of pancreatic pseudocysts and concluded that it should be a good method of treatment in high risk patients or in cases of previous surgical failure. Between March 1980 2 and December 1985, we had the opportunity to perform an endoscopic cystoduodenostomy (ECD) for 13 patients and a cystogastrostomy (ECG) for five patients, leaving a transnasal drain in the cyst for several days. Cystoduodenostomy was performed for painful pancreatic cysts protruding into the second portion of the duodenum and occurring in patients with chronic pancrea- titis. Disappearance of the cyst was observed in 12 cases (Fig. 1). After a median follow-up of 16 months, we observed two cases of relapsing pain and cysts, which were success- fully treated by another ECD. Endoscopic cystogastrostomy, performed for large retro- gastric pseudocysts, was followed by total regression in three cases without recurrence; in the first case, surgery was necessary to achieve complete drainage, and in the last one, which concerned a patient with three communicating cysts, total regression occurred after the addition of percutaneous external drainage. We did not observe any complications in either the ECDs or the ECGs. When the cysts are accessible, this type of endoscopic management is safe and less invasive than sur- gery. Therefore, this treatment need not be restricted to poor risk patients and, indeed, has become our first step for the management of pancreatic pseudocysts. M. Cremer, MD J. Ponsky catheter To the Editor: Recently, we have had difficulty with the use of a Medi- tech PEJ catheter (Ponsky catheter #324, American Endos- copy Co., Inc., 7150 Hard Street, Mentor, Ohio 44067). This information will, I suspect, be valuable to those who may be considering use of this device. This new catheter employs a 20 F mushroom catheter which contains two 8 F inner catheters. One serves as a jejunostomy tube and the other as a gastrostomy tube. The dilating stilette used in the placement of this catheter is the standard 16 F Medicath catheter. When we pulled the 20 F mushroom catheter through the abdominal puncture site, significant tension was placed on the mushroom catheter and it stretched. As it stretched, the inner plastic catheters were pulled farther into the mushroom catheter. The tension was relaxed, the mushroom catheter contracted to its former size, but the plastic catheters within it coiled and became 368 kinked and obstructed. In our patient, the catheter had to be removed. I think that some modification needs to be made in the construction of this catheter and the manner in which it is placed if it is to be used effectively. Martin S. In any event, obstruction of the feeding port should not prove dangerous to the patient. This situation, while frustrating, may be remedied in a number of ways. The tube may be left in place for a week or so and the tract allowed to mature. It may then be replaced with another tube. Tubes for percutaneous gastrostomy and jejunostomy, like the procedures themselves, are very new and will require constant improvement and development. With a quick, deft stroke, they have brushed aside the preliminary misconcep- tions engendered by "television" endoscopy and focused attention upon the essence of video endoscopy: the digital image. By utilizing this computer-generated image we have now transformed our visualization of the gastrointestinal tract into a form of information which offers boundless opportunity. Because it is digital information (computer data, if you wish), this new mode of viewing the gastrointes- tinal tract is placed smack in the middle of the vast computer revolution. Therefore, capabilities enumerated by Graham et al. represent but a few of the possibilities of this infant system. The following is a partial list of capabilities that video endoscopy could, with the appropriate software control, perform alone or integrated with any or all of the other functions: (1) video enhancement to intensify and/or subdue certain elements of the image to make subtle differences readily apparent; (2) video magnification to "zoom in" on details and "blow up" a small area to full screen size; (3) GASTROINTESTINAL ENDOSCOPY
