• include benzodiazepines & non-BZD BzRAs (Z-drugs)

• All these drugs act as agonists of the bz receptor on the GABA type A receptor complex to facilitate the effect of GABA on the receptor, opening chloride channels and inhibiting neural activity

THERAPEUTIC ACTIONS OF BENZODIAZEPINES (1)

• There are three subtypes of benzodiazepine receptors:

(i) Omega-1 : mediates hypnotic (& amnesic) effect

(ii) Omega-2 : mediates anxiolytic effect

(iii) Omega-3 : mediates myorelaxant (& anticonvulsant & r

espiratory depression) effect ( 小心 COPD, sleep apnea & myathenia gravis)

• Benzodiazepines act on all three receptor subtypes.

THERAPEUTIC ACTIONS OF BENZODIAZEPINES(2)

分級原則 : 藥品依其成癮性、習慣性、依賴性、濫用性及社會危 害性之程度，分四級管理。

第一級：禁止醫療使用，高成癮性之麻醉藥品，惟 cocaine、 opium 、 morphine 除外。 第二級：禁止醫療使用之影響精神藥品，或成癮性較第一級為 低之麻醉藥品，惟大麻除外。 第三級：醫療上使用，且成癮性較第二級為低之麻醉藥品，或 醫療上使用，國内有濫用之虞之影響精神藥品，濫用 可能導致中度至輕度心理或生理依賴。 第四級：濫用性較第三級為低之影響精神藥品，或有管制必要 之前驅化學物質，濫用產生之心理或生理依賴性較第 三級低者。

適應症及用法用量• 適應症與用途

• Rozerem 適用於治療入睡困難型失眠。• 用量與用法

• 本藥須由醫師處方使用• 成人劑量

• Rozerem 建議用法，就寢前 30 分鐘內，服用 8 毫克。

• Rozerem建議不要伴隨高脂肪餐點，或在食用高脂肪餐點後立即服用 。

• Rozerem 的每天總劑量不應超過 8 毫克。• 肝功能不全患者之用藥

• 輕度肝臟功能不全患者不需劑量調整 ; 重度肝功能不全的患者，不建議服用 Rozerem。

• 與其他藥物併用• Rozerem不應併用 fluvoxamine。
