Liebe Leser des APV-Plus Magazins,

an dem gewohnten Bild in dieser Rubrik, das Sie nunmehr erneut ertra-gen müssen, können Sie schon ablesen, dass die Mitgliederversammlungder APV mich erneut zum Mitglied des Vorstands gewählt hat. Außer-dem wurden satzungsgemäß per Personenwahl folgende Mitglieder ge-wählt – hier in alphabetischer Reihenfolge aufgeführt: Prof. Dr. JohannesBartholomäus (Pharmakreativ), Dr. Kathrin Bartscher (NextPharma), Dr.Karoline Bechthold-Peters (Novartis), Prof. Dr. Heribert Häusler (Pensio-när), Prof. Dr. Sandra Klein (Universität Greifswald), Dr. Hans Lindner(Bayer) und Dr. Martin Lück (Grünenthal). Ich bedanke mich ausdrücklichauch bei den unterlegenen Kandidatinnen und Kandidaten, die sich zurWahl gestellt hatten. Ich hätte mir auch mit ihnen eine erfolgreiche Zu-sammenarbeit im neuen Vorstand sehr gut vorstellen können. Die un-gewöhnlich hohe Beteiligung an der Mitgliederversammlung in Granadawar sicherlich auf die Spannung hinsichtlich der Wahl zurückzuführen,bei der es nach Jahren zum ersten Mal mehr Kandidat(inn)en als zu be-setzende Positionen gab. Insofern darf sich der neue Vorstand in einemhohen Maß legitimiert fühlen, die Geschicke der APV in den nächstenvier Jahren zu lenken. Nach der nächsten Vorstandssitzung werde ichdetailliert die Aufgabenverteilung innerhalb des Vorstands mitteilen.

Das PBP World Meeting in Granada wird allen Teilnehmern sicherlich als eine sehr erfolgreiche Konferenz in Erinnerungbleiben. Etwa 1.300 Teilnehmer, knapp 1.000 eingesendete Beiträge, die größte ResearchPharm® aller Zeiten; das PBPWorld Meeting ist eine feste Größe geworden und wächst – im Gegensatz zu vielen anderen wissenschaftlichen Kon-ferenzen – weiter. Es war in der Kontinuität der letzten PBP World Meetings schon fast vorauszuahnen, dass selbst inAndalusien nicht nur die Sonne vom Himmel lachen würde, sondern auch etliche Wolken, die diverse Regengüsseund auf den Bergen gar Schnee bereitstellten. Dies tat der Stimmung bei der Konferenz jedoch keinen Abbruch. Nebenden hochwertigen wissenschaftlichen Beiträgen wird besonders der Gesellschaftsabend in einer umwerfend schönenHazienda oberhalb von Granada, einem hervorragenden Mehrgänge-Dinner und einer Party bis spät in die Nacht inErinnerung bleiben. Auch wenn es schwer ist, derartige Ereignisse und Erlebnisse immer weiter zu toppen, bin ich festdavon überzeugt, dass das 12. PBP World Meeting, das vom 23. bis 26.03.2020 in Wien stattfinden wird, weitere Re-korde knacken wird.

Wir sehen uns auf einer der vielen interessanten APV-Veranstaltungen, die Sie in diesem Heft finden werden!

