-ción y estabilización de la cirugía de tumoral. Especialistas con gran experiencia personal demostrarán los “estándares de oro”, así como soluciones en los diferentes segmentos de la columna vertebral y diferentes patologías tumorales.

Además de las técnicas estándar, se discutirán detalladamente las posibles complicaciones intra y postoperatorias, con especial énfasis en las soluciones y los resultados finales. Se pretende que los asistentes participen en las discusiones de las diferentes mesas, tanto ci-rujanos, oncólogos… con diferente grado de experiencia que se involucran con los tumores vertebrales en cualquier etapa del diagnóstico, diagnóstico diferencial y tratamientos com-plejos…

El objetivo principal de esta reunión es dar una visión general de las opciones actuales en el manejo de la columna vertebral tumoral a través de los procesos de toma de decisiones, la planificación terapéutica, las técnicas quirúrgicas y la atención perioperatoria. El curso ofrecerá un conocimiento basado en la evidencia en el manejo de patologías complejas aso-ciadas a neoplasias vertebrales.

Tras la reunión, los participantes deberán ser capaces de:

• Identificar las diferentes opciones terapéuticas, así como indicaciones para la cirugía en tumores vertebrales.

• Identificar los objetivos del tratamiento en tumores primarios y metástasis. • Identificar los márgenes quirúrgicos óptimos para la resección en bloque en diferentes

tumores y diferentes topografías.• Seleccionar las técnicas quirúrgicas óptimas (intralesional, escisión marginal, sustitu-

ción y estabilización).• Reconocer las complicaciones intra y postoperatorias y decidir sobre su adecuado manejo.• Entender las direcciones de las actividades de trabajo del resto de especialistas implica-

dos en el manejo de estos pacientes.

Dear Colleagues,

Over the last two decades, the assessment and therapeutic management of vertebral tu-mours has evolved ostensibly, and rapid advances continue to be made in the field today. The greater understanding of systemic diseases, a more appropriate pre-operative evalua-tion and staging, a multidisciplinary approach (radiologist, oncologist and anaesthesiolo-gist), the proper training of the spine surgeon (orthopaedic surgeon or neurosurgeon) and his or her team, with adequate oncological knowledge and profile, together with the incorpora-tion of new surgical techniques and instrumentation, have made it possible to manage these patients more aggressively. This has resulted in more optimal prognoses and outcomes, and has enabled us to close in on the objective of improving quality of life.

The increased incidence and prevalence of cancer in the various organs have caused the number of patients with metastatic disease, with or without symptoms, to be increasingly higher. Lung, breast, lymphoma and myeloma are the most common origins, with the thorac-ic spine being the most frequently aff ected, followed by the lumbar and cervical spine, each with its own anatomical particularities.

The basic principles for providing the education, research, consulting and networking oppor-tunities for spine surgeons and specialists involved in managing these patients are reflected in the system of courses and meetings organized around the world. However, these gather-ings are oft en excessively compartmentalized by medical specialty.

That is why a major eff ort must be made over the coming years to establish multidisciplinary knowledge forums to structure and shape the diff erent activities into special fields, spe-cifically, tumours aff ecting the spinal column. Launching and organizing clinical research projects and publishing their results in international literature are some of the most highly sought-aft er objectives of this meeting.

This meeting provides us a unique opportunity to be able to organize an educational event with the input of the various specialists in the program who will be able to present a state-of-the-art perspective of the management of vertebral tumours.

We are convinced that the participants at this course are going to develop and deepen their comprehensive understanding of the field of cancer pathology.

The course covers everything from the patients’ baseline evaluation, diff erent oncological therapeutic options, and a general perspective on the basic techniques of reconstruction of the resection and stabilization of the tumour surgery. Specialists with extensive personal ex-perience will demonstrate the “gold standards”, as well as solutions in the various segments of the spine and range of tumour pathologies.

In addition to the standard techniques, potential intra- and post-operative complications will be discussed in detail, with special emphasis on their solutions and final outcomes. Our aim is for all attendees, surgeons, oncologists, etcetera, each with a diff erent degree of expe-rience in vertebral tumours at all stages of diagnosis, diff erential diagnosis, complex treat-ments, and so on, to actively participate in the diverse round table discussions.

The ultimate objective of this meeting is to provide a general outlook on the current options in vertebral tumour management that span the decision-making processes, therapeutic planning, surgical techniques and peri-operative care. The course off ers evidence-based ed-ucation on the management of complex pathologies associated with vertebral neoplasms.

Aft er the meeting, all participants should be able to:

• Identify the diff erent therapeutic options for vertebral tumours, as well as the indications for surgery.

• Identify the treatment objectives in primary tumours and metastases.• Identify the optimal surgical margins for en bloc resection in diff erent tumours and vari-

ous topographies.• Select the optimal surgical techniques (intralesional, resection, replacement and stabili-

zation).• Recognize intra- and post-operative complications and make decisions on their proper

management.• Understand the aims of the work of the rest of the specialists involved in the manage-

ment of these patients.
