After a five-month strike, nurses have finally agreed to resume duty. This comes after intensive deliberations between their officials and the Council of Governors, which is their current employer. The two-week talk led to an agreement that their 35% pay rise be paid latest by December 2017. The officials, however, asserted that any game paying, which will lead to non-payment will be met with another industrial strike. The Collective Bargaining Agreement (CBA) that was signed by both parties also addressed the need to have better working conditions, more employment of nurses to reduce workload, nursing risk allowance, off-duty allowance, per diem allowance, increased house allowance, commuter allowance, and an annual 5% increment on their salary. The governors have promised to uphold their part of the bargain and called on the nurses to fulfill their part as well.Their resumption of duty has begun immediately! ��� �� �� �

It is of great essence to note that a number of patients we talked to abuse drugs and take a lot of alcohol. We enquired of the reason why this happens, and we discovered a few facts. 1. Most of the patients who take alcohol are unemployed. 2. They take alcohol to forget about their frustrations, and lack of basic means of living. 3. If they are taking TB drugs, there are chances that they will not take them, and forget the strict routine. 4. failing to take medication leads to a more complicated TB, which takes an even longer time to treat. 5. When there is such a problem, the fight against Tb is weakened. It becomes of interest to note that two of the TB patients in Kangemi, have passed on. One of them was a young student.I discovered a few things when I was collecting responses from the patients. The fact that many of them cannot have good food to eat contributed greatly to their death, especially the young student. TB drugs require one to have energy, and generally, good nutrition. The connection between TB and HIV is also another problem which is very widespread. In fact, the relation between TB and HIV causes a lot of stigma among the patients. This makes them not to say they have TB, or do not go to hospital completely. I also noted with concern that most patients need more follow-ups since they say the medication takes a long time. To ensure this does not happen, the next session with CHVs will have to ensure these follow-ups are done, and very strictly. Relapse TB is more dangerous than initial TB. The Kenya Ministry of Health has issued a recommendation for a rapid and robust test to diagnose pulmonary tuberculosis (TB) in peripheral health centres.The test, known as TB-LAMP (loop-mediated isothermal amplification), requires minimal laboratory infrastructure and has been evaluated as an alternative to sputum smear microscopy, still the most widespread test in use in resource-limited settings.
The machines will be rolled out in the coming year with the first Machine to be installed in Kangemi Health centre."Currently, we are working on the policy framework to ensure that the rollout of the process is smooth and done most professionally", the health minister said in a meeting at Serena Hotel.The procurement, installation and use of such machines in Kenya will therefore become easier that before, when the government had not approved their use in the country. 2017 11 21

One of the most disturbing things to patients with TB, is the stigma that comes with their association with HIV Aids patients. Much as empirical studies have shown that a good number of TB patients have HIV, the stigma is much impeding to seeking medication. Given the long-lasting psychological effect of being associated with HIV, segregation from other people, and even refusing to share anything with you, it is high time we handle this problem.In our training of CHVs, we shall focus on the need to completely protect the patients from the stigma of any kind. That will be in the community, the health facility, and other social gatherings. The facilitators will have to come up with ways of making sure the patients get treatment with high levels of dignity, to fast-track the health-seeking behaviour of the Kangemi people. At that time as well, I will talk to the nurses to practice a much more private, friendlier, and more dignified approach in handling patients of this kind. The new CHVs will have to make this a success, for the record, and for the improved results of the project in Kangemi. It is zero tolerance to the stigma on TB patients as we set to begin this new project.

It is a very good thing to learn that the guidelines I developed for the training and management of CHVs have been taken as excellent by the Nairobi County and will be used to train CHVs even in other places in Kenya.The guidelines were developed in Nairobi to manage the activities of the CHVs and make it possible for their efforts to achieve the best results. We as Results Japan are happy that we are developing what is taken as a good example by many governmental organizations in the county.I thank all staff, who, directly or indirectly, contributed to the successful completion of that guideline.