Europäische wirtschaftlicheInteressensvereinigung gegründet

Am Rande des PBP World Meetings in Granada wurde Geschichtegeschrieben: zusammen mit unseren ständigen Partnern bei denPBP World Meetings, APGI aus Frankreich und ADRITELF aus Ita-lien, wurde der Gesellschaftervertrag einer Europäischen wirt-schaftlichen Interessensvereinigung (EWIV) unterschrieben. DieseGesellschaftsform ist weitgehend unbekannt in Deutschland. Siebasiert auf dem gültigen Recht der Europäischen Union undwurde im Jahr 1985 als erste europäische Unternehmensformvon der damals noch existenten EWG eingeführt. Die seltene Un-ternehmensform (engl.: EEIG, European Economic Interest Grou-ping; franz.: GEIE, Groupement européen d'intérêt économique)wurde zur Erleichterung und Förderung der grenzüberschreitenden Zusammenarbeit der Mitgliedstaaten eingeführt.Sie kann in Verbindung mit nationalem Recht jedes Mitgliedstaates von Gesellschaften (GmbH, AG, OHG etc.) oderanderen Organisationen (Gemeinnützigen Vereine, Stiftungen etc.) gebildet werden. In Deutschland gilt die EWIVals Handelsgesellschaft im Sinne des Handelsgesetzbuchs. Die neu gegründete Gesellschaft wird „Triple A CongressOrganisation EWIV“ heißen und Ihren Sitz in Mainz, in den Räumlichkeiten der APV an der Kurfürstenstraße haben.Geschäftsführer wird Dr. Martin Bornhöft, der zusätzlich die Rolle als APV-Geschäftsführer behalten wird. Mit derEWIV wird die Zusammenarbeit in allen Fragen der Organisation und Durchführung von Kongressen, wie dem PBPWorld Meeting und der European Conference on Pharmaceutics, organisatorisch und fiskalisch wesentlich erleichtertund die schon lange andauernde Partnerschaft mit APGI und ADRITELF manifestiert.

Nachruf

Die APV trauert um Ihr langjähriges Ehrenmit-glied Dr. Rolf Dolder. Dr. Dolder wurde 1963Mitglied bei der APV und unterstützt den Ver-ein 20 Jahre von 1966 bis 1986 als Vizepräsi-dent. Für seine langjährige, tatkräftigeUnterstützung sowie für besondere Verdien-ste erhielt Dr. Dolder die „silberne Ehrenna-del“ und zugleich wurde er zumEhrenmitglied ernannt.

Die APV wird ihm stets ein ehrendes Geden-ken bewahren.

The PBP World Meeting, which is jointly organised by APV, ADRITELF and APGI, tookplace in Granada this year, a wonderful city in the south of Spain. The meeting was heldfrom 19 to 22 March 2018 at the Exhibition and Conference Centre Granada with fourparallel symposia on basic research, R&D, industrial practice and analytics.Aim of the conference committee was to bridge the gap between industry and academicresearch. Almost 1300 participants, from 46 countries and 4 continents joined this year’smeeting in Granada and the organisers were pleased to announce that participants de-rived in almost equal parts from the three working disciplines industry, academia andstudents.The opening ceremony on Monday, 19 March 2018 was chaired by the local represen-tative José Luis Arias Mediano from the University of Granada, and the representativesof the hosting organisations: Anna-Maria Fadda (President of ADRITELF), Jörg Breitkreutz(President of APV) and Prof. Dr. Jürgen Siepmann (President of APGI).The first plenary lecture was given by Thomas Rades, University of Copenhagen, Den-mark, about “Enabling technologies for poorly soluble active pharmaceutical ingre-dients”. In his talk he reported on new research and development out of Copenhagenon amorphous and lipid based drug delivery systems. New and exciting research findingson glass forming ability, super-saturation, and glass stability were discussed togetherwith new developments in amorphous solid dispersions and co-amorphous systems. Inthe second part of the talk, new approaches in research and development of lipid baseddrug delivery systems, including super-SNEDDS, the chasing principle and the role of di-gestion in the efficacy of these systems were highlighted.

The first day of the meeting continued with a hot topic session with a talk by Jean-Chri-stophe Leroux, ETH Zürich, Switzerland, on “Oral delivery of peptides for gastrointestinaltargets”, pointing out that the systemic delivery of protein and peptide drugs via theoral route represents a considerable challenge. Abdul Basit, University College London,United Kingdom, gave the second lecture on “Translating pharmaceutics into oral the-rapeutics” and Maria José Alonso, University of Santiago de Compostela, Spain, closedthe session with her talk about transmucosal drug delivery (nasal, oral, ocular).

The welcome reception on the evening of the first day was held in the exhibition area.It was a great success and the participants had a chance to enjoy a glass of wine andcanapés while exploring the exhibition.

The Tuesday morning started with four parallel sessions, i. a. the symposium on newmanufacturing technologies with a talk of Robert O Williams III, University of Texas, Uni-ted States. The second symposium dealt with dermal and transdermal drug delivery. Fur-thermore, there were two parallel short lecture sessions with presentations selected fromthe submitted conference abstracts on “Controlled drug delivery” and “Peptide andprotein formulation”. Moreover, there were short lecture sessions during the course ofthe conference on nanoparticles, technical innovations - printing technologies, pediatricand geriatric drug delivery, preformulation and bioavailability.In total over 1000 scientific contributions were submitted to the conference, of whichthe abstract review committee chose 72 for oral presentation and over 800 for posterpresentations. The poster presentations took place every day in the exhibition hall, where

the participants had the opportunity to discuss the presented research results with theauthors.Before the Tuesday plenary lecture by Arzu Selen, FDA, USA on the “Pediatric formula-tions and dosage forms: Application of biopharmaceutics risk assessment roadmap (Bio-RAM)”, the APV announced this year’s APV award winners. The APV Research Awardfor Outstanding Achievements in the Pharmaceutical Sciences went to Prof. Dr. MaikeWindbergs from the Goethe University in Frankfurt. APV Award for the Most Outstan-ding Doctoral Thesis in the Pharmaceutical Sciences in the Years 2016/2017 was givento Dr. Neha Shreshta.The four symposia on Wednesday were focused on data integrity and advanced cha-racterisation techniques. Some highlight were the aspects discussed regarding the recent increasing demand forpersonalised medicine not only for diagnostics but moreover for individual dosages andtreatments by Maren Preis from Abo Akademi in Turku. Also the APGI award lecture took place this day. The Marie Maurice Janot Award (spon-sored by SANOFI) was given to Elias Fattal, University of Paris-Sud, France, and his awardlecture with the title “Tackling physiological resistances to drug delivery”. On Wednesday night the organisers invited all participants to the Gala dinner at La Ma-munia, where the “Coro Rociero” welcomed all participants with their traditional chant.A variety of local and international appetizers, spanish musicians and the tempting lo-cation in the gardens of La Mamunia made this evening start really special for all guests.The aperitif was followed by a tasty 5-course-menu in the appealing dining room. Afterthe excellent dinner the guests were invited to join a networking come together withbeer, wine, cava and a DJ, who took care of a good mixed music set up with Spanishflair.

During day 4 of the PBP World Meeting the participants had the opportunity to attendinvited lectures focusing on new materials and drug-drive combinations/combinationproducts. We were happy to welcome Hirofumi Takeuchi from the Gifu PharmaceuticalUniversity in Japan. He gave an overview on Continuous pharmaceutical process inJapan, their challenges in characterizing pharmaceutical processes including continuousgranulation and tableting. The plenary lecture was given by Sébastien LecommandouxENSCBP-Bordeaux – LCPO, France, who gave insides about the Innovative BiomimeticPolymer-based Biomaterials.Moreover on Thursday, the 1st International A.D.R.I.T.E.L. Award 2018 „Maria EdvigeSangalli“ was bestowed to Twan Lammers, RWTH Aachen University, Germany. The PBP World Meeting was accompanied by the largest ResearchPharm® exhibitionever in the history of the conference series with over 70 exhibiting companies. The par-ticipants had the opportunity to learn about the latest trends, products and equipmentsexhibited during the four days. This year we offered for the first time the exhibit hallscavenger hunt. All conference attendees had the chance to win an attractive prize, ifthey have had collected a stamp of each booth. For the first time of the scavenger huntwe were happy of a high resonance. The following prices were handed over on Thurs-day: a Twin-screw Extruder Model sponsored by Melt Prep, a 100 € Amazon gift cardsponsored by Technobis, a 100 € Amazon gift card sponsored by Merck and a singlemalt scotch whisky sponsored by ShinEtsu! Congratulations to the winners! The organisers furthermore announced the location for the 12th PBP World Meeting,which will be held at the Austrian Center Vienna in Austria from 23 to 26 March 2020.The PBP World Meeting is a biannual event in even years, and is now alternating with athe conference series “The European Conference on Pharmaceutics (ECP)”. The 3rd Eu-ropean Conference on Pharmaceutics with main topic “Bringing science into pharma-ceutical practice” will take place 2019 in Bolgona, Italy.

Introduction

In the last few years digitalization has leapt from the office and factory into the way health is managed both in hos-pital and in the community. Digitalization and, in particular, the arrival of Bluetooth technology and the popularityof the Smartphone opens up a myriad of opportunities to monitor health and its treatment in or “almost in” realtime, potentially promoting adherence to therapy and the tailoring of treatment to an individual patient’s needs. Itssophistication can vary from mobile applications which measure the number of steps taken to facial recognition sy-stems monitoring the administration of medicines. However, its increasing use raises issues of data ownership anduse, protection of patient confidentiality and the susceptibility of devices to security breaches. Its use had also widerimplications even for more wealthy societies as this technology is only open to the digital savvy and those with goodinternet connections. This article will not touch further on these security, societal and ethical issues but focus ontechnology developments in the field and their potential implications for drug development in the future.

Digital Platforms

mySugrOne of the main ways digital technology is used to manage health in the community is through monitoring, datamanagement and advice platforms. mySugr [1], based in Vienna Austria, is an open digital platform used by 1 millionpeople worldwide which aims to simplify diabetes management for patients. The mySugr bundle contains test strips,an Accu-Chek® Guide meter, the mySugr mobile application and diabetes advice on tap from a certified diabeteseducator. The Accu-Chek® Guide meter sends the blood glucose values to the mySugr app which enables the patientto review and analyse the data including estimated A1c levels easily and quickly, leading potentially to better ma-nagement of the disease. The patient has also the possibility to contact a diabetes educator with questions or foradvice or share the data with healthcare professionals and care-givers. The app is a class 1 medical device in the USand EU and the Bolus Calculator module has risk class IIb approval (available only in Europe at present). mySugr wasacquired by Roche in June 2017 which plans to integrate it into Roche’s new patient-centred digital health servicesfor diabetes [2]. As part of the agreement mySugr will remain a separate legal entity and the platform will remainopen to patients. Together with mySugr Roche plans to develop new technologies to further improve diabetes ma-nagement capitalizing on develop-ments in artificial intelligence and big data management.

Ingestible Markers and Adherence to Oral TherapyAdherence to therapy is a major issue both in clinical research and for ensuing successful therapeutic outcomes. Itis thought that 20% to 30% of clinical trials fail due to poor adherence to the study protocol, and even if poor ad-herence does not impact on the outcome of the trial itself, it results in increased numbers of patients having to beenrolled to ensure that the study results have the intended statistical power. For example, it has been calculated thatif 20% of patients do not comply with their medication regimen, then the cohort size needs to be increased by 60%[3]. Any im-provement in adherence rates therefore increases the reliability of the data collected and reduces trialcosts and the risk of trial failure. For approved medicines non-adherence has been estimated to cost the U.S. health-care system alone $290 billion annually [4] as a result of additional hospital admissions and other healthcare inter-ventions caused by the prescribed treatment not being taken as intended. With diseases like tuberculosis compliancewith the prescribed therapy is essential not only to ensure treatment, but also to prevent the development of resi-stance and spread of the disease. The causes of non-adherence are multi-factorial and difficult for doctors and otherhealthcare professionals to discern. Common reasons cited for poor compliance are the chronic nature of many the-rapies, side-effects experienced, lack of belief in medication effectiveness, denial that therapy is required, cost oftherapy (an issue in countries where patients must pay or contribute significantly to the costs of their medication)and dosage form and formulation factors such as ease of swallowing, taste, pain on injection and frequency of do-

sing. A number of companies have or are developing digital tracking of ingestion together with data collection andfeedback to monitor adherence and engage patients actively in their therapy.

Abilify™ MyCite™In November 2017 the FDA approved Otsuka Pharmaceutical Co.’s (Tokyo, Japan) Abilify™ MyCite™, the first everoral dosage form with a built-in tracking sensor to monitor ingestion [5,6]. Abilify™ MyCite™, which contains ari-piprazole, is licensed for the treatment of schizophrenia, acute treatment of manic and mixed episodes associatedwith bipolar I disorder and as an add-on treatment for depression in adults. The 1mm embedded sensor (IngestibleEvent Marker), manufactured by Proteus Digital Health (CA, USA), contains magnesium and cuprous chloride coatingswhich when wetted by gastric fluid form a circuit that results in a tiny electric charge. This signal is transmitted to awearable patch, which in turn relays a digital record to a mobileapplication and then to the Proteus Cloud so thatingestion can be tracked on a smartphone and also accessed via the Cloud. The patch also records the patient’s pe-riods of activity and the patient can add details via the app on periods of rest and mood. An icon shows the user ifthe patch is fully in contact with the skin and functioning properly. The patch is applied just above the lower edgeof the rib cage on the lefthand side of the body. The patient has the option of allowing carers and/or their doctor toaccess the data via a webbased portal.

In its press release [6] the agency highlighted that there is no proof as yet that prescribing Abilify™ MyCite™ aidsadherence to therapy, and warned that it should not be used to track real time digestion or in an emergency situationas detection may be delayed (up to 2 hours) or may not occur at all. However, in the majority of cases detectionoccurs within 30 min.

This was second time that approval had been sought from the FDA for this product. The first time round the agencyissued a complete response letter requesting more information about human factors testing and simplification ofsome of the smartphone app and online portal content to make it easier for patients, especially those with cognitiveimpairment, to understand and use [7]. The approval shows the commitment of the FDA to supporting innovationin medicines but some physicians have questioned the combination of this type of monitoring with a medication forschizophrenia, a condition where patients can suffer from delusions of being spied on or being poisoned [8, 9].

The tracking used in Abilify™ MyCyte™ is known as Proteus Discover™ and was first approved by the FDA as forthe ingestion tracking of medicines in 2012. At that time the sensor was not approved to be embedded in a tabletand pharmacies had to be specially contracted to encapsulate it together with the medicine to be tracked. Use ofthe sensor outside clinical trials only started in 2016 [10] but it has already been shown to have some success in im-proving adherence in patients with hypertension and diabetes [11]. It remains to be seen if this will also be the casefor Abilify™ MyCyte™.

ID™ sensor technologyEtectRx Inc (FL, USA) is another company employing an ingestible sensor to track the administration of medicines.Its ID™ sensor technology [12] is embedded in a standard gelatin or hydroxypropyl methylcellulose capsule andsends a signal when it comes into contact with stomach fluids to a reader which is worn round the patient’s neckand therefore can be removed between doses, unlike the Proteus patch. This of course has advantages (no potentialskin irritation) but also disadvantages (can be forgotten by the patient). No patch is required as the sensor generatesa low-power radio signal that can be detected by a small nearby antenna. The company also plan to fit the readerinto wrist-bands and mobile phone cases [9]. The data from the reader is forwarded to a mobile phone or other in-ternet connected device via Bluetooth technology. The output of the reader can accessed via a portal by the patientand/or healthcare professionals, and can be linked with data from other devices and sensors. Context specific mes-sages are sent to the patient or others based on adherence and/or other criteria. The capsule is currently only availablefor use in IRB approved clinical trials as it has not yet received FDA clearance which the company hopes will happenin 2018. So far it has been tested with opioids, HIV medication and some other drugs.

Use of Artificial IntelligenceProteus Digital and EtectRx are not the only companies developing interactive technology to track patients’ adherenceto medication through digitalization. AiCure Inc. (NY, USA) has developed an artificial intelligence platform basedon technology which can provide facial recognition, automatic medication identification, real time ingestion confir-mation and fraud and duplicate enrolment detection all by downloading an app onto a smartphone [13]. The appprompts the patient to hold the medication in front of the camera for identification purposes and then the patient

is filmed administering the medication. The data can be analysed to check for adherence and unusual activity to en-able intervention by healthcare staff at an early stage. The platform can be customized based on disease type, patientdemographics and risk factors. The advantages of this system are that it is interactive, there is no modification ofthe dosage form required and no detector system (patch or portable reader) is required to be placed on or be closeto the patient’s body. The disadvantage is that patients must film themselves talking the medicine which requiresprivacy and may not be acceptable to all individuals.

The AiCure’s technology is aimed at two different markets: clinical development and the monitoring of adherenceto medication in the community. For this type of application the AiCure software provides automatic titration, re-minders and scheduling information to the trial participants and enables the study site to log interventions and pro-tocol deviations. With regard to the second market for this technology, the AiCure app has been used e.g. to monitoradministration of tuberculosis therapy, a task normally carried out in person by a healthcare professional. In thisstudy a 94% mean adherence rate was achieved with the increased convenience of the patients not having to leavetheir homes and without the costs associated with in-person monitoring [13].

Interconnected Injection Devices

Biopharmaceutical medicines are expensive to manufacture and often prescribed for serious chronic conditions. Dueto the instability of these compounds in intestinal fluids and their inability to be orally absorbed, injection is the onlyoption for administration. This route of administration can be off-putting to patients and the nature of some admi-nistration schedules make it difficult for patients to keep track of when they should administer their next dose. Inaddition, the cost of poor adherence in terms of patient health and the serious side-effects often associated withsuch products make them ideal candidates for support and coaching through digital services. Bayer, Germany hasbrought to the market an electronic autoinjector designed for patients to use with its Betaseron™ product.

Betaconnect™ – An electronic autoinjectorThe Betaconnect™ autoinjector is an electronic autoinjector for the subcutaneous administration of Betaseron™,Bayer’s interferon beta-1b product for the treatment of multiple sclerosis [14]. It was recently showcased in a pre-sentation by Dr. Karym El Sayed at the 2nd European Conference on Pharmaceutics which took place on 3rd and4th April 2017 in Krakow, Poland [15].

The electronic autoinjector was first approved by the FDA in 2015 [16]. The autoinjector is provided by Bayer at nocost to the patient if they register for it on the dedicated website. The autoinjector has three adjustable injectiondepth settings (8mm, 10 mm and 12 mm) to enable the doctor to determine the correct depth setting and injectiontechnique and provide training to patients and/or caregivers wishing to administer the medication at home. Theneedle automatically inserts and retracts at a customizable speed on activation of the device, meaning that patientdoes not need to see the needle. A visual progress viewer enables confirmation that the injection is complete. Thedevice captures data from each injection with regard to injection date, time, speed and depth. Patients can downloada myBeta app to activate the Bluetooth® technology within the autoinjector from the Betaconnect™ website. Onceactivated the injection history data is automatically transferred to a designated Beta nurse and other members ofthe patient’s healthcare team via secure, cloud-based software called the BETACONNECT™ Navigator. The devicealso allows patients to set reminders of when their next injection is due. Through use of the app on their smartphone,computer or laptop, patients can receive a schedule of their past and future injections, keep track of their injectionsite rotation (the injection site for Betaseron™ should be rotated in order to avoid irritation at any one site), be re-minded about data recordings and enter how well they are feeling on a particular day.

Monitoring Inhaler Use

There are relatively low rates of adherence to preventative medication among asthmatics and sufferers of ChronicObstructive Respiratory Disease (COPD) resulting in avoidable acute episodes and associated visits to clinics and hos-pitali-sations. A number of companies are working on technologies to track inhaler usage in an attempt to improveadherence and clinical outcomes to therapy for respiratory disease.

Smartinhaler™The Smartinhaler™ technology by AdheriumLtd (Auckland, New Zealand) enables the application of digital techno-logy to the use of metered dose inhalers, dry powder inhalers and nebulisers [17]. It consists of sensors that are at-

tached to the patient’s prescribed medication, mobile applications for iOS and Android and a cloud-based portal forresearchers and clinicians. The company offers custom user interface options for an additional cost. The technologyhas been granted US FDA 510(K) clearance to market and a CE mark. On use the sensors automatically send usagedata to a Smartphone with the Smartinhaler™ app via Bluetooth® or over the Smartinhaler™ Home Hub to theSmartinhaler-live.com cloud-based portal. It is also possible to transfer the data via a personal computer using aSmartkey. The technology also is capable of sending audiovisual alerts to patients to remind them when preventativemedication has to be administered.

The use of the monitoring technology has been shown to improve adherence in a number of clinical studies in adultsand children. In one study use of the device was shown to improve the adherence rate to preventative medicationfrom 30 percent to 84 percent in asthmatic children [18]. AstraZeneca, Glaxo, Teva, Mylan and Ivax are among thosewho have partnered with Adherium for use of the technology with their asthma medications e.g. Astra Zeneca’sSmartturbo™ Pulmicort™. The company recently celebrated the sale of its 100,000th Smartinhaler™.

Others companies developing digital monitoring systems for inhalers include Propeller Health Inc. (WI, USA) [19]which has a clip-on sensor system and has signed deals with Glaxo Smith Kline (for the Diskus™ and Ellipta™ inhaler),Boehringer Ingelheim (for Respimat™) and most recently Vectura; Gecko Health Innovations [20] (MA, USA) (whichis now owned by Teva Pharmaceuticals) and Qualcomm which is collaborating with Novartis. The Novartis/Qualcommcollaboration [21] differs from the others in that the goal is to produce a sensor that is integrated into the next ge-neration of Novartis’ Breezhaler™ which is used with its portfolio of COPD products.

Comment

Electronically monitoring adherence to medication dosing regimens and providing digital feedback to patients hasbecome an area of intense activity in the last two or three years, with the global market for medication adherenceproducts predicted to grow from $1.6 billion in 2016 with a compound annual growth rate of 12.3% until 2023[22]. From a pharmaceutical perspective development of digital technology for inhalers appears to be the most ad-vanced probably as a result of the positive outcomes observed in clinical studies of adherence to respiratory therapy.Developments in the field of oral products seem to be further behind with the benefits of tracking of adherence tooral administration in out-patient settings not yet proven. Most benefit with digital systems seems occur when me-dication administration and/or disease management is complicated, of a chronic nature and the consequences ofnon-adherence are not immediately apparent. For oral products which tend to be relatively simple and cheap to ma-nufacture, the need to prove compatibility between the sensors and the formulation and the additional manufactu-ring costs will always act as a barrier to the use of such technology. Drivers for its use will be increased patientengagement in their therapy due to the support and information provided, the novelty of being able to track usageand receive reminders and, for healthcare providers and payers, the reduced costs due to improved clinical outcomes.However, based on the positive outcomes seen in monitored asthma and COPD treatment and the continuing un-necessary burden placed on cash-strapped healthcare systems due to poor compliance with therapy, the inclusionof digital technology in dosage forms and devices looks set to become more common. Its role in the monitoring ofadherence in clinical trials is also likely to expand due to the need for rapid interventions to reduce study costs, andensure that the study results are not compromised by poor compliance with the protocol.
